    Relationship between serum oxidative stress and blood brain barrier parameters in patients with major depression

    Objective: Research emphasises the critical role of blood-brain barrier (BBB) integrity in psychiatric disorders. This study aimed to investigate the relationship between serum Zonula occludens protein - 1 (ZO-1) and serum occludin levels, which are blood brain barrier markers, and serum oxLDL levels, which are known to be associated with oxidative stress and inflammation, in patients with major depression. Methods: The study included 44 drug-naïve patients diagnosed with Major Depressive Disorder (MDD) and 48 healthy controls matched for Body Mass Index (BMI), gender, age. Laboratory data, together with concentrations of Ox-LDL, Occludin, ZO-1, were evaluated. The Hamilton Depression Rating Scale (HADRS) was employed to evaluate the severity of depression in the major depression cohort. Result: Compared to healthy controls, patients with major depression exhibited significantly lower serum levels of ZO-1 (p = 0.001) and occludin (p = 0.001), while oxLDL levels were significantly higher (p = 0.001). The differences maintained as significant after corrections for age, sex, smoking, and BMI via ANCOVA (all p = 0.001). A positive correlation was identified between ZO-1 and occludin levels (r = .400, p = 0.001), whereas a negative correlation was found between ZO-1 and oxLDL levels (r = −.369, p = 0.001). Conclusion: This study's findings indicate that serum oxLDL levels, related with oxidative stress and inflammation in the pathogenesis of MDD, may influence the alteration of serum ZO-1 and occludin levels, recognised as indicators of the BBB

    Activity of ceftazidime-avibactam combinations against carbapenem-resistant Klebsiella pneumoniae assessed by checkerboard method

    Abstract: The aim of this study is to detect the carbapenemase type and to determine the in vitro effects of ceftazidime/avibactam-colistin, ceftazidime/avibactam-meropenem and ceftazidime/avibactam-tigecycline combinations against Carbapenem-Resistant Klebsiella pneumoniae (CRKP) isolates. A total of 35 CRKP isolates were included to the study. The minimum inhibitory concentrations of ceftazidime/avibactam, meropenem, colistin and tigecycline were determined by broth dilution method. Synergistic effects of ceftazidime/avibactam-colistin, ceftazidime/avibactam-meropenem and ceftazidime/avibactam-tigecycline were investigated by microdilution checkerboard method. Carbapenemase genes (blaOXA-48, blaNDM, blaKPC, blaIMP, blaVIM) were detected by multiplex PCR. All of the isolates were resistant to meropenem, whereas 77.1% of the isolates were resistant to ceftazidime/avibactam and 14.3% of the isolates were resistant to colistin.The carbapenemase genes of the CRKP isolates were determined as 17 OXA-48+NDM, 9 KPC, 6 OXA-48, 1 NDM, 1 KPC+NDM and 1 KPC+OXA-48. Ceftazidime/avibactam-colistin, ceftazidime/avibactam-meropenem and ceftazidime/avibactam-tigecycline combinations were synergistic against 5.7% (2/35), 17.1% (6/35), and 5.7% (2/35) of the isolates, respectively. Ceftazidime/avibactam-meropenem was the most effective synergistic combination in our study, showing synergism in 17.1% of isolates, however, the synergistic effect varied depending on the CRKP isolate tested

    GirişBilateral vokal kıvrım paralizisi (BVKP), her iki vokal kıvrımının hareket kabiliyetinin kaybı sonucu hava yolu açıklığında daralmayla karakterizedir. En sık etiyolojik neden cerrahi travma olup, tiroid cerrahisi sonrası görülme oranı %0,5’in altındadır. Tedavi, hava yolu güvenliği, fonksiyonel ses üretimi ve aspirasyonun önlenmesi hedeflerine dayanır. Cerrahi seçenekler arasında parsiyel/total aritenoidektomi, posterior kordotomi ve sütür lateralizasyonu yer alır.OlguElli üç yaşında erkek hasta, 3–4 aydır devam eden progresif nefes darlığı, stridor ve ses kısıklığı şikayetleri ile başvurdu. Fleksibl laringoskopide vokal kıvrımlarda hareket yokluğu ve glottik açıklığın ciddi derecede daraldığı görüldü. Trakeotomiye alternatif olarak sağ vokal kıvrım sütür lateralizasyonu uygulandı. Postoperatif dönemde inspiratuvar stridor tamamen düzeldi, hava yolu stabil hale geldi ve aspirasyon bulgusu izlenmedi.SonuçBVKP olgularında sütür lateralizasyonu, trakeotomiye etkili bir alternatif olarak hava yolu açıklığını sağlayabilir. Uygun hasta seçimi ve cerrahi deneyim ile bu yöntem, özellikle fonatuvar beklentileri olan hastalarda avantajlı bir seçenek sunmaktadır.</p

    The relationship between prenatal care satisfaction, prenatal breastfeeding self-efficacy and readiness for newborn hygienic care in pregnant women: a cross-sectional study

    The care satisfaction of pregnant women is very important for maternal and newborn health. This study aimed to examine the and affecting factors relationships among prenatal care satisfaction, prenatal breastfeeding self-efficacy and readiness for newborn hygienic care. This cross-sectional study was conducted in a hospital with 320 pregnant women. Data were collected using a personal information form, Prenatal Care Satisfaction Scale (PCSS), Prenatal Breastfeeding Self-Efficacy Scale (PBSS), and Scale for Readiness of Pregnant Women to Hygienic Care of the Newborn (SRPHN). Mann-Whitney U, Kruskal Wallis-H, and Spearman’s Correlation tests were used to analyze the data. The PCSS, PBSS and SRPHN mean scores were 72.32 ± 9.83, 87.01 ± 7.94 and 62.73 ± 9.08, respectively. There was a positive correlation between prenatal care satisfaction and prenatal breastfeeding self-efficacy and readiness for neonatal hygienic care. Prenatal care satisfaction was affected by income and health support. Prenatal breastfeeding self-efficacy was influenced by educational status, income status, family structure, previous breastfeeding duration, and regular follow-up visits. Age, marriage duration, educational status, number of pregnancies, number of children, previous mode of delivery, and regular follow-up visits affected readiness for newborn hygienic care. Holistic care and well-planned education should be provided to women to improve prenatal care satisfaction, breastfeeding self-efficacy, and readiness for newborn hygienic care

    Clinical characteristics, mid-term outcomes and management of asymptomatic primary hyperparathyroidism: A multicentre retrospective cohort study

    Purpose: The primary objective of this study was to retrospectively evaluate the demographic, biochemical, and clinical characteristics of patients with asymptomatic primary hyperparathyroidism (aPHPT), analyze their long-term outcomes, and discuss the effectiveness of current therapeutic strategies in light of the existing literature. We anticipate that our study will provide clinicians with guidance regarding surgical decision-making beyond the standard criteria for aPHPT. Methods: This was a nationwide, multicenter, observational, retrospective cohort study. All tertiary care endocrinology departments across the country were invited to participate. Center inclusion criteria required the enrollment of a sufficient number of aPHPT patients, confirmed by careful diagnostic evaluation in accordance with established guidelines, regular follow-up for at least one year, and systematic monitoring for complications. Results: Data from 27 centers representing various regions of Turkey were included in the study. A total of 829 patient records were reviewed, and after excluding 25 patients who did not meet eligibility criteria, 804 patients were included in the final analysis. The mean age was 55.59 ± 11.54 years, with a female predominance (85%, n = 683). The baseline prevalence of comorbidities was as follows: hypertension in 43% (n = 346), diabetes mellitus in 27.1% (n = 216), and cardiovascular disease in 10.8% (n = 87) of patients. Bone mineral density (BMD) measurements were available for 701 patients, among whom osteoporosis was present at 23.4% and osteopenia at 50.5%. Glomerular filtration rate (GFR) significantly declined by the third year compared to baseline (p = 0.003). The prevalence of cardiovascular disease significantly increased at year three compared to baseline and the first year (p = 0.002). A significant association was found between the presence of osteoporosis and lower baseline serum magnesium levels (p < 0.001), particularly among patients with magnesium levels below 1.6 mg/dL compared to those with levels within the normal range (1.6–2.5 mg/dL). Additionally, patients with osteoporosis demonstrated significantly lower serum phosphate levels compared to other groups (p < 0.001). Conclusion: The management of asymptomatic PHPT continues to present numerous unanswered questions. Hypertension, cardiovascular disease, and diabetes mellitus were found to be more prevalent among aPHPT patients compared to the general population. The observed increase in cardiovascular events over time suggests that cardiovascular disease could become a more prominent factor in future surgical decision-making. Furthermore, the significant decline in GFR over follow-up may necessitate revisiting the surgical threshold of GFR < 60 mL/min in future guidelines. Routine assessment of serum magnesium and phosphate levels should be considered, particularly in patients at high risk for osteoporosis. Future research may further redefine surgical indications in the management of aPHPT
