    Optimization of extraction and lyophilization conditions of Feretia apodanthera (Rubiaceae) leaf powders for galenic formulation

    Feretia apodanthera (Rubiaceae) is used in traditional medicine for the treatment of various pathologies. Its leaves are chewed and swallowed to treat stomach aches, sleep disorders, etc. With the aim of improving the extraction method in order to obtain high contents of phytochemical compounds and good antioxidant activity, this study aimed to highlight the extraction and lyophilization conditions of two extracts (aqueous and hydro-ethanolic) leaves of Feretia apodanthera. An aqueous and hydro-ethanolic extraction was carried out from the leaf powder of Feretia apodanthera, after a maceration plan (mass/volume ratio and extraction time). The extracts obtained were freeze-dried (flask size, filling volume and freeze-drying time) in order to determine the optimal extraction conditions. A phytochemical screening of the extracts with extreme yields of different solvents was carried out by TLC, which led to the determination of certains compounds of interest and the evaluation of the antioxidant activity by the DPPH and FRAP method. The optimal maceration conditions were 02 hours and m/V ratio of 1/20 for the aqueous extract and those of the ethanolic extract were 30 min and m/V ratio of 1/20. The optimal freeze-drying conditions were 60h and a 500ml flask 1/3 filled. The presence of flavonoids, tannins, saponosides, coumarins, triterpenes and sterols has been highlighted. The phenolics compounds contents of hydro-ethanolic lyophilisates were higher than those of aqueous lyophilisates. There was no statistical difference between the antioxidant activity of the different extracts. The optimal conditions will serve as a standard for the production of the extract necessary for the formulation.     Effects of Intravenous Lidocaine as an Analgesic Adjuvant in Oncological Pain

    Objectives: To verify that intravenous lidocaine is effective as an adjuvant analgesic in pain for hospitalized cancer patients at the National Institute of Cancerology, Mexico. Methods: Clinical records of patients hospitalized at the National Institute of Cancerology who received intravenous lidocaine infusion as an adjuvant to pain during the 4-year period from November 1, 2019, to October 31, 2023, were reviewed. Patients who met inclusion and exclusion criteria were included. A statistically significant measure was defined as a decrease equal to or greater than 4 points in the NRS value after lidocaine administration. Results: A total of 179 patients were included; 46.4% were men, and 53.6% were women. The most common painful syndrome was somatic (54%). Patients with neuropathic pain received a higher lidocaine dose (mg/kg/hr) of 1.9 ± 0.79. Regarding adverse effects, 1.7% of patients experienced them. It was established that a reduction of 4 points with respect to the previous value on the numeric rating scale (NRS) would be considered statistically significant. A statistically significant association was found between patients under 38 years old and a higher risk of not achieving the objective (p 0.05, CI 0.95–1 OR 0.9). Patients who received lidocaine infusions for less than 4.63 days also had a risk of not reaching the goal (p 0.02, CI 0.7–0.9, OR 0.8). Conclusions: This study demonstrated that the analgesic adjuvant use of intravenous lidocaine infusion is an effective and well-tolerated analgesic intervention for oncology patients.     A Case Report on Iatrogenic Cushing Syndrome with Diabetes Mellitus

    Cushing syndrome (CS) is a condition caused due to high levels of cortisol in which weight gain and fat tissue build-up take place. Generally, patients with Cushing syndrome also suffer from diabetes. A 57-year-old female patient with aggravated cortisol levels. Laboratory investigations of the patient were performed and they showed decreased haemoglobin levels, but increased vitamin B12 levels, and TSH levels. The patient was prescribed a combination of medications to manage anemia, anemia, and diabetes. The patient\u27s medical history included splenomegaly, cholelithiasis, panniculitis, and kidney enlargement.     Evaluation of Interleukin 4, High Sensitivity C-Reactive Protein and Activity of Alanine Transaminase Among Cooks in Nnewi

    Soot arises from burning wood, oils, tires, and other hydrocarbon or organic compounds. Soot is widespread and has a massive detrimental effect on human health, climate, and air quality. The serum levels of Interleukin 4, High sensitivity C-reactive protein, and alanine transaminase activity were evaluated. This cross–sectional study recruited 90 participants consisting of 45 participants exposed to soot (test group) and 45 participants not exposed to soot (control group). The Faculty of Health Sciences and Technology Ethics Committee at Nnamdi Azikiwe University granted ethical approval, and participants’ informed consent was acquired. Interleukin 4 and high sensitivity C-reactive protein levels were determined using the Sandwich ELISA method while the activity of alanine transaminase was determined using the spectrophotometric method. Questionnaires were used to obtain the socio-demographic data of the participants and the body mass index (BMI) was calculated using weight (kg)/height2 (m2). Independent t-test and Pearson’s correlation coefficient were used for statistical analysis and significance was set at p<0.05. The results showed that the mean serum activity of alanine transaminase (9.30 ± 0.28) was significantly lower in the participants exposed to soot compared with the control (9.48 ± 0.37) (p<0.05). However, there was no significant difference in the mean serum levels of Interleukin 4 and high sensitivity C-reactive protein of participants exposed to soot compared with the control (p>0.05). Hence, exposure to soot through the use of firewood did not predispose cooks to systemic inflammation, or cardiovascular and hepatic dysfunctions.     Strategies for Managing Anemia in Chronic Kidney Disease

    Anemia is a common complication in chronic kidney disease (CKD) and is associated with decreased quality of life, deteriorating kidney function, increased illness, higher mortality rates, and elevated healthcare costs. Management of CKD-related anemia aims to enhance renal function (when feasible) and bolster red blood cell production. Anemia in chronic kidney disease (CKD) stems from a variety of factors, including reduced erythropoietin (EPO) production, inadequate iron availability or functionality, and inflammation leading to heightened levels of hepcidin, among other causes. Typical treatment for CKD-related anemia involves administering oral or intravenous iron supplements in conjunction with erythropoiesis-stimulating agents (ESAs). Over the past decade, erythropoiesis-stimulating agents (ESAs) have proven effective in boosting hemoglobin (Hb) levels for many patients. However, concerns about their safety, particularly regarding increased cardiovascular risk—especially at high doses and in hyporesponsive patients—have emerged. As a result, there\u27s been a shift towards adopting more cautious Hb targets. Iron deficiency is widespread among CKD patients, particularly in those not undergoing dialysis. While iron supplementation has traditionally been used to support anemia management, there\u27s growing recognition of the potential benefits of intravenous iron therapy, especially in the context of heart failure. HIF prolyl hydroxylase inhibitors are promising new drugs being clinically evaluated as alternatives to ESAs for anemia treatment in CKD patients. Initial data suggest they effectively boost endogenous erythropoietin production with potential benefits like lower peak concentrations, efficacy in inflamed patients, and improved iron utilization. However, further research is needed to confirm their safety and efficacy in the long term use.     Piliostigma thonningii leaves are used in Nigeria folk medicine for the treatment and management of diverse ailments. This study was carried out to investigate the antimicrobial activity of the secondary metabolites produced from endophytic fungi isolated from the leaves of Piliostigma thonningii. The isolation of endophytic fungi was carried out according to the standard procedures. The fungi were subjected to solid-state fermentation on rice medium and the secondary metabolites extracted using ethyl acetate. The fungal crude extracts were screened for antimicrobial activity against selected clinically important microbes using the agar well diffusion method. The fungus with the best antimicrobial activity was molecularly characterized and its secondary metabolites profiled using Gas Chromatography - Mass Spectroscopy to establish the correlation between the observed activity and its phytochemical composition. The fungal extracts showed antimicrobial activity with inhibitory zone diameter ranging from 2.00 to 9.00 mm. The secondary metabolites of Aspergillus fumigatus (the most bioactive endophyte) contain 50 compounds with several of them having established antimicrobial activity. The results of this study suggest that Aspergillus fumigatus an endophytic fungus associated with P. thonningii could be a promising source of novel bioactive compounds with pharmaceutical, agricultural and industrial importance.     Nutritional and Therapeutic Values of Cow Milk: A Comprehensive Review

    Milk is the most necessary food for humans because it provides almost all nutrients such as the most nutritious protein, calcium, essential fatty acids, water-soluble vitamins, and other bioactive compounds responsible for maintaining health and other physiological functions. Milk has also been used effectively in the Unani system of medicine for centuries to improve complexion and to treat other ailments (especially dermatological disorders). For skin brightening, Unani physicians mentioned the use of the paste made up of powdered tukhm e kharpazah (seeds of musk melon), aarad e nakhud (powdered chickpea) each 2 parts, arad e baqla(powdered broad beans), aarad e jau (powdered barley) each 1 part, adas muqashshar (powdered lentil), nishasta (corn flour), kateera (tragacanth gum) each half part, tukhm e turb (radish seeds) 1/4th part, za’fran (saffron) a little and milk at night on the face and advice to wash the face with the decoction of banafsha (sweet violet) and post kharpaza (musk melon fruit epicarp) in the morning. Milk has several therapeutic actions too e.g. Musammin e Badan (adipogenous), Muhallil (anti-inflammatory), Mugharri (glutinous- Mugharri drug is a cold and dry substance with a small amount of viscid moisture that coats the surface and constricts pores. It moisturizes the skin and makes it appear lighter), Mohsin e laun (fairness activity), Ghassal (irrigator), Tiryaq (antidote), and Muqawwi (general tonic), etc. There are some scientific reports on the anti-hyperpigmentation effect of cow milk in which a paste of cow milk and powdered roasted red lentils was tested to enhance the complexion. They found significantly effective result on the Fitzpatrick scale, enhanced complexion, and reduced tanning as well as dullness.     Medication errors are regarded as a significant contributor to the morbidity and mortality rates among patients. These errors encompass any mistake made during the prescription, dispensing, administration, or consumption of medication, as well as the monitoring of therapy. Such errors often stem from factors such as insufficient knowledge, limited staff and resources, issues with medication naming and labeling, and inadequate communication among patients, physicians, and pharmacists. Nonetheless, it is crucial to detect these errors, as shortcomings in healthcare systems can lead to seemingly minor mistakes like misuse and abuse, which may ultimately result in more serious errors. The act of reporting a medication error holds great importance in enhancing the quality of life for patients and reducing the financial burden of re-hospitalization. The primary responsibility of a clinical pharmacist is to identify and assess medication errors with the aim of enhancing patient safety and therapeutic results. By closely monitoring the drug therapy administered to patients in a hospital setting and promptly informing healthcare providers of any inconsistencies discovered, the likelihood of medication errors occurring in hospitalized patients can be minimized.     Examining the Bioactive Elements of Urtica urens Hydroalcoholic Extract and Its Possible Antidepressant Effects in Mice

    The majority of antidepressant medications reduce depression symptoms, but they also have a number of undesirable side effects. The hunt is on for medications that are both more effective and well-tolerated. In light of this, the current investigation set out to assess the hydroalcoholic extract of Urtica Urens L.\u27s aerial parts\u27 (HAUU) antidepressant potential in mice. The Tail Suspension Test (TST) and the Forced Swimming Test (FST) on mice were used to assess it. For 14 days, the HAUU (250 and 500 mg/kg, po) was given orally to distinct groups of 20–25-weight Swiss albino mice in order to conduct TST and FST testing. The presence of alkaloids, glycosides, phenols, flavonoids, and tannins was discovered through phytochemical examination. The HAUU demonstrated a dose-dependent decrease in the amount of time that mice were immobile. Mice\u27s immobility duration was considerably shortened in both the FST and TST when 500 mg/kg of HAUU was administered. The extract\u27s effectiveness was discovered to be on par with fluoxetine (20 mg mg/kg, po). It was discovered that when given orally at a dose of 2000 mg/kg, it was toxicologically safe and did not cause any mouse deaths. The results of the current investigation indicate that the HAUU is toxicologically safe and has strong antidepressant activity, as demonstrated by the TST and FST tests.
