Resilience and perinatal mental health : development and evaluation of an intervention

    No full text
Background and objectives Perinatal mental health is a significant public health concern with a rising global burden. Its detrimental effects and consequences can have lasting impacts on a mother, child, and subsequently the entire family. This situation is more serious in developing countries like Pakistan. Numerous factors, such as poverty, economic instability, illiteracy, gender discrimination, sociocultural beliefs, a lack of social support, awareness, and stigma, etc. are associated with poor mental health in women. Given that the majority of Pakistan's population lacks access to adequate resources, it is crucial to draw attention to the prevention and promotion of mental health during pregnancy. Positive interventions to build resilience have been shown to have a strong protective effect against anxiety and depression during pregnancy because of their capacity to mediate the relationship between stress and its effects on mental health. It is a construct that helps a person in developing internal strength, capability, flexibility, and the capacity to cope successfully with difficulties. In the context of Pakistan, this has not yet been tested therefore, the overall aim of this research is to help disadvantaged pregnant women of Pakistan in building resilience. Our study objectives were to explore resilience core characteristics, develop, validate, and evaluate a resilience-based intervention to improve pregnant women’s resilience and marital adjustment and reduce depressive and pregnancy-related anxiety. Methods Two-phased intervention research was used to achieve the objectives. The first phase was the formative/ pre-intervention phase, and the second was the intervention phase. In phase I, we used an exploratory-descriptive design to better understand this resilience phenomenon and to develop the content of the intervention. We identified the core characteristics of resilience by interviewing 25 stakeholders: 17 pregnant women and 8 key informants. All of the women were married, aged 18 years and above, and at least 12 weeks of gestation. The key informants were professionals with more than five years of experience in the field of mental health. Participants were enrolled using purposive sampling techniques. For pregnant women and KIs, a separate semi-interview guide was developed. Data collection was stopped until saturation was obtained. Based on the input and responses from the pregnant women and KIs, the intervention content was considered and developed using a variety of teaching and learning techniques. The Gunning Fog Index (GFI) was used to assess each sentence, paragraph, and module to ensure that the language was clear, concise, and comprehensive. Then for content validation, eight expert members (two mental health nurses, two psychiatrists, two psychologists, and two maternal and child health experts) were approached. They evaluated our contextually developed Safe Motherhood- Accessible Resilience Training (SM-ART) intervention based on a self-developed 15-item checklist. The whole formative/pre-intervention phase was conducted over the course of a year and nine months (January 2019 to September 2020). In the second intervention phase, a single-centered, single-blind, two-group Randomized Control Trial design (RCT) was adopted to test the effectiveness of the SM-ART intervention on improving resilience, marital adjustment, and decreasing depression and pregnancy-related anxiety in a sample of pregnant women presenting to midwifery-led, Koohi Goth Hospital of Karachi, Pakistan. Our total sample size was 200 with 100 in each group. Permuted block randomization with blocks of four was employed. To ensure equal representation in each block, pregnant women were randomly assigned in a 1:1 ratio to either the intervention or control group. The intervention group received six weeks of SM-ART intervention, one session lasting 60 to 90 minutes each week, on resilience-based skills. CMWs (Community Midwives) were trained to deliver this intervention. Both intervention and control groups had access to standard care, a self-developed pamphlet on positive mental health, and a comprehensive list of local mental health service providers. Validated scales were used to measure outcomes (resilience, depression, pregnancy-related anxiety, and marital adjustment) in both the intervention and control groups at baseline (pre-intervention) and post-intervention. This intervention phase was conducted over the course of six months. Results In phase I, our study identified six core components of resilience that may promote mental health during pregnancy. These include: 1) finding the purpose of life, 2) dealing with emotions, 3) believing in yourself, 4) adopting an optimistic approach, 5) strengthening support and relationship, and 6) internalizing spirituality and humanity. Six themes emerged from their in-depth interviews and became part of the SM-ART intervention. The intervention comprised six modules based on these six different themes. Every module has a unique purpose that sets it distinct from the others and allows it to stand on its own. The GFI varied between modules, ranging from an index of nine to an index of seven. As per the expert's evaluation, The SM-ART intervention consisting of six modules showed strong to perfect CVI scores for each of the modules. Their responses also supported the intervention's strengths as having innovative and engaging activities, contextual and cultural relevance, and a detailed comprehensive facilitator guide. In Phase II, the intervention and control groups were found comparable regarding their socio-demographic, pregnancy, marriage, and family-related factors. Additionally, there is no statistically significant difference between their baseline mean scores for resilience, depression, marital adjustment, and pregnancy-related anxiety. Post-Intervention findings revealed that mean resilience scores in the intervention group (82.68 +14.54)) showed a significant difference (6.91) compared to the control group (75.77 +13.06), with p-value <0.001. Also, the mean depression scores of the intervention group were (8.86+6.67) significantly different (-2.12) compared to the control group mean (10.98+7.56), p-value <0.05. However, no significant change was observed in anxiety and marital adjustment scores between the intervention and control groups. Conclusion and recommendations The main findings of this study emphasize the use of these core competencies to define the resilience pathway, the availability of readily available SM-ART resource material, and the enhancement of women's resilience and reduction in depressive symptoms following a 6-week SM-ART intervention. The study also recognizes the potential for a non-medical, strength-based positive psychological intervention to improve resilience and reduce depression scores, which could ultimately lessen the burden of perinatal mental illness in Pakistan. We recommend that every woman seeking antenatal care be encouraged to take part in this intervention at least once while she is pregnant. Women will have the chance to express their emotions and worries in a secure environment. Moreover, this should be part of the core curriculum of midwives. Consequently, including such interventions in public health initiatives, especially in resource-limited countries like Pakistan, may help to improve the mental health of women and promote the development of healthy families.

Digitalisation and on-farm diversification : challenges and opportunities for multifunctional and diversified agriculture

    No full text
No matter how ambiguous it can be, the digitalisation of agriculture continues to be a top priority of industrialised economies. More and more evidence on the direct and indirect effects of digitalisation is emerging, and this feeds into all sides of the debate on the future trajectories of digital agriculture. However, the body of scientific research in digital agriculture tends to be less concerned with digital applications to, and impacts on, farming styles that are integrating agriculture, food, and rural development, such as on-farm diversification. On-farm diversification consists of mobilising farm assets, labour, and outputs in extra-farming activities related to the farm, such as food processing, social farming, agritourism, recreational and educational work, landscape management, and so forth. By using a qualitative and grounded theory approach in a journey marked by COVID-19 pandemic, this research explores the experiences of diversified farmers on the use and impacts of digitalisation in 13 farms across different contexts (Italy, Belgium, Ireland, Japan). The findings reveal that digitalisation can be adapted to the relational, creative, and on-site nature of diversified farms. At the same time, farmers are quite conscious of the costs, flaws, and potential threats to their own autonomy. What are quite important for diversified farmers are also elements outside the agri-tech realms, such as landscape features, housing conditions, wellbeing, and the relations with the living beings. These on-site, socio-physical elements are pre-conditions for diversifying and cannot be eclipsed by the hype of digitalisation. In terms of impacts, the research sheds light on the ambiguous role of digitalisation in on-farm diversification by exposing the positive and negative experiences of farmers. To deal with these ambiguities, policy-makers can create an enabling framework to balance these positive and negative effects, as well as take responsibility in the direction setting of digitalisation, by starting from agri-food and rural policies. For instance, the thesis suggests that an EU-wide target to substantially increase the share of farmers engaged in on-farm diversification could be integrated into the EU Farm-to-Fork strategy and CAP, and monitored through available Eurostat data

The first translation of the Passio Theclae in Old Church Slavonic : a new approach to critical text-editing in Palaeoslavic studies

    No full text
This dissertation focuses on the hagiographical literature devoted to the first female saint and martyr Thecla in its medieval Slavic Orthodox context. It is part of the growing scholarly interest in Saint Thecla in different languages and their corresponding manuscript traditions. The core of the dissertation concerns the Old Church Slavonic translation of the Passio Theclae or passion of Saint Thecla, also known as the originally apocryphal acts of apostle Paul and his female pupil Saint Thecla (APTh, or BHG 1710). This text was written in Greek towards the end of the second century: its popularity throughout Late Antiquity and the early Middle Ages is reflected in the abundance of extant Greek manuscript copies, and the many paraphrases and additional texts based on this Passio. Only a selection of this corpus in Greek has been translated into Old Church Slavonic, namely the original Passio BHG 1710 and the alternative ending BHG 1713e, together with the additional miracula BHG 1718m and 1718n, in Palaeoslavic studies referred to as the Passio BHBS 1a (Ivanova 2008: 221) and its second version 1b, accompanied by the miracula BHBS 3, 0 and 2 (Ivanova 2008: 222-223). This entire hagiographical corpus dedicated to Saint Thecla has most likely been translated as a whole at the same moment in time, although this hypothesis cannot be confirmed entirely because of lacking evidence and a gaping hiatus in data from that period, that is, from 893 until 969. This translated Passio has been preserved in forty-eight manuscripts, dating from the eleventh until seventeenth centuries and presenting different Church Slavonic spellings. Only six copies of this text are known to the broader scholarly community through diplomatic editions, and digital reproductions are available online of an additional six manuscripts. No attempt has yet been made to provide a critical edition of this text: with this dissertation, I aim to fill this gap in the scholarly investigation of Saint Thecla. Although the traditional methodological approach of text criticism known as the reconstructive method has not gained full recognition in the field of Palaeoslavic studies, I present the advantages and importance of this approach with the current case study of the Passio Theclae in its first translation into Old Church Slavonic. Through this approach, I reconstruct two hyparchetypes of this text: first, the original version of this translation into Old Church Slavonic of the Passio Theclae, and, second, its redacted version, created on the basis of a new Greek exemplar. The critical edition further explores the presence of two Greek exemplars as well. Following the preferred diplomatic text-editing technique, the eight most important text witnesses on stemmatological grounds have been selected as representatives of the Slavic textual transmission and presented diplomatically in the edition. This way, the current edition offers the opportunity to explore the origins of this text and its relation to the original Greek source text material on the one hand, and to investigate the Slavic textual transmission into more detail on the other hand.

Linked data event streams as an open data interface for live data consumption in integrated services

Optical methods, uncertainties, and algorithms for remote sensing of coastal and inland water quality

Hydrologic optics and aquatic remote sensing are fields of applied optics that aim to retrieve information of state and process of aquatic systems from optical signals. Such an inference link depends on a rich set of paired optical and biogeochemical data, robust development and evaluation of algorithms and deep understanding of the underlying processes in order to support meaningful ecological interpretation. In this research we explore those different aspects by evaluating and proposing improvements for methods of field spectroscopy, describing a dataset with detailed compositional information covering a diverse range of environmental conditions in Belgian waters, and by providing algorithms to retrieve information of the phytoplankton assemblage for use with in situ optical instrumentation and remote sensors. In Chapter 1 we provide an overview of the key concepts in hydrologic optics and aquatic remote sensing that provide the context for this study. In Chapter 2 we provide a detailed evaluation of the uncertainty in downwelling irradiance estimations with diffuse reflectors, a topic overlooked despite large sets of data, particularly in inland waters, collected with this methodology. Our results help to shed light into uncertainty budgets of past measurements and provide basis for suggestion for an improved measurement protocol. Based on the magnitude of the uncertainty, we are also able to ascertain the suitability of this method for all uses of downwelling irradiance in hydrologic optics. In Chapter 3 we provide another detailed evaluation related to errors in water-leaving radiance measurements with the on-water approach, a relatively recent method for field spectroscopy. We specifically evaluate the deployment setup used in this research, in order to provide corrections to our measurements. Additionally, we provide evaluations of the sensitivity of the error and simulations to parameters like backscattering ratio and the angular distribution of the downwelling radiance. We further provide evaluation of different deployment setups and suggest possible improvements for the method. In Chapter 4 we describe the full set of paired optical and biogeochemical data collected in Belgian inland and coastal waters. Extensive evaluations are provided to assert data consistency as a proxy for its accuracy. Of particular interest is the pairing of inherent and apparent optical data with particle size distribution, mineral fraction and metabarcoding, providing detailed composition information that can be used for devolvement of hyperspectral algorithms. The data is made freely available in a stable repository. In Chapter 5 we describe and validate a method to extract additional spectral information from a system of overlapping wavebands and apply this concept to the Operational Land Imager (OLI) onboard Landsat 8. We show that a virtual band, termed contra-band, with information on the yellow-orange spectra region covering the absorption peak of phycocyanin can be extracted from the panchromatic band of OLI. We provide extensive evaluations of the uncertainty and generality of the algorithm and of the independence of the retrieved information. In Chapter 6 we revisit the problem of optical detection of Phaeocystis globosa blooms in the Belgian Coastal Zone (BCZ) by evaluating the origin of the signal detected by previously proposed hyperspectral algorithms. We show that the signal detected is not specific of Phaeocystis globosa even when considering the phytoplankton assemblage composition of the BCZ, due to synchronous phenology of other blooming species producing similar optical signals. We further uncover an apparent general pigmentation pattern associated with presence of chlorophyll c 3 . We conclude by providing a new algorithm to detect this pigmentation pattern and provide a regional ecological interpretation linked to Harmful Algal Blooms (HABs). The research in those chapters is extensively supported with additional information in the form of theory, data and analysis provided in various Appendices. Lastly, in Chapter 7 we provide a synthesis of this research, discussing the relevance of our findings in the scope of hydrologic optics and aquatic remote sensing
