Epidemiology of human papillomavirus-associated anogenital cancers in Granada: a three-decade population-based study
Introduction: HPV infection is a common risk factor for all anogenital cancers. However, there are important differences in the epidemiology of anogenital cancers and these have not been compared considering diverse epidemiological indicators over a long period of time. To fill this gap, we investigated incidence, mortality, and survival trends of anogenital cancers over a period of three decades.
Methods: We conducted an observational registry-based study using data from the population-based cancer registry of Granada in southern Spain. We collected data on all incident cases of anogenital cancer (cervical, anal, penile, vulvar, and vaginal cancer) diagnosed between 1985 and 2017. We calculated crude and age-standardized incidence and mortality rates, and 1, 3, and 5-year overall and net survival. We further conducted time-trend analysis calculating annual percent changes (APC) for each cancer site.
Results: The incidence of anogenital cancers decreased slightly during the past 30 years, with the exception of vulvar cancer, where a slight increase was observed. Mortality decreased significantly for cervical cancer over the study period but increased non-significantly for the remaining cancer sites. Survival rates were similar to those reported in comparable countries and increased for cervical and vulvar cancer.
Discussion: Cervical cancer was the greatest contributor to the burden of anogenital cancers and showed a marked improvement in all indicators in comparison to the remaining cancer sites.This work was partially supported by the Ministry of Economy and Competitiveness and the National Institute of Health: Instituto de Salud Carlos III (ISCIII) - PI18/01593 EU/FEDER and the Asociación Española Contra el Cáncer (AECC) (PROYE20023SÁNC).Ye

La BV-SSPA: Democratización del acceso a la información científica.
Esta Jornada está impulsada por el Centro de Documentación y Estudios y organizada por la Biblioteca del Instituto Andaluz de Patrimonio Histórico. Su propósito de establecer un punto de encuentro y reflexión entre los profesionales de las bibliotecas ubicadas en el Campus Cartuja, para debatir sobre temas de interés, estableciendo redes de colaboración y posibles proyectos futuros y que además pudiera tener continuidad en jornadas organizadas por otras bibliotecas. Las bibliotecas van evolucionando hacía nuevos ámbitos, ofrecen acceso a múltiples recursos digitales, acogen actividades diversas con el fin de convertirse en espacios de bienestar, para el encuentro y el diálogo y siguen siendo un importante lugar para el aprendizaje y el acceso a la información. En los últimos años las expectativas de nuestros usuarios/as han ido cambiando y por esta razón es tan importante crear una comunicación efectiva que posibilite un mayor acercamiento a las necesidades reales de este colectivo. La puesta en marcha de nuevas líneas de trabajo supone asumir por parte del profesional de la biblioteca nuevos roles para facilitar el conocimiento y el aprendizaje. Ante este panorama donde lo digital va adquiriendo protagonismo, en detrimento de los servicios tradicionales, vemos muy importante establecer una colaboración y unas líneas de trabajo conjuntas con las bibliotecas que están ubicadas en la Isla de la CartujaN

Clinical utility of a personalized and long-term monitoring device for Parkinson's disease in a real clinical practice setting: An expert opinion survey on STAT-ON (TM)
Background: STAT-ON (TM) is an objective tool that registers ON-OFF fluctuations making possible to know the state of the patient at every moment of the day in normal life. Our aim was to analyze the opinion of different Parkinson's disease experts about the STAT-ON (TM) tool after using the device in a real clinical practice setting (RCPS).Methods: STAT-ON (TM) was provided by the Company Sense4Care to Spanish neurologists for using it in a RCPS. Each neurologist had the device for at least three months and could use it in PD patients at his/her own discretion. In February 2020, a survey with 30 questions was sent to all participants.Results: Two thirds of neurologists (53.8% females; mean age 44.9 +/- 9 years old) worked in a Movement Disorders Unit, the average experience in PD was 16 +/- 6.9 years, and 40.7% of them had previously used other devices. A total of 119 evaluations were performed in 114 patients (range 2-9 by neurologist; mean 4.5 +/- 2.3). STAT-ON (TM) was considered "quite" to "very useful" by 74% of the neurologists with an overall opinion of 6.9 +/- 1.7 (0, worst; 10, best). STAT-ON (TM) was considered better than diaries by 70.3% of neurologists and a useful tool for the identification of patients with advanced PD by 81.5%. Proper identification of freezing of gait episodes and falls were frequent limitations reported.Conclusion: STAT-ON (TM) could be a useful device for using in PD patients in clinical practice. Barriers and proposals for a health care approach to trafficking for sexual exploitation
Objetivo: Explorar la percepción de la trata de mujeres con fines de explotación sexual, identificar las
dificultades y recoger las propuestas de profesionales en contacto directo con las víctimas en Andalucía.
Método: Estudio cualitativo basado en entrevistas a 10 informantes clave seleccionados en entidades de
apoyo y atención a víctimas de trata de mujeres con fines de explotación sexual en 2021. Dos investigado-
ras llevaron a cabo un análisis de contenido categórico temático. Se analizaron tres temas con diferentes
subtemas.
Resultados: Se han identificado dificultades relacionadas con las mujeres (tardan en reconocerse como
víctimas, diferencias culturales, desconfianza en el sistema), con los traficantes (modificación de estra-
tegias), con profesionales de la salud (falta de sensibilidad y poca homogeneidad en las actuaciones) y
con el sistema sanitario (ausencia de intermediación cultural, complejidad administrativa). Las perso-
nas informantes clave proponen una mayor capacitación de profesionales, el empleo de protocolos de
actuación efectivos y una mejor coordinación entre instituciones.
Conclusiones: El sector sanitario enfrenta diversos desafíos para dar una respuesta integral y efectiva a la
trata de mujeres con fines de explotación sexual. Se requieren mejoras en la sensibilización de profesio-
nales de la salud, el desarrollo de protocolos estandarizados, una mayor colaboración entre sectores, la
provisión de servicios especializados en salud mental y una mediación cultural eficaz.Ye
Does expert opinion match the definition of lupus low disease activity state? Prospective analysis of 500 patients from a Spanish multicentre cohort.
To apply the lupus low disease activity state (LLDAS) definition within a large cohort of patients and to assess the agreement between the LLDAS and the physician's subjective evaluation of lupus activity. We conducted a cross-sectional analysis of a prospective multicentre study of SLE patients. We applied the LLDAS and assessed whether there was agreement with the clinical status according to the physician's opinion. A total of 508 patients [92% women; mean age 50.4 years (s.d. 3.7)] were recruited and 304 (62.7%) patients were in the LLDAS. According to physician assessment, 430 (86.1%) patients were classified as remission or low activity. Overall agreement between both evaluations was 71.4% (95% CI: 70.1, 70.5) with a Cohen's κ of 0.3 [interquartile range (IQR) 0.22-0.37]. Most cases (96.1%) in the LLDAS were classified as remission or low activity by the expert. Of the patients who did not fulfil the LLDAS, 126 (70.4%) were classified as having remission/low disease activity. The main reasons for these discrepancies were the presence of new manifestations compared with the previous visit and a SLEDAI 2K score >4, mainly based on serological activity. Almost two-thirds of SLE patients were in the LLDAS. There was a fair correlation between the LLDAS and the physician's evaluation. This agreement improves for patients fulfilling the LLDAS criteria. The discordance between both at defining lupus low activity, the demonstrated association of the LLDAS with better outcomes and the fact that the LLDAS is more stringent than the physician's opinion imply that we should use the LLDAS as a treat-to-target goal
Opsoclonus-myoclonus syndrome: clinical characteristics, therapeutic considerations, and prognostic factors in a Spanish paediatric cohort
Introduction: Opsoclonus-myoclonus-ataxia syndrome is a rare neuroinflammatory disorderwith onset during childhood; aetiology may be paraneoplastic, para-infectious, or idiopathic. No biomarkers have yet been identified, and diagnosis is clinical. Better cognitive prognosis appears to be related to early onset of immunomodulatory therapy. Methods: We describe the epidemiological, clinical, therapeutic, and long-term prognostic characteristics of a cohort of 20 Spanish patients. Results: The mean age of onset was 21 months (range, 2-59). Ataxia and opsoclonus were the most frequent symptoms both at disease onset and throughout disease progression. The meantime from onset to diagnosis was 1.1 months. Neuroblast lineage tumours were detected in 45% of patients; these were treated with surgical resection in 7 cases and chemotherapy in 2. Cerebrospinal fluid analysis revealed pleocytosis in 4 cases (25%) and neither antineuronal antibodies nor oligoclonal bands were detected in any patient. Immunomodulatory drugs were used in all cases. Nine patients started combined immunomodulatory treatment at the time of diagnosis, and 5 patients after a mean of 2.2 months. In the long term, 6 of the 10 patients followed up for more than 5 years presented mild or moderate cognitive sequelae. Four patients presented relapses, generally coinciding with the decrease of corticosteroid doses. Conclusions: Early initiation of immunotherapy, as well as triple combination therapy, where needed, was associated with a lower frequency of cognitive impairment 2 years after onset.
