    Kadınların Sezaryen Deneyimleri ve Doğum Sonrası Dönemde Bakım Gereksinimleri: Nitel Bir Çalışma

    Giriş ve Amaç: Bu araştırmanın temel amacı lohusaların sezaryen deneyimlerini ve sezaryen sonrası bakım gereksinimlerini incelemektir. Gereç ve Yöntemler: Nitel araştırma tekniklerinden görüşme tekniği kullanılmış olup fenomenolojik araştırmadır. Örneklemi; maksimum çeşitlilik örnekleme tekniğiyle belirlenen Aydın Kadın Doğum ve Çocuk Hastalıkları Hastanesi’nde sezaryen olan 12 lohusa oluşturmuştur. Veriler yarı yapılandırılmış form ile bireysel görüşme ve ses kaydı alınarak toplanmış, içerik analizi yoluyla çözümlenmiştir. Bulgular: Bu çalışmada gebelik döneminde yaşanılan sağlık problemlerinin başında bulantı, kusma, bel ağrısı ve kas kramplarının geldiği, lohusaların yarısının doğum ve doğum şekliyle ilgili araştırma yaptığı, araştırma yapan lohusaların yarısının internetten araştırma yaptığı ve hepsinin çevrelerinden doğumla ilgili duyumlar aldığı, çoğu lohusaların vajinal doğumu tercih ettiği saptanmıştır. Sezaryen olan kadınların neredeyse tamamı sezaryen doğuma hekimin karar verdiğini belirtmiş olup lohusaların tamamının ameliyat ekibinden memnun kaldığı, ameliyat ortamının lohusalara korku ve heyecan hissettirdiği, postoperatif süreçte sezaryene bağlı sorun yaşandığı bulunmuştur. Yaşanan sorunların başında ağrı, acı ve hareket güçlüğü gelmekte, ağrı ve acı ile baş etme yöntemleri, öz aktivitelerini yerine getirme ve emzirme konularında sağlık profesyonellerinin bakımına ihtiyaçları olduğu ve lohusaların yarıdan fazlasının sezaryenden pişman olmadıkları tespit edilmiştir. Sonuç: Lohusaların sezaryen doğum deneyimlerinin olumlu olduğu ve en fazla ağrı ve acı ile baş etmede, öz aktivitelerini yerine getirmede ve emzirme konularında sağlık çalışanlarının desteğine gereksinimleri olduğu bulunmuştur

    The effect of intracerebroventricular amyloid beta 1-42 application on cognitive functions in aged rats supplemented with taurine and the change of peroxisomal proteins in this process.

    Objective: The aim of our study is to investigate the change of peroxisomal proteins in the neurodegenerative and oxidative process caused by the neurotoxicity of A beta 1 & ndash;42 in aged rats supplemented with taurine and to show the possible positive effects of taurine in this process. Methods: 30 Wistar albino rats were randomly divided into 5 groups as control, sham, A beta 1 & ndash;42, taurine, and A beta 1 & ndash;42+taurine. Taurine administration continued for 6 weeks (1000 mg/kg/day with drinking water). Stereotaxic surgery was applied to all groups (intracerebroventricular per lateral ventricle needle only or 5 mu l, PBS, or A beta 1 & ndash;42). Spatial learning and memory performances of the animals were evaluated with Morris water maze and elevated plus maze. The levels of MDA and GSH were measured as oxidative stress parameters in the cerebral cortex and hippocampus. Expressions of CAT, PEX14, PMP70 of peroxisomal membrane proteins were indicated by Western blot analysis. Results: Our results showed that injection of A beta 1 & ndash;42 decreased the spatial learning and memory performance, cortex CAT and hippocampus PEX14, PMP70 and GSH levels, and increased cortex and hippocampus MDA levels (p < 0.05). Although the administration of taurine partially ameliorated the adverse effects of A beta 1 & ndash;42 injection, a significant difference was found only at the hippocampus GSH levels (p < 0.05). Also, taurine caused anxiety at this dose (p < 0.05). Discussion: In conclusion, decreased peroxisomal proteins and antioxidant capacity in neurodegenerative and oxidative processes induced by intracerebroventricular A beta 1 & ndash;42 injection showed that peroxisomes may play a role in this process and taurine supplementation may have positive effects especially in increasing antioxidant capacity

    Posterior realignment of basilar invagination with facet joint distraction technique.

    Purpose: We describe our experience with management of basilar invagination (BI) with the atlantoaxial dislocation (C1/C2) joint reduction technique, including posterior atlantoaxial internal fixation. Materials and methods: From 2008 to 2018, eleven patients with atlantoaxial dislocation (AAD) and BI underwent surgical reduction using C1/C2 the joint reduction technique with a fibular graft/peek cage placement followed by C1 lateral mass/C2 pedicle screw fixation. In two cases that we originally planned to perform C1/C2 joint reduction, occiput-C2 pedicle screw fixation was performed instead due to intraoperative challenges. Post-operative course and surgical complications will be discussed. Results: A total of 13 patients, with an average age of 30.46 +/- 13.23 years (range 12-57), were operated. In one patient, iatrogenic vertebral artery injury occurred without any neurological complication. JOA score improved from 10.45 +/- 1.128 to 15.0 +/- 1.949 (p < 0.0001, paired t-test). All radiological indices were improved (p at least < 0.001). No construct failure was seen in any of the patients with C1-2 facet joint distraction technique during follow-up, and no additional anterior decompression surgery was required. Conclusions: C1/C2 joint reduction technique with fibular graft/cervical PEEK cage of BI patients together with AAD seems to be an effective and safe surgical method of treatment

    The pleiotropic effects of glucagon-like peptide-1 receptor (GLP-1R) agonists on the heart have been recognised in obese or diabetic patients. However, little is known regarding the molecular mechanisms of these agonists in cardioprotective actions under metabolic disturbances. We evaluated the effects of GLP-1R agonist liraglutide treatment on left ventricular cardiomyocytes from high-carbohydrate induced metabolic syndrome rats (MetS rats), characterised with insulin resistance and cardiac dysfunction with a long-QT. Liraglutide (0.3 mg/kg for 4 weeks) treatment of MetS rats significantly reversed long-QT, through a shortening the prolonged action potential duration and recovering inhibited K -currents. We also determined a significant recovery in the leaky sarcoplasmic reticulum (SR) and high cytosolic Ca -level, which are confirmed with a full recovery in activated Na /Ca -exchanger currents (I ). Moreover, the liraglutide treatment significantly reversed the depolarised mitochondrial membrane potential (MMP), increased production of oxidant markers, and cellular acidification together with the depressed ATP production. Our light microscopy analysis of isolated cardiomyocytes showed marked recoveries in the liraglutide-treated MetS group such as marked reverses in highly dilated T-tubules and SR-mitochondria junctions. Moreover, we determined a significant increase in depressed GLUT4 protein level in liraglutide-treated MetS group, possibly associated with recovery in casein kinase 2α. Overall, the study demonstrated a molecular mechanism of liraglutide-induced cardioprotection in MetS rats, at most, via its pleiotropic effects, such as alleviation in the electrical abnormalities, Ca -homeostasis, and mitochondrial dysfunction in ventricular cardiomyocytes

    The Importance of ACTN3 R577X Polymorphism in Athletic Performance and Modeling of R577X Mutant Type and Wild Type ACTN3 Protein by Bioinformatics Analysis

    In studies conducted on the relationship between ACTN3 R577X polymorphism and athletic performance, it is known that athletic performance is polygenic, but the most sensitive relationship is provided by the ACTN3 gene. Considering the results, the conformational difference is much less (<0.05 A) between wild type and R577X. R577X has fewer amino acids (324 amino acids) than the wild type. It has been observed that the subunits of the R577X model do not have 4 long and 5 short alpha-helical curves. According to the analysis results of physico-chemistry properties, each model is very close to alipatich endex, but the R577X mutant type is higher than the wild type. Each model contains a large number of hydrophilic amino acids. R577 was found to be more hydrophilic than wild type. Each model has more likely amino acids versus negative amino acids. However, it considers the R577X mutant type more likely than the wild type. The purpose of this study is; explaining physico and chemistry properties with 3D homology between the wild type of ACTN gene and the R577X mutation region and contributed to future studies on this subject by making homology models. Uniprot, Swiss Model and Chimera were used as bioinformatics transition. Our results showed that there was no significant change in the molecular structure, in which case the function of the protein was not impaired. However, athletes with this mutation at the end of intense muscle performances are more susceptible to muscle damage.     The Effectiveness of Client-Centered Intervention With Telerehabilitation Method After Total Knee Arthroplasty.

    There is known to be a decrease in quality of life and perceived occupational performance and satisfaction following total knee arthroplasty (TKA). This study was planned to examine the effectiveness of a client-centered (CC) intervention with the telerehabilitation (TR) method on the quality of life, perceived occupational performance, and satisfaction after TKA. A total of 38 patients who had undergone TKA were randomly assigned to the CC and control groups. A 12-day intervention program was applied to the CC group. The Nottingham Health Profile (NHP) and Canadian Occupational Performance Measure (COPM) were applied before and after intervention. The 12-day intervention showed a strong effect on all parameters in the CC group (<.001). In the postintervention comparisons, a significant difference was found in favor of the CC group ( < .001). CC interventions with the TR method can be used in post-TKA interventions

    Design and synthesis of novel electroactive 2,2′:5′,2″-terthiophene monomers including oxyethylene chains for solid-state flexible energy storage applications

    Here, we present the synthesis of novel poly(2,2′:5′,2″-terthiophene) derivatives containing oxyethylene pendant groups for the fabrication of high performance flexible redox-active electrode materials. The poly(3′,4′-bis(2-methoxyethoxy)-2,2′:5′,2″-terthiophene) (PSEDEN1), poly(3′,4′-bis(2-(2-methoxyethoxy)ethoxy)-2,2′:5′,2″-terthiophene) (PSEDEN2) and poly(3′,4′-bis(2-(2-(2-methoxyethoxy)ethoxy)ethoxy)-2,2′:5′,2″-terthiophene) (PSEDEN3) have been electrochemically polymerized on flexible stainless steel substrates without any binder and directly employed as redox-active materials. The effect of pendant group chain length on morphological characteristics of conducting polymer films have been systematically evaluated and correlated to the charge storage properties of redox-active electrode materials. Capacitive performance tests reveal that PSEDEN1, PSEDEN2 and PSEDEN3 could reach up to specific capacitances of 135 F g−1, 212.8 F g−1 and 403.3 F g−1, respectively, at constant current density of 2.5 mA cm−2 in the potential range of 0.4–1.8 V with good rate capability performances. In addition, symmetrical flexible solid-state supercapacitor devices based on polymer gel electrolyte have also been assembled using PSEDEN1, PSEDEN2 and PSEDEN3 coated flexible stainless steel substrates and tested by cyclic voltammetry, galvanostatic charge/discharge and electrochemical impedance spectroscopy techniques in detail. Fabricated devices (Cell 1, Cell 2 and Cell 3) have delivered maximum specific capacitances of Cspec= 29.3 F g−1, 92.1 F g−1 and 162.4 F g−1, energy densities of SE= 6.35 W h kg−1, 22.9 W h kg−1 and 41.1 W h kg−1 and power densities of SP= 929 W kg−1, 937.7 W kg−1 and 986.4 W kg−1 at a current density of 2.5 mA cm−2 in two-electrode cell configuration. Furthermore, flexible supercapacitor devices have achieved high cycle life performances with good capacitance retention values of 80.2%, 84.7% and 91.4% over 10 000 consecutive galvanic charge/discharge cycles at 2.5 mA cm−2 constant current density from 0.4 to 1.8 V. Similarly, excellent mechanical stabilities have also been observed with 3.4%, 4.66% and 1.97% capacitance losses under various bending conditions from 0° to 170° for all flexible supercapacitor devices. These results confirm that PSEDEN1, PSEDEN2 and PSEDEN3 redox-active materials with gratifying capacitive performances and excellent flexibilities have a great potential for utilization in innovative flexible or wearable energy storage solutions.     Investigation of double-carbapenem efficiency in experimental sepsis of colistin-resistant .

    OBJECTIVE: Klebsiella pneumoniae, a Gram-negative pathogen, especially which produces carbapenemase, is seen as a major threat to public health due to rapid plasmid-mediated spread of resistance and limited therapeutic options available for treatment. Although colistin has been recognized as a "last resort" antimicrobial for multidrug-resistant K. pneumoniae infections, these isolates have developed resistance to colistin as a result of its intensive use. The aim of this study was to evaluate the efficacy of double-carbapenem treatment of colistin-resistant K. pneumoniae experimental sepsis in mice. METHODS: In the study, 8-10-week-old Balb-c mice were divided as control groups (positive and negative) and treatment groups (colistin, ertapenem+meropenem, and ertapenem+meropenem+colistin). Sepsis was developed in mice by an intraperitoneal injection of colistin resistant K. pneumoniae. Antibiotics were given intraperitoneally 3 h after bacterial inoculation. Mice in each subgroup were sacrificed with overdose anesthetic at the end of 24-48 h and cultures were made from the heart, lung, liver, and spleen. Furthermore, homogenates of lung and liver were used to detect the number of colony-forming units per gram. Bacterial clearance was evaluated in lung and liver at different time points. RESULTS: When the quantitative bacterial loads in the lung and liver tissues are evaluated, no statistically significant difference was observed between different antibiotic treatments (p>0.05). All three treatment options were not effective, especially in 24 h. Only the decrease in bacterial load at the 48th h of the group treated with ertapenem + meropenem + colistin was found significant (p<0.05) compared to the 24 h. CONCLUSION: In the light of these data, it was understood that double-carbapenem application was not sufficient in the treatment of experimental sepsis in mice with colistin-resistant K. pneumoniae. Furthermore, ertapenem + meropenem + colistin combined therapy was not found to be superior to colistin monotherapy or double-carbapenem therapy
