A Technology-Supported Guidance Model to Support the Development of Critical Thinking among Undergraduate Nursing Students in Clinical Practice: Concurrent, Exploratory, Flexible, and Multimethod Feasibility Study
Background: There is widespread recognition and acceptance of the need for critical thinking in nursing education, as it is necessary to provide high-quality nursing. The Technology-Supported Guidance Model (TSGM) intervention was conducted during clinical practice among undergraduate nursing students and aimed to support the development of critical thinking. A major element of this newly developed intervention is an app, Technology-Optimized Practice Process in Nursing (TOPP‑N), combined with the daily guidance of nursing students from nurse preceptors and summative assessments based on the Assessment of Clinical Education.
Objective: The main objective of this study was to assess the feasibility of a newly developed intervention, TSGM, among undergraduate nursing students, nurse preceptors, and nurse educators. Further objectives were to assess the primary and secondary outcome measures, recruitment strategy, and data collection strategy and to identify the potential causes of dropout and barriers to participant recruitment, retention, intervention fidelity, and adherence to the intervention.
Methods: This study was designed as a concurrent, exploratory, flexible, and multimethod feasibility study of the TSGM intervention that included quantitative and qualitative data from nursing students, nurse preceptors, and nurse educators. The primary outcome measures were the feasibility and acceptability of the intervention. The secondary outcomes included the suitability and acceptance of the outcome measures (critical thinking, self-efficacy, clinical learning environment, metacognition and self-regulation, technology acceptance, and competence of mentors); data collection strategy; recruitment strategy; challenges related to dropouts; and hindrances to recruitment, retention, and intervention fidelity and adherence.
Results: Nursing students, nurse preceptors, and nurse educators had varied experiences with the TSGM intervention. We identified factors that make the intervention feasible and challenging and may influence the feasibility, acceptability, dropout rate, adherence, and fidelity of the intervention. We also identified areas for future improvement of the intervention.
Conclusions: The use of a newly developed intervention, TSGM, is feasible and accepted by undergraduate nursing students, nurse preceptors, and nurse educators; however, refinement and improvement of the intervention and the TOPP‑N app, improvement in intervention management, and mitigation of negative factors are necessary before a randomized controlled trial can be performed.publishedVersio
eHealth literacy among hospital health care providers: a systematic review
Background: eHealth literacy is a key concept in the implementation of eHealth resources. However, most eHealth literacy definitions and frameworks are designed from the perceptive of the individual receiving eHealth care, which do not include health care providers’ eHealth literacy or acceptance of delivering eHealth resources.
Aims: To identify existing research on eHealth literacy domains and measurements and identify eHealth literacy scores and associated factors among hospital health care providers.
Methods: This systematic review was reported in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 checklist. A systematic literature search was conducted in MEDLINE, Cinahl, Embase, Scopus, PEDro, AMED and Web of Science. Quantitative studies assessing eHealth literacy with original research, targeting hospital health care providers were included. Three eHealth literacy domains based on the eHealth literacy framework were defined a priori; (1) Individual eHealth literacy, (2) Interaction with the eHealth system, and (3) Access to the system. Pairs of authors independently assessed eligibility, appraised methodological quality and extracted data.
Results: Fourteen publications, of which twelve publications were conducted in non-Western countries, were included. In total, 3,666 health care providers within eleven different professions were included, with nurses being the largest group. Nine of the included studies used the eHealth literacy scale (eHEALS) to measure eHealth literacy, representing the domain of individual eHealth literacy. A minority of the studies covered domains such as interaction with the eHealth system and access to the system. The mean eHEALS score in the studies ranged from 27.8 to 31.7 (8–40), indicating a higher eHealth literacy. One study reported desirable eHealth literacy based on the Digital Health Literacy Instrument. Another study reported a relatively high score on the Staff eHealth literacy questionnaire. eHealth literacy was associated with socio-demographic factors, experience of technology, health behaviour and work-related factors.
Conclusions: Health care providers have good individual eHealth literacy. However, more research is needed on the eHealth literacy domains dependent on interaction with the eHealth system and access to the system. Furthermore, most studies were conducted in Eastern and Central-Africa, and more research is thus needed in a Western context.publishedVersio
The Value of Nursing Competence and Staffing for Patients in Critical Care: A Scoping Review
Background: Nurse competence and staffing in critical care are of interest to hospital managers and politicians, especially if a relationship exists between educated nurses and patient outcomes. Observational data can describe and measure the impact of critical care nursing practices in intensive care units and may be linked to outcomes.
Objective: The aim of this study was to provide an understanding of how levels of nursing competence and staffing in intensive care units are described and associated with nurse-sensitive outcomes.
Methods: A scoping review was conducted. Electronic databases, including CINAHL, Medline, Embase, the British Nursing Index, and SweMed+, were searched for English language studies from 2006 to 2023. The inclusion criteria were nurses working in critical care, nurse staffing ratio or assessment, and nurse-sensitive outcomes for intensive care patients.
Results: Twenty-one studies were included. As only few studies mentioned the educational level of nurses and the studies used several job titles, it was difficult to interpret nursing competence. Studies that used valid tools for workloads often reported significant results of nursing staffing related to nurse-sensitive outcomes. Adverse events increased with higher levels of nurse workload, and some events were also affected by the level of competence. Lower mortality rate was linked to more nursing hours per patient.
Conclusion: We found associations between education, nurse staffing, and nurse-sensitive outcomes in intensive and critical care units. To highlight its effects, advanced nursing competence in critical care and research must be better described. Patients’ need for nursing competence must be assessed using validated tools.

Sykepleiers kommunikasjonserfaringer med etniske minoritetspasienter
Problemstilling: Hvilke kommunikasjonserfaringer har sykepleiere i behandlingen av etniske minoritetspasienter?
Teoretisk perspektiv: Denne bacheloroppgaven tar for seg fenomenet personsentrert sykepleie underbygget av rammeverket Personsentrert Praksis. Dette rammeverket tar for seg alle aspekter som påvirker behandlingssituasjonen med et personsentrert fokus.
Metode: Dette er en litterær oppgave som tar for seg fire forskningsartikler, samt faglitteratur, for å besvare problemstillingen. Det er foretatt søk i CINAHL Complete og PubMed med følgende søkeord: «Nursing Care+», «Communication Barriers+», «Transcultural Nursing»,
Drøfting: Diskusjonen tar for seg faglig relevant litteratur og artikler for å belyse sykepleiers kommunikasjonserfaringer i behandlingen av etniske minoritetspasienter, i lys av rammeverket Personsentrert praksis. Hensikten med arbeidet er å frembringe kunnskap om hvordan best ivareta det personsentrerte perspektivet på tvers av språk- og kulturbarrierer.
Konklusjon: Sykepleiere har flere kommunikasjonserfaringer, som språkbarrierer, bruk av tolk og kulturell kompetanse i behandlingen av etniske minoritetspasienter. De bruker en rekke kreative strategier for å overkomme kommunikasjonsbarrierer i behandlingssituasjonen, dette kan være alt fra non-verbale gester og berøring til formelle og uformelle tolker. Denne bacheloroppgaven fremhever viktigheten av kulturell bevissthet og kompetanse hos sykepleiere, samt etableringen av system og effektive rutiner for anskaffelsen av formelle tolketjenester
Trust in healthcare before and during the COVID-19 pandemic
Background: Public trust is often advantageous for health authorities during crises such as the COVID-19 pandemic. Norwegian health authorities used the public´s high trust to control the pandemic, resulting in relatively few casualties.
Methods: We wanted to describe and compare the Norwegian public trust in GPs, public healthcare, information and treatment in hospitals before and during the early phases of the COVID-19 pandemic. Further, we wanted to investigate the relationship between somatic or mental illness, and trust in GPs and public health information, and to develop a theoretical understanding of the relationship between trust in healthcare institutions, generalised trust and the societal situation caused by the COVID-19 pandemic. We performed two surveys, the first in December 2019; the second in May 2020, thus providing two snapshots of the Norwegian public’s trust in healthcare and healthcare actors before and during the COVID-19 pandemic.
Results: There was statistically significant increased trust in public healthcare, in treatment at hospital and in information at hospital after the outbreak of the COVID-19 pandemic. There was a non-significant rise in trust in GPs. We found that trust in public health information was not related to mental health nor having a chronic, somatic disease.
Conclusion: The findings confirm that the Norwegian public’s trust in healthcare and healthcare actors is high. The trust levels are also relatively stable, and even show an increase during the early phases of the pandemic. We suggest that there is a dynamic relationship between trust in public health information, healthcare institutions, generalised trust and a societal crisis situation such as the COVID-19 pandemic. However, the GP-patient trust seems less affected by a crisis situation, than the public´s trust in healthcare institutions. This difference may be explained by the relative stability caused by mandates of trust obtained from the patient.publishedVersio
Advantages and Challenges of Using Telehealth for Home-Based Palliative Care: A Systematic Mixed Studies Review.
Background: Owing to the increasing number of people with palliative care needs and the current shortage of health care professionals (HCPs), providing quality palliative care has become challenging. Telehealth could enable patients to spend as much time as possible at home. However, no previous systematic mixed studies reviews have synthesized evidence on patients’ experiences of the advantages and challenges of telehealth in home-based palliative care.
Objective: In this systematic mixed studies review, we aimed to critically appraise and synthesize the findings from studies that investigated patients’ use of telehealth in home-based palliative care, focusing on the advantages and challenges experienced by patients.
Methods: This is a systematic mixed studies review with a convergent design. The review is reported according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) statement. A systematic search was performed in the following databases: Allied and Complementary Medicine Database, CINAHL, Cochrane Central Register of Controlled Trials, Embase, Latin American and Caribbean Health Sciences Literature, MEDLINE, PsycInfo, and Web of Science. The inclusion criteria were as follows: studies using quantitative, qualitative, or mixed methods; studies that investigated the experience of using telehealth with follow-up from HCPs of home-based patients aged ≥18; studies published between January 2010 and June 2022; and studies published in Norwegian, Danish, Swedish, English, Portuguese, or Spanish in peer-reviewed journals. Five pairs of authors independently assessed eligibility of the studies, appraised methodological quality, and extracted data. The data were synthesized using thematic synthesis.
Results: This systematic mixed studies review included 41 reports from 40 studies. The following 4 analytical themes were synthesized: potential for a support system and self-governance at home; visibility supports interpersonal relationships and a joint understanding of care needs; optimized information flow facilitates tailoring of remote caring practices; and technology, relationships, and complexity as perpetual obstacles in telehealth.
Conclusions: The advantages of telehealth were that patients experience a potential support system that could enable them to remain at home, and the visual features of telehealth enable them to build interpersonal relationships with HCPs over time. Self-reporting provides HCPs with information about symptoms and circumstances that facilitates tailoring care to specific patients. Challenges with the use of telehealth were related to barriers to technology use and inflexible reporting of complex and fluctuating symptoms and circumstances using electronic questionnaires. Few studies have included the self-reporting of existential or spiritual concerns, emotions, and well-being. Some patients perceived telehealth as intrusive and a threat to their privacy at home. To optimize the advantages and minimize the challenges with the use of telehealth in home-based palliative care, future research should include users in the design and development process.publishedVersio
Antisipatorisk kvalme: Forebyggende faktorer i klinisk praksis
Innledning: Oppgaven tematiserer faktorer som har betydning i forebyggingen av antisipatorisk kvalme (AK) hos pasienter som gjennomgår cytostatikabehandling på sykehus. Cytostatikabehandling er et sentralt aspekt av kreftbehandling, og kvalme er en av de vanligste bivirkningene av cytostatika. En av de vanligste formene for kvalme i cytostatikabehandling er antisipatorisk kvalme; et subjektivt og psykologisk betinget pasientfenomen som er vanskelig å behandle. Antisipatorisk kvalme kan oppleves så plagsomt og ubehagelig at det regelmessig medfører utsettelse eller avbrytelse av cytostatikabehandlingen. Oppgavens problemstilling er: Hvilke faktorer har betydning for forebygging av cytostatikaindusert antisipatorisk kvalme?
Metode: Oppgaven er et generell litteraturstudie og baserer seg derfor på eksisterende litteratur og forskning, samt aktuelt lovverk og nasjonale retningslinjer for sykepleie. Det ble gjort et strukturert litteratursøk i CINAHL med søkeordene "Nursing", "Prevention", "Anticipatory nausea" og "CINV".
Resultat: Oppgaven tar i bruk fire forskningsartikler for å besvare problemstillingen. Artiklene avdekker flere faktorer med betydning i sykepleiers forebygging av AK: Sykepleiers forståelse av AK, informasjon, personsentrert sykepleie og pasientens opplevelse og forståelse av AK.
Diskusjon: Faktorene med betydning i sykepleiers forebygging av AK som ble avdekket i de aktuelle artiklene diskuteres i lys av eksisterende teori, aktuelt lovverk og etiske aspekter. Sykepleiers forståelse for fenomenet legger grunnlag for sykepleiers forebyggende funksjon. Pasientenes opplevelse og forståelse av eget behandlingsforløp legger grunnlag for deres forventning av AK, noe som er en sentral faktor i forebygging av AK. Informasjon fra sykepleier til pasient spiller også en sentral faktor i forebyggingen av AK, da det underbygger pasientens individuelle forståelse av egne behandlingsforløp. Personsentrert sykepleie utgjør en sentral faktor i forebyggingen av AK, da individualisering er sentralt i forebyggende behandling. Styrker og svakheter ved egen metode er diskutert; dette innebærer vurdering av litteraturstudie som metode, søkeprosessen, inkludering av artikler og artiklene i seg selv
Foreldres erfaringer med kengurumetoden
Innledning: Temaet for oppgaven er foreldres erfaringer med utøvelsen av kengurumetoden (hud-mot-hud kontakt). Premature barn blir født før de er ferdig utviklet, og utgjøre en sårbar pasientgruppe. Barnas foreldre har også behov for oppfølging. Temaet er relevant for sykepleie, da kunnskap om erfaringer bidrar til at sykepleiere kan utøve individuelt tilpasset og personsentrert sykepleie. Problemstillingen for oppgaven er: “Hvilke erfaringer har foreldre med å utøve kengurumetoden på premature barn, som er innlagt på nyfødtintensiv avdeling?”
Metode: I denne oppgaven er litteraturstudie brukt som metode, med utgangspunkt i kvalitative forskningsartikler. Databasen Cinahl ble brukt for å finne artikler relevant for problemstillingen. Følgende søkeord ble brukt: Mothers OR Fathers OR Parents AND Kangaroo care OR Skin to skin AND Experiences OR Postpartum experiences AND Infant, premature OR Preterm birth. Det resulterte i 32 artikler, hvor 5 artikler ble inkludert.
Resultat: Hovedfunnene fra studiene var at deltakerne hadde flere ulike erfaringer med bruk av kengurumetoden. Positive erfaringer som ble trukket frem var forsterket foreldrefølelse, positiv effekt på amming, samt praktisk/enkel metode. Utfordringer var mangel på utstyr og skjerming, tid til å ivareta sine behov, ønsker ble ikke hørt, samt varierende holdninger, erfaringer og kompetanse blant sykepleierne.
Diskusjon: Deltakernes erfaringer diskuteres i lys av faglitteratur, etikk, lover og forskrifter. De hadde flere ulike erfaringer ved utøvelsen av kengurumetoden. Til tross for enkelte utfordringer, bidro metoden til at foreldrene følte seg som viktige omsorgspersoner for sitt premature barn. Flere deltakere uttrykte at sykepleierne hadde en viktig assisterende rolle. Tilstrekkelig informasjon bidro til økt trygghetsfølelse. Basert på resultatene vurderer vi det som viktig at sykepleiere får kunnskap og forståelse for foreldres perspektiv og erfaringer ved bruk av KM. Mer forskning på temaet hadde vært ønskelig for å kunne undersøke og belyse flere erfaringer
Ivaretakelse av kvinner som gjennomfører provosert abort
Problemstilling: Hvilken betydning har personsentrert sykepleie for kvinner som gjennomfører provosert abort på sykehus?
Teoretisk perspektiv: I det teoretiske kunnskapsgrunnlaget presenteres teoretisk kunnskap om provosert abort på sykehus, personsentrert sykepleie som et rammeverk, sykepleierens funksjon og rolle i arbeidet, hva god kommunikasjon innebærer, samt om sykepleiefaglige rammer. Det teoretiske kunnskapsgrunnlaget vil ta utgangspunkt i pasientperspektivet.
Metode: Oppgaven er utført gjennom metoden generell litteraturstudie. Det er gjort et strukturert søk i databasen CINAHL. Litteratursøket besto av søkeordene «Abortion, Induced» AND «needs» AND [«Scandinavia» OR «Europe»]. Søket ga et smalt treff med 37 artikler totalt, hvorav fem artikler ble inkludert i litteraturstudien.
Drøfting: Det teoretiske kunnskapsgrunnlaget drøftes opp mot resultatene fra de inkluderte forskningsartiklene. Hovedelementene i rammeverket personsentrert sykepleie, forutsetninger, omgivelser, prosess og utbytte, er styrende for resultatdiskusjonen. Funn knyttet til kvinnenes behov for bedre forberedende informasjon, betydningen av relasjonen mellom pleier og pasient og god kommunikasjon er et utdrag av hva som diskuteres.
Konklusjon: Oppgaven konkluderer med at personsentrert sykepleie kan ha en stor betydning for kvinner som gjennomfører provosert abort på sykehus. Kvinnene har svært ulike opplevelser og derav også ulike behov knyttet til aborten. En individuell og tilpasset sykepleie er dermed essensielt for kvinnene
Hvilken betydning har sykepleierens undervisende funksjon for å fremme fysisk aktivitet hos brystkreftpasienter?
Innledning: Tema for oppgaven er sykepleierens undervisende funksjon og informasjon om fysisk aktivitet til brystkreftpasienter. Pasienter som mottar adjuvant behandling reduserer aktivitetsnivået. Sykepleieren har en sentral rolle i å informere om fysisk aktivitet gjennom sin undervisende funksjon. Hensikten med denne litteraturstudien er å undersøke hvordan forskningslitteraturen beskriver betydningen av informasjon om fysisk aktivitet til pasienter med brystkreft under adjuvant behandling.
Metode: For å besvare problemstillingen, er metoden for denne oppgaven en generell litteraturstudie. Søket etter forskningsartikler er utført i databasen CINAHL. Søkeordene er «physical activity» OR «exercise» AND «information» AND «chemotherapy, cancer” AND “breast cancer” OR “cancer patients”. Dette resulterte i 22 treff, og basert på valgte inklusjons- og eksklusjonskriterier er 4 artikler inkludert fra tidsperioden 2015-2022.
Resultat: Flere brystkreftpasienter opplevde et umøtt behov knyttet til informasjon. Flere av deltakerne hadde ikke fått informasjon, eller opplevde utilstrekkelig med informasjon om fysisk aktivitet. Pasienter som hadde tidligere erfaringer med fysisk aktivitet, eller de som hadde mottatt informasjon om det, hadde økt etterlevelse av fysisk aktivitet under behandling. Økt kompetanse blant sykepleiere om fysisk aktivitet, kan føre til at pasienter mottar bedre og mer informasjon av betydning.
Diskusjon: Den didaktiske relasjonsmodellen er et verktøy som sykepleier kan benytte seg av for å strukturere informasjonsgiving. Dersom sykepleierens undervisende funksjon skal ha betydning, bør sykepleieren gi en personsentrert og tilpasset informasjon om fysisk aktivitet. Sykepleieren bør bruke sin funksjon til å jobbe sammen med pasienten, og etablere hvilke mål, behov og ønsker som er viktig for hver enkelt. Sykepleieren bør anvende en metodikk i utøvelsen av den undervisende funksjonen som samsvarer med pasientens mål og forutsetninger, og informasjon bør være motiverende og oppfordrende
