To explore the impact of sarcopenia on quality of life among patients with liver cirrhosis:A pilot study

肝硬化為國人十大死因之一,肝病患者隨著年齡增加,肌肉量與質量流失,會造成身體活動表現下降,快速骨骼肌損耗和肌肉減少會嚴重影響肝病患者的存治及生活品質。
目的:
探討肝硬化病患肌少症之現況及其對生活品質之影響。
方法:
本研究為描述性相關性研究,採方便取樣,以南部某醫學中心之126位肝硬化初期病患為收案對象,收案時間為2020年1月至12月,我們收集握力、行走速度測定及雙能量X光吸收儀檢查作為肌少症診斷,並填寫生活品質問卷及收集血液維生素D值和白蛋白檢驗值。以百分比或平均值與標準差描述研究樣本之基本屬性、疾病特性、身體組成及量表得分,並利用卡方檢定、皮爾森積差相關分析及逐步複迴歸模式來分析生活品質之差異。
結果:
本研究結果顯示初期肝硬化病患有無肌少症的比?各31.7%及68.3%
。影響肝硬化合併肌少症之相關因素為年齡(χ2=8.984;p<.001)、身體質量指數(BMI)(χ2=42.474;p<.001)、肌肉量(t=40.735;p<.001)、握力(t=13.731;p<.001)、行走速度(t=10.767;p<.001)與骨骼肌指數(Fat mass/Height2)(t=8.697;p<.001)。整體生活品質與有無合併有肌少症無顯著差異,然,在心理範疇子構面中肌少症較無肌少症者有顯著呈現較低的分數 (t=2.916;p=0.012),影響肝硬化肌少症患者生活品質之相關因素為低的BMI值(p=0.009)。
結論:
本研究顯示在肝硬化初階段,有約1/3的病患合併有肌少症的狀況,年齡、低的BMI值、心理範疇的生活品質與肌少症有關,建議未來針對肝病病患能早期識別出高風險族群,盡早提供預防的介入措施
Investigation on the Management Model of Leisure Farm Combined with Agricultural Biotechnology in Taiwan
指導教授 林雅菁傳統農場的轉型結合農業生技是現今的新方向。藉由生物科技發展新的技術不僅可以改善傳統農業技術不足的問題,且透過不斷的創新進步,也可將高端的農業技術運用在農業場域,改變原有農場經營方式,以多元特色展現休閒空間,滿足都市民眾的機能需求,提升良好生活品質,發揮多元功能。台灣的農業生技不管是技術上的發展,都跟的上時代腳步邁進,也希望將農業生技導入傳統農場,突破原有?念來呈現新的經營模式。本研究以台灣農場做調查,探訪數家生技農場,以SWOT分析探究農場的優勢、劣勢、機會、威脅,提出農業生技運用結合休閒農場的經營改善的建議。希望台灣農場在現有的環境轉變下,透過農業生物技術的結合,能謀求農場未來轉型的最佳經營方式
Ambient air pollution and asthma onset in Taiwanese adults
Background
Ambient nitrogen dioxide is associated with asthma occurrence in adults in Western countries. However, two recent large-scale studies have provided inconclusive results regarding this association. Moreover, the association remains unknown in an Asian population.
Objective
We aimed to investigate the association between ambient air pollution exposure and incident adult asthma.
Methods
From 2016 to 2017, we conducted a cross-sectional self-administered questionnaire survey with retrospective follow-up of incident asthma cases among schoolchildren's parents from 41 schools within 1 km of air monitoring stations distributed throughout the main island of Taiwan. To estimate participants' exposure, air pollution data of air monitoring stations within 1 km of schools from where participants were sampled were inputted into the proximity model. We used mixed logistic regression models to determine the association between air pollution exposure and asthma onset within five years prior to the survey.
Results
The questionnaire was completed for a total of 11 585 participants out of 13 424 candidates (response rate, 86.3%). Among 9131 participants aged from 26 to 50 years, 101 were diagnosed with asthma within the five years prior to the survey. The prevalence of incident asthma cases was 2.2 per 1000 person-years. Those exposed to higher ambient nitrogen dioxide (odds ratio = 1.20 per interquartile [5.7 ppb] of nitrogen dioxide, 95% CI = 1.02–1.42) were more likely to develop adult-onset asthma.
Conclusion
In an Asian population, exposure to higher ambient nitrogen dioxide is associated with increased prevalence of incident asthma cases among adults aged 26–50 years
