    How much does a forensic autopsy cost in Spain?

    The autopsy is considered the gold standard in death investigation. Performing an autopsy requires human and material resources that must be programmed in order to meet the demands of the judicial system. However, as far as we know, the cost of forensic autopsy in Spain has not been determined. Thus, the aim of this study was to estimate the cost of a standard autopsy in order to organise Forensic Pathology Services more efficiently. A micro-cost analysis was carried out. The nominal group technique was applied using a panel of 10 forensic experts in order to identify and quantify the resources associated with a forensic autopsy. The results showed that analysis and studies are the most important item in the total cost (54.7%), followed by staff (20.5%), preservation of body (14%), single-use products (7%), equipment and stock (1.6%), cleaning and disinfection (1.5%), facilities maintenance (0.5%) and IT (0.2%). The total cost of a standard autopsy was €1501.45, which is lower than the European average. This study is the first in Spain to calculate the unit price of a forensic autopsy by means of micro-cost analysis. This may help to address the way forensic pathology centres are organised at different levels of complexity.S

    Allostatic Load and Depression Symptoms in Cancer Survivors: A National Health and Nutrition Examination Survey Study.

    Background: Individuals with cancer often experience stress throughout the cancer trajectory and have a high risk of experiencing depression. Objective: The aim of this study was to examine the relationship between allostatic load (AL), a measure of cumulative stress-related physiologic dysregulation of different body systems, and symptoms of depression in cancer survivors. Methods: Participants were 294 adult cancer survivors from the US National Health and Nutrition Examination Survey (NHANES 2007-2018). Allostatic load was measured using 14 indicators representing cardiometabolic risk, glucose metabolism, cardiopulmonary functioning, parasympathetic functioning, and inflammation. Depressive symptoms were measured with the Patient Health Questionnaire-9. The relationship between AL and depressive symptoms was investigated using multiple regression adjusted for diverse sociodemographic and diagnosis variables. Results: Higher AL was associated with higher depressive symptom scores. The higher risk of depression was concentrated among those survivors in the highest AL quartile, with 21% (95% confidence interval, 11%-32%) of survivors presenting a high risk of depression compared with 8% to 11% of survivors in the lower quartiles. In exploratory analyses, the relationship between AL and depressive symptoms was only significant among survivors with a lower income. In contrast, in survivors in the highest income group, depressive symptoms were lower and unrelated to AL. Conclusion: High AL is associated with more depressive symptoms among cancer survivors. Implications for practice: Nurses have an important role in identifying psychological distress in cancer patients and survivors. Further research is needed to investigate the usefulness of AL as a marker in the context of cancer follow-up care and screening for psychological distress.Ye

    Terapia de campo de tumores (TTF) para el tratamiento del cáncer de pulmón de células no pequeñas

    Se ha evaluado la eficacia y seguridad de la TTF en el tratamiento de pacientes con cáncer de pulmón de células no pequeñas avanzado. Para ello se realizó una revisión sistemática de la literatura en la que se localizó un estudio donde se siguió a una serie de 42 pacientes. El tratamiento fue seguro y los pacientes tuvieron una mediana de supervivencia global fue 13,8 meses, con una supervivencia al año de 57%, que, aunque la muestra del estudio fue pequeña y no incluyeron grupo control, podría suponer un aumento con respecto a lo publicado en esta población, donde el tratamiento habitual obtiene una mediana de supervivencia de 8,3 meses. Si bien no se localizaron estudios de evaluación económica, por similitud al tratamiento en el glioblastoma, el coste se estimaría en 21.000€ por mes de tratamiento y paciente.Ye
