Experiences of Patients with Dilated Cardiomyopathy Who Underwent Cardiac Resynchronization Therapy Defibrillator Placement
application/pdfThis study investigates the experiences of a patient with dilated cardiomyopathy (DCM) who underwent the cardiac resynchronization therapy defibrillator (CRT-D) placement to obtain useful findings to support nursing care practice. A series of semi-structured interviews were conducted with a male patient in his 70s. The recorded narratives were analyzed using a ualitative integration method (the KJ method). Those narratives were subsequently categorized into six categories and defined as follows: acceptance of DCM—the current condition would not last forever because it is an intractable disease; recognition of CRT-D—being aware that the device helps his heart to function normally; health awareness—feeling healthy due to the freedom from the anxiety of arrhythmia; possibility of a painless death—no longer worried about death; self-awareness due to device placement—awareness of his old age; and a new emotion due to device placement: concerns about family members left behind. The result suggests that the patient’s improved cardiac failure prognosis due to CRT-D placement often exhibits developmental challenges similar to those observed in people of the same age. Nurses should support such patients in facing these challenges by enabling them to be self-sufficient and independent.本研究は、拡張型心筋症患者が除細動機能付き両心室ペースメーカー植込み後にどのような体験をしているかを探究し、看護支援の一助に繋げることを目的とした。70歳代男性1名を対象に半構成的面接を実施し、語りの内容を質的統合法(KJ法)で分析した。6枚の最終ラベルに統合され、そのシンボルマークとして【DCMの受け止め:治らない病気だから、いつまでもこの状態は続かない】、【CRT-Dの認識:正常に心臓を動かしていることを知る】、【健康の認識:不整脈の不安から解放されて健康だと感じる】、【苦痛のない死の可能性:死を思い悩まない】、【植込みによって得られた自覚:老いてしまった自分に気づく】、【植込みによって得られた新たな思い:残される家族への気がかり】が抽出された。除細動機能付き両心室ペースメーカーの植込みにより心不全症状が改善した患者は同年代者と近似した発達課題を有しており、看護者は患者が課題に取り組みその人らしく生きる支援をする必要がある。journal articl
The role of governmental normal schools in the establishment of regional education systems: Based on an analysis of the academic and career paths of graduates of Niigata Governmental Normal School
application/pdfThis study seeks to clarify the role of state-run teacher training schools (shihan gakko) during the Gakusei period (1872–1879) by focusing on one such school in Niigata (Kanritsu Niigata Shihan Gakko). Their role has hitherto been thought to have been restricted to training teachers from their respective educational administrative districts. We began by creating a database by searching for comments related to graduates of the Niigata school within materials such as a series of histories of education of each prefecture and city (Toudoufuken kyouiku-shi). We then grouped the school’s graduates based on the career path they chose in the three years following graduation and comprehensively analyzed the fields they worked in.As a result of this analysis, it was clarified that graduates of the state-run teacher training school in Niigata became i) members of the teaching staff at prefectural institutes for training in-service teachers (kyōin kōshū-sho ) or teacher training schools (shihan gakko, ii) regional educational officials, and iii) elementary school teachers. Such graduates were found to not only play a role in teacher training in educational administrative district 6, but also to move into other educational administrative districts. The roles taken on by those graduates who became members of the teaching staff at prefectural institutes for training in-service teachers or at teacher training schools shifted from training in-service teachers to training new teachers. In addition, cases in which the graduates moved to roles in other educational administrative districts were seen to become particularly numerous from October 1876.本論の目的は、これまで各大学区内の教員養成を行うことが役割であるとされてきた学制期の地方官立師範学校について、官立新潟師範学校を対象にその役割を明らかにすることである。都道府県教育史などから官立新潟師範学校卒業者に関する記述部分を検索・資料化し、卒業後3年間の進路別にグループ化した後、卒業生がどの分野に活躍したかを総合的に分析した。その結果、官立新潟師範学校卒業生は府県教員講習所・師範学校教員、地方教育行政官、小学校教員となって第6大学区内に限らず、他の大学区に進出して教員養成の役割を果たしていたこと、府県教員講習所・師範学校教員となった卒業生は現職教員講習から教員養成へ転換させる役割を担ったことが明らかになった。また卒業生が大学区を越えて進出する事例は、明治9年10月以降、特に多くなっていたことが明らかになった。journal articl
