    Laparoscopic Sciatic Nerve Dissection: a Reproducible Approach

    BackgroundTo present the anatomical landmarks of a standardized and reproducible surgical approach enablinglaparoscopic sciatic nerve dissection utilising the ‘’ medial’’ approach.MethodsNarrated surgical video. Local institutional review board not required, patient consent was obtained.The sciatic nerve can be compressed by several gynaecological and obstetrical causes leading tosciatica, with endometriosis being the commonest gynaecological aetiology. Sciatic nerve dissectioncan be performed laparoscopically via a ‘’lateral’’ or a ‘’medial’’ approach. The ‘’lateral’’ approachutilises a plane of dissection lateral to external iliac vessels, whereas, in the ‘’medial’’ approach, thesurgeon dissects medially to the external iliac vessels. 28-year-old patient with chronic pelvic painradiating to the posterior aspect of her right lower limb. Pre-operative imaging did not identify anypathology accounting for her symptomatology, however, due to the severity of symptoms, anexploratory laparoscopy was decided upon.ResultsLaparoscopic sciatic nerve dissection utilising the ‘’medial’’ approach can be performed by followingthese steps: 1. Horizontal incision of the peritoneum, parallel to the gonadal vessels, in the trianglethat is formed by the adnexa, the external iliac vessels and the round ligament. 2. Identification ofthe ureter that is kept on the medial border of the dissection plane. 3. Identification of the obturatornerve and vessels laterally. 4. Deepening the dissection caudally, to reach the hypogastric vein.Careful skeletonization of this vessel is necessary to avoid haemorrhage. 5. Fine dissection lateral andcaudal to the hypogastric vein leads to the identification of the lumbosacral trunk and sciatic nerve.ConclusionsThe ‘’medial’’ approach described here represents a safe and standardized approach to dissect thesciatic nerve laparoscopically, employing a dissection plane medial to the external iliac vessels.https://player.vimeo.com/video/1085650679?autoplay=1</p

    The Effect of Wet Cupping Therapy on Recovery in Athletes

    The purpose of this study is to evaluate the effects of wet cupping therapy (WCT) on post-exercise recovery in active athletes. A total of 11 healthy and active athletes, with a mean age of 21.4 ± 1.77 years, an average height of 1.77 ± 0.1 m, a mean body weight of 75.4 ± 5.65 kg, and an average training experience of 10.6 ± 2.35 years, voluntarily participated in the study. Initially, the participants' resting heart rate and blood lactate levels were measured, followed by the Yoyo-1 test conducted until exhaustion. During the first measurement, heart rate and blood lactate levels were recorded post-exercise recovery at 3, 6, 9, 12, 15, 20, and 30 minutes. In the second measurement, the same test protocol was applied; however, this time, WCT was administered post-exercise recovery, and the same measurements were repeated at identical intervals. The data were analyzed using SPSS 26.0, and groups were compared using the Paired-Samples T-Test, with a significance level set at p&lt;0.05. The results indicated that WCT had a significant effect on reducing blood lactate levels and heart rate after exercise. These findings suggest that WCT may be an effective method for enhancing recovery

    Unilateral Ektopik Mandibular Daimi Kanın Dişin Teşhisi ve Tedavi Yaklaşımı: Olgu Sunumu

    Amaç: Ektopik mandibular daimi kaninler, mandibula içinde çeşitli anormal pozisyonlardakonumlanan nadir dental olgulardır. Bu olguların tanısı genellikle erken puberte döneminde vetesadüfen radyografik incelemeler sırasında konulmaktadır. Etiyolojisi tam olarakaydınlatılamamış olup, son yıllarda yapılan çalışmalar süt kanin eksenine dayalı yeni birgelişimsel referans tanımlamaktadır. Bu yaklaşım, ektopik kaninlerin başlangıç noktaları,maturasyon düzeyleri, kök ve kron morfolojileri ile lokasyonlarına ilişkin tanımlamaları elealmaktadır. Bu olgu sunumunda, nadir görülen ektopik yerleşimli unilateral mandibular daimikanin dişin, güncel tanımlamalar ışığında değerlendirilmesini ve tedavi yaklaşımının ortayakonulmasını amaçlanmaktadır. Olgu Tanımlaması: On bir yaşında kız hasta, rutin dental muayene amacıyla kliniğimize başvurmuştur. Anamnezinde hastanın sistemik olarak sağlıklı ve bilinen bir alerjisinin bulunmadığı öğrenilmiştir. Ekstraoral muayenede bir patolojiye rastlanılmamış; intraoral muayenede 43 numaralı dişin sürmediği gözlenmiştir. Radyografik muayenede 43 numaralı dişin, sağ mandibular bölgede yatay pozisyonda gömülü olduğu saptanmıştır. İlgili dişin morfolojik özellikleri ve çevre anatomik yapılarla ilişkisini değerlendirmek amacıyla konik ışınlı bilgisayarlı tomografi (KIBT) alınmıştır. Bulgular: KIBT incelemesinde, dişin mandibula alt kenarına yakın, premolar dişlerin apekslerinin altında konumlandığı; kronun posterior ve inferior yönelimli olduğu belirlenmiştir. Başlangıç noktası, süt kanin dişin eksfoliasyonu nedeniyle tanımlanamamıştır. Maturasyon düzeyi 5, kök morfolojisi koronal ve apikal eksenler arasındaki açının 10°’den büyük olması nedeniyle eğri ve kron morfolojisi lateral olarak tespit edilmiştir. Tedavisine yönelik multidisipliner değerlendirilme yapılmış; operasyonel riskler, postoperatif komplikasyonlar ve semptom gözlenmemesi nedeniyle altı ayda bir periyodik olarak takip edilmesine karar verilmiştir.Sonuç: Bu olgu literatürle uyumlu olarak tesadüfen saptanmış ve anatomik yapılarla yakınlığınedeniyle konservatif yaklaşım ile yönetilmiştir. Bu iki yönüyle olgu erken tanının öneminivurgulamaktadır. Buna ek, ektopik daimi kaninlerin güncel literatüre uyumlu tanımlayıcıparametrelere dayalı raporlanmasının sürme paternlerinin aydınlatılmasına katkı sunacağıdüşünülmektedir.Anahtar Kelimeler: Ektopik erüpsiyon; gömülü diş; konik ışınlı bilgisayarlı tomografiAim: Ectopic mandibular permanent canines are rare dental anomalies found in abnormalpositions within the mandible and are typically diagnosed incidentally during radiographicexaminations in early puberty. Although the etiology remains unclear, recent studies propose adevelopmental reference based on the primary canine axis, classifying these teeth by origin,stages of maturation, root and crown morphology, and anatomical location. This case reportaims to evaluate a rare unilaterally ectopic mandibular permanent canine and its managemetapproach in accordance with current definitions. Case Description: An 11-year-old female patient presented a routine dental examination. She was systemically healthy and had no known allergies. Extraoral examination revealed no pathological findings, whereas intraoral examination revealed the absence of eruption of tooth 43. Radiographic examination revealed tooth 43 impacted horizontally position in the right mandibular region. Cone-beam computed tomography (CBCT) was performed to assess the morphology and relation to surrounding structures. Results: CBCT showed the tooth was positioned near the inferior mandible border, below the premolars apices, with the crown oriented posterior-inferiorly. The origin point could not be determined due to exfoliation of the primary canine. The maturation stage was classified as stage 5. The root morphology was characterized as curved, due to an angle greater than 10° between the coronal and apical axes, while the crown morphology was determined to lateral. A multidisciplinary evaluation was conducted determine the treatment approach. Considering surgical risks, possible postoperative complications, and the absence of symptoms, it was decision to monitor the patient periodically at six-month intervals. Conclusion: This case, consistent with literature, was incidentally detected and conservatively managed due to its proximity to critical anatomical structures. In these two respects, the case emphasizes the importance of early diagnosis. Furthermore, reporting ectopic permanent canines using descriptive parameters consistent with current literature may enhance understanding of their eruption patterns.Keywords: Cone-beam computed tomography; ectopıc eruption; impacted tooth</p
