Factors affecting Urethral Catheter Placement Following Flexible Ureterorenoscopy: RIRSearch Study Group
Background/Purpose: To investigate the factors affecting UC placement following flexible ureterorenoscopy (fURS) and the effect of urethral catheter (UC) placement on patient quality of life. Methods: The present study was performed in prospective manner from 1st January 2015 to 30th December 2023, and patients with renal stones smaller than two centimeters who underwent fURS were analyzed for study inclusion. Patients’ demographic characteristics, operative parameters, success of procedure, complications, and VAS score were recorded. Patients were categorized into two groups according to UC placement or not. These groups were compared according to preoperative parameters, intraoperative data, complications, success and VAS at postoperative 6th hour. Results: In total, 324 patients were enrolled into the study. UC was inserted in 170 patients following fURS and was not inserted to 154 patients. In the patient group with UC placement, ratio of male patients (p= 0.002), ratio of anticoagulant use (p= 0.002), preoperative creatinine level (p=0.001), stone size (p= 0.001), stone burden (p= 0.001), and ratio of multiple stones (p= 0.001) were significantly higher. Operation time was significantly longer (p= 0.003) and intraoperative complications (p= 0.045) were significantly higher in patients with UC insertion. Need for additional analgesia and VAS score was significantly lower in patients without UC placement (p= 0.004 vs. p= 0.001). Multivariate analysis revealed that male gender, higher preoperative creatinine level, higher stone size and stone burden, and longer operation time were predictive factors for UC placement following fURS (p= 0.008, p= 0.001, p= 0.001, p= 0.010, and p= 0.001, respectively). Conclusion: The present study demonstrated that UC placement following fURS was associated with increased analgesia requirements and more pain. Moreover, our study demonstrated that male gender, higher preoperative creatinine level, higher stone size and stone volume, and longer operation time resulted in UC insertion after fURS.</jats:p
CARL CZERNY OPUS 599/30 ve 36 NUMARALI ETÜTLERİN PİYANO İCRASINA YÖNELİK KARŞILAŞTIRMALI ANALİZİ
Bu çalışma, Carl Czerny'nin Op. 599 etüt kitabındaki 30 ve 36 numaralı etütlerin teknik ve biçimsel analizleri üzerine odaklanmaktadır. Araştırma, piyano öğrencilerinin bu etütleri daha etkin bir biçimde çalışabilmesi için detaylı çalışma yöntemleri sunmayı amaçlamaktadır. Nitel bir araştırma olan bu çalışmada doküman analizi yöntemi kullanılmış olup, 30 ve 36 numaralı etütler bu yöntemle analiz edilmiştir. Etütlerin yapısal özellikleri, kullanılan motif ve cümleler hakkında elde edilen bilgiler ışığında, geliştirilen çalışma yöntemleri sayesinde piyano öğrencilerinin bu etütleri rahatlıkla icra edebilecekleri düşünülmektedir. Çalışma sonucunda, Czerny Op. 599 etütlerinin diğer numaralarının da benzer bir şekilde analiz edilmesinin faydalı olacağı, piyano öğrencilerinin bu çalışmada sunulan yöntemleri kullanırken, kendi yaratıcılıkları yoluyla yeni çalışma yöntemlerini ortaya koymaları öngörülmektedir.</jats:p
Can HALP (Hemoglobin, albumin, lymphocyte, and platelet) score distinguish malignant and benign causes of extrahepatic cholestasis in patients with extrahepatic bile duct obstruction?
Cholestatic diseases are common and classified as benign or malignant based on their etiology. HALP is a unique nutritional immune marker that combines indicators of nutritional status, including hemoglobin and albumin, with immune function markers like lymphocyte and platelet counts. We investigated the HALP score's ability to differentiate between benign and malignant causes in extrahepatic cholestasis patients.This research was designed as cross-sectional and retrospective. Between 1 January 2020-1 January 2022, patients diagnosed with extrahepatic cholestasis were included. The diagnoses were confirmed using non-invasive imaging methods, ERCP (endoscopic retrograde cholangiopancreatography), and tissue biopsy results. Based on the type of extrahepatic biliary obstruction, either benign or malignant, the patients were divided into two groups. The HALP score was calculated by multiplying the patient's albumin (g/L), hemoglobin (g/L), and lymphocyte count (/L) and dividing by the platelet count (/L).In 121 of 216 patients, extrahepatic cholestasis was caused by benign factors, mostly choledocholithiasis, while malignant causes, predominantly pancreatic head cancer, were responsible for extrahepatic cholestasis in 95 patients. The malignant cholestasis group had significantly higher bilirubin levels (p<0.001), lower hemoglobin levels (p=0.005), lower albumin levels (p<0.001), higher lymphocyte counts (p<0.001), and higher platelet levels (p=0.001) compared to the benign cholestasis group. There was no considerable difference in the HALP score between the two groups, as indicated by a p-value of 0.741.The HALP score could not distinguish between benign and malignant causes of extrahepatic cholestasis
Application of Action Observation Therapy in Swallowing Rehabilitation: A Randomised Controlled Study
ABSTRACTBackgroundAction Observation Therapy (AOT) has emerged as a potential neurorehabilitation therapy to promote motor control for individuals with neurological dysphagia (ND), facilitating neural plasticity through activation of the mirror nervous system.ObjectiveThe aim of this study was to investigate the effect of AOT‐based dysphagia rehabilitation on suprahyoid (SH) muscle activation, swallowing difficulties, swallowing function, depressive symptom and quality of life in ND patients.MethodsThirty‐four participants with ND were randomly allocated to the AOT group (n = 17) or control group (n = 17). All participants underwent a 4‐week (5 sessions per week) conventional swallowing therapy. The AOT group watched exercise videos before doing these exercises, and the control group performed the same exercises without the videos. Surface electromyographic (sEMG) activation of suprahyoid muscles, Eating Assessment Tool 10 Turkish Version (T‐EAT 10) and Swallowing Ability and Function Evaluation (SAFE) for swallowing difficulties and swallowing function, Swallow Quality of Life Questionnaire (SWAL‐QOL) and Beck Depression Inventory (BDI) for quality of life and depressive symptom severity were evaluated.ResultsPost‐treatment SH amplitude level and T‐EAT‐10 scores were significantly lower in the AOT group compared to the control group (p = 0.001, p = 0.012). There was a significant improvement in SAFE after treatment in the AOT group (p < 0.05); however, there was no significant difference between the two groups (p > 0.05). Both groups showed similar improvement in quality of life and depression.ConclusionThis study showed that AOT can be a good option for improving the SH muscle activation and dysphagia difficulties in ND patients.Trial RegistrationClinicalTrials.gov identifier: NCT05782790</jats:sec

Influence of Gd2O3 reinforcement on the mechanical, physical and radiation shielding properties of Al-30Sm2O3 composites
In this comprehensive study, we meticulously fabricated Al-30Sm2O3-XGd2O3 (X = 3, 6, 9, 12, and 15) composites through a series of tests and analyses to determine the composites' mechanical, physical, corrosion, and radiation shielding properties. Scanning electron microscopy (SEM-EDX) and X-ray diffraction (XRD) techniques were used for microstructural analysis. The main objective of the research is to investigate the effects of Gd2O3 reinforcement on the mechanical properties (hardness, wear resistance, and corrosion resistance) and radiation shielding performances of the composites. MCNP6.2 simulations evaluated thermal, fast neutron, and gamma-ray attenuation properties, while mechanical properties were analyzed by hardness, wear, and corrosion tests. XRD analyses showed no intermetallic phase was formed, while SEM-EDX analyses revealed that the composites were homogeneous and had a partial increase in void ratio with Gd2O3 reinforcement. Gd2O3 reinforcement increased the hardness and wear resistance of the composites, but a slight decrease in relative density was observed. The highest hardness and wear resistance were obtained at 15% Gd2O3 reinforcement. The corrosion rate decreased with Gd2O3 reinforcement, while an increase in corrosion rate was observed in the composite containing 15% Gd2O3. MCNP6.2 simulations showed that Gd2O3 reinforcement increased the thermal neutron macroscopic cross-section and linear attenuation coefficient while decreasing the fast neutron macroscopic cross-section.</jats:p
EARLY-STAGE DIABETES RISK PREDICTION USING MACHINE LEARNING TECHNIQUES BASED ON ENSEMBLE APPROACH
Diabetes Mellitus which is considered as one of the deadliest is a common, chronic disease. It also causes the emergence of many diseases, especially neuropathy, nephropathy and retinopathy. In this context, early diagnosis of the disease by accurately evaluating the symptoms and initiating a rapid treatment process is very important. The aim of this study is to present an effective model that can determine the diabetes risk in eary-stage with the best accuracy. To do so, the classification algorithms that are frequently used in diabetes risk estimation are supported with ensemble approaches. Firstly, the performance of Naive Bayes (NB), Trees-J48, k Nearest Neighbor (kNN) and Sequential Minimal Optimization (SMO) classifiers is analyzed separately by using a dataset of 520 samples collected with direct questionnaires from Sylhet Diabetes Hospital patients in Sylhet, Bangladesh. Then, the effects of Adabost, Bagging and Random Sub-Space (RSS) algorithms on classifier success are investigated and it is shown that the j48 classifier based on Adabost approach has the best accuracy in this dataset. Finally, the Wrapper Subset Eval (WSE) feature extraction algorithm is applied to reduce the estimation cost of diabetes and increase classification success. Thus, the best accuracy at 99% is achieved using reduced data set with proposed classifier method.</jats:p
