    Tools to support self-care monitoring at home: Perspectives of patients with heart failure

    Abstract: Self-care monitoring at home can be a challenge for patients with heart failure (HF). Tools that leverage information and communication technology (ICT), comprise medical devices, or have written material may support their efforts at home. The aim of this study was to describe HF patients’ experiences and their prioritization of tools that support, or could support, self-care monitoring at home. A descriptive qualitative design employing semi-structured interviews was used with HF patients living at home and attending an HF outpatient clinic in Norway. We used a deductive analysis approach, using the concept of self-care monitoring with ICT tools, paper-based tools, medical devices, and tools to consult with healthcare professionals (HCPs) as the categorization matrix. Nineteen HF patients with a mean age of 64 years participated. ICT tools are used by individual participants to identify changes in their HF symptoms, but are not available by healthcare services. Paper-based tools, medical devices, and face-to-face consultation with healthcare professionals are traditional tools that are available and used by individual participants. HF patients use traditional and ICT tools to support recognizing, identifying, and responding to HF symptoms at home, suggesting that they could be used if they are available and supplemented by in-person consultation with HCPs.publishedVersio

    I need to be who I am : a qualitative interview study exploring the needs of people with dementia in Norway

    Objective: Many people with dementia receive community services. These services are often based on the needs of informal caregivers and professional caregivers’ assessment. User involvement and participation are main objectives in the Norwegian National Dementia Plan 2020. To enhance user involvement and individual tailoring of healthcare services, more information is required about the needs of people with dementia. The aim of this study was to explore the experienced needs of people with dementia in order to facilitate user involvement in provided services. Methods: An explorative cross-sectional study design was used. We performed semistructured interviews with people with dementia who were recruited from all regions of Norway. The sample comprised 35 participants diagnosed with dementia. The interviews were audio-recorded and transcribed, and the data material was analysed using a qualitative content analysis. Results: Three main categories emerged from the interviews: (1) to stay connected; (2) to be active and participate; and (3) to live for the moment. The overarching theme was: the need to be who I am. Conclusions: People with dementia participating in the study were heterogeneous regarding wants and requirements. Most of them expressed the need and wish to hold on to who they are. Close and robust relations with family and friends can give significant support to people with dementia. However, living with dementia might put considerable strain on relations. Services should provide support to enhance relationships, encourage existing networks to remain stable and facilitate participation in meaningful activities for people living with dementia.publishedVersio

    Problemstilling: Gjennom praksisperiodene har jeg undret meg på hvorfor det er så vanskelig å få til god dokumentasjon i journalen, og dette er derfor et tema jeg ønsket å lære mer om. Med utgangspunkt i dette temaet ble følgende problemstilling formulert: Hvorfor er det utfordrende å oppnå systematisk og god dokumentasjon av sykepleie i sykehjem? Teoretisk perspektiv: I fagbøker og forskning er det enighet om at sykepleieprosessen skal ligge til grunn for dokumentasjonspraksis. Rapportene skal i hovedsak være evaluering av tiltaksplanen, men undersøkelser viser at rapportene som regel er tilfeldige, kronologiske og lite relatert til tiltaksplanen. I teorien er det i tillegg begrunnet hvorfor standardisering og bruk av kunnskaps- og beslutningsstøtte har en positiv effekt på kvalitet av dokumentasjon. Metode: Dette er en litterær oppgave der eksisterende fag- og forskningslitteratur er brukt til å undersøke problemstillingen. Det er ikke gjort systematisk søk for å finne all litteratur, men fokusert på å finne et utvalg nyere relevante artikler for problemstillingen. Det ble først tatt utgangspunkt i pensum og søkt i ulike databaser, og etter hvert som søkeord ble identifisert ble det gjennomført et systematisk søk i PubMed ved bruk av MeSH-termer og tekst-søk. Drøfting: Funnene i forskningsartiklene kan deles inn i tre hovedområder. 1) Hvordan informasjonspraksis generelt på sykehjemmene påvirker journalføring. 2) Hvordan ytre faktorer som teknologi, arbeidsbelastning, opplæring og ledelse påvirker dokumentasjonspraksis. 3) Hvordan lite bruk av standardisering samt kunnskap- og beslutningsstøtte påvirker. Funnene innenfor disse tre områdene er diskutert opp mot teori og egne erfaringer. Konklusjon: Hovedutfordringen er at den sirkulære tankegangen bak sykepleieprosessen ikke følges opp i dokumentasjon. Det er også for mye bruk av uformelle kanaler. Dette kan skyldes at opplæringen i og fokus på dokumentasjon er ikke god nok i tillegg til at EPJ systemene ikke er optimale. Det brukes for lite standardisering og kunnskapsstøtte

    Problemstilling: Hvilke tiltak kan sykepleiere veilede rusavhengige pasienter om for å forebygge utvikling av hud- og bløtvevsinfeksjoner og kroniske sår. Teoretisk perspektiv: Oppgavens sykepleieteoretiske grunnlag er Orems egenomsorgsmodell og Martinsens sykepleieteori. Metode: Litteraturstudie basert på nyere fagbøker og sju relevante forskningsartikler som ble funnet ved hjelp av usystematiserte og systematiserte søk i PubMed og CINAHL. Drøfting: Sykepleierens rolle er veiledning og undervisning av rusavhengige om mer forsvarlig sprøytebruk. Besvarelsen fokuserer på ulike skadereduserende tiltak som kan iverksettes for å forebygge utvikling av kroniske sår og hud- og bløtvevsinfeksjoner hos rusavhengige. Tiltakene som drøftes i oppgaven er knyttet til hygiene, valg og tilberedning av rusmidler, valg av innstikkssted, injeksjonsteknikk og undervisning om mer generelle risikofaktorer. Konklusjon: Sykepleieren har en viktig veiledende og forebyggende funksjon når det gjelder skadereduksjon. Ved hjelp av tiltakene diskutert i drøftingen kan mange komplikasjoner av sprøytebruk forebygges. Videre forskning er nødvendig for å vurdere effekten av enkelte tiltak, og for å undersøke konsekvenser av øvrige skadelige vaner og ritualer knyttet til injisering av rusmidler

    Kartlegging av ernæringsmessig risiko hos pasienter med kreft

    Problemstilling: Hvilke kartleggingsverktøy bør sykepleier benytte ved vurdering av ernæringsmessig risiko hos pasienter med kreft? Teori: Behandling av kreftsykdom gir store ernæringsmessige konsekvenser for pasientene. Stor utbredelse av komplikasjoner og bivirkninger gjør at sykepleier tidlig må forebygge utvikling av ernæringsmessig risiko og underernæring. Det er utarbeidet en rekke kartleggingsverktøy som skal bidra til å forebygge dette, eksempelvis Nutritional Risk Screening 2002 (NRS 2002) og Scored Patient-Generated Subjective Assessment (PG-SGA). Samtidig er det juridiske og etiske føringer som sykepleier er pålagt å følge. Metode: Vi har benyttet de medisinske databasene PubMed og Cinahl for å søke etter relevant forskning som kan besvare problemstillingen. Følgende søkeord er benyttet i litteratursøket: Nutritional risk screening, Hospitals, Patient-generated subjective global assessment, Adult og Malnutrition. Resultater: NRS 2002 og PG-SGA er kartleggingsverktøy som kan vurdere og fange opp pasienter med kreft i ernæringsmessig risiko. Funn viser at NRS 2002 fanget opp at 30,7% (n=284) av pasientene var i ernæringsmessig risiko og PG-SGA fanget opp at 86,3% (n= 800) av pasientene var underernærte. Fire forskningsartikler er inkludert i oppgaven og funnene indikerer at NRS 2002 og PG-SGA er blant de to kartleggingsverktøyene som er anbefalt i nasjonale og internasjonale studier. Drøfting: Funn fra forskningsartikler blir diskutert på bakgrunn av teori for å besvare oppgavens problemstilling. Sykepleierens forebyggende funksjon i kartlegging av ernæringsmessig risiko, fordeler og ulemper ved NRS 2002 og PG-SGA, tid, kunnskap og interesse. Konklusjon: NRS 2002 og PG-SGA er to kartleggingsverktøy norske helsemyndigheter anbefaler sykepleier å benytte for å kartlegge ernæringsmessig risiko og underernæring hos pasienter med kreft. Likevel viser flere internasjonale studier at det er en uenighet om kartleggingsverktøyene krever for mye tid og kunnskap hos sykepleier

    Problemstilling Hvilken kompetanse bør sykepleieren ha om sykehusmiljø, om omsorgsfull sykepleie og god kommunikasjon, for å kunne bidra til å redusere stress og fremme mestring hos foreldre til premature barn, på en nyfødt intensivavdeling? Teoretisk perspektiv Sykepleierens teoretiske kunnskapsgrunnlag består av kompetanse om det premature barnet, foreldrenes opplevelse av sykehusmiljøet og det å få et prematurt barn, kommunikasjonsteori og Benner og Wrubels sykepleieteorier om omsorg, stress og mestring. Til slutt beskrives sykepleierens veiledende funksjon, i tillegg til relevante juridiske og etiske føringer for sykepleierens yrkesutøvelse. Metode Oppgaven er en litteraturstudie hvor fag- og forskningslitteratur danner grunnlaget for diskusjonsdelen til slutt i oppgaven. Metodekapittelet består av fremgangsmåte for valg av litteratur, søkehistorikk for forskningsartikler, kildekritikk og etiske vurderinger. Drøfting I diskusjonsdelen drøftes presentert teoretisk kunnskapsgrunnlag opp mot forskningsartiklene. Her diskuteres sykehusmiljøets påvirkning på foreldrenes stress og mestringsopplevelse, foreldrenes behov for informasjon og god kommunikasjon, familiebasert omsorg og emosjonell støtte i praksis, og betydningen av sykepleieveiledning for å fremme mestring. Konklusjon Sykepleieren bør ha kompetanse om det premature barnet og foreldrenes opplevelse av det å få et prematurt barn som legges inn på nyfødt intensivavdeling. I tillegg er Benner og Wrubels sykepleieteori om omsorg, stress og mestring, kommunikasjonsteori, sykepleierens veiledningsfunksjon og relevante etiske og juridiske føringer vesentlig å ha kunnskaper om. Sykepleierveiledning er hensiktsmessig for å fremme mestring og redusere stress. Sykehusmiljøet har stor påvirkning på foreldrenes opplevelse. Sykepleieren bør benytte familiebasert omsorg for å fremme familien som en enhet og at foreldrene er barnets viktigste omsorgsgivere. Kommunikasjonen bør være personsentrert.     A new treatment for eating disorders combining physical exercise and dietary therapy (the PED-t): experiences from patients who dropped out

    Purpose: Eating disorders (ED) are complex and severe illnesses where evidence-based treat- ment is needed to recover. However, about half of the patients with ED do not respond to treatments currently available, which call for efforts to expand the portfolio of treatments. The aim of this study was to explore experiences from patients who dropped out of a new treatment for bulimia nervosa and binge ED, combining physical exercise and dietary therapy (PED-t). Methods: We conducted open-ended face-to-face interviews. The interviews were tran- scribed verbatim and the data were analysed with a phenomenological hermeneutical approach. Results: Three themes emerged: “standing on the outside”, “unmet expectations” and “parti- cipation not a waste of time”. Feelings of standing on the outside were elicited by being different from other group members and having challenges with sharing thoughts. Unmet expectations were related to treatment content and intensity, as well as the development of unhealthy thoughts and behaviours. Finally, some positive experiences were voiced. Conclusion: A need to clarify pre-treatment expectations and refining criteria for treatment suitability is indicated. The findings have contributed to the chain of clinical evidence regarding the PED-t and may lead to treatment modifications improving the treatment and thereby reducing drop out.publishedVersio

    When a common language is missing: Nurse–mother communication in the NICU. A qualitative study

    Abstract Aims and objectives: To explore how communication in neonatal intensive care units (NICUs) between immigrant mothers and nurses take place without having a common language, and how these mothers experience their NICU stay. Background: Admission of infants to NICU affects both parents and infants. Immigrant mothers constitute a vulnerable hospital population in need of culturally, linguistically and individually tailored information. Design and methods: The study had a qualitative design reported according to the COREQ criteria. Eight mothers who spoke neither Scandinavian nor English went through individual semi-structured interviews. Six mother–nurse interactions were observed, and eight nurses' experiences were explored through focus-group inter- views. All interviews were audio recorded and transcribed verbatim. The analysis was thematic and hermeneutic in character. Results: Interpreters were present during the consultations with the physicians, but rarely during the daily nurse-mother interactions. Nurses focused on daily routines, infant care guidance and mother–infant attachment. The mothers learned through demonstrations and hands-on guidance. Language barriers made it difficult to assess the mothers' understanding, but the mothers expressed that they felt adequately in- cluded in the care of their infant and well informed and guided. Even so, both mothers and nurses expressed desire to use interpreters more regularly. The pictorial com- munication boards available lacked important vocabulary needed in neonatal nursing contexts and their use furthermore interrupted the mother–nurse conversation. Conclusion: Body language, simple words, guesswork, trial and error characterised the nurse–mother interaction. The nurses adopted various communication strategies to help the mothers understand and give them a voice. Competent interpreters were used during meetings with physicians, but not during daily bedside guidance and in- formation giving by nurses. Relevance to clinical practice: Knowledge of immigrant mothers' and nurses' communication strategies and how both parties think, feel and act to overcome communication problem is necessary to improve clinical practice and reduce com- munication barriers.publishedVersio

    Available, but not always accessible: A nationwide, qualitative study of multidisciplinary healthcare providers’ experiences with follow-up care after paediatric brain tumour

    Objective Paediatric brain tumour (PBT) survivors face high risks of disabling long‐term and late effects. Whether survivors’ needs are met in a system with publicly funded services, but in the absence of a formal long‐term follow‐up model, is uncertain. Empirically based recommendations for a national model are needed. We explored multidisciplinary healthcare providers’ (HCP) experiences with providing such care. Methods We conducted five focus‐group interviews and five individual interviews with a nationally representative sample of 33 Norwegian HCPs. Focus‐group interviews and individual interviews were analysed using systematic text condensation. Results Three main themes were identified: (a) ‘Providing care above and beyond system constraints’, describing a perceived discrepancy between HCPs’ knowledge of, and their ability to meet, the survivors’ needs. (b) ‘System barriers to providing optimal follow‐up care’, describing a perceived lack of routines for communication and coordination between the HCPs and existing care services. (c) ‘Nurses and shared‐care to improve care’, including empowering nurses and establishing routines for collaborations and areas of responsibilities. Conclusion The current healthcare system was perceived not to fully meet the survivors’ needs. Nurse‐led care models, including standardised patient‐care pathways, were suggested to increase the accessibility of already‐existing services and thus to improve long‐term follow‐up care.submittedVersio
