DİYABET YÖNETİMİNDE BÜTÜNCÜL BİR BAKIŞLA TAMAMLAYICI VE İNTEGRATİF SAĞLIK UYGULAMALARI
Diyabet, yüksek morbidite ve mortalite oranları ve hastalığın giderek artan yaygınlığı ile ilişkili, dünyadaki en ciddi sağlık sorunlarından biridir. Diyabetin akut ve kronik komplikasyonlarını önlemek ve mortaliteyi azaltmak için diyabetin yönetimi hayati öneme sahiptir. Az gelişmiş ve gelişmekte olan ülkelerde diyabet yönetimi yetersizdir. Etkili diyabet yönetimi, kanıta dayalı kılavuzlar, ekip iletişim becerileri ve sağlıklı yaşam tarzı davranışlarının teşviki kullanılarak multidisipliner bir ekip tarafından bütünsel bakım yoluyla sağlanır. Bütünsel sağlık hizmeti sunan kuruluşlar ayrıca tamamlayıcı ve bütünleştirici tedavi uygulamalarından da yararlanır. Bu noktada, multidisipliner ekip üyelerinin bir parçası olarak hemşirelerin uygulamalardaki rolü, sağlığın iyileştirilmesine ve sürdürülebilirliğine katkıda bulunarak daha da önem kazanmaktadır. Hemşireliğin önemli teorisyenleri Florence Nightingale ve Jean Watson da bütünsel bakımı vurgulamışlardır. Literatürde Reiki'nin kolesterol ve kan şekeri kontrolünde, lavanta yağı aromaterapisinin insülin enjeksiyonu ağrısını azaltmada, Qigong'un açlık kan şekerini düşürmede, yoganın kan şekeri kontrolünde ve refleksolojinin periferik nöropati ve kan şekeri kontrolünü azaltmada etkili olduğu belirtilmiştir. Bu derlemenin amacı, diyabet yönetiminde tamamlayıcı ve bütünleyici terapilerin kullanımı hakkında bilgi vermek ve literatüre katkıda bulunmaktır.Diabetes is one of the most serious health problems in the world, associated with high rates of morbidity and mortality and a growing prevalence of the disease.In order to prevent acute and chronic complications of diabetes and to reduce mortality, the management of diabetes is of vital importance.Diabetes management in underdeveloped and developing countries is inadequate. Effective diabetes management is achieved through holistic care by a multidisciplinary team using evidence-based guidelines, team communication skills, and promotion of healthy lifestyle behaviors.Organizations that provide holistic health care services also use complementary and integrative treatment practices. At this point, the role of nurses as part of the multidisciplinary team members in practices becomes even more important by contributing to the improvement and sustainability of health. Florence Nightingale and Jean Watson, important theorists of nursing, also emphasized holistic care. In the literature, Reiki is effective in cholesterol and blood sugar control, lavender oil aromatherapy is effective in reducing insulin injection pain, Qigong is effective in reducing fasting blood sugar, yoga is effective in blood sugar control, and reflexology is effective in reducing peripheral neuropathy and blood sugar control. Integrating Clinical and Laboratory Markers to Predict Transfusion in Pediatric Trauma Patients
Trauma-related hemorrhage represents a leading contributor to avoidable deaths in pediatric patients and often necessitates prompt transfusion decisions. Unlike adults, children may maintain hemodynamic stability until significant blood loss occurs, making early recognition of transfusion needs particularly challenging. This investigation sought to determine dependable clinical and laboratory predictors of blood transfusion during the initial 24-hour period of pediatric trauma and to establish a practical risk stratification tool to aid emergency decision-making. This retrospective study included pediatric trauma patients under 18 years of age who were admitted to a Pediatric Intensive Care Unit. Clinical and laboratory data at admission were recorded, and patients were grouped based on whether they received blood transfusions during the initial 24-hour period. Univariate and multivariate logistic regression analyses were conducted to determine independent predictors. Receiver operating characteristic curve analysis was employed to establish optimal threshold values and develop a combined risk score for transfusion prediction. Among 95 pediatric trauma patients, 34.7% required blood transfusion within 24 hours. Transfused patients had significantly lower pediatric trauma scores, higher glucose levels, lower platelet counts, and were more frequently intubated. Multivariate analysis identified intubation, glucose, and platelet count as independent predictors of transfusion. A combined risk score incorporating these variables demonstrated high discriminative ability (area under the curve=0.831). Risk stratification revealed transfusion rates of 3.8% in low-risk patients, 30.2% in moderate-risk patients, and 100% in high-risk patients. The combination of intubation, glucose, and platelet count provided a simple yet effective tool for predicting transfusion needs in pediatric trauma. The resulting risk score outperformed individual parameters and allowed for early identification of high-risk patients. This model may help guide timely transfusion decisions and optimize care in emergency settings. Prospective validation is warranted to confirm its clinical utility and broader applicability
Comparative performance analysis of two matrix-assisted laser desorption/ionization time-of-flight mass spectrometry systems for direct identification of urinary tract pathogens from clinical samples
Abstract Background/Aim: Urinary tract infections (UTIs) are a significant global health concern that necessitates expedited diagnostic methods to guide appropriate antimicrobial treatment and mitigate the spread of multidrug-resistant organisms. This investigation was conducted to assess the effectiveness of two distinct matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) platforms for the direct identification of UTI-causing agents from urine samples: VITEK MS (bioMérieux, France) and MALDI Biotyper Sirius (Bruker Daltonics, Germany). The findings were evaluated by comparison of the outcomes with those of conventional urine culture, which is considered the diagnostic gold standard. Methods: The study included 60 urine specimens consisting of 40 specimens from patients presenting with UTI symptoms and 20 specimens from a control group that showed no bacterial growth in culture. A differential centrifugation method was employed to prepare the samples, which were subsequently analyzed using both MALDI-TOF MS systems. Concurrently, all samples underwent conventional urine culture. Results: Among the 40 culture-positive samples, the MALDI Biotyper Sirius system demonstrated an overall identification sensitivity of 69.2% and a specificity of 95.2%. The VITEK MS system showed a sensitivity of 79.5% and a specificity of 95.2%. For the 29 samples with a bacterial concentration of ≥1×105 colony-forming units per milliliter (CFU/mL), the sensitivity was 75.9% for the MALDI Biotyper Sirius system and 79.3% for the VITEK MS system. A statistical evaluation using McNemar's test determined that the difference in sensitivity between the two platforms was not statistically significant (P=0.125). Conclusion: The findings suggest that both MALDI-TOF MS platforms have considerable promise for the rapid and direct identification of uropathogens, particularly in monomicrobial urine samples with a high bacterial load (≥1×105 CFU/mL). While the VITEK MS system exhibited a marginally higher overall identification sensitivity, the data indicate that both systems can be considered valuable assets in clinical diagnostic laboratories.
