    Rising Testicular Cancer Incidence in Spain Despite Declining Mortality: An Age-Period-Cohort Analysis

    Objetivo Actualizar la información sobre la incidencia y mortalidad por cáncer testicular (TC) en España entre 1990 y 2019. Métodos Se analizaron datos de la Carga Global de Enfermedades (GBD) para calcular las tasas de incidencia y mortalidad estandarizadas por edad (TIEE y TMEE). Se utilizó la regresión Joinpoint y el modelo edad-periodo-cohorte (EP- C ) para analizar las tendencias y los efectos de la edad, el período y la cohorte de nacimiento. Resultados Se observó una sorprendente duplicación de la incidencia de CT, de 3,09 a 5,40 por 100.000 hombres (aumento anual del 1,9%), mientras que las tasas de mortalidad se mantuvieron estables e incluso disminuyeron en los grupos de menor edad (de 0,34 a 0,26 por 100.000, disminución anual del 0,8%). El análisis Joinpoint reveló cuatro periodos distintos de aumento de la incidencia, con una ralentización reciente. El modelo EPC puso de manifiesto un aumento constante del riesgo de incidencia con cada generación sucesiva nacida después de 1935, que contrasta con un descenso progresivo del riesgo de mortalidad en todas las cohortes, especialmente marcado para los nacidos a partir de los años sesenta. Conclusiones La incidencia del CT aumenta en España, mientras que la mortalidad se mantiene estable o incluso disminuye. Existe un efecto generacional en la incidencia, con mayor riesgo para las generaciones más jóvenes. Se necesitan más investigaciones para comprender las causas del aumento de la incidencia y desarrollar estrategias de prevención.Ye

    This article is part of a clinical practice guideline update on the risk assessment, diagnostic imaging, and microbiological evaluation of complicated intraabdominal infections in adults, children, and pregnant people, developed by the Infectious Diseases Society of America. In this article, the panel provides recommendations for diagnostic imaging of suspected acute cholecystitis and acute cholangitis. The panel's recommendations are based on evidence derived from systematic literature reviews and adhere to a standardized methodology for rating the certainty of evidence and strength of recommendation according to the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach.Ye

    Background: There is insufficient evidence to provide recommendations for leisure-time physical activity among workers across various occupational physical activity levels. This study aimed to assess the association of leisure-time physical activity with cardiovascular and all-cause mortality across occupational physical activity levels. Methods: This study utilized individual participant data from 21 cohort studies, comprising both published and unpublished data. Eligibility criteria included individual-level data on leisure-time and occupational physical activity (categorized as sedentary, low, moderate, and high) along with data on all-cause and/or cardiovascular mortality. A 2-stage individual participant data meta-analysis was conducted, with separate analysis of each study using Cox proportional hazards models (Stage 1). These results were combined using random-effects models (Stage 2). Results: Higher leisure-time physical activity levels were associated with lower all-cause and cardiovascular mortality risk across most occupational physical activity levels, for both males and females. Among males with sedentary work, high compared to sedentary leisure-time physical activity was associated with lower all-cause (hazard ratios (HR) = 0.77, 95% Confidence interval(95%CI): 0.70-0.85) and cardiovascular mortality (HR = 0.76, 95%CI: 0.66-0.87) risk. Among males with high levels of occupational physical activity, high compared to sedentary leisure-time physical activity was associated with lower all-cause (HR = 0.84, 95%CI: 0.74-0.97) and cardiovascular mortality (HR = 0.79, 95%CI: 0.60-1.04) risk, while HRs for low and moderate levels of leisure-time physical activity ranged between 0.87 and 0.97 and were not statistically significant. Among females, most effects were similar but more imprecise, especially in the higher occupational physical activity levels. Conclusion: Higher levels of leisure-time physical activity were generally associated with lower mortality risks. However, results for workers with moderate and high occupational physical activity levels, especially women, were more imprecise. Our findings suggests that workers may benefit from engaging in high levels of leisure-time physical activity, irrespective of their level of occupational physical activity.Data and/or sample material have been provided by the Study of Health in Pomerania (SHIP) from the Community Medicine Research Alliance of the University Medicine Greifswald. First of all, we would like to highlight the importance of documents of this nature, which are invaluable for professionals serving the trans collective, of high quality given not only given the professional trajectory of its authors, but also its scientific contents. However, we would like to make some comments to enrich this source with additional details. The main point which we would like to reflect on concerns the conditions under which informed consent is obtained

    Real-world clinical experience of dupilumab in the treatment of chronic palmo-plantar eczema in patients with moderate-severe atopic dermatitis: 52-week follow-up

    Background: Hand-foot eczema (HFE) are special locations of dermatitis, which are often associated with atopic dermatitis (AD) and have a significant negative impact on the quality of life, demanding a clinically relevant improvement. Objectives: To evaluate the effectiveness and safety of dupilumab in the treatment of eczema localized in hands and/or feet in patients with moderate-to-severe AD. Methods: Retrospective multicenter study of adult patients with HFE treated with dupilumab for their AD. Patients with other concomitantly systemic immunosuppressive treatments did not undergo a washout period. The severity of palmar and/or plantar involvement was assessed using the Physician Global Assessment (PGA) scale on a scale of 0 ( = clear) to 5 ( = very severe). Eczema Area and Severity Index (EASI) and NRS-pruritus scales were also evaluated. One hundred percent of patients reached week 16, while 67/84 reached week 52 of follow-up. Results: A total of 84 patients were included 86.69% of patients showed a reduction in PGA-Hand, and 80.34% in PGA-Foot at week 52, EASI improvement was reached by 83.55% of patients at week 16 and 87.35% at week 52. Reduction of pruritus (≥4 points in NRS-pruritus scale) was 73.01% at week 16 and 80.67% at week 52. No differences in response to dupilumab were observed in the different subtypes of palmo-plantar dermatitis. Conclusions: The results obtained in our study suggest that dupilumab may be an effective and safe therapeutic option for the treatment of dermatitis localized in hands and/or feet.Ye
