    矯正歯科治療における圧下力により歯根吸収を誘導する新規動物実験モデルの開発

    大阪歯科大学Osaka Dental University博士(歯学)There is a high risk of external apical root resorption (EARR) following the application of intrusive orthodontic forces to the apical root. However, there is a lack of suitable animal models to study this phenomenon in depth. This study compared the usability of three different types of loops, namely, vertical helical loop, box loop, and L loop, for preparing a stable rat model of orthodontic tooth movement with invasive forces. Results showed a significant downward movement in the first molar of the rat after L loop placement for 14 days. Three-dimensional reconstructed images showed root resorption and length shortening on the apical root and decreased bone volume and trabecular thickness in the alveolar bone under compression. Histological staining revealed odontoclasts on the root resorption fossa. This study showed that orthodontic tooth movement using the L loop provides an effective experimental animal model of external apical root resorption.doctoral thesi

    歯科保健指導における口腔内スキャナーの応用―指導効果の検証―

    大阪歯科大学Osaka Dental University博士(口腔科学)目的:我々は,歯科保健指導に口腔内スキャナー(IOS)を活用することで,歯周組織の変化の定量化と可視化を図り,患者が歯周組織の状態を理解しやすくするとともに治療効果の診査の確度を向上させることを目的として検討を進めてきた.本研究では,従来の歯科保健指導方法とIOSを用いた歯科保健指導を比較検討し,IOSを用いた歯科保健指導の有用性について検討した.研究対象と方法:研究対象は,十分な歯科保健指導を受けた経験が無い本学教職員および学生 計9名(平均年齢41.6歳)とし,歯科衛生士3名が,以下の歯科保健指導を研究対象者ごとに2週間以上の間隔をあけ,順序を変えて実施した.①従来法1:鏡のみを利用した歯科保健指導,②従来法2:鏡に加え口腔内写真とスタディーモデルを利用した歯科保健指導,③IOS法 :IOS記録のみを利用した歯科保健指導。IOSの記録は,3D測定データ評価ソフトウェアにて分析した.結果:IOSでの記録では,従来の歯周組織検査よりも詳細に歯肉の変化を把握できた.IOSによる記録を重ね合わせて求めた辺縁歯肉表面の偏差,プロービングデプス,歯肉の腫脹,発赤の検査結果及びBOPの変化との関係性が低かった.IOSを用いた歯科保健指導と従来の指導法との間には指導効果の差は認められなかった.IOSによる記録を不快に感じる研究対象者がいたが,多くはIOSによる保健指導は分かりやすく,受けたい指導であると回答した.結論:現状ではIOSを用いた歯科保健指導の臨床的有用性は必ずしも高いとはえいないが,IOSによる記録は,これまでの歯周組織検査とは異なる視点で歯肉を詳細に評価できる方法であり,患者・術者両者にとって理解しやすい方法である.今後,研究を進めることで歯科保健指導に効果的に活用できる可能性があると考えられる.」と考えられる.doctoral thesi

    肥満2型糖尿病モデルラットにおける上顎第一臼歯口蓋側歯肉の粘膜上皮の形態学的研究

    大阪歯科大学Osaka Dental University博士(歯学)In Japanese, obesity is a lifestyle-related disease and major risk factor for type 2 diabetes mellitus. Asians, including the Japanese, are reported to have a higher incidence of diabetes mellitus because they have a lower body mass index and higher body fat percentage. In this study, we performed a morphological analysis of the mucosal epithelium of the palatal gingiva of the maxillary first molar in normal rats and in Spontaneously Diabetic Torii fatty rats, a model of obese type 2 diabetes mellitus (ODM). The superficial morphology of the mucosal epithelium showed epithelial cells of various sizes and nearly exfoliated epithelial cells in the ODM group. The mucosal epithelium was significantly thicker in the ODM group than in the normal group (p < 0.01). The number of cells in the granular and prickle layers was significantly greater in the ODM group than in the normal group (p < 0.01), and the number of epithelial papilla was significantly lower in the ODM group than in the normal group (p < 0.01). In conclusion, hyperglycemia with ODM thickens the mucosal epithelium of the palatal gingiva of the maxillary first molar causing the epithelial papillae to be lower and epithelial cells of the keratinized layer to detach more easily.doctoral thesi

    ヒト歯髄由来線維芽細胞様細胞におけるマトリックスメタロプロテアーゼ‐1産生に対するS-PRGフィラー溶出液およびムラミルジペプチド(MDP)による共刺激の影響

    大阪歯科大学Osaka Dental University博士(歯学)The present study investigated the effects of a co-stimulation with surface reaction-type pre-reacted glass-ionomer (S-PRG) filler eluate and muramyl dipeptide (MDP) on matrix metalloproteinase (MMP)-1 production by human dental pulp fibroblast-like cells (hDPFs). S-PRG filler eluate contains 6 ions (F, Na, Al, B, Sr, and Si) released from S-PRG filler. Each S-PRG filler eluate and MDP stimulation enhanced MMP-1 production by hDPFs. The co-stimulation with S-PRG filler eluate and MDP enhanced MMP-1 production more than the MDP stimulation alone. A similar stimulation induced the phosphorylation of ERK 1/2. The increased secretion of MMP-1 and enhanced phosphorylation of ERK 1/2 by the co-stimulation with S-PRG filler eluate and MDP were suppressed by the selective and potent CaSR antagonist NPS 2143. Since strontium binds to CaSR, these results suggest that the enhanced production of MMP-1 by the co-stimulation with S-PRG filler eluate and MDP was due to the effects of strontium.doctoral thesi

    インプラント補綴治療による舌圧の改善

    大阪歯科大学Osaka Dental University博士(歯学)Implant-supported prostheses have been widely accepted for edentulous or partially edentulous patients. However, few studies have examined the relationship between masticatory performance and implant-supported prostheses treatment. The aim of the study was to assess changes in biting abilities, maximum tongue pressure, and oral health-related quality of life in subjects with implant-supported restoration. Twenty partially edentulous patients who received dental implant therapy were included in this study. Biting abilities including occlusal contact area, bite force, maximum occlusal pressure, and average occlusal pressure, maximum tongue pressure, and oral health-related quality of life (OHRQoL) were evaluated. The assessments were performed at the baseline (pre-implant prostheses), post-implant prostheses, 1, 3, 6, and 12 months after implant prostheses placement. Biting abilities showed no statistically significant differences throughout the entire period. Maximum tongue pressure measurement was significantly increased at 12 months compared to before implant-supported prostheses treatment. OHRQoL was significantly increased at 1 month, 3 months, 6 months, and 12 months after prostheses placement compared to before prostheses placement. Placement of implant-supported prostheses for partially edentulous patients significantly improved the maximum tongue pressure and OHRQoL.doctoral thesi

    頭部エックス線規格写真分析による成人skeletal Class I 顎顔面形態の研究

    大阪歯科大学Osaka Dental University博士(歯学)We used cephalograms to investigate each measurement item of the maxillofacial morphology of skeletal Class I Japanese adult males and females with individualized normal occlusion. We also calculated the standard values of the Steiner analysis, which is widely used for setting treatment goals in orthodontic practice. In comparison with pre-existing Japanese measurements on growing children, in males, our results showed larger values for ∠SNB, ∠SND, U1 to NA (mm), U1 to NA (angle), Po to NB, Interincisal angle, SL, and SE, and smaller values for ∠ANB, L1 to NB (angle), ∠Occl to SN, and ∠GoGn to SN. In females, ∠SNB, ∠SND, U1 to NA (angle), Po to NB, Interincisal angle, SL, and SE showed large values, while ∠ANB, L1 to NB (angle), ∠Occl to SN, and ∠GoGn to SN, showed smaller values. Based on the results of our study, it is necessary to consider a new ratio for the relationship between the distances L1 to NB (mm) and Po to NB (mm) in Japanese because of Japanese facial features.doctoral thesi

    COVID-19と非COVID-19によるARDS患者における嚥下障害の比較検討

    大阪歯科大学Osaka Dental University博士(歯学)We compared the clinical characteristics and incidence of dysphagia after mechanical ventilation in patients with acute respiratory distress syndrome (ARDS) occurring with or without coronavirus disease 2019 (COVID19). Sixtyseven patients who were diagnosed with ARDS due to COVID19 and were managed with mechanical entilation between January 2020 to March 2021 were included in the COVID19 group. This group was compared with 32 patients who were diagnosed with ARDS due to diseases other than COVID19 between January 2017 and March 2021 ( the nonCOVID19 group) . Clinical characteristics and dysphagia during the time after being in the intensive care unit and hospital discharge were compared for the two patient groups. The Functional Oral Intake Scale ( FOIS ) was used to evaluate dysphagia. FOIS levels 1 to 5 are defined as dysphagia. There were no statistically significant differences in patient characteristics between the two groups other than their Acute Physiology and Chronic Health Evaluation II scores (18.82±5.949 vs 25.97±6.977, p<0.001). Multivariate analysis showed that the incidence of dysphagia at hospital discharge in patients with COVID19associated ARDS was statistically significantly lower than in patients with ARDS due to other causes (OR 0.238, 95% CI 0.071−0.798; p=0.02).doctoral thesi

    大規模診療報酬請求データベースを用いた口腔・咽頭がんの二次原発がんの発症に関する疫学研究

    大阪歯科大学Osaka Dental University博士(歯学)Background/purpose: As the occurrence of second primary cancers (SPCs) is strongly related to the survival rate of patients with oral and pharyngeal cancers, early detection and treatment are important.Therefore, this study aimed to clarify the incidence of SPCs and their risk factors in patients with oral and pharyngeal cancer. Materials and methods: This observational study was conducted using data from the administrative claims database of 21,736 participants with oral and pharyngeal cancer from January 2005 to December 2020. We evaluated the cumulative incidence of SPCs among patients with oral and pharyngeal cancers using the KaplaneMeier method. The Cox proportional-hazard model was used for multivariate analysis. Results: Of the 1633 patients with oral and pharyngeal cancer who qualified for analysis, 388 developed SPCs (incidence rate, 7.994/1000 person-months). The multivariate analysis showed that the risk of developing SPCs was affected by age at diagnosis of oral and pharyngeal cancer, cancer treatment, and anatomical site of the primary cancer. Conclusion: Patients with oral and pharyngeal cancers are at a high risk of developing SPCs. The data from this study may be useful in providing accurate information to patients with oral and oropharyngeal cancer.doctoral thesi

    タバコ煙成分で培養したヒト歯根膜線維芽細胞の伸展刺激による細胞老化の増悪化

    大阪歯科大学Osaka Dental University博士(歯学)Understanding the effect of smoking on oral pathologies at the molecular and cellular level is required for the stable progress of oral medicine. In the present study, we investigated the effect of tobacco smoke components (TSCs) and mechanical stimulation on the senescence of periodontal ligament cells. We cultured human periodontal ligament fibroblasts (HPdLFs) for different durations that were exposed to TSCs collected by our own original procedures. Proliferative activity and cellular senescence after TSC treatment was examined at different points in time. Our results demonstrated that HPdLFs continuously treated with TSCs exhibited characteristics of senescence. Moreover, there was an increased expression of the senescence-related gene when the senescent HPdLFs were cultured with a stretching stimulation. This result clearly suggests that stretching stimulation aggravates cellular senescence.doctoral thesi

NGF acts as an important mediator of the cascade, acting downstream of C5a receptor 1 (C5aR1). Although NGF is known to be involved in inflammatory pain, the mechanism by which it causes pain remains unclear. It is known that an injection of complement 5a (C5a) into the hind paw skin of mice elicits painful behavior and enhances NGF expression in dermal macrophages. Sorting nexin 25 (SNX25) is part of a family of proteins that plays an important role in membrane trafficking, intracellular signaling, membrane remodeling, and organelle motility, some of which affect the immune system. Using the macrophage cell-line J774.1, we examined whether SNX25 is involved in C5a-induced NGF expression at the cellular level. Treatment of J774.1 with C5a enhanced the expression of NGF and SNX25. Western blotting revealed that C5a-induced NGF expression was reduced by SNX25 siRNA. SNX25 +/- mice had lower NGF levels and showed a pain-insensitive phenotype in response to C5a. We propose that the expression level of C5a-induced NGF is controlled by macrophages through SNX25 signalling.doctoral thesi
