    Highly sensitized patients awaiting kidney transplantation face substantial challenges due to the presence of potential donor-specific anti-HLA antibodies (DSA). These antibodies increase the risk of antibody-mediated rejection (ABMR), but also complicate their access to HLA compatible transplantation. Although advancements in allocation priority programs, such as the Spanish Program for the Access of Highly Sensitized Patients to Kidney Transplantation (PATHI), have introduced virtual crossmatching (v-XM) to streamline compatibility assessments, patients with >99,5 % virtual panel reactive antibodies (vPRA) often remain on waiting lists for extended periods with minimal chances of receiving a transplant. This article summarizes Spanish guidelines for a harmonized and comprehensive framework for the management of highly sensitized patients. These guidelines focus on strategies to facilitate transplantation in the presence of DSA, including a stepwise approach to delist HLA antigens, prioritizing those recognized as "less deleterious" antibodies, to expand transplant options while minimizing immunological risks. Conventional desensitization techniques are discussed, alongside the innovative use of imlifidase to enable transplants in particularly complex cases. Post-transplant monitoring protocols are also exposed, with a focus on early detection of antibody rebound and effective management of ABMR. Ultimately, this resource offers clinicians a structured framework to navigate the intricate challenges of kidney transplantation in high-risk populations, aiming to enhance access to life-saving procedures and improve patient outcomes.Ye

    To analyze catharsis techniques used with professionals assisting victims of emergencies and disasters. Systematic review carried out in seven bibliographic databases: MEDLINE-Pubmed, Web of Science, Cochrane Library, CINAHL, Scopus, PsycINFO, and other secondary sources, for articles published between 2008 and 2017, with the terms "catharsis", "emergencies", "debriefing" y "post-traumatic stress". Publication and selection biases were taken into account. Five reviews met the study inclusion criteria. Distress and traumatic experiences following an emergency or disaster are very common among health professionals, with adverse repercussions at the physical, psychological and cognitive levels. Some authors find that debriefing offers an opportunity for emotional release and toreduce the impact of stress on the lives of emergency responders, acting as a support network where they can reconstruct their experiences, express their thoughts and feelings, and reduce the impact on their lives. For other authors, however, this technique is not always effective for all professionals and may lead to negative consequences. Debriefing is a common intervention that promotes catharsis. However, there is considerable controversy over its implementation and effectiveness.     Background: Unemployment is associated with poorer disease outcomes in chronic conditions. Current high rates of unemployment in Spain may lead to a higher burden of disease in axial spondyloarthritis (axSpA) patients. Objectives: To evaluate the differences in sociodemographic factors and patient-reported outcomes (PROs) between employed and unemployed axSpA patients in the same sample. Methods: Data from 680 unselected patients of the Spanish Atlas of Axial Spondyloarthritis from an online survey were collected in 2016 were analysed. Active workforce participants were divided into employed and unemployed according to International Labour Organization (ILO) standards. Socio-demographic characteristics, and Patient-reported Outcomes (PROs) [BASDAI (0-10), spinal stiffness (3-12), functional limitation (0-54) and psychological distress (0-12, General Health Questionnaire GHQ-12)] were compared between employed and unemployed participants. The Χ² test was used for qualitative variables and the Mann-Whitney test for quantitative variables.S

    Background: West Nile Virus (WNV) exemplifies the complexities of managing vector-borne diseases, expanding globally due to human activities and ecological changes. Originating from Africa and transmitted by Culex mosquitoes, WNV is now reported across multiple continents. The aim of this study was to identify the environmental, ecological, and individual factors influencing WNV transmission. Methods: An umbrella review was conducted. Comprehensive searches were performed in PubMed, Scopus, Web of Science, Embase, and LILACS. Inclusion criteria were reviews involving WNV transmission agents (reservoirs, vectors, hosts) and associative analyses between environmental, ecological, or individual factors and WNV transmission. Data extraction and quality appraisal were performed using templates and the AMSTAR 2 tool. Results: From 404 retrieved studies, 23 systematic reviews and meta-analyses were included. Almost 70 % were low or critically low quality. The co-occurrence network highlighted emerging research on climate change and environmental factors. Temperature, precipitation, and land use significantly influence WNV transmission. Warmer temperatures enhance mosquito populations and viral replication, while extreme weather events like droughts increase mosquito-human contact. Climate change significantly contributes to WNV dynamics by altering temperature and precipitation patterns, enhancing vector proliferation, and extending transmission seasons. Ecological factors such as higher avian diversity, vegetation indexes, and distribution of mosquito species can impact WNV transmission significantly. Education and income levels influence preventive behaviors and infection risk, with lower socioeconomic status linked to higher WNV risk. Certain occupational groups are also at elevated risk of WNV infection. Conclusion: Environmental factors like temperature and precipitation critically affect WNV transmission by influencing mosquito behavior and avian reservoir dynamics. Socio-economic status and education levels significantly impact individual preventive behaviors and infection risk. Multifactorial influences on infection risk make necessary integrated surveillance systems and public health strategies. Longitudinal studies with One Health approaches are necessary to better understand WNV dynamics and reduce WNV transmission.Ye
