Fever and dehydration as the first COVID-19 presentation in a 2-months old boy in northeast of Iran
Introduction: After the worldwide pandemic of coronavirus 19 disease (COVID-19), pediatric involvement has been seen as case reports with various initial symptoms. Case Presentation: Here, we report a 2-months old boy with severe dehydration, fever, and electrolyte imbalance with lung involvement compatible with COVID-19 in the Northeast of Iran (North Khorasan). Conclusions: Pediatric case reports in COVID-19 are limited, and it seems that the disease has a milder course compared to the adult patients, but the range of the clinical features is wide. © 2020, Author(s)

Efficacy of mediterranean diet on blood biochemical factors in type II diabetic patients: A randomized controlled trial Tip II diyabetik hastalarda akdeniz diyetinin kan biyokimyasal faktörleri �zerindeki etkinli�i: Randomize kontrollü bir �alı�ma
Introduction: Diabetes mellitus is one of the most common endocrinologic disorder and the seventh cause of mortality worldwide. In Mediterranean dietary, consumption of plant source food such as vegetables, fruits, seeds, beans etc. is much emphasized. There are many studies showed that this regimen can decrease the risk of cardiovascular disease and factors associated in diabetes mellitus. Therefore, in this study we investigated the efficacy of this regimen on blood biochemical factors in diabetic patients referred to diabetes clinic in Gorgan. Methods: This was a clinical trial study in which 228 well-controlled type 2 diabetes mellitus patients between 40-60 years were included in the study. They were randomly assigned to two different groups: control and Mediterranean educational intervention group. They followed up for 6 months and before and after the intervention, we evaluated the glycemic and lipid profile and physiological parameters in them. Data were analyzed by SPSS software. Results: Totally 228 patients with diabetes mellitus type II between 40 to 60 years old were recruited. The mean age of patients was 57.3±9.28 years old. Majority of participants were female (77.2) with elementary level of literacy (77.6). The mean of fasting blood glucose was 192.50± 64.17 mg/dL in intervention group versus 208.48 ± 87.90 mg/dL in control group (P>0.05). This index was changed to 165.49± 50.39 mg/dL in intervention group which was significantly lower than the amount in control group (P<0.001). A clinically and statistically significant fall in HbA1c was observed in intervention group rather than control group (P<0.001). Conclusion: The present study represented a significant relationship between Mediterranean dietary intervention and both anthropometric and laboratory findings in patients with type 2 diabetes mellitus. We showed that this dietary could significantly lower the FBS, HbA1c, and LDL. Although we showed this meaningful difference, it should be more evaluated in Iranian race people to investigate more in this topic. © Copyright 2020 by Gazi University Medical Faculty
Non-ionic surfactant vesicles as novel delivery systems for sulfasalazine: Evaluation of the physicochemical and cytotoxic properties
Sulfasalazine (SSZ) is a sulfa drug that is used to treat various forms of arthritis and is known for its ability to induce apoptosis in T lymphocytes and cancer cells. To enhance its solubility and efficacy, we encapsulated SSZ within a micellar/niosomal formulation. The micellar/niosomal formulation was prepared using an amphiphilic self-assembly method involving squalene (S) and tween 80 (T8) for the first time. The encapsulation and release of SSZ from the nanocarrier was characterized. The percent encapsulation efficiency of SSZ was 99.5 ± 0.2 , 90.5 ± 0.5 , and 80.5 ± 0.8 for the 5:100, 7.5:100, and 10:100 (w/w) ratios of SSZ:total weight of ST8 micellar/niosomal vesicles (ST8MNVs), respectively. A higher loading of 20:100 (w/w) led to SSZ diffusion from the vesicular systems within 24 h. The cytotoxic activity of the SSZ-encapsulated ST8MNVs was evaluated against different cancer cell lines. Encapsulation and the use of the nanoformulations improved the effectiveness of SSZ in promoting apoptosis in MDA-MB-231, MCF-7, and HeLa cell lines, by reducing the IC50 value by at least five folds. The high loading and encapsulation of SSZ within the micellar/niosomal formulation together with its greater water solubility enhanced the bioavailability of the drug, improving its potential for biomedical and therapeutic applications. © 202
Global prevalence of microsporidia infection in cats: A systematic review and meta-analysis of an emerging zoonotic pathogen
Microsporidiosis in pet and stray cats is an emerging zoonotic threat with public health significance worldwide. However, the epidemiological patterns of feline microsporidiosis is still neglected around the world. Hence, current systematic review and meta-analysis aimed at characterizing the prevalence estimates and genotypes of microsporidian parasites among cats of the world. Several databases (PubMed, Web of Science, Scopus, and Google scholar) were systematically explored to find relevant studies. Evaluation of the weighted prevalences among included studies was done using random-effects model. Totally, 30 studies (34 datasets) reported from 19 countries were included in the present work. Microsporidia infection demonstrated higher prevalence rates using microscopy 29.7 (19.7�42.2 ), followed by serology and molecular techniques with 11 (4.6�24.2 ) and 8.2 (5.9�11.4 ), respectively. Moreover, molecular data showed Enterocytozoon bieneusi as the most dominant reported species with 7.4 (5.1�10.5 ). Also, investigations (11 studies) mostly isolated D genotype among all E. bieneusi genotypes. These results highlight cats as a potential reservoir for acquisition of microsporidia infection in humans, and surveillance programs should be implemented in high-risk areas. © 2021 Elsevier B.V
The oncogenic role of both lncRNA PANDA and BCL2 gene in glioblastoma
Glioblastoma multiform (GBM) is the most prevalent primary tumor of the brain with aggressive behavior. This type of cancer accounts for 60 of all brain tumors in humans. More recently, the participation of long non-coding RNAs (lncRNAs) in the regulation of many biological processes in tumors has been evidenced. In the present study, we measured expression levels of lncRNA PANDA (P21 associated non-coding RNA DNA damage activated, hereafter termed PANDA) and anti-apoptotic gene B-cell lymphoma 2 (BCL2) in tissue samples of 50 GBM patients and 50 healthy-matched subjects. According to our data, both lncRNA PANDA and BCl2 mRNA were significantly overexpressed in tumor tissues of patients with GBM in comparison to healthy control specimens. Furthermore, a significant correlation was observed between the expression of lncRNA PANDA and BCL2 transcripts in the patients group. Additionally, the results of receiver operating characteristic (ROC) curve analysis and the area under the curve (AUC) unraveled that PANDA had a significant value in the diagnosis of GBM. The present data uncovered the up-regulated expressions of PANDA and BCL2 transcripts in patients with GBM, which are positively correlated with tumor grade. This may suggest the oncogenic function for these two markers and propose them for the interpretation of prognostic and diagnostic data as well as the promising potential treatment target in GBM patients. © 202
The fatty acid-binding protein (FABP) decreases the clinical signs and modulates immune responses in a mouse model of experimental autoimmune encephalomyelitis (EAE)
Background: An increasing body of studies has shown that Fasciola hepatica can affect immune responses. This study explored whether the fatty acid-binding protein (FABP) of F. hepatica can modulate the immune system in a mouse model of experimental autoimmune encephalomyelitis (EAE). Methods: EAE-induced C57BL/6 mice were treated with vehicle, F. hepatica total extract (TE) or FABP. The clinical signs, body weights, and the expression of IFN-γ, T-bet, IL-4, GATA3, IL-17, RORγ, TGF-β, FOXP3, IL-10, TNF-α genes and proteins were determined in the isolated CD4+ splenocytes. Besides, the percentage of Treg cells and degree of demyelination were evaluated. Results: We found that TE and FABP treatments decreased the clinical scores, lymphocyte infiltration rate, and demyelinated plaques in EAE mice. The expressions of IL-4 and GATA3 were increased, whereas IL-17 and TNF-α were down-regulated. FABP did not affect the expression of IFN-γ, RORγ, IL-10, and TGF-β genes or proteins but reduced the expression of T-bet. TE administration did not affect the expression of IL-10 and the Tbet genes, and increased the expression levels of IFN-γ and FOXP3 in CD4+ lymphocytes. Both FABP and TE treatment did not affect the Treg cell percentage. Conclusion: This study indicates that F. hepatica FABP and TE can suppress the inflammatory responses in EAE-induced mice and shift the immune system toward Th2 responses. However, FABP exerts stronger anti-inflammatory effects and seems to be more effective than TE for EAE treatment. © 2021 Elsevier B.V
Effects of inhaling jasmine essential oil on anxiety and blood cortisol levels in candidates for laparotomy: A randomized clinical trial
Context: Anxiety is an unpleasant emotion and inhaling Jasmine Essential Oil (JEO) may decrease anxiety before laparotomy. Aims: Determining effects of inhaling JEO on anxiety and blood cortisol levels in patients undergoing laparotomy. Setting and Design: The setting was 5th Azar Hospital in Gorgan (Iran) and the study was a single blind parallel, randomized, controlled clinical trial that was conducted in 2016. Materials and Methods: The subjects (84 patients) were randomly allocated in two intervention and control groups. The intervention and control group inhaled two drops of JEO and two drops of distilled water respectively for 60 min. Blood cortisol and anxiety levels were measured immediately after intervention by ELISA kit, and the Spielberger's State Anxiety Inventory, respectively. Statistical Analysis: The data were described with mean, Median, Interquartile Range (IQR) and analyzed using t-test, Chi-square, Wilcoxon signed-rank, Mann-Whitney U and correlation coefficient tests. Results: The mean score of anxiety in JEO group before and after intervention was 50.90 ± 7.71 and 36.42 ± 6.62 (P = 0.001), respectively and median (IQR) of the cortisol was 160.7 (60.88) and 93.15 (52.38), respectively (P = 0.001). In the control group, cortisol (IQR) level increased (124.1 (67.42) to 127.9 (62.42), P = 0.02) and the mean anxiety score decreased slightly (P = 0.43). Median and IQR of difference cortisol level before and after in the intervention and control group were significant (P = 0.001). Conclusion: Our findings suggest that aromatherapy with JEO may have beneficial effects on preoperative anxiety in patients undergoing laparotomy. © 2021 Journal of Nursing and Midwifery Sciences | Published by Wolters Kluwer-Medknow
Human gene polymorphisms and their possible impact on the clinical outcome of SARS-CoV-2 infection
The SARS-CoV-2 pandemic has become one of the most serious health concerns globally. Although multiple vaccines have recently been approved for the prevention of coronavirus disease 2019 (COVID-19), an effective treatment is still lacking. Our knowledge of the pathogenicity of this virus is still incomplete. Studies have revealed that viral factors such as the viral load, duration of exposure to the virus, and viral mutations are important variables in COVID-19 outcome. Furthermore, host factors, including age, health condition, co-morbidities, and genetic background, might also be involved in clinical manifestations and infection outcome. This review focuses on the importance of variations in the host genetic background and pathogenesis of SARS-CoV-2. We will discuss the significance of polymorphisms in the ACE-2, TMPRSS2, vitamin D receptor, vitamin D binding protein, CD147, glucose�regulated protein 78 kDa, dipeptidyl peptidase-4 (DPP4), neuropilin-1, heme oxygenase, apolipoprotein L1, vitamin K epoxide reductase complex 1 (VKORC1), and immune system genes for the clinical outcome of COVID-19. © 2021, The Author(s), under exclusive licence to Springer-Verlag GmbH Austria, part of Springer Nature
Epigenetic alterations and genetic variations of angiotensin-converting enzyme 2 (ACE2) as a functional receptor for SARS-CoV-2: potential clinical implications
Receptor recognition is a crucial step in viral infection and is a critical factor for cell entry and tissue tropism. In several recent studies, angiotensin-converting enzyme 2 (ACE2) has been demonstrated to be the cellular receptor of SARS-CoV-2 as it was previously well known as the receptor of SARS-CoV. SARS-CoV-2 can bind with high affinity to human ACE2 and engages it as an entry receptor. It seems that the genetic, notably epigenetic variations of ACE2 are less known in different populations, indicating the need for its further investigation. These variations have the potential to affect its contribution to the pathogenicity of COVID-19. The contribution of epigenetics in the interindividual variability of ACE2 merits more attention because epigenetic processes can play important roles in ACE2 alterations in various tissues and different people and populations. Analyzing different DNA methylation patterns and microRNAs, contributing to the ACE2 modulation in the lungs will have a high priority. The epigenetic and genetic variations of ACE2 become even more important when considering that some people have mild clinical symptoms despite having COVID-19. The pathogenicity of SARS-CoV-2 infection is complex; therefore, a better understanding of the underlying pathobiology, especially binding the virus to its receptors, could help improve therapeutic and preventive approaches.
