    Evaluating the overt extracardiac malformations in children with congenital heart disease in Khuzestan Province

    Background: Extracardiac malformations can be seen in 20-45% of infants with congenital heart disease (CHD). Chromosomal abnormalities exist in 5-10% of patients with CHD. The aim of this study was to assess the frequency of overt extra cardiac malformations in children with CHD. Methods: This descriptive epidemiologic study was conducted on 720 patients with CHD referred to the pediatric cardiac clinic of the most important south west referral center of Iran in 2014. Data were collected by studying patients' files during this time. Results: Totally, 401(55.7%) and 319 (44.3%) of patients were male and female, respectively. Extracardiac malformations were diagnosed in 53 cases (7.36%) and multiple extracardiac malformations involving more than one anomaly were present in 10 cases (1.38%). The most frequent extracardiac malformations in the current study included skeletal and visceral abnormalities and then genitourinary was seen. Of all patients, 33 (4.58%) cases were affected by syndromic features of which Down syndrome was the most common. Conclusions: Investigation for extra cardiac abnormalities in patients with CHD is very important, since proper diagnosis and early treatment of these abnormalities can improve the patients' outcomes. On the other hand all the patients with overt and syndromic abnormalities require investigation for a possible CHD

    Pediatric vesicoureteral reflux approach and management

    Vesicoureteral reflux (VUR), the retrograde flow of urine from the bladder toward the kidney, is congenital and often familial. VUR is common in childhood, but its precise prevalence is uncertain. It is about 10–20% in children with antenatal hydronephrosis, 30% in siblings of patient with VUR and 30–40% in children with a proved urinary tract infection (UTI). Ultrasonography is a useful initial revision but diagnosis of VUR requires a voiding cystourethrography (VCUG) or radionuclide cystogram (DRNC) and echo-enhanced voiding urosonography (VUS). Although for most, VUR will resolve spontaneously, the management of children with VUR remains controversial.     Asthma awareness among primary school teachers in Zanjan in 2014

    Background: Asthma is the most common chronic respiratory disease and a leading cause of school absenteeism. Awareness, attitude, and knowledge of asthma can play an effective role in controlling this disease and its consequences. Accordingly, this study aimed to determine the level of asthma awareness among primary school teachers in Zanjan. Methods: This descriptive cross-sectional study was conducted on 403 teachers of boys- and girls-only primary schools, who were selected using single-stage cluster sampling. Data were collected using a researcher-made questionnaire, in which higher scores indicated higher level of asthma awareness. Findings were analyzed using descriptive and analytical statistics in SPSS. Results: Data analysis showed that 58 (14.4%) and 345 (85.6%) out of 403 participants were male and female, respectively. The means of age and work experience of the participants were 45±5.53 and 22±5.92 years old, respectively. In addition, teachers’ mean score for asthma awareness was at a good level (12±2.2). Teachers’ mean level of asthma awareness had a significant relationship with gender and history of dealing with asthmatic children (p = 0.03, p = 0.04), while it had no significant relationship with age, work experience, and education level. Conclusion: In this study, the mean level of asthma awareness among the primary school teachers was good. Due to the importance of the disease in children and effective role of teachers in helping them, effective measures must be taken to increase teachers’ awareness

    The effect of dexamethasone therapy on prolonged latent phase of labor: a randomized double-blind clinical trial

    Background: Shortening the interval between the onset of labor and delivery is useful for both mothers and fetuses. The aim of the study was to investigate the effect of dexamethasone on the augmentation and the duration of labor in prolonged latent phase of labor. Method: The design of the study was a randomized double-blind clinical trial design, which was conducted in delivery center of Rouhani hospital in Babol (Iran). A total 121 nulliparrous women with a singleton pregnancy and cephalic presentation at 40–42 gestational weeks in prolonged latent phase were randomly assigned to receive 2 ml ampoule dexamethasone 4 mg/mL (the intervention group) and 2 ml ampoule of sterile water for injections (the control group), which were both intramuscularly administered. Then, the Then, the augmentation of labor with the use of intravenous oxytocin infusion (Caspian Tamin Company Iran) (2.5 m units/ per minute) began in both groups. The primary outcome was the duration of time between the onset of augmentation and the second stage of labor. Results: The duration of time between the onset of labor augmentation and the second stage of labor (hours) was 5.6±1.9 in the study group, whereas it was 7.7±1.5 in controls with a significant difference (p ≤ <0.001). In the study group, the duration of time between the onset of labor augmentation and the active phase of labor was lower than that of the control group (p= 0.02). In addition, the duration of the second stage of labor (p <0.001) and the third stage (p <0.001) was lower in the study group compared with that of the control group. Conclusion: It is imperative that midwives administer dexamethasone to improve the prolonged latent phase in women during the labor

    Comparison of coronal microleakage of resin modified glass ionomer and composite resin as intra-orifice barriers in internal bleaching

    Introduction: Internal bleaching is a treatment option for wightening endodontically treated discolored teeth. Cervical resorption is one of the side effects of this method. The aim of this study was to compare the sealing ability of resin composite and light-cured resin modified glass ionomer (RMGI) as intra-orifice barriers in internal bleaching. Materials &Methods: In this study, 34 single-canal anterior teeth were used. All samples were endodontically prepared and divided into two experimental groups (n=12) and two control groups (n=5). In the experimental groups, Gutta-percha was removed up to 3 mm below the cemento enamel junction (CEJ). RMGI and composite resin was placed over gutta-percha in the experimental groups up to the level of CEJ. After 24-hours incubation period, the bleaching agent (a mixture of sodium perborate and 30% hydrogen peroxide) was placed in the access cavities. The bleaching agents were replaced every 3 days over 9 days. Then, the access cavity was filled with 2% methylene blue for 48 hours. All samples were longitudinally sectioned and the dye penetration range was evaluated using stereomicroscope. Data was statistically analyzed by using T-student test and variance analysis. Results: The microleakage in RMGI group was 0.945mm and in composite resin group was 0.641mm. Statistically, no significant difference was observed in microleakage between the experimental groups (p=0.121). Conclusion: Both materials can be applied as the intra-orifice barriers for internal bleaching

    Antibacterial efficacy of lavandula officinalis extract, sodium hypochlorite and chlorhexidine gluconate solutions as root canal irrigations: A comparative analysis

    Introduction: This in vitro study aimed to compare the antimicrobial effect of lavandula -0fficinalis extract, with sodium hypochlorite (NaOCL) and chlorhexidine gluconate (CHX), as root canal irrigants, on Enterococcus faecalis (EF). Materials &Methods: Seventy five extracted single-rooted premolars were selected. Root canals were prepared using rotary ProTaper system and were infected with the culture of E. faecalis. Specimens were randomly divided into five groups (n = 15), Group 1, 2: lavandula extracts (0.26 and 0.52 mg/mL), Group 3: 2.5%NaOCL, Group 4: 2%CHX, Group 5: Normal Saline. Irrigation was performed for each group for 5, 10 and 15 min. The viable bacteria obtained by collecting the canal dentin chips. Data analysis was performed with Kruskal-Wallis and Mann-Whitney u tests. Results: The mean number of viable bacteria was significantly reduced after 5 min exposure to lavandula solutions (p<0.05). A significant difference also existed between different times in the NaOCL group, being significant between 5 and 15 min (p<0.05), but there was no significant difference between different times in the CHX group. Comparison of the mean number of viable bacteria between different groups at different exposure times revealed that the difference between lavandula and NaOCL solutions with CHX was significant at 5 and 10 min (p<0.05), however, no statistically significant difference was observed between lavandula solutions and NaOCL. Conclusion: lavandula extract was effective in killing of EF. Further studies are necessary to fully understand its other properties such as tissue solubility, removal of smear layer and impact on dentin structure

    Evaluating the effect of dentin surface pretreatment on the static contact angle of a drop of a bonding agent: an in vitro study

    Introduction: The aim of this study was to investigate the effect of dentinal pretreatment on the static contact angle of a bonding agent as a measure of dentin surface wettability. Materials &Methods: Twenty mid-coronal dentin surfaces were prepared and randomly allocated to four groups (n=5) according to the priming solutions. All segments were etched with 35% phosphoric acid gel for 15 s, rinsed for 30 s and dried. Each group was rehydrated with 10 µL of distilled water, 0.2 % chlorhexidine, 70% ethanol and 5.25% Sodium Hypochlorite respectively and the excess solution was removed after 60 sec using an absorbent paper. Using a micro syringe, a droplet of the Adper Single Bond 2 was placed on each prepared surface. Then the profile and the static contact angle of the droplet were analyzed with a video-based optical contact angle measuring system. The statistical analysis was performed using One-way ANOVA and Dunnett’s t tests (p<0.05). Results: There was a statistically significant difference between the water and sodium hypochlorite groups which indicates the negative effect sodium hypochlorite may have on dentinal surface energy. (p=0.013). The differences between the water and ethanol groups (p=0.168) and between the water and chlorhexidine groups (p=0.665) were not significant. Conclusion: The use of 5.25% sodium hypochlorite as a priming solution in bonding procedure is not recommended. There is no improvement in dentinal surface wettability by using 70% ethanol or 0.2% chlorhexidine instead of water and the recommendation for use of any of the two should be based on other long-term or short-term effects they may have on the bonding procedure

    Effects of position on oxygen saturation and heart rate in very low birth weight neonates

    Background: Optimal oxygenation in preterm neonates is very important, therefore different measures are recommended to improve their oxygenation. One of these measures is the position of these infants. The studies on the effects of prone and left lateral positions showed conflicting results. So, the aim of this study was to determine the effect of position on arterial oxygen saturation (SaO2) and heart rate (HR) in very low birth weight (VLBW) neonates. Methods: This non-randomized simple convenient interventional study was conducted on 40 VLBW 7-28-day infants with 29-35-weeks gestational age using in 2014-2015. The infants were hospitalized in the neonatal intensive care unit of Rouhani Hospital in Babol. Based on the inclusion criteria, each of them was initially kept in supine position for 120 minutes and then in prone and finally left lateral position for 120-min after 10-min rest. During this period, SaO2 and HR parameters were recorded every 15-min and data were analyzed. Results: The mean of SaO2 was 97.41±1.91%, 96.74±2.09% and 96.14±2.36% in prone, supine and left lateral positions, respectively and this difference was statistically significant (P=0.032). The mean of HR was 146.09±9.65, 148.15±11.46 and 146.02±10.54 (beat/min) in prone, supine and left lateral positions, respectively. HR was normal in all three positions but the HR variability in prone position was slightly less than other positions (P=0.596). Conclusions: The results of the current study indicated that in preterm newborns, the prone position made more desirable oxygenation and HR variability compared to the supine and left lateral positions

    Evaluation of the accuracy of two apex locators in endodontic treatment and retreatments: an ex vivo study

    Introduction: Working length determination is important in successful endodontic treatment and retreatment. This study evaluated the accuracy of two electronic apex locators Root ZX and Raypex®6 (EALs) in determining the electronic working length (EWL) of the root canals in endodontic treatment and retreatment. Materials &Methods: Access cavities were prepared on forty extracted, single-rooted human teeth and the actual working length (AWL) of the canals was determined. In the first phase of the study, primary EWL of un-instrumented teeth was measured and compared between two EALs. In phase II, all of the teeth were pre-flared and divided into the control (n=10) and the retreatment groups (n=30). Canals in the retreatment group were obturated by the lateral condensation technique using Gutta percha and sealer. After 15 days, gutta-percha was removed, and then the secondary EWL was recorded and compared between the two devices, in treatment and retreatment groups. Data were analysed by paired t-test and t-test. Results: Significant differences were found between both EALs in treatment and retreatment phases of the study (p<0.001). Both EALs showed increased accuracy in retreatment group (p<0.001). However, no statistically significant differences were found between the control and retreatment groups in the second phase of the study for Root ZX (p=0.929), and Raypex®6 (p=0.937). Conclusion: Accuracy of the two EALs was similar and acceptable. EWLs determined by Root ZX were closer to the AWL. The EWL determination after pre-flaring improved the accuracy of EALs and root canal obturation remnant materials did not have any clear effect on the accuracy of these EALs

    Evaluation of the concavity depth and inclination in jaws using CBCT

    Introduction: Nowadays, using implants as a choice in patient's treatment plans has become popular. The aim of this study was to determine the prevalence of mandibular lingual and maxillary buccal concavity, mean concavity depth and angle and its relation to age and gender. Materials &Methods: In 200 CBCT, concavity depth and angle were measured in 2 mm superior to the inferior alveolar canal in the mandibular first molar area and in 1 mm distance from nasal floor in the midpoint region of maxillary lateral incisor and canine. Concavity depth and angle relationships with age and gender have been evaluated using Spearman Correlation and a t-test. Results: Mean and standard deviation of lingual concavity, concavity angle and ridge angle in mandible were 1.3±1.54 mm, 15.45±16.19 and 10.13±6.1. Mean and standard deviation of buccal concavity and concavity angle in maxilla were 5.35±1.03 mm and 30.6±5.75. Mandibular concavity depth was zero in 44% of subjects and more than zero in 56%. Results were more than 3 mm in maxillary samples. There was a linear relationship between mandibular concavity depth and age equaled to -0.27, p=0.007 and for mandibular concavity angle and age equaled to -0.25, p=0.01. There was no significant relationship between mandibular ridge angle and age. In maxilla, there was no linear relationship between age and gender with any other variables. Conclusion: It is necessary to provide more information on these regions' anatomy using CBCT cross sections before implant placement
