The Effect of Listening to Music Presurgery on the Anxiety Level of Individuals who are Planned for Urological Surgery: Randomized Controlled Study
This study aims to evaluate the effects of listening to music on preoperative anxiety levels in individuals planned for urological surgery.The study was a pretest-post-test randomized controlled study.This research was conducted with 80 (intervention=40, control=40) individuals who applied to the urology clinic of a university hospital between September 2021 and February 2022 with surgery planned. The data were collected via the information form, state-trait anxiety inventory (STAI= STAI-S: state anxiety, STAI-T: trait anxiety), anxiety specific to surgery questionnaire (ASSQ).The mean STAI-S scores for the pretest and post-test intervention group were 37.15 ± 8.04 and 34.67 ± 9.09; while scores for the control group were 36.23 ± 9.44 and 37.23 ± 9.78. While the difference in the intervention group was statistically significant (P = .006), there was no difference observed in the control group (P = .256). The pretest and post-test STAI-T mean scores for the intervention group were 39.73 ± 7.71 and 39.88 ± 8.22; the control group scores were 40.68 ± 4.91 and 41.25 ± 5.87; the difference between the intervention (P = .840) and control (P = .346) groups was not statistically significant. The pretest and post-test surgery-specific anxiety scale mean scores of the intervention group were 18.08 ± 4.52 and 16.55 ± 4.79; the control group were 20.50 ± 7.31 and 20.95 ± 7.69. While the difference in the intervention group was statistically significant (P = .011), no difference (P = .498) was determined in the control group.Individuals planned for urological surgery who listened to music for 30 minutes had reduced state and surgery-specific anxiety for 15 minutes after listening, however, this did not affect trait anxiety. The practice of listening to music before surgery may be recommended for individuals who are scheduled for urological surgery to reduce patient anxiety
Egg White and Eggshell Mortar Reinforcing a Masonry Stone Bridge: Experiments on Mortar and 3D Full-Scale Bridge Discrete Simulations
In this study, experimental and numerical investigations were conducted to examine the time-dependent creep and earthquake performance of the historical Plaka stone bridge, which was constructed in 1866 in Arta, Greece. During the original construction of the bridge in 1866, Khorasan mortar with an egg white additive was used between the stone elements. Furthermore, when the bridge underwent restoration in 2015, Khorasan mortar with an eggshell additive was employed between the stone elements. Consequently, two distinct 3D finite-difference models were developed for this study. In the first bridge model, egg white was used in the Khorasan mortar, replacing water at various proportions of 0%, 25%, 50%, 75%, and 100%. In contrast, for the second model, eggshell was incorporated into the Khorasan mixture at percentages of 25%, 50%, 75%, and 100%, relative to the lime amount. Subsequently, the mortars were subjected to curing periods of 1 day, 7 days, and 28 days, and their mechanical properties were determined through unconfined compression strength experiments. Taking into account the determined strengths of the mortars, the kn and ks stiffness values of the interface elements between the stone elements and Khorasan mortar were calculated. In the 3D model, each stone element was individually represented, resulting in a total of 1,849,274 stone elements being utilized. Non-reflecting boundary conditions were applied to the edge boundaries of the bridge model, and the Burger creep and Mohr–Coulomb material models was employed for time-dependent creep and seismic analyses, respectively. Subsequently, time-dependent creep analyses were conducted on the bridge, and seismic events that occurred in the region where the bridge was located were simulated to assess their impact. Based on the results of the time-dependent creep and seismic analyses, we observed that the use of 50% eggshell-mixed Khorasan mortar between the stone elements had a positive influence on the earthquake and creep behaviors of both restored and yet-to-be-restored historical bridges.</jats:p

Experimental and numerical studies on the proposed stiffener detailing of integral bridge steel H piles to alleviate the combined adverse effects of axial load and cyclic thermal displacements
Past research studies demonstrated that local buckling may be observed when the steel H piles (SHPs) at the abutments of integral bridges are simultaneously subjected to axial load and lateral plastic displacements. The large curvatures and strains associated with local buckling may lead to earlier low cycle fatigue (LCF) failure of the pile under cyclic thermal displacements. Accordingly, first, the detrimental effect of axial load combined with lateral plastic displacements leading to local buckling and accelerated LCF failure of the SHP is demonstrated through experimental testing and finite element modeling and analysis. Then preventive measures to eliminate or delay local buckling are developed and assessed numerically as well as by experimental testing. Numerical simulation and experimental test results revealed that welding longitudinal stiffeners on the flanges successfully prevent the local buckling of the flanges and improve the fatigue life of the pile. </jats:p
Paediatric nurses' experiences of success and failure in first-time peripheral intravenous catheter insertion: A qualitative study
To explore in-depth the thoughts, anxiety factors, and experiences of paediatric nurses regarding the success and failure of first-time peripheral intravenous catheter insertion.The study employed a qualitative research method using the phenomenology design. Face-to-face semi-structured in-depth interviews were conducted with nurses (N = 12) working in the paediatric service and neonatal intensive care unit of a teaching and research hospital. The transcripts were analysed using a phenomenological approach and an inductive process.Four themes were identified: Peripheral Intravenous Catheter Insertion, Experience of Success on the First Attempt, Experience of Failure on the First Attempt, and Proposed Solutions. Nurses felt a sense of happiness when they successfully inserted a peripheral intravenous catheter on their first attempt in paediatric patients. Conversely, they experienced emotions such as sadness, worry, stress, and anxiety when they failed and had to make repeated attempts.Unsuccessful peripheral intravenous catheter insertions cause distress and anxiety for nurses, patients, and parents alike. Therefore, nurses' experiences of peripheral intravenous catheter insertions in healthcare settings should not be ignored and require improvement.Unsuccessful peripheral intravenous catheter insertions affect children, parents, and nurses negatively, thus reducing the success rate of repeated peripheral intravenous catheter insertions. Therefore, it is advisable for nurses conducting paediatric peripheral intravenous catheterisations to undergo periodic training. A specialised intervention team should be established for cases of initial failure, and a mentoring system between seasoned and new nurses is recommended
Exploring shared triggers and potential etiopathogenesis between migraine and idiopathic/genetic epilepsy: Insights from a multicenter tertiary-based study
Migraine and epilepsy are two episodic disorders that share common pathophysiological mechanisms. The aim of our research was to assess the possible shared etiopathogenesis by analyzing the relations of headache, and seizure triggers, based on information obtained from a national cohort surveying the headache characteristics of 809 patients who had been diagnosed with idiopathic/genetic epilepsy.Our study utilized data from a multi-center, nationwide investigation of headaches in 809 patients with idiopathic/genetic epilepsy. Out of these, 508 patients reported complaints related to any type of headache (333 Migraines, 175 Headaches of other types). In the initial phase of the study encompassing the entire sample of 809 epilepsy patients, differences in seizure triggers were assessed between the migraine group (n = 333) and the non-migraine group (n = 476). Additionally, the subsequent part of the study pertains to a subgroup of the entire patient group, namely those affected by all types of headaches (n = 508), and differences in headache triggers were assessed among migraine patients (n = 333) and those with other types of headaches (n = 175). Similar differences were observed between epilepsy patients with and without a family history of epilepsy.The most frequently reported seizure triggers in all I/GE group (n = 809) were stress (23%), sleep deprivation (22%) and fatigue (18%), respectively. The most frequently reported headache triggers in migraine patients were stress (31%), sleep deprivation (28%), and noise (26%). The occurrence of menstruation-triggered seizures in individuals with migraine and I/GE was found to be considerably higher than those without migraine. The most common triggers for seizure and headache among the individuals with a positive family history of epilepsy were determined to be light stimuli and sleep deprivation.In conclusion, our study provides valuable insights into the overlapping triggers including sleep patterns, stress levels, and menstrual cycles, etc. and potential shared etiology of migraine and I/GE. Recognizing these connections may facilitate the development of more precise therapeutic strategies and underscore the significance of adopting a holistic, multidisciplinary approach to the management of these intricate neurological conditions. Further research is essential to explore in greater depth the shared mechanisms underpinning these associations and their implications for clinical practice
Impact of Laparoscopic Sleeve Gastrectomy on Fatigue in Obese Patients
Background: Fatigue with obesity negatively affects the motivation to lose weight and causes failure of treatment. So, obesity and fatigue are two related entities that worsen each other in a vicious circle. This study aimed to examine the changes in fatigue levels in obese patients undergoing laparoscopic sleeve gastrectomy (LSG). Methods: Preoperative and postoperative BMI, fat percentage, and obesity degree were determined based on Tanita test results, and the rates of change were calculated. The Fatigue Assessment Scale (FAS) was used to assess the impact of obesity on mental and physical fatigue. Results: Six months after LSG, there was a 29.7% decrease in Body Mass Index (BMI) across all patients (45.0 to 31.4, p < 0.001). Significant reductions in the percentage of body fat, obesity grade, and HgA1C were observed in both women and men and overall (p < 0.001 for all comparisons). Scores on the Fatigue Assessment Scale (FAS) for total, mental, and physical scales decreased significantly for women, men, and all participants (p < 0.001 for all comparisons). There was a weak but significant positive correlation between the percentage change in FAS total and FAS physical scale scores and the change in BMI% (p = 0.015 and 0.004; r = 0.089, 0.106, respectively). Change in obesity grade and FAS physical subscale scores negatively correlated (p = 0.029, r = 0.080). Conclusions: LSG not only leads to significant weight reduction but also improves fatigue levels in patients with obesity.</jats:p
Farklı İndüktif Etkilere Sahip Yan Gruplar İçeren Naftalimit Türevlerinin DNA Bağlanma Aktivitelerinin Belirlenmesi
Naftalimit türevleri, son yıllarda yapılan çalışmalarda yüksek biyolojik aktiviteye sahip bileşikler olarak karşımıza çıkmaktadır. Bu doğrultuda yeni türevlerin DNA etkileşim yollarının belirlenmesi ve literatüre kazandırılması büyük önem taşımaktadır. Bu çalışmada farklı yan gruplar içeren naftalimit türevlerinin DNA etkileşimleri incelenmiştir. UV titrasyonu ile bağlanma türünün interkalasyon olduğu anlaşılmıştır. Etidyumbromür ile yarışmalı floresans deneyleri gerçekleştirilerek bağlanma türü hakkında daha kesin bilgiler elde edilmiştir. Bileşiklerin plazmid DNA üzerinde gerçekleştirdikleri form değişiklikleri agaroz jel elektroforez yöntemiyle incelenmiş ve aktif konsantrasyona karar verilmiştir.</jats:p
Digital analysis of unilateral ptosis repair: external levator advancement vs. Müller's muscle conjunctival resection
Evaluation of lid contour and marginal peak point changes to compare outcomes of external levator advancement and Miiller's muscle conjunctival resection surgery in unilateral ptosis.We reviewed the charts of unilateral ptosis patients who underwent external levator advancement or Miiller's muscle conjunctival resection. Eyelid contour analysis was conducted on preoperative and 6-month postoperative digital images. This was performed with the multiple margin reflex distances technique, measuring the vertical distance from a line intersecting the center of the pupil to the eyelid margin at 10 positions at 2 mm intervals. The marginal peak point changes were analyzed digitally using the coordinates of the peak point according to the pupil center. Each position's mean distance was compared preoperatively, postoperatively, and with the fellow eyelid.Sixteen patients underwent external levator advancement and 16 patients had Miiller's muscle conjunctival resection. The mean margin reflex distance was improved by both techniques (1.46 vs. 2.43 mm and 1.12 vs. 2.25 mm, p=0.008 and p=0.0001 respectively) and approached that of the fellow eyelid (2.43 vs. 2.88 and 2.25 vs. 2.58 mm, p=0.23 and p=0.19, respectively). However, statistically significant lid margin elevation was limited to between the N6 and T6 points in the external levator advancement group. Whereas, significant elevation was achieved along the whole lid margin in the Miiller's muscle conjunctival resection group. The marginal peak point was shifted slightly laterally in the external levator advancement group (p=0.11).Both techniques provide effective lid elevation, however, the external levator advancement's effect lessens toward the canthi while Müller's muscle conjunctival resection provides more uniform elevation across the lid margin. The margin reflex distance alone is not sufficient to reflect contour changes
