Role of neoadjuvant treatment in clinical T2N0M0 oesophageal cancer: results from a retrospective multi-center European study
International audienceAims: The aims of this study were to compare short-and long-term outcomes for clinical T2N0 oesophageal cancer with analysis of (i) primary surgery (S) versus neoadjuvant therapy plus surgery (NS), (ii) squamous cell carcinoma and adenocarcinoma subsets; and (iii) neoadjuvant chemoradiotherapy versus neoadjuvant chemotherapy.Methods: Data were collected from 30 European centres from 2000 to 2010. Among 2944 included patients, 355 patients (12.1%) had cT2N0 disease; 285 (S) and 70 (NS), were compared in terms of short-and long-term outcomes. Propensity score matching analyses were used to compensate for differences in baseline characteristics.Results: No significant differences between the groups were shown in terms of in hospital morbidity and mortality. Nodal disease was observed in 50% of S-group at the time of surgery, with 20% pN2/N3. Utilisation of neoadjuvant therapy was associated with significant tumour downstaging as reflected by increases in pT0, pN0 and pTNM stage 0 disease, this effect was further enhanced with neoadjuvant chemoradiotherapy. After adjustment on propensity score and confounding factors, for all patients and subset analysis of squamous cell and adenocarcinoma, neoadjuvant therapy had no significant effect upon survival or recurrence (overall, loco-regional, distant or mixed) compared to surgery alone. There were no significant differences between neoadjuvant chemotherapy and chemoradiotherapy in short-or long-term outcomes.Conclusion: The results of this study suggest that a surgery alone treatment approach should be recommended as the primary treatment approach for cT2N0 oesophageal cancer despite 50% of patients having nodal disease at the time of surgery

Tandis que dans la ville préindustrielle leur existence se déroulait principalement dans la rue, les enfants ont été parmi les premiers concernés par le transfert d’un nombre important d’activités vers les espaces privés, qui a entraîné le déclin des espaces publics en tant qu’espaces de travail et de sociabilité dans les villes occidentales. Cet article s’appuie sur une enquête menée à Paris et Milan pour interroger la perception qu’ont les parents actuels des évolutions de la présence des enfants dans les espaces publics, au regard de leur double expérience d’enfant puis de parent. En exposant ce qui ressort de leurs souvenirs de manière transversale sur les deux terrains, il vise à éclairer le déclin de la présence des enfants dans les espaces publics des sociétés urbaines occidentales, au-delà des cas de ces deux villes
Involvement of Ion Channels in Endothelin-1-induced Signalling in Human Prostate Cancer Cells
International audienceObjective: Endothelin-1 (ET-1), a potent vasoconstrictor secreted primarily by endothelial and various epithelial cancercells has been implicated in prostate cancer progression and the ET axis has been suggested to represent a novel andexciting target in the treatment of prostate cancer (PCa). ET-1, acting primarily through the endothelin receptors (ETRs)is integrally involved in multiple facets of PCa progression, including cell growth, inhibition of apoptosis, angiogenesis anddevelopment of bone metastases. ET-1 and ETRs are expressed in PCa tissues and their expression is modulated duringthe evolution of these cancers. The purpose of the present work was to study the effects of ET-1 on proliferation of humanPCa cells PC-3 and the mechanisms by which the activation of ETRs may promote the PCa cells growth.Methods: Prostate cancer cell lines and primary cultured epithelial cells from prostate cancer, RT-PCR and calciumimaging techniques were used to study the expression and functionality of the Endothelin receptors and involvement ofion channels in the effects of ET-1 in prostate cancer cells.Results: We show for the first time that the application of ET-1 induces a dose-dependent cell proliferation andan increase in intracellular free Ca2+ concentrations ([Ca2+]i) via a mobilisation of the internal calcium stores and bya capacitative calcium entry (CCE). These effects of ET-1 were completely abolished by BQ123, a selective ETARantagonist, but not by BQ788, a selective ETBR antagonist. By use of pharmacological inhibitors and siRNA targetingcalcium-activated (IKCa1 and BKCa) potassium channels and calcium channels (TRPC1, TRPV6, Orai1), we showedthat these ion channels play an important role in calcium entry and cell proliferation induced by ET-1 in PCa cells.Conclusion: These results suggest that these ions channels evidenced here might constitute potential targets toblock the ET axis in human prostate cancers
Decentralized Collaborative Learning of Personalized Models over Networks
We consider a set of learning agents in a col-laborative peer-to-peer network, where each agent learns a personalized model according to its own learning objective. The question addressed in this paper is: how can agents improve upon their locally trained model by communicating with other agents that have similar objectives? We introduce and analyze two asynchronous gossip algorithms running in a fully decentralized manner. Our first approach , inspired from label propagation, aims to smooth pre-trained local models over the network while accounting for the confidence that each agent has in its initial model. In our second approach, agents jointly learn and propagate their model by making iterative updates based on both their local dataset and the behavior of their neighbors. Our algorithm to optimize this challenging objective in a decentralized way is based on ADMM

International audienceIl s'agit de montrer que et comment les textes biographiques de Dicéarque peuvent avoir contribuer à la critique de la conception contemplative de la philosophie, hypothèse proposée par Jaeger qui doit être modifée sur plusieurs points. Après une analyse de ce que pouvait être le traité "Sur les vies" de Dicéarque, on se concentre sur la vie de Pythagore pour montrer que ce dernier n'était pas un modèle mais plutôt un contre-modèle pour Dicéarque, et on montre les liens des textes biographiques avec les critiques de la vie contemplative, des doctrines pythagoriciennes de l'âme et on fait l'hypothèse d'un débat avec la représentation de Pythagore et de ses doctrines chez Héraclide Pontique et, plus généralement, dans l'Académie
Squirrel: Architecture Driven Resource Management
International audienceResource management is critical to guarantee Quality of Service when various stakeholders share the execution environment , such as cloud or mobile environments. In this context, providing management techniques compatible with standard practices, such as component models, is essential. Resource management is often realized through monitoring or process isolation (using virtual machines or system containers). These techniques (i) impose varying levels of overhead depending on the managed resource, and (ii) are applied at different abstraction levels, such as processes, threads or objects. Thus, mapping components to system-level abstractions in the presence of resource management requirements can lead to sub-optimal systems. We propose Squirrel, an approach to tune component deployment and resource management in order to reduce management overhead. At run-time, Squirrel uses an architectural model annotated with resource requirements to guide the mapping of components to system abstractions, providing different resource management capabilities and overhead. We present an implementation of Squirrel, using a Java component framework, and a set of experiments to validate its feasibility and overhead. We show that choosing the right component-to-system mappings at deployment-time reduces performance penalty and/or volatile main memory use
