    Reduksjon av kateterassoisert urinveisinfeksjon på norske sykehus

    Problemstilling Hvordan kan etterlevelsen av kunnskapsbaserte retningslinjer for reduksjon av kateterassosiert UVI på sykehus forbedres? Teoretisk perspektiv I teoridelen blir det teoretiske grunnlaget for oppgaven presentert. Her belyses temaer som er nødvendig å ha en forståelse av for å kunne besvare problemstillingen på en god måte. Blant annet beskrives temaer som helsetjenesteassosierte infeksjoner, urinveisinfeksjon, kunnskapsbaserte retningslinjer, kvalitetsforbedring samt relevant lovverk og etikk. Metode Oppgavens metode er av typen Litterær oppgave. Dette innebærer at det blir gjort et utvalg av relevant fag- og forskningslitteratur hvor informasjonen som kommer frem, deretter blir gjennomgått og sammenfattet for å belyse og besvare problemstillingen. Oppgaven presenterer funnene fra 4 kvantitative oppgaver hvorav en er en systematisk kunnskapsoppsummering. Drøfting De fleste barrierene for etterlevelse av kunnskapsbaserte retningslinjer passer inn under kategoriene miljømessig kontekst og ressurser, kunnskap og forståelse av konsekvenser, samt sosial påvirkning. Tiltak som rettes mot å overkomme disse barrierene diskuteres i lys av etiske retningslinjer, lovverk, og fag- og forskningslitteratur. Lederens rolle i kvalitetsarbeidet dras også inn, og sykepleierens etiske og juridiske ansvar for å forbedre egen og avdelings praksis i forbindelse med bruk av KAD. Konklusjon Tiltak med fokus på ansvarliggjøring, og forbedring av kulturen på arbeidsplassen rundt bruk av KAD kan bidra til å forbedre sykepleierens etterlevelse av kunnskapsbaserte retningslinjer for reduksjon av kateterassosiert urinveisinfeksjon. I tillegg har barrierer for etterlevelse av retningslinjene blitt identifisert og tiltak som rettes mot å overkomme disse bør testes for å kunne forløse uutnyttet potensiale i forbedringen av etterlevelsen til disse

    The gap between expectations and reality: A qualitative study of psychosocial challenges of young childhood cancer survivors from the PACCS study

    Objective: In this sub-study from the ‘PACCS’ study, we explored the psychosocial experiences of children and adolescents in everyday life post-cancer treatment and the possible factors that can moderate these experiences. Methods: This is a qualitative explorative study using semi-structured interviews with 43 childhood cancer survivors between the ages of nine and 18 from Norway and Denmark. We conducted a secondary thematic analysis using Malterud's systematic text condensation. Results: Two main themes were identified: ‘The post-treatment gap between expectations and reality’ comprised two subthemes: (1) lack of mastery and feeling different and (2) lack of understanding and acceptance. The second main theme, ‘Managing the gap’, comprised three subthemes: (1) information and knowledge, (2) adjustments and adaptions and (3) social support and openness. The findings reveal that the psychosocial challenges resulted from the remaining ability gap(s). Measures such as tailored information, school adjustments and social support were potential dynamic factors affecting the gap(s) positively or negatively. Psychosocial challenges post-treatment are experiences of lack of acceptance and understanding from others. Conclusion: To safeguard a positive transition back to everyday life, health care providers should support the survivors' psychosocial care, including getting back to school and re-entering social interactions.publishedVersio

    Technology-Supported Guidance Models Stimulating the Development of Critical Thinking in Clinical Practice: Mixed Methods Systematic Review

    Background: Nursing education has increasingly focused on critical thinking among nursing students, as critical thinking is a desired outcome of nursing education. Particular attention is given to the potential of technological tools in guiding nursing students to stimulate the development of critical thinking; however, the general landscape, facilitators, and challenges of these guidance models remain unexplored, and no previous mixed methods systematic review on the subject has been identified. Objective: This study aims to synthesize existing evidence on technology-supported guidance models used in nursing education to stimulate the development of critical thinking in nursing students in clinical practice. Methods: This mixed methods systematic review adopted a convergent, integrated design to facilitate thematic synthesis. This study followed the guidelines of the Joanna Briggs Institute Manual for Evidence Synthesis. Results: We identified 3 analytical themes: learning processes implemented to stimulate critical thinking, organization of the learning process to stimulate critical thinking, and factors influencing the perception of the learning process. We also identified 4 guidance models, all based on facilitator or preceptorship models using tailored instructional or learning strategies and one or several technological tools that were either generic or custom-made for specific outcomes. The main facilitators of these technology-supported guidance models were nurse educators or nurse preceptors, and the main challenges in using technology-supported guidance models were the stress associated with technical difficulties or increased cognitive load. Conclusions: Although we were able to identify 4 technology-supported guidance models, our results indicate a research gap regarding the use of these models in nursing education, with the specific aim of stimulating the development of critical thinking. Both nurse preceptors and nurse educators play a crucial role in the development of critical thinking among nursing students, and technology is essential for such development. However, technology-supported guidance models should be supervised to mitigate the associated stress.publishedVersio

    Problemstilling Hvilke ikke-medikamentelle metoder kan sykepleier anvende for å lindre smerte hos barn mellom 6-12 år ved nålrelaterte prosedyrer på sykehus? Teoretisk perspektiv Oppgaven tar for seg teori om barnets smertefysiologi og smertevurdering, barn på sykehus og prosedyrer på sykehus. Videre er det innhentet teori om avledning og distraksjon som ikke-medikamentell metode, barnets kognitive og psykososiale utvikling, da også teori om teoretikeren Jean Piaget, lovverk og ulike sykepleiefaglige rammer. Metode Oppgaven er en litterær oppgave som baserer seg på allerede eksisterende forskning og faglitteratur. Det er anvendt både pensumlitteratur, selvvalgt litteratur, forskning og lovverk. De fire inkluderte hovedartiklene er funnet i CINHAL med søkeord som er relevante for problemstillingen. Drøfting Det ses på hvordan avledning og distraksjon er fungerende i forhold til barnets kognitive funksjon og alder, samt hvilke ikke-medikamentelle metoder sykepleier kan anvende for å best lindre barnets smerte. Allerede presentert forskning og teori ses i lys av hverandre, og problemstillingen skal forsøkes besvart. Konklusjon Sykepleier kan anvende distraksjonskort som en avledningsteknikk for på best mulig måte lindre barnets smerte. Både musikk og videospill som metode blir for passive teknikker for barnet, da de ikke får ta en like stor deltagende del som ved bruk av distraksjonskort. Ytterligere forskning som tar for seg både distraksjonskort, musikk og videospill i samme studie er nødvendig for å sammenligne deres smertelindrende effekt

    Integrating simulation training during clinical practice in nursing homes: An experimental study of nursing students´ knowledge acquisition, self-efficacy and learning needs

    Background Limited access to supervision, feedback and quality learning experiences pose challenges to learning in the clinical setting for first-year nursing students who are beginning their clinical experiences. Prior studies have indicated that simulation training, as a partial replacement of clinical practice hours, may improve learning. However, there has been little research on simulation training integrated as a partial replacement during first-year students’ clinical practice in nursing homes. The primary aim of this study was to examine first-year nursing students’ knowledge acquisition and self-efficacy in integrating a partial replacement of clinical hours in nursing homes with simulation training. Its secondary aim was to examine perceptions of how learning needs were met in the simulated environment compared with the clinical environment. Design The primary aim was addressed using an experimental design that included pre- and post-tests. The secondary aim was investigated using a descriptive survey-based comparison. Methods First-year students at a Norwegian university college (n = 116) were asked to participate. Those who agreed (n = 103) were randomly assigned to the intervention group (n = 52) or the control group (n = 51). A knowledge test, the General Self-efficacy Scale and the Clinical Learning Environment Comparison Survey were used to measure students’ outcomes and perceptions. The data were analysed using independent samples t-tests, chi-square tests and paired samples t-tests. Results Knowledge scores from pre- to post-tests were significantly higher in the intervention group than in the control group with a medium to large effect size (p < 0.01, Hedges’ g = 0.6). No significant differences in self-efficacy were identified. Significant differences (p <  0.05) were observed between the simulated and the clinical environment with regard to meeting learning needs; effect sizes ranged from small and medium to large (Cohen’s d from 0.3 to 1.0). Conclusion Integrating the partial replacement of clinical hours in nursing homes with simulation training for first-year nursing students was positively associated with knowledge acquisition and meeting learning needs. These findings are promising with regard to simulation as a viable partial replacement for traditional clinical practice in nursing homes to improve learning.publishedVersio

    Problemstilling Hvilke barrierer opplever sykepleiere for å rapportere om feil knyttet til legemiddelhåndtering? Teoretisk perspektiv Sykepleiers ansvar om å drive kvalitetsarbeid danner grunnlaget for oppgaven, der avviksrapportering vektlegges. Teoretisk kunnskap om pasientsikkerhet, organisasjonskultur, aktuelle lovverk og etikk inkluderes. Det belyses også hvordan ledelsen på sykehus er av relevans. Metode Bacheloroppgaven er en litterær oppgave basert på fag- og forskningslitteratur. Det er hovedsakelig anvendt faglitteratur fra Nasjonalbibliotekets nettbibliotek og pensumlitteratur, samt forskningsartikler fra søk i databasene Idunn og Cinahl. Det anvendes i tillegg rapporter, retningslinjer og forskrifter hentet fra internett. Drøfting Problemstillingen drøftes basert på forskning om hva sykepleiere opplever som barrierer for å rapportere feil i legemiddelhåndtering. Barrierene som fremheves drøftes opp mot tidspress, samt sykepleiers pliktige og etiske ansvar om å drive kvalitetsarbeid. I tillegg sees barrierene i lys av konsekvensene de har for pasientsikkerheten. Konklusjon Sykepleieres opplevde barrierer for å rapportere feil i legemiddelhåndtering kan deles inn i følgende; ledelsens håndtering av avvik, kulturen på sykehuset og rapporteringsprosessen

    Problemstilling: Hvilke utfordringer eksisterer ved identifisering av sepsis hos eldre på akuttmottak? Teoretisk perspektiv: Det teoretiske kunnskapsgrunnlaget tar for seg patofysiologien til sepsis, sykepleieres funksjon og hva slags observasjonskompetanse som kreves for å kunne identifisere sepsis. Videre blir de eldres aldersfysiologiske forandringer beskrevet, før vi tar for oss aktuelle kartleggingsverktøy. Relevant etikk og lovverk blir presentert sammen med modellen for kunnskapsbasert praksis. Metode: I denne bacheloroppgaven har vi brukt litterær oppgave som metode. Relevant pensum, fag- og forskningskunnskap er anvendt i besvarelsen. Artikkelsøk ble gjort i CINAHL og PubMed. Det er benyttet fem ulike forskningsartikler i denne oppgaven. Drøfting: Drøftingen belyser problemstillingen ved å ta for seg fire utfordringer som vi identifiserte som hovedfunn fra artiklene. Utfordringene vi kom frem til var at eldre har atypiske tegn og uspesifikke symptomer, kartleggingsverktøy er for dårlig egnet for den eldre pasientgruppen, sykepleiere har manglende kunnskap om sepsis og mangel på tid hos pasientene. Modellen for kunnskapsbasert praksis (KBP) brukes for å drøfte hvorfor det er viktig at sykepleiere har kunnskap og erfaring i det de driver med. KBP støtter også opp viktigheten med forskningsbasert praksis. Konklusjon: Sykepleiere har en viktig rolle i å identifisere sepsis, men det er flere utfordringer som kan være til hinder for at det oppdages tidlig nok. Atypiske symptomer og tegn kan være vanskelig å oppdage, og forhindrer at sepsis oppdages. Kartleggingsverktøyene som brukes mest er ikke godt nok tilpasset eldre pasienter, og er et problem ettersom pasienter med en potensiell sepsis kan bli oversett. Ved bruk av forskningsbasert kunnskap kan sykepleiere oppdatere sin kunnskap om sepsis hos eldre. Samtidig kan tidspresset sykepleiere føler på, i et akuttmottak, gå utover identifiseringen av sepsis

    Problemstilling Hvilke former for behandling kan fremme tilheling av kroniske venøse leggsår i hjemmesykepleien? Teoretisk perspektiv Teoretisk grunnlag tar utgangspunkt i kunnskap om venøs dysfunksjon, sårtilhelingsprosessen og eldre pasienter, samt lokal sårbehandling. Verktøyet TIMES blir presentert i tilknytning til observasjon og innsamling av data. Sykepleiers behandlende funksjon, relevant lovverk og etikk blir gjort rede for. Også hjemmesykepleiens relevans blir belyst. Metode Problemstillingen er besvart som en litterær oppgave. Det er blitt benyttet fag- og forskningslitteratur, retningslinjer og lovverk, samt egne erfaringer for å besvare problemstillingen. Faglitteraturen er hentet fra Lovisenberg diakonale høgskole sin tidligere og nåværende pensumlitteratur, samt skolens bibliotek. Forskningslitteraturen er hentet ut fra databasene CINAHL og PubMed. Drøfting Besvarelsen diskuterer funn i forskningen med relevans for problemstillingen. Teori er benyttet supplerende. TIMES sin relevans til observasjon og innsamling av data blir diskutert. Først drøftes debridering som form for behandling, så belyses behandlingsformen kompresjon og sykepleiers rolle i etterlevelse av behandlingen. Egne kliniske erfaringer blir også trukket med i drøftingen. Konklusjon TIMES synes å være et strukturert verktøy i vurderingen av kroniske venøse sår. Verktøyet kan fungere som en støtte for hjemmesykepleier med å ta avgjørelser ved valg av behandling. Debridering og kompresjon kan fremme tilheling av kroniske venøse sår. Den ene formen for behandling utelukker ikke nødvendigvis den andre. Mekanisk debridering fører til reduksjon av bakterier i såret. Kompresjon er forholdsvist skånsomt for pasienten, men stiller krav til god etterlevelse over lengre tid. Endelig må hjemmesykepleier unngå rutinemessig bruk av behandlingsformer, men vurdere hver enkelt pasient

    Preferences for home care to enable home death among adult patients with cancer in late palliative phase - a grounded theory study

    Background: The wish to be cared for and to die at home is common among people with end-stage cancer in the western world. However, home deaths are declining in many countries. The aim of this study was to explore the preferences for home care over time to enable home death among adult patients with cancer in the late palliative phase. Methods: A qualitative method was applied according to grounded theory (Corbin & Strauss, 2008). Data was collected using individual interviews (n = 15) with nine adult patients. One to two follow up interviews were conducted with four patients. Sampling, data collection and constant comparative analysis were undertaken simultaneously. Results: The findings are presented as a conceptual model of patients’ preferences for care to enable home death. The core category “Hope and trust to get the care I need to die at home” showed that the preference to die at home seemed stable over time and did not change with deterioration in health status and progression in illness. Five categories were related to the core category. The categories “being in the present”, “be safe and in charge” and “be seen and acknowledged” describe the patients’ preferences to live a meaningful life until death and be the same person as always. These preferences depended on the categories describing characteristics of healthcare personnel and the organisation of care: “reliable, compassionate and competent healthcare personnel” and “timely, predictive, continuous and adaptive organisation”. Conclusion: An important preference over time was to be here and now and to live as meaningful a life as possible until death. Moreover, the patients preferred to retain control over their lives, to be autonomous and to be seen as the person they had always been. To achieve this, person-centred care provided by healthcare personnel with competence, skills and enough/ample time were required. In addition, home care needed to be organised in a way that ensured continuity and predictability. Systematic implementation of a person-centred care model and the use of advanced home care plans with continued re-evaluation for patients’ preferences of home care were proposed measures to enable home death.publishedVersio

    The Parent Health Literacy Questionnaire (HLQ-Parent). Adaptation and validity testing with parents of children with epilepsy

    Aims: The aim of this study was to adapt the Health Literacy Questionnaire (HLQ) in English and Norwegian for use with parents. Methods: The research group evaluated all HLQ items and, where relevant, modified them to refocus the attribution of care to that of a child by a parent. Five cognitive interviews with parents were undertaken to gain a detailed depiction of the meanings and processes they used to respond to the HLQ items. Assessment of the psychometric properties of the revised HLQ was undertaken using data from a cross-sectional survey of 254 parents of children with epilepsy. Analysis included internal consistency (Cronbach’s alpha) and confirmatory factor analysis (CFA). Results: Some 22 out of 44 items and the names of three domains were modified (e.g. attribution changed from ‘me’ to ‘my child’). Cognitive interviews indicated that parents interpreted the HLQ-Parent items in the way intended. All but three factor loadings were high to acceptable. All nine HLQ scales showed satisfactory to good internal consistency (Cronbach’s alpha 0.70 to 0.87). When fitting one-factor CFA models, correlated residuals were required for four scales to generate an acceptable fit. One scale, ‘8. Ability to find good health information’, required inclusion of two correlated residuals to generate an acceptable CFA model fit, indicating that further work on this scale is warranted. Conclusions: The results from both the adaptation process and the CFA analysis supported the relevance, understanding and theoretical structure of the instrument in a parental context.publishedVersio
