Alterations of the splicing machinery components in leukocytes from patients with systemic lupus erythematosus influences its development and atherothrombotic profile and drives the therapeutic response
Recent studies emphasize the relevance of alternative splicing in the development of genetic and autoimmune diseases and suggest therapeutic possibilities based on the modulation of this process. This study aimed to identify alterations in the leukocyte splicing machinery of patients with systemic lupus erythematosus (SLE) and to evaluate their influence on disease development and activity, the atherothrombotic profile, and response to specific therapies. An array of selected components of the major (n=12) and minor spliceosome (n=4), along with associated splicing factors (n=28), was developed, and their expression levels were evaluated using Fluidigm methodology in purified leukocytes from 36 SLE patients and 29 healthy donors (HD). In parallel, an extensive clinical and serological evaluation was carried out. Carotid intima-media thickness (CIMT) was used as a marker of atherosclerosis, endothelial activity was monitored by laser-doppler flowmetry, and pro-inflammatory and oxidative stress markers were quantified by flow cytometry and RT-PCR. Additionally, in a parallel cohort of SLE patients (n=12), the effects of in vivo treatment with ubiquinol on spliceosome components were evaluated.S

A substantial share of the global population continues to face barriers to accessing essential medicines. While the pharmaceutical industry's business model has successfully facilitated the development of innovative medications, efforts to promote universal access to medicines (UAM) remain ineffective. This paper critically assesses the existing barriers to global access to medicines, including the role of unsuitable governance, the protection of intellectual property rights, and other market barriers such as shortages, quality shortcomings, and high prices. Furthermore, we explore a number of promising potential strategies that can help towards achieving the UAM. Specifically, we evaluate the evidence from various initiatives, including alternative models of innovation, manufacturing, procurement, intellectual property management, and structural/organisational operations. We argue that the effective realisation of UAM requires a robust framework to implement these initiatives. This framework must strike a delicate balance between addressing public health needs, incentivising research and development, and ensuring affordability. Achieving such a balance encompasses a careful oversight and collaboration between national and international regulatory bodies.Ye

The Spanish GIRO Guideline: A Paradigm Shift in the Management of Obesity in Adults
Current obesogenic environments, along with intrinsic factors, contribute to the obesity pandemic, which impacts the quality of life and healthcare for individuals with obesity. In addition, discrimination and stigma related to obesity remain widespread in our society. In this scenario, the Spanish Society for the Study of Obesity (SEEDO), in collaboration with 38 recognized scientific societies and 12 patients' organization, has elaborated the Spanish guideline for obesity management in adults, referred to as the GIRO guideline. GIRO aims to drive a shift in obesity management and serve as a guide for healthcare professionals (HCPs) to address this chronic and multifactorial disease. A comprehensive systematic review was conducted and completed with experts' contribution, with a particular focus on Spanish society. The quality of evidence was assessed using the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) system. Experts selected the recommendations and determined their strength through consensus. A total of 121 recommendations were proposed, including 32 adopted from the Canadian Adult Obesity Clinical Practice Guidelines and 89 specific recommendations created for the Spanish context, and were distributed across five areas of application: (1) recognition of obesity as a chronic disease, (2) obesity assessment, (3) multidisciplinary approach to obesity treatment, (4) recommendations for obesity management in special populations, and (5) implementation of the GIRO guideline and future challenges. The GIRO recommendations are intended to serve as a useful and interactive tool for HCPs, policymakers, and other stakeholders to ensure access to and quality of healthcare for individuals living with obesity.Ye

Los Procesos Asistenciales son las herramientas que desde la Consejería de Salud y Consumo se ponen a disposición de las personas y profesionales para conseguir disminuir la variabilidad y facilitar la coordinación mediante una serie de recomendaciones que aportan calidad a la asistencia sanitaria. Fibrilación y flutter auricular ya fueron incluidos en el Proceso Asistencial Integrado Arritmias en 2003 como un subproceso. Actualmente debido a la evolución de los conocimientos y evidencias surgidas en los últimos años, se ha considerado necesaria la elaboración de un Proceso Asistencial específico.Ye
Trends in the incidence of brain cancer and the use of mobile phones: analysis of the Spanish Network of Cancer Registries (REDECAN).
Purpose: The association between the use of mobile phones use and the risk of brain cancer remains controversial. The aim of this study is to describe trends in the incidence of central nervous system (CNS) cancers in Spain and its possible relationship with mobile phone use.
Methods: Trends and trend changes from 1985 to 2015 in adjusted incidence rates of CNS cancers by sex, age (adults and children), site, and histological type were assessed using data from 14 general and paediatric cancer registries.
Results: The study included 20,325 CNS malignancies in adults and 2,372 in children. For adults, the overall rate of malignant brain tumours showed a slight increase of 0.2% (95% CI: 0.1 - 0.4) per year. This increase was concentrated in the early years up to 1996 (1.7% per year, 95% CI: 0.9 - 2.6) followed by 20 years of a non-significant slight decline of -0.1% (95% CI: -0.4 - 0.1) per year until 2015. In children, an increase of 7.6% (95% CI: 2.4 - 13.1) per year until 1991 followed by a decrease of -1.0% (95%CI: -1.7 - -0.3) per year until 2015 was observed. This increase in the incidence in 1980s and early 1990s could be explained by diagnostic improvements especially in imaging techniques implemented during these years.
Conclusion: The present findings do not support a possible relationship between the use of mobile phones and the incidence of malignant brain tumours. However, the possibility of the presence of a weak correlation or that a longer latency period would be needed to observe a possible ecological correlation cannot be discarded.Ye
