    MEDICATION ERRORS IN THE PRESCRIBING AND TRANSCRIBING PHASE: CASE STUDY

    Medication errors are a leading cause of patient safety incidents in primary healthcare facilities and often occur during the prescribing and transcribing phases , the initial stages of the medication use process. This case study aims to examine in-depth a medication error incident involving the erroneous prescription of gentamicin skin ointment for an eye complaint in a patient. Data were collected through medical record reviews, documentation audits, incident reports, and in-depth interviews with healthcare workers involved in the prescribing, transcribing, and dispensing processes. The study results revealed several discrepancies in the prescribing phase , including illegible prescriptions, incomplete drug names, omissions of instructions for use, and the omission of important clinical information such as allergy history, prescription date, and patient physiological parameters. In the transcribing phase , errors occurred due to incorrectly transcribing the drug name and instructions for use. Systemic factors contributing to this incident include high workload, limited human resources, and suboptimal implementation of medication safety procedures in accordance with national pharmaceutical service standards. These findings emphasize the importance of improving prescription clarity, strengthening verification mechanisms, and adherence to SOPs in efforts to minimize medication errors and strengthen patient safety

    Hospitalization is a process that requires a person to be hospitalized for treatment, until they are allowed to go home. This process often causes stress and anxiety, especially for children, because they have to adapt to a foreign environment, be separated from their families, and undergo medical treatment. The purpose of this study was to determine the effect of bibliotherapy with picture story books on reducing children\u27s anxiety levels in the Lily Room of Anwar Medika Hospital, Sidoarjo. The research design used a Quasi-experiment with a one-group pre-test–post-test design. The population in this study were all children treated in the Lily Room of Anwar Medika Hospital. The sample was taken using an accidental sampling technique, obtaining 37 respondents. The research instrument used a questionnaire. Data were analyzed using the Wilcoxon Signed Rank Test. The results of the Wilcoxon test obtained a p-value of 0.000 <0.05, which means there is an effect of bibliotherapy with picture story books on reducing the level of hospitalization anxiety in children in the Lily Room of Anwar Medika Hospital, Sidoarjo. Bibliotherapy with picture books is highly effective in reducing anxiety in children because the text and illustrations in books are engaging and provide a distraction that can reduce anxiety

    Mental health is part of the health status that must be achieved at the optimal level. Indicators that can be used include a decrease in psychosocial problems and the incidence of people with mental disorders (ODGJ). A service that plays a fundamental role and directly impacts the community is Posyandu Jiwa. This service is provided by the community, from the community, and for the community. The main service providers at Posyandu are mental health cadres under the guidance and support of health workers at the local Community Health Center. Posyandu Jiwa provides services for people with mental disorders (ODGJ). Services provided include regular health screenings, skills or vocational development, spiritual guidance, Group Activity Therapy (TAK), and family therapy. The purpose of this study was to analyze the utilization of Posyandu Jiwa in the working area of Bloto Health Center in 2025. This type of research is quantitative with a Cross Sectional approach. The sampling technique is total sampling. The research data were obtained from questionnaires on ODGJ knowledge, cadre behavior, family support, and ODGJ motivation. Data analysis using Chi square. There is a relationship between knowledge and the utilization of Posyandu Jiwa in Bloto Health Center in 2025 with a p-value = 0.000 (p <0.05). There is a relationship between cadre behavior and the utilization of Posyandu Jiwa in Bloto Health Center in 2025 with a p-value = 0.001 (p <0.05). There is a relationship between family support and the utilization of Posyandu Jiwa in Bloto Health Center in 2025 with a p-value = 0.001 (p <0.05). There was a correlation between patient motivation and utilization of the Posyandu Jiwa (Mental Health Post) at the Bloto Community Health Center in 2025, with a p-value of 0.002 (p < 0.05). The factor most closely related to Posyandu Jiwa utilization was knowledge, with a p-value of 0.001 (p < 0.05). One effort that needs to be improved to further optimize Posyandu Jiwa utilization is to improve knowledge among cadres, families, and cooperative individuals with mental disorders

    Lifting Moving Management with Role Play Method On Volunteers Mojokerto Tanggap Bencana (MTB)

    Lifting and moving disaster victim management skills  are important competencies for volunteers. Incorrect techniques can aggravate the victim\u27s injury, but it is also at high risk of causing back injury to volunteers. Mojokerto Tanggap Bencana (MTB) volunteers need effective training methods to ensure safe mastery of techniques. The Role Play method was chosen because it offers a simulation of real situations of hands-on practical learning. analyzes the effect of the application of the Role Play method  on the improvement of the knowledge and skills of lifting moving victims in MTB volunteers. This study used a quasi-experimental design of One-Group Pretest-Posttest with a total sampling technique of 56 active MTB volunteers. The intervention provided was lifting moving training using the Role Play method. Knowledge was measured using  an intervention pre-post test questionnaire  . Skills were measured using  a standard checklist on a score scale of 0-100. Data analysis was carried out by paired t-test. The implementation of lifting moving management by volunteers prior to role play training  is still low, with nearly half of respondents (48%) rated "Not Appropriate." However, role play training succeeded in substantially improving the competence of volunteers, as shown by a significant surge in respondents who carried out according to the SOP to (86%). The results of paired t-test confirmed that there was a significant influence of the method with a p value of 0.000<α(0.05), this proves the effectiveness of the role play method  in improving lifting moving management. Simulations that are close to field conditions, coupled with live feedback, allow volunteers to internalize  safe lifting moving techniques  and practice decision-making skills. The Role Play method  is effective in increasing the knowledge and management skills  of  lifting moving victims in MTB volunteers

    THE EFFECT OF DASH DIET EDUCATION USING THE PEER GROUP METHOD ON THE EATING PATTERNS OF DIABETES MELLITUS PATIENTS IN KALIBUNTU VILLAGE

    The management of diabetes mellitus (DM) includes diet management as an important  component  in  maintaining  blood  sugar  stability.  The  DASH  diet helps maintain electrolyte balance and cardiovascular health through calcium, potassium,  and  magnesium  intake.  Dietary  education  using  the  peer  group method  is  useful  for  sharing  experiences,  exchanging  information,  and providing and receiving emotional support.. This study aimed to determine the effect  of  DASH  diet  education  using  the  peer  group  method  on  the  eating patterns  of  Diabetes  Mellitus  (DM)  patients.  The  research  design  was  pre- experimental  with  a  one-group  pretest-posttest  approach.  The  study population consisted of 40 respondents, with a total sampling technique, and the data were analyzed using the Wilcoxon Sign Rank Test. After receiving DASH  diet  education,  most  patients  had  good  eating  patterns.  The  study results showed that DASH diet education using the peer group method had an effect on the eating patterns of Diabetes Mellitus patients. Based on the study findings,  it  was  expected  that  community  nurses  could  use  DASH  diet education interventions to improve blood sugar levels, lipid profiles, and blood pressure in diabetes mellitus patients

    FACTORS ASSOCIATED WITH DEPRESSION AMONG CHRONIC KIDNEY DISEASE PATIENTS RECEIVING HEMODIALYSIS

    Chronic  kidney  disease  (CKD)  is  one  of  the  therapies  for  CKD  is hemodialysis. Hemodialysis can have psychological effects, one of which is depression. The purpose of this study was to identify factors associated with depression in patients with chronic kidney failure undergoing hemodialysis. This  is  a  quantitative  study  with  a  descriptive  correlational  design  using  a cross-sectional design on 52 samples with purposive sampling. The study was conducted  using  the  Beck  Depression  Inventory  II  questionnaire,  Fatigue Severity  Scale,  Pittsburgh  Sleep  Quality  Index,  and  data  were  tested  using Kendall\u27s Tau b and c tests and Ordinal Logistic Regression. The study found that 46.2% of CKD patients experienced severe depression. Depression was more  prevalent  among  CKD  patients  aged  41–55  years  (40.0%),  females (61.5%),  married  individuals  (65.4%),  those  with  a  high  school  education (40.4%), those who had undergone hemodialysis for 13–36 months (44.2%), respondents  with  poor  sleep  patterns  (55.8%),  and  those  with  comorbid conditions  (hypertension)  (34.6%).  The  results  of  Kendall\u27s  tau  analysis showed that factors associated with depression include age, gender (p=0.001), marital  status  (p=0.001),  duration  of  hemodialysis  (p=0.016),  education (p=0.001),  sleep  patterns  (p=0.001),  fatigue  (p=0.001),  and  comorbidities (p=0.001), while age did not show a significant association (p=0.057). Among the  factors  analyzed  using  the  partial  test,  no  single  factor  was  dominant  in influencing  depression  levels  in  CKD  HD  patients.  The  odds  ratio  for education was the highest, with a value of 603,801.0287. Clinically, the odds ratio  results  indicate  that  education  is  the  dominant  factor,  with  a  60-fold influence on depression in CKD HD patients.

    K3 maintains the safety of nurses in every health service facility to carry out their performance and maintain the safety of recipients or users of health services. Purpose of the study: to determine the effect of the implementation of SMK3 on the performance of nurses in the operating room of Sidowaras Hospital, Mojokerto Regency. This study uses a quantitative method using an analytical observational research design with a cross-sectional approach.. PopulationIn this study, all employees of the Sidwaras Hospital operating room, totaling 45 nurses, were involved and a sample of 35 respondents was obtained.Questionnaire data collection technique. Kthen tested using the Chi-Square Test and Logistic Regression Test. Results :The results of the study show that 0.005 <0.05 means that there is an influence between occupational health and performance. And 031 <0.05 means that there is an influence between occupational safety and performance. Occupational safety has a sig wald value of 0.955> 0.05 which means that occupational safety does not provide a significant partial influence on performance. Occupational health has a sig Wald value of 0.031> 0.05 which means that occupational health does not provide a significant partial influence on performance. Conclusion and suggestions:Hospitals are expected to continue to pay attention to and improve the implementation of the occupational safety management system. They need to provide facilities and infrastructure for occupational safety in each work unit, especially operating rooms, to reduce the risk of work accidents

    THE EFFECT OF BALLOON BLOWING THERAPY IN PATIENTS WITH PPOK IN IMPROVING OXYGEN SATURATION

    COPD is a disease of the respiratory system characterized by progressive and irreversible airway obstruction, accompanied by inflammation and systemic effects leading to airway narrowing and increased sputum production. This results in difficulty breathing and shortness of breath, which can be seen through increasingly strong contractions of the respiratory muscles. Prevention can be done through respiratory rehabilitation, such as blowing balloon exercise, which aims to better improve oxygen saturation. One way to measure this is by monitoring oxygen saturation. This study aims to determine the effect of balloon blowing on improving oxygen saturation in COPD patients. The research design used a one group pre-post design. The population of this study were patients with COPD, 30 samples were selected purposive sampling technique and data analysis using Wilcoxon test. The results showed that there was an effect of balloon blowing therapy on increasing oxygen saturation in COPD patients at UNS Hospital (p value 0.000). Researchers recommend that blowing balloon exercise be done regularly and consistently, with three blows every morning for three consecutive days, so that COPD patients can achieve more optimal oxygen saturation and reduce the risk of exacerbations.     EFFECTIVENESS OF ERGONOMIC EXERCISE ON BLOOD SUGAR IN DIABETES MELLITUS PATIENTS

    Diabetes is a chronic metabolic disorder characterized by hyperglycemia because the pancreas is unable to produce enough insulin and the body is unable to use insulin effectively. If diabetes mellitus is not treated properly, it will cause acute and chronic complications. Physical activity training is one of the non-pharmacological management of diabetes mellitus. One form of physical activity training is ergonomic gymnastics. This study aims to determine the effect of ergonomic exercises on blood sugar levels in patients with diabetes mellitus in Krembung Public Health Center. This research is a quantitative study using Pre-Experimental Design One Group Pre- Test Post-Test with 36 respondents. This study shows that there is a decrease in the average blood sugar level on the first day of 234 mg/dL to 209.5 mg/dL, on the second day the average blood sugar level from 211.5 mg/dL to 187.7 mg/dL while the average blood sugar level of the third meeting from 175.7 mg/dL to 148.8 mg/dL. Ergonomic gymnastics involves the main muscles that can make insulin receptors increase and more sensitive so as to reduce blood sugar levels. This is evidenced by the results of the analysis using the Paired T-Test test obtained a p-value of 0.000 (<0.05) which shows the effect of ergonomic exercises on blood sugar levels in patients with diabetes mellitus in Krembung Public Health Center. Based on the results of this study, ergonomic exercises are important to be done routinely in patients with diabetes mellitus to reduce blood sugar levels.

    FACTORS RELATED TO THE PRACTICE OF SELLING LIMITEDRESTRICTED OTC DRUGS IN SMALL SHOPS IN NORTH OBA DISTRICTIN 2025

    Based on data obtained from the Tidore Islands City Health Office, an inspection was conducted on kiosks/small stalls/stalls in several areas of North Oba District in 2023. The inspection results showed that of the 96 kiosks/small stalls in North Oba District, 74 (77%) were found to be selling restricted over-the-counter drugs despite not having a permit as a legal drug kiosk. The purpose of this study was to determine the factors related to the practice of selling restricted over-the-counter drugs in small stalls. This type of research used quantitative research with a descriptive design.cross sectional.There are four variables that have a relationship with the practice of selling restricted over-the-counter drugs: knowledge (p = 0.001), social environment (p = 0.003), economy (p = 0.000), supervision (p = 0.000). The knowledge of business actors and residents in the social environment greatly influences the practice of selling restricted over-the-counter drugs by business actors in small shops. Poor knowledge of both sellers and buyers about the dangers of restricted over-the-counter drugs sold without a doctor\u27s prescription is still very high. In addition, low economic conditions and government supervision of the distribution of restricted over-the-counter drugs in small shops without official permits are among the factors that continue to be widespread in the practice of selling restricted over-the-counter drugs
