    KNEE FLEXION EXTENSION AND STRENGTHENING (FELS) EXERCISE TO PAIN INTENSITY IN KNEE OSTEOARTHRITIS PATIENTS

    Introduction. Osteoarthritis (OA) is a degenerative disease due to the thinning of the joint cartilage, mostly in the knee joint (89.91%) with the pain manifestation. If the pain is not managed, it could lead to the limitation of movement and disruption of daily activities. Having physical exercise regularly could reduce pain. The purpose of study was to determine the effect of knee fl exion extension and strengthening (FELS) exercise in reducing pain intensity. Methods. This was a quasi-experimental study with pretest-posttest control randomized design, where 70 respondents were recruited and consisted of 53 in intervention and 17 in control group. Result. The statistical analysis used ordinal regression revealed (1) FELS exercise was effective in reducing pain intensity (p = 0.013); (2) a signifi cant difference in pain intensity before and after FELS exercise (p = 0.000); (3) a signifi cant difference of pain intensity in the intervention group compared to the control group (p = 0.004); (4) effect of other activities in reducing pain intensity (p = 0.042). While the factors of age, gender, pain experience, anxiety and support system were not signifi cantly effective in reducing pain. Discussion. FELS exercise was helpful in reducing pain intensity in patients with knee osteoarthritis. It was then suggested to OA patients are requested to perform regular FELS exercise while the families are keep on giving support to the patient to do FELS exercise.Keywords: Knee fl exion extension, strengthening exercise, pain intensity, knee O

    LOW BACK PAIN IN ICU AND EMERGENCY UNIT NURSES AT SARI MUTIARA GENERAL HOSPITAL MEDAN

    Introduction. The nurses who works at Intensive Care Unit (ICU) and Emergency Unit often suffering Low Back Pain (LBP) because they have high mobility and unergonomic condition. The purpose of this study were to identify the correlation among age, body mass index and working position of nurses with the low back pain who work in ICU and emergency unit at Sari Mutiara Indonesia general hospital. Methods. The data obtained was analyzed by analytic correlational with cross-sectional approach. The Sampel of this study was recruited with total sampling as 31 nurses. The data was analyzed by spearman correlation with α = 0.05. Result. The result of this study found that there was a signifi cant correlation between age with low back pain (r = 0.466 ; p 0.05 and r = - 0.172;p >0.05), respectively. Discussion. It can be recommended to occupational safety team to organize training the manner of working according to ergonomic standard, in addition to develope standar operating procedure for nurses in the hospital.Keywords : Risk factors, low back pain, nurs

    LOW OXYGENATION STATUS INCREASES NAUSEA-VOMITING INCIDENCE IN HEMODIALYSIS PATIENTS

    Background and Objective: Data from Indonesia Basic Health Research (2013) states that the chronic renal failure in Indonesia is increasing, especially in Yogyakarta with the prevalence of chronic renal failure of 0,3%. If the patients of chronic renal failure are in End Stage Renal Disease (ESRD), the kidney needs replacement therapy to help its function. This therapy is called Continuous Renal Replacement Therapy (CRRT) or Hemodialysis (HD). Hemodialysis therapy may influence to the imbalance of oxyhemoglobin in the blood. Patients undergoing hemodialysis may experience intradialytic nausea and vomiting. The objective of this study is to identify whether there is a relationship between pre-dialysis oxygenation status through oxygen saturation (SpO2) and respiratory rate (RR) examination and the intradialytic nausea-vomiting occurrence. Method: This research is a comparative research with analytical cross sectional design. This research was conducted in hemodialysis room in Public Hospital of Panembahan Senopati Bantul. The subject of the research was taken using total sampling, by paying attention to research ethics. The total research subjects are 183 respondents. The researchers measured the oxygen saturation and patients’ respiratory rate and examined the intradialytic nausea and vomiting complaints. Result: The analysis result of Fisher’s exact in this research shows p value of 0,000 both in bivariate analysis of oxygen saturation to the nausea and vomiting occurrence and in bivariate analysis of the respiratory rate to the nausea and vomiting occurrence. The multivariate analysis employing regression logistic shows that the OR of oxygen saturation is 73,57, this means that the measurement of the abnormal oxygen saturation has the chance of seventy three times more to the nausea and vomiting occurrence, if compared to the patients with normal oxygen saturation. Conclusion and Suggestion: Intradialytic nausea and vomiting is one of the causes of the imbalance of oxyhemoglobin. The readiness of pre-dialysis oxygenation status needs to be noticed so that the nausea and vomiting complaints can be anticipated earlier

    OUTCOME PRESENT TEST METHOD (OPT) IMPROVE THE SATISFACTION OF CLINICAL REASONING STUDENT PROFESSION IN MEDICAL NURSING SURGERY STAGE

    Nursing clinical education is an instrument in improving the clinical reasoning skills of students. One of learning models of clinical reasoning that is used to improve clinical reasoning skills in nursing education is the Outcome-Present Test Model (OPT). OPT is a nursing process model that is designed to develop the skills of learners’ clinical reasoning and the focus of the model are the results (outcomes) and thinking behind (backward) to change the client from his health status at this time (the present state) to the desired state (outcome ). In other words, a very simple model that consists of only 1 (one) page makes the learning improving clinical reasoning skills more easily and reduce the element of copy and paste in the documentation. While peer learning is an effective strategy to address the problem in which the students will be divided into several groups of 2-3 students.To obtain the result, this learning model uses research development method with the respondents 10 clinics and institutions preceptor and 30 students who practice in medical surgical nursing stase.The research results shows the level of student satisfaction after using Outcome-Present Test (OPT)-peer learning for 4 weeks, it affected with the value of α: 0,001. By using this model, not just a clinical reasoning that they get but they also will understand about how to use the diagnosis of NANDA, NIC and NOC. The next suggestion, this model can be a format in clinical learning in other departments

    MODERN WOUND IRRIGATION DEVICE (MWID) REDUCE THE NUMBER OF BACTERIA IN DIABETIC ULCER PATIENTS

    Background and Purpose: One of the micro-vascular complications in patients with diabetes mellitus is a diabetic ulcer. One of he goals of care in diabetic ulcers is to reduce infection in the wound area by doing a cleansing method particularly utilizing irrigation techniques. Irrigation technique aims to reduce the number of bacteria in the wound, so that the inflammatory process is expected to be more quickly achieved. Many diabetic ulcer care-related research but researches on irrigation tools on diabetic ulcers are very minimal. This study aims to determine the effect of Modern Wound Irrigation Device (MWID) to decrease the amount of bacteria on patients with diabetic ulcers. Method: This study was utilized quasi-experimental post test only with control group design. A total of 20 respondents in this study were divided into 10 respondents for intervention group that was given wound care using MWID. 10 respondents as control group that was given manual irrigation wound care. Results: The average number of bacteria in intervention group is 30,4x109 colonies/ml, while the average number of bacteria for control group is 59x109 colonies/ml. Analysis with independent t test was found that there is no significant difference between the average number of bacteria on the intervention and control groups with p value of (0.001 < 0.005). Conclusion and recommendation: the use of MWID for current wound irrigation is proven can reduce the amount of bacteria on diabetic ulcers as compared with manual irrigation technique. Further developmental researches utilize MWID tool is to modify the output of irrigation to reduce anaerobic bacteria for gangrene wound.Key words: Diabetic Ulcer, Wound irrigation, amount of bacteri

    Background and objectives. During this time the unit cost of clinical practice in hospitals that practice becomes a vehicle which has not been done calculation costing more Clinical practice based on estimates, propriety and comparison with the cost of clinical practices from other hospitals. The costing is sometimes caused problems. Currently the cost of student practice in teaching hospitals varies between Rp. 300,000 - Rp. 500,000 / person / month. The problem that exists is how the unit cost and how much it costs the right based on unit cost and other considerations that apply to students practice at the Hospital Clinic as a vehicle for clinical practice. The purpose of this study to determine the unit cost and set the cost of professional practice of nurses that will be proposed applicable professional nurses students at Hospital.Method. This research was a descriptive observational study by conducting a case study in the dr. Soewandhi hospital East Java. Calculation of unit cost carried by Activity Based Costing (ABC), costing more to do content analysis after conducting focus group discussions, as well as other considerations.Research result. The results using the ABC method showed that the unit cost to the cost of professional practice nurses in the hospital Dr. Soewandhi Surabaya is Rp. 460,000, - / month / person, while the cost of practice set by the RS Dr. Soewandhi is Rp. 1.200.000, - / month / person. There is a practice of the profession excess costs Rp. 740,000, -. RS Dr. Surabaya Soewandhi practice fees apply without using a standard based on the unit cost accordingly. Some educational institutions felt the cost was very heavy so the impact to the cancellation of the filing of the professional nurses student practice.Conclusions and recommendations. The hospital used a professional students practice nurses should use the standard fees in accordance with needs through precise calculations that the fees charged to students through educational institutions in accordance with the appropriate calculations.Keyword: ABC system - Profession nurses - Fee Practice

    PARENT NURTURE MODEL IN SHAPING BEHAVIOR OF ADOLESCENCE 12-15 AGES

    Introduction: The deviation problem of smoking activity an adolescent is come to anxious level for parents, teachers, and society. The correlation between parents nurture model and smoking activity of adolescent needs to be examined further. The purpose of this study was to analyze the correlation between parents nurture model with smoking activity of adolescent (12-15 years old). Method: This was correlational research with cross sectional approach. The sample were 84 adolescent (12-15 years old) at MTs Mojosari Nganjuk. The independent variables was parents nurture model and the dependent variable was adolescent smoking activity. Data were collected by using questionnare, then examined by using chi square with the level of significant α=0,05. Result: Statistical analysis had showed the low correlation between permissive parents nurture model with smoking activity of adolescent (12-15 years old) at MTs Mojosari Nganjuk (p=0,049; r = 0,210) and no correlation between democratic nurture model (p=0,554) and authoritative nurture model (p=0,418) with smoking activity of adolescent (12-15 years old) at MTs Mojosari Nganjuk, but only permissive model which correlate with smoking activity. The permissive parents with no control and demand caused adolescent to be feeling unimpeded to do smoking activity since there is no warning and punishment from the parents. Discussion: So that, School nurses should provide health promotion to parents in making appropriate parenting in adolescence. Parents should have the right parenting provided in accordance with the age and development of adolescents because appropriate parenting will have a positive impact on adolescent behavior. Further research on parenting questionnaires must be checked for cross-compatibility between questionnaire answers given adolescent and parents to know the truth in filling out the questionnaire.     RIGHT MEDICATION RELATED TO DRUG CENTRALIZED IN RSUD SIDOARJO

    Introduction. Centralized drug is a management of the entire drug which is entirely done by nurses to administration to patients. Right medication is the process of right drug administration which is done by nurses based on 6 rights of medication, and wary of side effects. The purpose of this study was to analyze the corelation between centralized drug, team leadership, and nurse`s knowledge with right medication among nurses. Methods.The design of the study was descriptive corelational with cross-sectional approach. The population was inpatient nurses in RSUD Sidoarjo. Total sample was 114 respondents was selected by purposive sampling. The independent variables in this study: centralized drug, team leadership, and nurse`s knowledge. The dependent variable was right medication. Data were collected by using questionnares for independent variables and dependent variable. Data were analyzed by using Binary Logistic Regression with degree of significance α>0,05. Results. Binary Logistic Regression test showed non significance level between centralized drug with right medication (P=0.501), team leadership with right medication (P=0.874), and nurses`s knowledge with right medication (P=0.243). Discussion. This study concluded centralized drug, team leadership, and nurse`s knowledge were good. But, there are nurses that have negative value at right medication, however right medication in RSUD Sidoarjo has majority positive value. Keywords: centralized drug, right medicatio

    Abstract The balance of body is one of the main factor in doing fungsional activity. In every activity, the body always need control of the body balance, because basically every phisic activity right static or dinamic will take someone on unstable position with big risk to having fell. Swiss Ball exercise as support is believed on labile surface will make spine have big chance to stabling the muscle between vebra and increase the dinamic balance to restrain the repeats stability. The research object to know The Effect of Swiss Ball Exercise toward The Body Balance to Less The Risk Fall of Older at UPT Social Service of Older and Children Under Five district Binjai and Medan in Year 2016. The research type is Quasi Experiment with pre test-post test one group only design method. Population in this research all of older at UPT Social Service district Binjai as much as 172 older with number of man is 81 and woman is 91, with Purposive sampling Technique that is 15 respondences. Data collecting using observation sheet with analysis that used is univariat: respondence characteristics, the body balance of older before and after doing Swiss Ball practice, and bivariat with Paired t-Test. The research result showing the average of body balance before doing Swiss Ball as much (Mean 38,07) and after (Mean 46,33). Conclusion: there is the effect of Swiss Ball toward body balance to less the risk fall of older (p=0,000 ; p=< 0,05). Sugessted for older to practice the balance himself more steady so that not easy to have risk fall further.     Parent Perceptions on The Obedience in Implementing Primary Immunization to Infants Reviewed with Health Belief Model

    Introduction: universal coverage immunization achievements that have not reached national targets, including in Puskesmas Kualin, were only 61.5% of the 90% target by 2015. One of the causes of parents' negative perceptions of immunization. The Health Belief Model Theory describes a person's judgment in a healthy way influenced by perceptions about benefits and barriers. This study aims to explain the relationship of perception of benefits and barriers with obedience in implementing of primary immunization to infants. Methods: The study was cross-sectional with the population of mothers with infants aged 12-18 months in the working area of Puskemas Kualin. The sample size of 80 persons was selected using cluster sampling. Independent variable in the study was the perception of benefit and perception of barriers. The dependent variable was the implementation of primary immunization to infants. Data were collected using questionnaire and health monitoring card. Data analysis using Spearman Rho statistic test with significance level α ≤ 0,05. Results: There was a relationship with a positive direction between perception and obedience to the application of primary immunization to infants (p = 0.0001, r=0.428). There was a relationship with a negative direction between the perception of barriers and obedience to the application of primary immunization to infants (p=0.0001, r=-0.525). Conclusion: High perceptions of the benefits and low perceptions of parental barriers are associated with obedience to the implementation of primary immunization to infants. It is expected that parents are diligent to Posyandu every month and health workers visit home for sweeping immunization
