Economic value of atopic dermatitis prevention via partially-hydrolyzed whey-based infant formula (PHF-W) use in high-risk, non-exclusively breastfed, Indonesian urban infants: results of a cost-effectiveness model
Background: Early nutritional intervention with partially-hydrolyzed whey-based formula (PHF-W) instead of standard cow’s milk formula (CMF) has been found to reduce the risk of atopic dermatitis (AD) development in non-exclusively breastfed infants with familial heredity of AD.Objective: To estimate the 6-year economic impact of this nutritional intervention in non-exclusively breastfed Indonesian urban infants with family history of AD.Methods: A mathematical model simulated AD incidence and burden of using PHF-W vs. CMF in the target population from birth to age 6. The model integrated literature, current cost and market catalogues, and expert clinician opinion. Modeled outcomes included AD risk, time spent post-AD diagnosis, days without flare, quality-adjusted life-years, and costs.Results: Using PHF-W instead of CMF resulted in an estimated absolute 14% (95% CI: 4%, 23%) AD risk reduction, a 0.69 year (95% CI: 0.26, 1.13) per-child reduction in time spent post-AD diagnosis, a 38 (95% CI: 12, 67) increase in days without AD flare, and a 0.046 gain in quality-adjusted life-years. The AD-related 6-year cost estimates when feeding high-risk urban infants with PHF-W were Indonesian Rupiah (IDR) 8,695,057 (95% CI: IDR 4,519,447, IDR13,995,605) and IDR13,139,569 (95% CI: IDR 7,098,794, IDR 19,216,068) per child, respectively, resulting in a net per-child difference of IDR 4,444,512 (95% CI: IDR1,893,080, IDR 8,557,946) favoring PHF-W.Conclusion: PHF-W for the first 17 weeks of non-exclusively breastfed Indonesian urban infants with a hereditary risk of AD demonstrated a reduction in AD incidence, increased days without flare, and increased quality-adjusted life-years and net cost reductions
Correlation between Zinc Intake and Zinc Serum Levels with C-reactive Protein Level in Head and Neck Cancer Patients
The inflammatory process of head and neck cancer leads to an increase the proinflammatory cytokines and the synthesis of c-reactive protein (CRP), which then causes metabolic alteration and anorexia in the patients. Zinc is one of nutrient that has an important role in suppressing inflammation, but it is reported that about 65% of head and neck cancer patients have zinc deficiency. The aim of this cross sectional study is to determine the correlation between zinc intake and serum zinc levels with CRP level as an effort in reducing inflammation process in head and neck cancer patients. Subjects were collected by consecutive sampling in the Oncology Polyclinic Rumah Sakit Kanker Dharmais, from 49 subjects 67,3% were men, most subjects were in the age range between 46–65 years. The highest frequency (65,3%) is nasopharyngeal cancer and 69,4% are already in stage IV. All subjects (100%) in this study have a zinc intake below the recommended dietary allowance (RDA) in Indonesia. The mean serum zinc level of the subjects was 9.83±2.62 μmol/L. Most subjects have elevated CRP levels. There was a weak negative correlation between zinc concentration and CRP levels of subjects (r =-0.292, p =0.042), but there was no correlation between zinc intake and CRP levels of subjects (r =-0.25)
Correlation Between Docosahexaenoic Acid Intake and It’s Content in Breast Milk of Lactating Mothers in Jakarta
Objective: Docosahexaenoic acid (DHA) is the predominant structural fatty acid in the brain and one of the most important nutrient for cognitive development in early life. During early life, there is very limited metabolic capability to convert omega-3 fatty acids to DHA. Therefore, newborn intake of DHA completely depends on preformed DHA in mother’s breast milk. This study aims to determine DHA intake among lactating mothers and their association with breast milk’s DHA.Method: This cross-sectional study was conducted in Grogol Petamburan and Cilincing Public Health Centre in Jakarta. Eighty healthy lactating mothers aged 20–35 years old in 1–6 months postpartum was taken using consecutive sampling method. Mother’s DHA intake was assessed with a semiquantitative food frequency questionnaire. Post-feed breast milk specimens were collected in the morning, transported with cool boxes, and stored in the laboratory at -70° C before the analyses. Breast milk DHA content was analysed using Gas Chromatography with Mass Spectrometry. Descriptive analyses and Spearman rho test was used with a 95% confidence level.Result: This study showed the median of subjects’ DHA intake was 158.5(13.9–719.7) mg/day, i.e. 67.5% of the subjects was below Food and Agriculture Organization (FAO) recommendation (200 mg/day). The median of breast milk DHA was 51.7(19–184.7) mg/day, only 42.5 % of the subjects had breast milk DHA to meet the minimal requirement of their infant based on FAO recommendation (0.1% of total energy requirement). There was a moderate correlation between subject DHA intake with breast milk’s DHA content (r = 0.478, p < 0.001). Conclusion: More than half of the subjects had DHA intake below FAO recommendation. Our finding showed a positive moderate correlation between DHA intake and breast milk DHA among lactating mothers
Association between exclusively breast fed infant and picky eating behavior in children below five years old: a cross-sectional study
Background: Picky eating behaviors are prevalent during childhood and often linked to nutritional problems. Environmental factors play a role in taste and eating preferences, such as genetics, learning experiences, and culture including exclusive breast fed for six months. Nutritional problems deserve special attention for its long term consequences such as malnutrition, stunting, infection, social and cognitive impairment. In Indonesia, 50-60% parents have problems with picky eating behavior with their children.1 Objective: To investigate association between exclusively breast fed infant and picky eating behavior. Method: A cross-sectional study with purposive sampling was performed in 208 children at age below 5th years old in Serang. To assess picky eating behavior, the writer use Children Eating Behavior Questionnaire especially for food fussiness section (CEBQ-FF). Based on WHO recommendation, breast fed infant is infants that receive only breast milk for 6 months as an optimal way of feeding infants. Statistical analysis using chi square with p value < 0.05 considered being significant and odds ratio > 1 considered have causal effect. Result: Among 208 children, there are 123 children (59%) that have picky eating behavior and 43 children of them are exclusively breast fed. There is a relationship between exclusively breast fed infant and picky eating behavior (p=0.037). Correlation between Serum Hugh Sensitivity C-Reactive Protein With Dietary Intake of Indonesian Lactating Mothers
Background and Objectives:Chronic low-grade inflammation has emerged as important pathophysiology of non-communicable diseases, which can cause negative effects to mother and baby. Dietary intake has been known as important factor to affect inflammation, which can be measured by high-sensitivity C-reactive protein (hs-CRP). This study aimed to examine the correlation between serum hs-CRP with dietary intake in lactating mothers.Methods:A cross-sectional study was conducted by consecutively enrolling 71 lactating mothers, 3-6 months post-partum, age 20-35 years old, visiting Grogol Petamburan and Cilincing community health center between February and April 2019. Dietary assessment was conducted using semi-quantitative food frequency questionnaire. Anthropometric measurements included were pre-pregnancy weight, post-partum weight, and body height. Serum hs-CRP was measured by immunoturbidimetry method. Spearman correlation was used, with p<0.05 considered significant.Results:Correlation was found between serum hs-CRP and energy (r = 0.372, p = 0.001), carbohydrate (r = 0.295, p = 0.013), and vitamin B6 (r = -0.285, p = 0.016) intake. We also found that serum hs-CRP was correlated with pre-pregnancy (r = 0.296, p = 0.012) and post-partum BMI (r = 0.430, p<0.001).Conclusion:Energy, carbohydrate, and vitamin B6 intakes are positively correlated with serum hs-CRP level
Plasma Folate, Vitamin B6 and B12 in Their Relationship to the Presence of Probiotic Strain Bifidobacterium animalis subsp. Lactis HNO19 (DR10TM) Among Indonesian Pregnant Women in Their Third Semester
Introduction Vitamin B12 plays a role during pregnancy in maintaining folate metabolism. Together with folate and vitamin B6, B12 is involved as a coenzyme in DNA synthesis and various methylation reactions in developing embryos. This study aims to compare plasma folate, vitamin B6 and B12 blood levels with respect to the presence of probiotic strain Bifidobacterium animalis subsp. lactis HNO19 (DR10TM) among pregnant women in Indonesia. Methods: This study is part of a larger study entitled “Effects of Bifidobacterium animalis subsp. lactis HNO19 (DR10TM), inulin, and micronutrient fortified milk on fecal DR10TM, immune markers, and maternal micronutrients among Indonesian pregnant women.” Further analyses were performed using independent-t test or Mann Whitney test, GLM-repeated measures and chi-square test, to compare folate, vitamin B6 and B12 intake and blood concentration during pregnancy with presence (n=22) and absence (n=55) subjects of fecal DR10TM at third trimester. Results: At the first trimesters there was no difference in plasma vitamin B6 and vitamin B12 levels between the two groups, based on the presence or absence of fecal DR10TM. However, at the second and third trimester, vitamin B6 blood concentration (p=0.034 and p=0.001) and vitamin B12 blood concentrations at the third trimester (p=0.035) were significantly higher in the fecal DR10TM positive group, while having a similar vitamin B6 and B12 intake. Conclusion: Consumption of pre- and probiotics during the periconceptional period may be a useful strategy for improving maternal vitamin B’s vitamins, especially vitamin B6 and B12 status and therefore provide benefits for the offspring’s quality of life

The advancing of economic and technology in the last 1–2 centuries that characterized by industrialization, urbanization, and globalization have changed human lifestyle worldwide. Abundance of food and the ease of getting it, the availability of calorie-dense processed foods, changes in dietary patterns, and sedentary lifestyles force the rapid accretion of the incidence of obesity and non-communicable diseases such as type 2 Diabetes mellitus, cardiovascular diseases (CVD), and cancer. Non communicable diseases (NCDs) are the leading cause of death worldwide, and one of the major health challenges of the 21st century.1 In 2016, NCDs were responsible for 71% (41 million) of the 57 million deaths which occurred globally. The major NCDs responsible for these deaths included cardiovascular, cancers, chronic respiratory diseases, and diabetes.2 Many epidemiological, observational, and clinical studies have shown that nutrition, as the main environmental factor, plays a pivotal role in these high-cost degenerative diseases
Correlation between Hair Zinc Level and Cognitive Function in Elderly
Background and Objectives : Neurodegenerative disease is the most problem in elderly. Amyloid β (Aβ) accumulation is the major cause of cognitive impairment. Zinc has an important role in antioxidant and Aβ accumulation process. This study aimed to evaluate the correlation between hair zinc level and cognitive function in elderly.Methods : A cross sectional study was conducted involving 58 subjects of elderly in Jakarta. Subjects were recruited by consecutive sampling. Hair zinc level was measured by inductively coupled plasma emission spectrometer (ICPS) and cognitive function assessed by abbreviated mental test (AMT). Data analysis was done by spearman rank correlation test[DN1] and p-value less than 0.05 was considered statistically significant.Results : The mean of age was 65.4 ± 4.4 years old and 56.9% of subjects were female. The mean of hair zinc level was 123.23 ± 69.71 µg/gram hair and 32.8% subjects had hair zinc deficiency. There was 91.4% subjects had normal cognitive function. The study showed no correlation between hair zinc level and cognitive function in elderly (p=0.871 ; r=-0.022).Conclusion : There was no correlation between hair zinc level and cognitive function in elderly. Further research is expected to be performed with different level of cognitive function
Correlation between the Consumption Frequency of Sugar-Sweetened Beverages with Serum Triglyceride Levels in Female Adolescents
Background: The prevalence of overweight and obesity in adolescence increases significantly from year to year particularly in Depok. Consumption of sugar-sweetened beverages is one of the causes. Excess triglyceride levels are one of the risk factor for metabolic syndrome and cardiovascular disease. Consumption frequency of sugar-sweetened beverages is associated with increased serum triglyceride levels. Method: This study used a cross-sectional design involving 47 subjects recruited through the consecutive sampling method. The consumption frequency of sweetened soft drinks is taken by the semiquantitative FFQ method. Samples of serum triglyceride levels were taken from venous blood and measured using enzymatic methods.Result: Forty-seven subjects finished the study protocol The result showed that there is a significant positive correlation with very strong degrees (p = <0.001, r = 0.88) between the consumption frequency of sugar-sweetened beverages with serum triglyceride levels.Conclusion: There is a significant positive correlation with very strong degrees between the consumption frequency of sugar-sweetened beverages with serum triglyceride levels.Keywords: overweight, female adolescence, sugar-sweetened beverages, triglycerid
Comparing The Anthropometric Measurements of Intra-Extra Uterine Period between Stunting and Non-stunting Children Aged 6-24 Months Old in Bogor Tengah Subdistrict, Bogor City, West Java
Introduction: The prevalence of stunting in children under two years old is high in developing country. Stunting in the early of life has many negative health consequences in later life, while the positive view is stunting could be prevent earlier by detecting a predisposing factors in intrauterine and extrauterine periode. Anthropometry of intrauterine predisposing factors (IntraPF) and Extrauterine predisposing factors (ExtraPF) could detect growth retardation. These study aim was to prove that there were significant different of IntraPF and ExtraPF between stunting and non stunting children aged 6-24 months.Methods: This comparative cross sectional study was nested to (Bogor longitudinal study on child growth and development (BLSCGD) held by National Health Research and Development (NHRD), Health Ministry of Indonesia. Subjects who defined as stunting if their height for age was more than two standard deviations below the WHO Child Growth standards median. There were 38 stunting and 46 non-stunting aged 6-24 months who met the study criteria. Independent variable was (1)IntraPF follow as maternal height, weight and length of neonatal, (2)ExtraPF: weight gain and body length gain at first six months of neonatal. Primary and secondary data collection was carried out from July 2017 to Februari 2018, held at the BLSCGD. All of body measurement using WHO standard. Appropriate statistical analysis was used to compare the stunting and non-stunting groups with a significance limit of p <0.05.Results: Statistical analysis tests showed that: significantly different and lower IntraPF in the stunting group was maternal height, birth weight and length of the subjects compare to non-stunting and there were significant different and lower of ExtraPF follow as weight gain in the first six months, average weight gain per month in the first six months, increase in body length in the first six months, average increase in body length every month in the first six monthssignificantly lower in stunting children compare to non stunting children.Conclusion: The antropometric of intrauterine, extrauterine predisposing factors have been showed to have an impact on the child's height. It is important to monitor the anthropometry of the mother before pregnancy and provide nutritional interventions in the first 1000 days
