    Get PDF
Full mouth rehabilitation with fixed implant supported prostheses is one of the challenging treatments, because important factors in aesthetic and function should be considered. Implants' alignments and locations should be planned before the insertion unless the improper inclination and location of implants would lead to difficulties in patient's comfort, oral hygiene problem and inability to reconstruct the acceptable functional and aesthetical prosthesis. This clinical report described the step by step prosthetic treatment procedure of full mouth rehabilitation and 1-year follow-up of a patient who underwent 10 and 8 implants in upper and lower jaw respectively, which were unaligned and malpositioned. In the cases of improper alignment and position of implants, modified prosthetic design with consideration of biomechanical principles should be incorporated to overcome the limitations

Objectives: The primary infection of pregnant women by Toxoplasma gondii can cause severe complications in the fetus, and can also lead to neurological complications. The aim of the present study was to determine the seroprevalence and seroconversion rates of Toxoplasma gondii in female students who were at childbearing age in Iran. Methods: This cross sectional study was performed on female students at Babol University of Medical Sciences (Babol, Iran) over a three-year period between 2012 and 2014. Two ml blood sample was obtained from volunteers, and the specific antibodies (IgG) to T. gondii were detected by Enzyme-Linked Immunosorbent Assay. Second, the blood samples obtained from seronegative cases were used to evaluate the seroconversion during a year. Results: The mean age of the subjects was 21±2.2, ranging from18 to 35 years. Overall, 28% of the participants were positive and 232 (72%) were negative for anti-Toxoplasma antibodies (IgG). The rate of this infection was higher among female students from Mazandaran province (31.7% vs 12.5%) (P=0.031). Based on the results of Enzyme-Linked Immunosorbent Assay, the seroconversion rate of Toxoplasma infection among female students was estimated to be 2.5%. Conclusion: This study found a noticeable rate of seroconversion in female students. Therefore, there is an urgent need for a national screening project to determine the seroconversion rate of Toxoplasma infection in women at childbearing age, particularly pregnant women from all over Iran

Background: Preterm labor is a leading cause of fetal and neonatal morbidity and mortality. There are various kinds of drugs used to suppress the preterm labor, but they are not thoroughly effective. The aim of this study was to compare the effectiveness of oral nifedipine with intravenous magnesium sulfate in delaying the preterm labor. Methods: A randomized, clinical trial was conducted in a hospital in Babol, Iran. One hundred twenty singleton pregnant women with preterm labor, 24-37 weeks of gestation, were randomly assigned to receive oral nifedipine or intravenous magnesium sulfate. The main outcome of the study was the inhibition of uterine and the secondary outcome was the side effect related to drugs and neonatal outcome. The data were analyzed with SPSS software, using chi-squared test and independent t test. Results: According to the results, in 35% of women in the nifedipine group and 23.3% of women in the magnesium sulfate group, the inhibited uterine contraction was less than 48 hours. Also, in 65.0% of women in the nifedipine group and 76.7% of women in the magnesium sulfate group, the inhibited uterine contraction was more than 48 hours. There was no significant difference between the nifedipine and the magnesium sulfate groups in the inhibition of uterine contraction in both less and more than 48 hours. The total side effects of medication were found to be lower in patients receiving oral nifedipine than those who received intravenous magnesium sulfate. (26.6 vs. 45.0) (p= 0.036). There was no significant difference in neonatal outcome between the two groups. Conclusion: Oral nifedipine should be a suitable alternative to intravenous magnesium sulfate in suppression preterm labor with fewer side effects

Introduction: Cervical root resorption is one of the most important complications of intra coronal bleaching. A way of preventing this type of resorption is using a coronal barrier under the bleaching materials. The aim of this study was to compare the sealing ability of glass ionomer cement and Pro Root Mineral Trioxide Aggregate (MTA) as a coronal barrier in intra coronal bleaching. Materials &Methods: In this study, 40 single-root maxillary anterior teeth were endodontically prepared and divided into two experimental groups (n= 15) and two positive and negative control groups (n=5). In the experimental groups, gutta percha was removed up to 3 mm below the cemento enamel junction (CEJ).RMGI and MTA were placed over gutta percha up to the level of CEJ. After a 24-hour incubation period, the bleaching agent (a mixture of sodium perborate and 30% hydrogen peroxide) was placed in the access cavities. The bleaching agents were replaced every 3 days over 9 days. Then, the access cavity was filled with 2% methylene blue for 48 hours. All samples were longitudinally sectioned and the dye penetration range was evaluated using a stereomicroscope. Data were statistically analyzed using Kruskal-Wallis and Mann–Whitney tests (α=0.05). Results: Leakage mean indicated that there was a significant difference between these two groups and leakage was less in ProRoot than glass ionomer. Conclusion: It seems that the MTA can provide a better coronal seal during the bleaching

Background: The aim of this study was to investigate the validity, reliability and factor structure of the child and parent's version of psychological inflexibility in pain scale (PIPS) in the population of children with chronic pain and their parents. Methods: The sample consisted of 112 pairs of children and parents, selected through available sampling method from the Tehran Children's Hospitals. The PIPS questionnaire along with KIDScreen scale (to measure well-being), Child Behavior Checklist (CBCL) (the Youth Self-Report (YSR)) (to measure negative mood) and Visual Analogous Scale (VAS) (to measure pain severity) was implemented on them. Results: The reliability results of PIPS showed that the Cronbach's alpha for the child and parent versions was 0.66 and 0.82, respectively. In addition, the rerun correlation of child and parent versions was significant and acceptable, ranging from 0.47 to 0.78. Moreover, the highest correlation between PIPS dimensions in two versions of the child and parent was 0.89 and 0.92, respectively. The convergent and divergent validity of the PIPS showed that this questionnaire had a negative and significant correlation with the KID Screen scale and had a positive and significant correlation with the CBCL, YSR and VAS. Conclusions: The results of the exploratory factor analysis of this questionnaire revealed new factors. The exploratory factor analysis of child version indicated four factors (the factors 1, 3 and 4 are related to avoidance, and factor 2 measures the fusion) and in the confirmation factor analysis, the good fitting of these new factors was confirmed

Background: Ovarian response to gonadotropin stimulation can predict the success rate of Assisted Reproductive Technology. The purpose of this study was not only to determine the ovarian response rate in patients on whom intracytoplasmic sperm injection (ICSI) did not successfully work but also to assess the influence of factors on ovarian response. Methods: In a retrospective study, the medical records of 220 women undergoing their first cycle of ICSI from March 2012 to October 2016 were thoroughly examined. Special attention was paid to ovarian response with regards to age, body mass index, the history of menstrual cycle, the type of infertility, the duration of infertility, and the cause of infertility with respect to measures of women’s hormones. The ovarian response expressing the number of mature MII oocytes after oocyte retrieval was regarded as “poor response” representing a yield of 1–4 oocytes, and “appropriate response” representing a yield of 5 or more oocytes. Results: Out of 220 infertile women, 45 subjects (20.5%) had poor response, 175 subjects (79.5%) had appropriate response to ovulation stimulation with gonadotropin. The results of indicated that lower female age (p=0.005) and lower female FSH (p=0.006) were consistent with appropriate ovarian response. After adjusting the variables, the age (age <30 years) (OR= 2.45, 95% CI= 1.19-5.07) and the female FSH (FSH <30 years) (OR= 2.59, 95% CI= 1.16-5.71) were found to be associated with an appropriate ovarian response. Conclusion: It can be concluded that entering the first cycle of ICSI with FSH test and considering the age of women seem to be acceptable strategies

    No full text
Background: Osteoporosis and osteopenia are common diseases in every population. Type 2 diabetes mellitus (T2DM) can lead to the development of various complications, such as bone disorders especially among elderly individuals. Studies suggested that ectonucleotide pyrophosphatase/phosphodiesterasel (ENPP1) is contributed in insulin resistance and also the inhibition of bone mineralization. In this study, association of K121Q (rs1044498) polymorphism of the ENPP1 gene with T2DM and bone disorders is evaluated. Methods: Four-hundred-and-ninety females who were classified based on bone mineral density (BMD) at lumbar spine and femur were included in this study. In addition, participants were classified according to their diabetes status. K121Q polymorphism was evaluated by the PCR-PFLF technique. One-way ANOVA was used for comparison of various analyzed factors in diseases subgroups and K121Q genotypes. Association of K121Q polymorphism with diabetes and bone disorders was evaluated by logistic regression. Results: Significant association was observed between K121Q polymorphism with osteoporosis and osteopenia (p = 0.041, p = 0.029, respectively), but a similar pattern was not observed in T2DM status (p = 0.723). Moreover, in diabetic patients, K121Q polymorphism showed a better prediction potential for the development of bone disorders in comparison to non-diabetic subjects (p = 0.018; OR = 4.63,p = 0.540; OR = 1.31). There were no significant differences between K121Q genotypes with FBS, Ca, P, vitamin D, PTH and BMD status. Conclusions: The present study implies that K121Q polymorphism of ENPP1 gene is able to modulate the development of bone disorders in T2DM. Therefore in diabetic patients screening of this polymorphism is suggested for the monitoring of these persons
