    Preschool children�s knowledge of correct names of genital body parts in Gorgan, Iran

    This study sought to identify Iranian preschool children�s (N = 318) knowledge of genital and non-genital body parts. Assessment consisted of children naming selected external body parts in drawings of same-sex counterparts. Results indicated that almost all children knew the correct terms for their non-genital body parts, but few knew the correct terminology for their genitals. Many used slang terms and some appeared embarrassed or reluctant to reply (girls were more likely than boys not to respond). No statistically significant difference between the sex of children in knowledge of correct names of all genitals emerged. Boys knew the correct name of their genitalia (penis) significantly more than girls. Results indicated children are learning the names of their body parts but not their genitals, perhaps indicative of a lack of sexual health education that may be representative of the conservative culture in Iran. It is critical for young children to learn about their genitals as this knowledge provides the foundation for subsequent sex education and facilitates discussion about bodily safety to prevent sexual abuse. When children learn these names, it can facilitate disclosure of abuse and can assist those working with young children in medical settings, child protection and legal arenas. © 2021 Informa UK Limited, trading as Taylor & Francis Group

    Radiation-Free myeloablative allogeneic hematopoietic stem cell transplantation for adult acute lymphoblastic leukemia: A comparison of outcomes between patients with and without central nervous system involvement

    For patients with acute lymphoblastic leukemia (ALL) undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT), total body irradiation (TBI) has been particularly advocated as a part of the conditioning regimen in case of extramedullary involvement in sanctuary sites such as the central nervous system (CNS), to ensure greater tissue penetration. In resource-limited countries lacking TBI facilities; however, ALL patients undergo radiation-free myeloablative conditioning, though its impacts on post-HSCT outcomes of the patients with pre-HSCT CNS involvement have not been analyzed. In this 14-year series of 278 adult (> 18 y) ALL patients undergoing TBI-free busulfan/cyclophosphamide conditioning allo-HSCT, we found that the long-term probabilities of overall survival, disease free survival, relapse and non-relapse mortality were not significantly different between CNS-involved and CNS-spared patients. Moreover, there was no statistically significant difference in the incidence of post-HSCT CNS relapse between CNS-involved and CNS-spared patients. Pre-HSCT cranial radiation therapy (CRT) showed no significant preventive effect on the likelihood of post-HSCT CNS relapse. Through multivariable regression analysis, grade III-IV acute graft-versus-host disease (GvHD), extensive chronic GvHD and post-HSCT relapse were ascertained as independent determinants of mortality (Adj.R2 = 53.9 , F(12,265) = 28.1, P < 0.001), while other parameters including Philadelphia translocation, pre-HSCT CNS involvement and CRT were found to have no independent effect. Although this study was not an attempt to compare TBI-based vs. non-TBI conditioning, the TBI-free myeloablative allo-HSCT was shown to be feasible and an option for adult ALL patients with CNS involvement, considering the comparable outcomes between patients with and without CNS involvement. © 2021 Elsevier Lt

    Discoloration and mineralization of a textile azo dye using a hybrid UV/O3/SBR process

    Most synthetic dyes are toxic and hardly biodegradable compounds that enter the environment mainly through the discharged of non-treated textile industry effluents. The present study investigated the removal of the textile monoazo dye Reactive Red 198 (RR-198) from aqueous solutions using the ultraviolet light and ozonation alone and in combination (i.e., UV/O3) followed by a Sequencing Batch Reactor (SBR). The pH (5 � pH � 9) and dye initial concentration (50�300 mg/L) parameters were optimized in the ozonation process at reaction time of 0�60 min. Then, TOC removal and dye discoloration percentage was compared with the O3, UV and O3/UV processes. In order to compare the performance of the SBR in dye discoloration of RR-198 and TOC removal, four types of effluent, including Raw dye, O3-pretreated dye, UV-treated dye and UV/O3-pretreated dye were separately treated in the SBR system. In the ozonation process, by increasing the pH and reducing the initial dye concentration increased the discoloration percentage. The highest dye discoloration percentage and TOC removal obtained in the hybrid UV/O3/SBR process. Combining biological systems and Advanced Oxidation Processes is an appropriate option for the decomposition of resistant pollutants and increasing the biodegradability of these compounds and is applicable in the water and wastewater industry. © 2021, The Author(s)

    Fixed-dose combination therapies with and without aspirin for primary prevention of cardiovascular disease: an individual participant data meta-analysis

    Background: In randomised controlled trials, fixed-dose combination treatments (or polypills) have been shown to reduce a composite of cardiovascular disease outcomes in primary prevention. However, whether or not aspirin should be included, effects on specific outcomes, and effects in key subgroups are unknown. Methods: We did an individual participant data meta-analysis of large randomised controlled trials (each with �1000 participants and �2 years of follow-up) of a fixed-dose combination treatment strategy versus control in a primary cardiovascular disease prevention population. We included trials that evaluated a fixed-dose combination strategy of at least two blood pressure lowering agents plus a statin (with or without aspirin), compared with a control strategy (either placebo or usual care). The primary outcome was time to first occurrence of a composite of cardiovascular death, myocardial infarction, stroke, or arterial revascularisation. Additional outcomes included individual cardiovascular outcomes and death from any cause. Outcomes were also evaluated in groups stratified by the inclusion of aspirin in the fixed-dose treatment strategy, and effect sizes were estimated in prespecified subgroups based on risk factors. Kaplan-Meier survival curves and Cox proportional hazard regression models were used to compare strategies. Findings: Three large randomised trials were included in the analysis (TIPS-3, HOPE-3, and PolyIran), with a total of 18 162 participants. Mean age was 63·0 years (SD 7·1), and 9038 (49·8) participants were female. Estimated 10-year cardiovascular disease risk for the population was 17·7 (8·7). During a median follow-up of 5 years, the primary outcome occurred in 276 (3·0) participants in the fixed-dose combination strategy group compared with 445 (4·9) in the control group (hazard ratio 0·62, 95 CI 0·53�0·73, p<0·0001). Reductions were also observed for the separate components of the primary outcome: myocardial infarction (0·52, 0·38�0·70), revascularisation (0·54, 0·36�0·80), stroke (0·59, 0·45�0·78), and cardiovascular death (0·65, 0·52�0·81). Significant reductions in the primary outcome and its components were observed in the analyses of fixed-dose combination strategies with and without aspirin, with greater reductions for strategies including aspirin. Treatment effects were similar at different lipid and blood pressure levels, and in the presence or absence of diabetes, smoking, or obesity. Gastrointestinal bleeding was uncommon but slightly more frequent in the fixed-dose combination strategy with aspirin group versus control (19 0·4% vs 11 0·2%, p=0·15). The frequencies of haemorrhagic stroke (10 0·2% vs 15 0·3%), fatal bleeding (two <0·1% vs four 0·1%), and peptic ulcer disease (32 0·7% vs 34 0·8%) were low and did not differ significantly between groups. Dizziness was more common with fixed-dose combination treatment (1060 11·7% vs 834 9·2%, p<0·0001). Interpretation: Fixed-dose combination treatment strategies substantially reduce cardiovascular disease, myocardial infarction, stroke, revascularisation, and cardiovascular death in primary cardiovascular disease prevention.     Mucinous cystadenocarcinoma of breast in a 69-year-old woman with positive hormone receptors, the first case reported

    Background: Mucinous cystadenocarcinoma is a relatively uncommon histological subtype of breast cancer that is a cystic form of papillary mucinous carcinoma. It is regularly negative for estrogen and progesterone receptors and it is most often diagnosed in older than 55-60 years old. The incidence of breast mucinous cystadenocarcinoma is about 1-6 of primary breast cancers. Here, we present a case of breast mucinous cystadenocarcinoma of left breast in a 69-year-old female which is positive for estrogen and progesterone receptors. Case Presentation: In this article, we describe a case of a-69-year-old female with a painful mass in her left breast. Based on intraoperative pathology consult, neoplastic tissue mostly floating in mucinous lakes with invasion to surrounding stroma was seen. Immunohistochemistry profile showed positive estrogen and progesterone receptors and negative for HER2. Conclusion: Mucinous cystadenocarcinoma of breast is typically triple negative for hormone receptors. But ER and PR positive variant of this tumor is rare, giving the chance of a better prognosis for the patient with hormonal therapy. © The Author(s

    Expression and production of protoscolex recombinant P29 protein and its serological evaluation for diagnosis of human hydatidosis

    Cystic Echinococcosis/Hydatidosis considered as one of the most important parasitic diseases in humans and animals across the world. The goal of the present study was to determine a native antigen with an acceptable sensitivity and specificity to be used in the human hydatid cyst diagnostic methods. In the present study, recombinant P29 antigen was used to detect the antibodies in the serum of patients with hydatid cysts of the liver. In fact, purified recombinant P29 protein is used as an antigen in ELISA. In order to evaluate the recombinant P29 protein for diagnostic ELISA, 25 serums obtained from people harboring the hydatid cysts were tested. The result of the gene expression on a 12 SDS-PAGE gel showed a band with a length of 28 KD. Also, 28KDa band was observed through the reaction of recombinant P29 protein with Anti T7-tag monoclonal antibody in the western blotting method. This protein showed satisfactory results in detecting the hydatid cyst antibodies in the serum of patients having hydatid cysts. Twenty two of 25 hydatidosis serums positively reacted in ELISA using with P29 protein, indicating in 92 of ELISA sensitivity, 95 of specificity, 95.83 of positive predictive value, and 90.42 of negative predictive value for recombinant P29 protein. Whereas the produced recombinant protein P29 showed promising results in the diagnosis of hydatidosis but of more research needs to be done to reach a more accurate conclusion. © 2021, Indian Society for Parasitology

    Cardiac fibrosis and curcumin: a novel perspective on this natural medicine

    Background: According to WHO statistics, cardiovascular disease are the leading causes of death in the world. One of the main factors which is causing heart failure, systolic and diastolic dysfunction, and arrythmias is a condition named cardiac fibrosis. This condition is defined by the accumulation of fibroblast-produced ECM in myocardium layer of the heart. Objective: Accordingly, the current review aims to depict the role of curcumin in the regulation of different signaling pathways that are involved in cardiac fibrosis. Results: A great number of cellular and molecular mechanisms such as oxidative stress, inflammation, and mechanical stress are acknowledged to be involved in cardiac fibrosis. Despite the available therapeutic procedures which are designed to target these mechanisms in order to prevent cardiac fibrosis, still, effective therapeutic methods are needed. Curcumin is a natural Chinese medicine which currently has been declared to have therapeutic properties such as anti-oxidant and immunomodulatory activities. In this review, we have gathered several experimental studies in order to represent diverse impacts of this turmeric derivative on pathogenic factors of cardiac fibrosis. Conclusion: Curcumin might open new avenues in the field of cardiovascular treatment. © 2021, The Author(s), under exclusive licence to Springer Nature B.V

    In vitro Selection of Synbiotics and in vivo Investigation of Growth Indices, Reproduction Performance, Survival, and Ovarian Cyp19α Gene Expression in Zebrafish Danio rerio

    In this study, we tested the compatibility of two extracts from the plant Jerusalem artichokes and button mushrooms with two different Lactobacillus probiotics (Lactobacillus acidophilus; La and Lactobacillus delbrueckii subsp. Bulgaricus; Lb) to develop a synbiotic formulation to improve the growth, survival, and reproductive performances of farmed fishes. Initially, we employed in vitro approach to monitor the growth of the probiotic lactobacilli in the presence of the different doses of the plant-based prebiotics, with the aim of selecting interesting combination(s) for further verification under in vivo conditions using zebrafish as a model. Results from the in vitro screening assay in the broth showed that both the probiotic species showed a preference for 50 mushroom extract as a source of prebiotic. A synbiotic formulation, developed with the selected combination of L. acidophilus, L. bulgaricus, and 50 mushroom extract, showed a positive influence on the growth and reproductive performances of the zebrafish. Our findings also imply that the improvement in the reproductive indices was associated with the upregulation of a cyp19a gene. Overall results suggest that a combination of L. acidophilus, L. bulgaricus, and mushroom extract can be considered as a potential synbiotic for the successful production of aquaculture species. © Copyright © 2021 Zakariaee, Sudagar, Hosseini, Paknejad and Baruah

    Contribution of HLA class II genes, DRB4*01:01, DRB1*07:01, and DQB1*03:03:2 to clinical features of Vitiligo disease in Iranian population

    Background: Vitiligo is a multifactorial depigmentation condition, which is due to skin melanocyte destruction. Increased expression of HLA class II genes in patients with pre-lesions of Vitiligo suggests a crucial role for the participation of immune response in Vitiligo development. Recent studies progressively focused on HLA-DRB1 and DQB1 genes. In this study, we have evaluated the association and role of HLA-DRB4*01:01, -DRB1*07:01, and -DQB1*03:03:2 genes in different clinical subtypes of Vitiligo in the Iranian population. Methods: First, Genomic DNA from peripheral blood of 125 unrelated Vitiligo patients and 100 unrelated healthy controls were extracted through the salting-out method. Then, HLA class II genotyping was performed using the sequence-specific primer PCR method. Finally, the clinical relevance of the testing for these genotypes was evaluated by applying the PcPPV (prevalence-corrected positive predictive value) formula. Results: Our results indicated the positive associations of DRB4*01:01 and DRB1*07:01 allelic genes with early-onset Vitiligo (p = 0.024 and 0.022, respectively). DRB4*01:01 also showed strong protection against late-onset Vitiligo (p = 0.0016, RR = 0.360). Moreover, our data revealed that the DRB1*07:01 increases the susceptibility to Sporadic Vitiligo (p = 0.030, RR = 1.702). Furthermore, our findings proposed that elevated vulnerability of Vitiligo patients due to DRB4*01:01 and DRB1*07:01 alleles maybe is correlated with the presence of amino acid Arginine at position 71 at pocket 4 on the antigen-binding site of the HLA-DRB1 receptor. Conclusion: Our findings on different subtypes of Vitiligo suggest that, despite a more apparent autoimmune involvement, a non-autoimmune nature for the etiology of Vitiligo should also be considered. © 2021, The Author(s), under exclusive licence to Springer Nature B.V

    Administration of dietary antioxidants for patients with inflammatory bowel disease: A systematic review and meta-analysis of randomized controlled clinical trials

    Objective: Accumulating evidence has been reported regarding the effect of dietary antioxidants on clinical variables in IBD patients, however, findings are controversial. This systematic review and meta-analysis aimed to investigate effect of dietary antioxidants on clinical variables in patients with IBD or its subtypes. Methods: We searched PubMed, Scopus, and ISI Web of Science from inception to January 2021 using relevant keywords. Data were pooled by using the random-effect model. All statistical analyses were done using STATA version 14. Results: Our meta-analysis was exclusively done on studies about the effect of curcumin on IBD patients, because limited studies were done on other antioxidants. Curcumin administration resulted in significant increment of clinical remission in patients with IBD (SMD: 0.86, 95 CI: 0.16, 1.56, p = 0.016), significant remission in clinical symptoms (SMD: �0.96 score, 95 CI: �1.34, �0.57, p < 0.001), and significant increment in endoscopic remission in IBD patients (SMD: 0.51, 95 CI: 0.16, 0.85, p = 0.004), comparing to control group. Curcumin supplementation also made better clinical response than control group (SMD: 0.74, 95 CI: 0.22, 1.26, p = 0.005) and also resulted in significant improvement in quality of life of patients with IBD, as compared to control group (SMD: 1.23 score, 95 CI: 0.72, 1.74, p < 0.001). Conclusions: Our meta-analysis showed that curcumin significantly improved clinical and endoscopic remissions in IBD patients. This supplementation also caused significant reduction in clinical symptoms of IBD patients along with better clinical response and the increased quality of life. Further researches with larger sample size and longer period of intervention are required to evaluate efficacy of dietary antioxidants on clinical variables in patients with IBD. © 202
