Injuries of the facial skeleton in the elderly. Observations of the Maxillofacial Surgery Clinic of the Fryderyk Chopin University Clinical Hospital in Rzeszów
Promotor: dr hab. med. Bogumił Lewandowski, prof. UR - 89 s.Wraz ze wzrostem średniej długości życia i populacji osób w podeszłym wieku (powyżej
60. roku życia wg WHO) rośnie liczba urazów części twarzowej czaszki w tej grupie wiekowej.
Praca obejmuje materiał kliniczny 505 chorych po urazie części twarzowej czaszki, leczonych
w Klinice Chirurgii Szczękowo-Twarzowej USK im. Fryderyka Chopina w Rzeszowie, w latach
2000–2019. Celem pracy jest retrospektywna wieloczynnikowa analiza danych dotyczących
przyczyn, diagnostyki, sposobów i wyników leczenia chorych w dwóch okresach 2000–2009
i w latach 2010–2019.
Zaobserwowano wzrost liczby leczonych chorych w latach 2010–2019 o 37%,
porównując do lat 2000–2009. Wiek hospitalizowanych chorych wahał się pomiędzy 60 a 94
lata, mediana wynosiła 69. Do celów analizy statystycznej podzielono wiek podeszły na 3
grupy, tj.: 60–70, 71–80 i powyżej 80 lat. Najliczniejszą grupę stanowili chorzy w wieku 60–70
lat, tj. 58%, chorzy w wieku 71–80 lat stanowili 28,1%, chorzy w wieku powyżej 80 lat 13,9%.
Przeważającą grupę pacjentów stanowili mężczyźni 70%, zaś kobiety 30%.
Główną przyczyną obrażeń części twarzowej czaszki w każdej badanej grupie wiekowej
i w dwóch badanych okresach był upadek z własnej wysokości – 35,5% przyczyn złamań.
Dominowały złamania kompleksu jarzmowo-szczękowego – 42,2% oraz żuchwy – 31,1%.
Nie stwierdzono współistniejących obrażeń u 66,3% chorych. Złamaniom szczękowo-
twarzowym najczęściej towarzyszyły urazy OUN – w 16,2% oraz narządu ruchu w 11,3%.
U większości chorych, tj. 64,6% stwierdzono choroby współistniejące, wśród których
najczęściej występowały schorzenia kardiologiczne (46,5%) oraz endokrynologiczne
i metaboliczne (17,4%).
Większość chorych – 60% leczono chirurgicznie, 15% za pomocą metod zachowawczo-
ortopedycznych, u 25% wykorzystano metody chirurgiczno-ortopedyczne. Stwierdzono
najmniejszą liczbę powikłań po leczeniu w grupie chorych leczonych za pomocą metod
zachowawczo-ortopedycznych.With the increase in life expectancy and the elderly population (over 60 years of age
according to WHO), the number of injuries of the facial part of the skull in this age group is
increasing. The work covers the clinical material of 505 patients after trauma to the facial part
of the skull, treated at the Fryderyk Chopin Maxillofacial Surgery Clinic USK in Rzeszów, in
the years 2000–2019. The aim of the study is a retrospective analysis of data on the causes,
diagnostics, methods and results of treatment of patients in two periods 2000–2009 and in the
years 2010–2019.
An increase in the number of treated patients was observed in the years 2010–2019 by
37% compared to the years 2000–2009. The age of hospitalized patients ranged between 60 and
94 years, the median was 69. For the purposes of statistical analysis, the age of the elderly was
divided into 3 groups, i.e. 60–70, 71–80 and over 80 years. The largest group were patients aged
60–70, i.e. 58%, patients aged 71–80 accounted for 28.1%, patients over 80 years old 13.9%.
The majority of patients were men 70% and women 30%.
The main cause of injury to the facial part of the skull in each examined age group
and in the 2 examined periods was a fall from one’s own height, i.e. 35.5% of fracture causes.
Fractures of the zygomatic maxillary complex 42.2% and mandible 31.1% predominated. No
concomitant injuries were found in 66.3% of patients. Maxillofacial fractures were most often
accompanied by CNS injuries, i.e. 16.2% and musculoskeletal injuries (11.3%). The majority
of patients, i.e. 64.6%, had comorbidities, among which the most common were cardiological
(46.5%) and endocrine and metabolic diseases (17.4%).
Most patients, i.e. 60%, were treated surgically, 15% by conservative-orthopedic
methods, and 25% were treated with surgical and orthopedic methods. The lowest number of
complications after treatment was found in the group of patients treated with conservative-
orthopedic methods
Atypical presentation of extra-skeletal Ewing’s sarcoma in a 57-year-old female – a case report
Approved by Bharat Heavy Electricals Limited, Power Sector Southern Region, Chennai, Tamilnadu, India.Introduction and aim. Malignant soft tissue tumors exhibiting similar histological and immunohistochemical characteristics to Ewing sarcoma of the bones are referred to as extra-skeletal Ewing sarcoma within the pathology research domain. These tumors fall under the broader classification of Ewing sarcoma family of tumors, which encompasses Ewing sarcoma of the bones, extra-skeletal Ewing sarcoma, and primitive neuroectodermal tumor, the latter demonstrating a more pronounced neural differentiation compared to Ewing sarcoma of the bone. Extra-skeletal Ewing sarcoma stands out as a rare, aggressive, and rapidly growing malignant soft tissue tumor characterized by a notable recurrence rate and a predilection for occurrence in males. The roots of recognizing extra-skeletal Ewing sarcoma trace back to 1975 when Angervall and Enzinger reported the inaugural case. This study aims to underscore the significance of recognizing diverse clinical presentations for precise diagnosis and effective patient care of Extra-skeletal Ewing sarcoma in an elderly patient.
Case description. In the context of our pathology research, a noteworthy case involves a 57-year-old female presenting with a mass in the left iliac fossa. The diagnosis, established through a comprehensive approach involving Imaging, histopathological examination, immunohistochemistry, and molecular studies such as fluorescence in situ hybridization, confirms the nature of the tumor as extra-skeletal Ewing sarcoma. This case adds to the understanding and documentation of this distinct variant through a multi-modal investigative process.
Conclusion. This case report contributes to the existing literature by shedding light on an atypical presentation of extra-skeletal Ewing sarcoma in an older patient. Understanding the varied clinical manifestations and incorporating advanced diagnostic techniques, such as fluorescence in situ hybridization and immunohistochemistry, is pivotal for accurate diagnosis and optimal patient management
Information overload in a contemporary human’s education process
Information overload may disturb human’s personality shaping process. In this unfavourable space an excessive absorption of an attention takes place and it results in data dumping of his/her mental stowage. Overfilling with “info-rubbish” may preclude collecting needed, substantive and reliable data. Efficiency and productivity of an educational influence may become limited because of shallow and uncritical approach to the information sources.
The exploration is an attempt of information overload analysis in aspect of human’s educational process. On the base of the literature of the subject, the exploration of the chosen educational aspects of “info-mass” was instigated. The author analyzed an information overload phenomenon from a historical perspective, lining out a terminology interpretative context. It is, undoubtedly, significant to find out such strategies that may reduce an influence of unfavourable incentives for human’s personality shaping process. The prophylaxis may become an only effective way of human’s protection for civilization changes’ side effects
Assessment of behavior and barriers of post-exposure prophylaxis for prevention of rabies in patients attending a tertiary care center in eastern India – a mixed method approach
All subjects gave informed consent for inclusion before participating in the study. The study was conducted under the Declaration of Helsinki, and the protocol was approved by the Ethics Committee of Kalinga Institute of Medical Sciences (KIIT/KIMS/IEC/1325/2023).Introduction and aim. Rabies from animal bites, although preventable, is almost 100% fatal, with the dog being the most common infected animal. Several factors influence the timely initiation of post-exposure prophylaxis (PEP) which needs to be explored. This study aimed to determine the facilitating and inhibiting factors in initiating PEP among animal bite victims attending a tertiary care center in Eastern India.
Material and methods. A cross-sectional study was conducted among 150 consenting animal bite victims attending casualty and medicine as out-patients in a tertiary care hospital. A mixed-method study approach was undertaken to explore the facilitating and inhibiting factors for the timely initiation of PEP using a pre-tested and semi-structured questionnaire and in-depth interviews. Data were analyzed using SPSS software version 20.
Results. The most common biting animal were dogs (96.2%). Around 102 patients (68%) were males and 48 (32%) were females. Timely initiation of PEP was seen in 112 (74.7%) of the study sample while delay was seen in 38 (25.3%). The inhibiting factors were the absence of an accompanying person (54.67%), the absence of vaccines and immunoglobulins in the nearest health facility (50.67%), referral from other health facilities (44%) and preferred other modalities of treatment (24.67%). Delays in initiation of PEP was significantly associated with gender (p=0.018), place of residence (p<0.001), those living more than 20 km from a health facility (p=0.014), educational status (p<0.001), and those with monthly income <10000 INR (p<0.01).
Conclusion. Lower economic status, education, and accessibility to health facilities were major inhibiting factors causing a delay in the initiation of PEP. Further, community-based studies are warranted

The physiological efficiency of cells largely depends on the possibility of metabolic adaptations to changing conditions, especially on the availability of nutrients. Central carbon metabolism has an essential role in cellular function. In most cells is based on glucose, which is the primary energy source, provides the carbon skeleton for the biosynthesis of important cell macromolecules, and acts as a signalling molecule. The metabolic flux between pathways of carbon metabolism such as glycolysis, pentose phosphate pathway, and mitochondrial oxidative phosphorylation is dynamically adjusted by specific cellular economics responding to extracellular conditions and intracellular demands. Using Saccharomyces cerevisiae yeast cells and potentially similar fermentable carbon sources i.e. glucose and fructose we analyzed the parameters concerning the metabolic status of the cells and connected with them alteration in cell reproductive potential. Those parameters were related to the specific metabolic network: the hexose uptake - glycolysis and activity of the cAMP/PKA pathway - pentose phosphate pathway and biosynthetic capacities - the oxidative respiration and energy generation. The results showed that yeast cells growing in a fructose medium slightly increased metabolism redirection toward respiratory activity, which decreased pentose phosphate pathway activity and cellular biosynthetic capabilities. These differences between the fermentative metabolism of glucose and fructose, lead to long-term effects, manifested by changes in the maximum reproductive potential of cells. This research was funded in whole by the Polish National Science Centre (Grant No.: 2021/05/X/NZ3/00162)

The Orthodox Church and its significance in Siberia
Almost all of Siberia is part of the Russian Federation. As it is well known, the main, and once the state religion prevailing in Russia is Orthodoxy. It is clear that after the capture of new Siberian territories it began to spread there. This publication describes how the spiritual development of Siberia took place, how long it took, with what purpose it was carried out and what results were achieved by the missionary activity of the Russian Orthodox Church among the indigenous population of Siberia, who professed and confessed paganism, as well as Islam and Buddhism. The methods to which the missionaries resorted in order to induce the natives to accept the new faith are also mentioned. Examples of several Siberian peoples and their attitude to the Orthodox faith at present are also considered
