Necessidades de Cuidados em Saúde Mental: A Perceção da Pessoa com Doença Mental Versus a do Técnico
Shared decision‑making supports users to experience greater empowerment, including a subjective sense of involvement in treatment, self‑efficacy and autonomy. Health care should be provided on the basis of need, with an intended goal of improving quality of life. It is known that the number of unmet needs has been directly implicated in a lower quality of life for people. The studies differ as to the level of agreement between the unmet needs identified by patients and technicians but the needs of a social and psychosocial nature corresponded to the majority of unmet needs identified. The needs assessment should be part of the psychiatric clinical practice and should not be used only for research. A policy of actively assessing and addressing patient‑rated unmet needs may lead to improved quality of life.A tomada compartilhada de decisões entre técnicos e pacientes permite que estes experienciem uma maior capacitação, incluindo uma sensação subjetiva de envolvimento no tratamento, autoeficácia e autonomia. Os cuidados ao doente devem ser prestados com base nas necessidades identificadas pelo mesmo, de modo a promover a qualidade de vida. A quantidade de necessidades não satisfeitas tem sido diretamente implicada numa menor qualidade de vida das pessoas. Os estudos realizados divergem quanto ao nível de concordância entre as necessidades não satisfeitas identificadas pelos doentes e pelos técnicos, mas as necessidades de natureza social e psicossocial corresponderam à maioria das necessidades sem resposta identificadas. A avaliação de necessidades deve fazer parte da prática clínica psiquiátrica e não deve ser apenas utilizada para investigação. Uma política de avaliar ativamente e abordar as necessidades não atendidas identificadas pelo paciente pode levar a uma melhor qualidade de vida
Síndrome da Boca Ardente: Casos Clínicos
Burning mouth syndrome (BMS) is a relatively common chronic disease of unclear etiopathogenesis. It is defined as a distinctive nosological entity that includes all forms of burning sensation in the mouth, including stinging sensation or pain, in association with an oral mucosa that appears clinically normal, in the absence of local or systemic diseases. It was hypothesized that psychological factors could influence its appearance, but also physiological factors, with some authors suggesting a neuropathic etiology. Regarding the treatment of BMS, benefits were found in individual or group psychotherapy, as well as in psychopharmacological interventions, however its current therapeutic approach is not completely satisfactory. In this paper we describe two cases of BMS and discuss the possible etiopathogenesis and current therapeutic approaches.A síndrome da boca ardente (SBA) é uma doença crónica relativamente comum de etiopatogenia desconhecida. É definida como uma entidade nosológica distinta que inclui todas as formas de sensação de ardência bucal, incluindo sensação de picada ou dor, em associação com uma mucosa oral que aparenta ser clinicamente normal, na ausência de doenças locais ou sistémicas. Foi colocada a hipótese de que fatores psicológicos pudessem influenciar o seu aparecimento, assim como fatores fisiológicos, com alguns autores a sugerir uma etiologia neuropática. No que diz respeito ao tratamento da SBA foram encontrados benefícios em psicoterapia individual ou de grupo, assim como em intervenções psicofarmacológicas, no entanto a sua abordagem terapêutica atual não é totalmente satisfatória. Neste artigo descrevemos dois casos de SBA e discutimos a sua possível etiopatogenia e abordagens terapêuticas vigentes
Impacto da Cirurgia Bariátrica na Imagem Corporal e Saúde Mental
Body image, in the subsequence of bariatric surgery and the inherent fast and massive experience of weight loss, may suffer many changes such as dissatisfaction or distortion. This narrative review aims to explore the conception of body image in obese patients and the experience of their own body after bariatric surgery, as well as the reflection of this intervention in their mental health. Additionally, this study further clarifies the potential dissociation between the time of physical‑metabolic adaptation and psychological adaptation, the use of aesthetic correction procedures after bariatric surgery, as well as the motivations that sustain the choice for such surgical strategies.The acquisition of the bibliographic information was performed through a research on PubMed and SCOPUS platforms, starting from the following combination of terms: body image and bariatric surgery. Studies obtained in the last five years were analyzed, corresponding to investigations conducted or based on the obese population. Most of the studies corroborate the benefits of bariatric surgery in the conception of body image; however, the existence of unsatisfactory results, limited improvements in time or negative repercussions in the field of mental health have also been reported. The psychological transformation may even take a different pace of development from the fast and simultaneous physical‑metabolic transformation. The evidence of the benefit of having subsequent aesthetic corrective surgery, such as body lifting, has been increasingly consistent, associated with generalized psychosocial improvements.However, the motivations that underlie the use of bariatric surgery and body lifting remain unclear.The changes caused by bariatric surgery may expose the patient to stresses never experienced or projected before surgery, so that the long‑term follow‑up of the post‑bariatric patient by psychology or psychiatry is an essential step for the whole evaluation of the effect of this type of surgery.Introdução: A imagem corporal (IC), na sequência do recurso a cirurgia bariátrica (CB) e da experiência rápida e massiva de perda de peso inerente, poderá sofrer alterações como satisfação, insatisfação ou distorção. Esta monografia pretende explorar a concepção da IC do doente obeso e a sua experiência perante o próprio corpo após recurso a CB, bem como o reflexo desta intervenção ao nível da saúde mental. Aditivamente, este estudo esclarece ainda a potencial dissociação entre o tempo de adaptação físico-metabólica e de adaptação psicológica, o recurso a técnicas de correção estética após CB, bem como as motivações na base da opção por estratégias cirúrgicas.
Metodologia: A aquisição da informação bibliográfica foi conduzida através de uma pesquisa realizada nas plataformas PubMed e SCOPUS, partindo da seguinte combinação de termos: body image e bariatric surgery. Estudos obtidos nos últimos 5 anos foram analisados, correspondendo a investigações conduzidas ou tendo por base a população obesa.
Resultados: A maioria dos estudos corrobora o efeito benéfico da CB ao nível da concepção da IC, contudo, a existência de resultados insatisfatórios, melhorias limitadas no tempo ou de repercussões negativas no domínio da saúde mental têm também vindo a ser reportadas. A transformação psicológica implícita pode, inclusive, tomar um ritmo de desenvolvimento discrepante da rápida e simultânea transformação físico-metabólica. A evidência do benefício de recurso a cirurgia estética corretora, como lifting corporal (LC), tem-se apresentado cada vez mais consistente, associado a melhorias psicossociais generalizadas. No entanto, as motivações que servem de base ao recurso a CB e LC subsequente permanecem pouco esclarecidas
Conclusões: As mudanças causadas pela CB poderão expor o doente a tensões nunca antes por si experienciadas ou projectadas antes da cirurgia; assim, o seguimento a longo prazo do doente pós-bariátrico por psicologia e/ou psiquiatria assume-se uma etapa de avaliação imprescindível
Discriminação e Barreiras ao Acesso ao Serviço Nacional de Saúde Percecionados por Pessoas Trans
Introduction: Trans people, who need specific medical, surgical and psychological healthcare due to their condition, are confronted with discrimination, prejudice and stigma thus limiting their access to health services. The present study aims to assess barriers in trans health in three distinct public services (emergency, primary care and surgery).
Material and Methods: A non‑controlled cross‑sectional study was conducted, using interviews with 71 trans people, after their sexology consult. An interview model was elaborated by the authors, adapting questions from the Trans Pulse Project and the 2015 US Transgender Survey.
Results: Half of the trans people who used the services studied perceived discrimination by a healthcare provider. The use of prejudiced language was the most frequent perceived episode of discrimination and the emergency department was the place where it occurred with higher frequency. We found an association between episodes of perceived discrimination in family medicine and the discomfort felt in addressing trans health with the family physician.
Discussion: The perceived discrimination felt by trans people was similar to the literature data, in emergency and family medicine services. The lack of knowledge of standards of care by health providers may reinforce barriers to trans healthcare, as seen in the literature.
Conclusion: We found the need to study the impact of trans discrimination, around the country, as welll as evaluate and promote trans healthcare amongst healthcare providers.Introdução: A população trans, que necessita de tratamentos médicos, cirúrgicos e psicológicos específicos pela sua condição, é confrontada com discriminação, preconceito e estigma, na procura de serviços de saúde, que limitam o seu acesso. Este estudo pretende avaliar barreiras no acesso à saúde, por esta população, em três ambientes distintos do Serviço Nacional de Saúde (urgência, medicina geral e familiar e cirurgia).
Material e Métodos: Realizou‑se um estudo transversal, não controlado, com recurso a entrevista a 71 pessoas trans, após consulta de Sexologia. Um modelo de entrevista foi elaborado pelos autores, adaptando questões do Trans Pulse Project e do 2015 US Transgender Survey.
Resultados: Metade dos trans que recorreram aos três contextos estudados percecionaram discriminação por um profissional de saúde. O uso de linguagem preconceituosa foi o episódio de perceção de discriminação mais frequente e o serviço de urgência o local onde ocorreu com mais frequência. Encontramos relação entre episódios de perceção de discriminação em medicina geral e familiar, com o desconforto sentido em abordar saúde trans com o médico de família.
Discussão: A discriminação percecionada por pessoas trans foi semelhante à verificada na literatura, em contexto de urgência e medicina geral e familiar. O incumprimento e desconhecimento de guidelines de boas práticas clínicas, por profissionais de saúde, à semelhança da literatura, podem constituir barreiras de acesso a cuidados de saúde, na população estudada.
Conclusão: Verificamos que existe a necessidade de estudar, a nível nacional, o impacto da discriminação na população trans, bem como de avaliar e promover a formação de profissionais de saúde nesta área
Pandemia COVID‑19 e Perturbação Mental: Breve Revisão da Literatura
The COVID‑19 pandemic has been declared a global health emergency. It is estimated that it may lead to a high level of suffering, and the crisis period is expected to give rise to various challenges in the sphere of mental health, both in the general population and in people with a previous mental disorder. Psychological suffering and generalized anguish due to the immediate impact of the virus on health, but also the need to implement quarantine measures, physical isolation associated with loss in educational and work activities, predict the fear of the disease, the fear of death and uncertainty about the future. On the other hand, it is estimated that the COVID‑19 pandemic may disproportionately affect the most vulnerable populations, namely health professionals and those with a previous history of mental disorder.
Due to the threat of mental health problems that the general population may face and the possible worsening of the underlying condition in patients with previous mental disorders, it is important to assess the various emerging problems. Indeed, mental health services need structural adaptation to deal with the increased risk of psychiatric morbidities, developing and implementing screening and early intervention programs, aimed not only at acute effects but also long‑term or chronic effects of this pandemic.
The preparation of this review article aims to gather, in a rigorous manner, the information dispersed in various scientific sources currently existing on the impact of COVID‑19 infection on mental health, as well as clinical experience in the field. In this way, the authors reviewed the literature, through research through PubMed and Medscape using the keywords “Pandemic”, “COVID‑19”, “Mental Health”, “Mental Disorder”.
The article proposes to develop concisely and summarily some of the particular problems inherent to the pandemic COVID‑19 and mental health, proposing some strategies and interventions to face the situation of crisis experienced. However, the future need for continuous studies and evaluation of the practical experience must be considered, to reorient mental health intervention policies and measures. A pandemia COVID‑19 foi declarada uma emergência de saúde mundial. Estima‑se que poderá originar um elevado nível de sofrimento, prevendo‑se que o período de crise origine variados desafios na esfera da saúde mental, tanto na população geral como nas pessoas com perturbação mental prévia. O sofrimento psicológico e angústia generalizada devido ao impacto imediato do vírus na saúde, mas também a necessidade de implementar medidas de quarentena, de isolamento físico associado à perda nas atividades educativas e laborais, adivinham o medo da doença, o receio da morte e incerteza quanto ao futuro. Por outro lado, estima‑se que a pandemia COVID‑19 poderá afetar de modo desproporcional populações mais vulneráveis, nomeadamente os profissionais de saúde e aqueles com antecedentes prévios de perturbação mental.
Pela ameaça de problemas da saúde mental que a população em geral pode enfrentar e a possível agudização da condição de base nos doentes com perturbação mental prévia éimportante uma avaliação dos vários problemas emergentes. Com efeito, os serviços de saúde mental necessitam de uma adaptação estrutural de modo a lidar com o risco acrescido das morbidades psiquiátricas, desenvolvendo e implementando programas de rastreio e intervenção precoce, que visem não apenas os efeitos agudos, mas também os efeitos a longo prazo ou crónicos desta pandemia.
A elaboração deste artigo de revisão tem por objetivo reunir, de forma rigorosa, a informação dispersa em variadas fontes científicas atualmente existentes sobre o impacto da infeção por COVID‑19 na saúde mental, assim como a experiência clínica no terreno. Deste modo, os autores realizaram uma revisão da literatura, pela pesquisa através da PubMed e Medscape usando as palavras‑chave “Pandemic”, “COVID‑19”, “Mental Health”, “Mental Disorder”.
O artigo propõe‑se a desenvolver de forma concisa e sumária algumas das problemáticas particulares inerentes à pandemia COVID‑19 e saúde mental, propondo ainda algumas estratégias e intervenções para fazer face à situação de crise vivida. Considera‑se, no entanto, a necessidade futura da contínua realização de estudos e avaliação da experiência prática, de modo a reorientar as políticas e medidas de intervenção na saúde mental. 
O Consumo Alcoólico Estará Aumentado em Adolescentes com Fat Bingeing?
Evidence has been suggesting that neurochemical and behavioral adaptations emerging during one addictive behavior may enable a subsequent, different, addiction, even when the first one has ended. Such association seems remarkably accurate to fat bingeing and ethanol consumption, which constitute a relevant example of addictive patterns sequential association, particularly in adolescence. Therefore, the main purpose of the following review is to comprehend if adolescents who have engaged on fat bingeing escalated their ethanol consumption, after fat bingeing terminus, and secondly, in a positive scenario, to highlight why such association may happen. After searching databases such as MEDLINE (PubMed) and Clarivate Analytics Web of Science, all articles focused on adolescent humans or adolescent rats or adolescent mice, that included topics on fat bingeing and consequent ethanol consumption, were considered. Gathered results strengthened the referred hypothesis. Reports stating that adolescent fat bingers engage on escalated ethanol consumption were fundamentally supported by a prevalent fast food and ethanol cultures, bingers ‘shared personality traits, adolescence ‘susceptibilities for addictive behaviors and neurochemical and behavioral craving installment after fat bingeing cessation. Given that adolescence is a key structural and functional phase of human development, and that fat and ethanol consumption are linked to harmful physiological and social effects, priority must be given to multidisciplinary interventions aiming to challenge addictions, both pharmacologically and psychotherapeutically.A evidência tem sugerido que existem alterações, de índole neuroquímica e comportamental, que ocorrem durante uma perturbação aditiva e que propiciam uma perturbação aditiva subsequente, inclusive após a primeira ter terminado. Esta associação, entre dependências de substâncias, parece ser bastante representativa para fat bingeing e consumo alcoólico, que constituem, então, um exemplo representativo de associação sequencial entre perturbações aditivas, particularmente na adolescência. Assim, o objetivo principal da seguinte revisão é verificar se adolescentes com fat bingeing, tiveram aumento do consumo alcoólico, após término do fat bingeing, e num cenário positivo, compreender porque é que tal associação acontece. Depois de uma pesquisa em bases de dados, como a MEDLINE (PubMed) e a Clarivate Analytics Web of Science, foram considerados os artigos que incluíram fat bingeing e consumo alcoólico em adolescentes, quer humanos quer ratos. Os resultados reunidos defendem fortemente a hipótese questionada. Os diferentes estudos, que afirmaram que adolescentes com fat bingeing têm consumo alcoólico aumentado, baseiam‑se, fundamentalmente: na vigente cultura de fat food e consumo alcoólico, nos traços de personalidade transversais a doentes com dependência de substâncias, nas suscetibilidades inerentes à própria adolescência para dependência, e na instalação neuroquímica e comportamental de craving, após a cessação de fat bingeing. Tendo em conta que a adolescência é uma fase chave no desenvolvimento estrutural e funcional do ser humano, e conhecendo os efeitos pejorativos, fisiológicos e sociais, do fat bingeing e do consumo alcoólico, é recomendado que seja dada prioridade a intervenções multidisciplinares, de nível psicoterapêutico e farmacológico, que possam combater as perturbações consideradas
Doença de Marchiafava‑Bignami: A Importância do Diagnóstico e Tratamento Precoces por uma Equipa Multidisciplinar
Marchiafava‑Bignami disease is a rare condition associated with chronic alcohol consumption and/or malnutrition, characterized by demyelination of the corpus callosum, generally attributed to a deficiency in B complex vitamins. We report the case of a 34‑year‑old male with a 10‑year history of alcohol dependence who was admitted to the hospital, after having been found lying on the floor of his house, malnourished and with pressure ulcers on his chest and knees. On clinical observation he was found to be alert but mute. He followed some simple orders. Generalized spastic hypertonia was present. Magnetic resonance imaging showed demyelination of the corpus callosum, suggesting the diagnosis of Marchiafava‑Bignami disease. He was admitted to the Psychiatry Inpatient Unit and evaluated by a multidisciplinary team. He received thiamine, corticosteroids and rehabilitation. After a week, his speech was slurred but comprehensible and he could walk with aid; magnetic resonance imaging findings had improved. After three months in a Convalescence Unit, he was discharged with total autonomy.A doença de Marchiafava‑Bignami é uma condição rara associada ao consumo crónico e abusivo de álcool e/ou desnutrição, caracterizada por desmielinização do corpo caloso, sendo geralmente atribuída a uma deficiência de vitaminas do complexo B. Apresentamos o caso de um homem de 34 anos com 10 anos de dependência de álcool que foi trazido ao hospital, após ter sido encontrado caído no chão de sua casa em decúbito ventral, desnutrido, com úlceras de pressão no tórax e joelhos. Estava vígil, não emitia discurso, cumpria ordens simples e apresentava hipertonia espástica generalizada. Na ressonância magnética identificou‑se desmielinização do corpo caloso, sendo diagnosticado com doença de Marchiafava‑Bignami. Foi internado no serviço de psiquiatria e avaliado por uma equipa multidisciplinar, iniciando terapêutica com tiamina e corticosteróides e um programa de reabilitação. Após uma semana, apresentava melhoria clínica e imagiológica. Após um período de 3 meses numa Unidade de Convalescença, teve alta totalmente autónomo
