    Evaluation of clinical efficacy of Dentofresh® Mouthrinse

    Introduction. The role of antiseptics used today in chemical supragingival plaque control is inhibiting its formation and an independent anti-inflammatory effect. Aim of the study. Clinical evaluation of Dentofresh® mouthwash used as a supplement to tooth cleaning during the treatment of gingivitis in adults. Material and methods. The study included 76 patients with gingivitis aged 18 to 50. Patients were divided at random into 3 groups: Group one – 26 people who had oral hygiene instruction performed and were recommended to brush two times a day with the triclosan toothpaste and rinse two times a day for 1 min each time with Dentofresh mouthwash for a period of 4 weeks; Group two – 25 people who had oral hygiene instruction performed and were recommended to brush three times a day with the triclosan toothpaste; Group three (control) – 25 people during conservative treatment: no change of behavior in the oral hygiene (brushing frequency was recorded and the type of paste). This study evaluated: the effectiveness of tooth brushing using PCR by O'Leary index, interdental cleaning efficiency using the API index by Lange et al., the extent of gingival inflammation with BoP index by Ainamo and Bay, the intensity of gingival inflammation by means of PBI index by Saxera and Mühlemanna, presence of plaque using an index CI by Ennevera, breadth and intensity of teeth staining using the DI index by Loebena and all side effects of therapy. The survey was conducted four times: survey 0 – before the randomization of patients into three groups, survey 1–2 weeks after the implementation of preventive and therapeutic action in group I and II, survey 2–4 weeks after the implementation of preventive and therapeutic action in group I and II – the day of termination of antiseptic rinsing in group I, survey 3–8 weeks after randomization. Conclusions. As the Dentofresh mouthrinse meets the requirements set for plaque inhibitors and has an anti-inflammatory effect, it can be used as a supplement to gingival inflammation therapy. In specific clinical situations it may also broaden the preventive and therapeutic scope of proper brushing and interdental cleaning. After four weeks of using Dentofresh no local adverse effects were recorded

    Rare case of a huge odontogenic cyst of the mandible

    Dentigerous cysts are one of the most common odontogenic cysts of the jaws, being associated with the crowns of permanent teeth, especially unerupted mandibular third molars. Bilateral and multiple cysts are usually found in connection with a number of syndromes that include cleidocranial dysplasia and Gorlin syndrome. In the absence of these syndromes, multiple dentigerous cysts are rare. Dentigerous cysts are the second most common odontogenic cysts after radicular ones, accounting for approximately 24% of all true cysts of the jaws. This paper presents an unusual case of a huge odontogenic cyst in a 49-year-old man associated with mandibular impacted premolars. During the last 30 years a similar case was observed in our material only once. They are randomly diagnosed in routine dental radiographs. The patients do not usually report pain or discomfort. Clinical examination reveals a missing tooth or teeth, facial asymmetry and a lesion of the bone which predisposes to pathologic fracture

    Tinnitus, diagnosis and therapy based on selected literature – preliminary report

    The paper presents the definition, diagnosis and therapy of tinnitus on the basis of selected and current literature. Tinnitus involves the inconvenient sound impressions received by the patient in one or both ears in the absence of an acoustic stimulus in the environment. They may coexist with various disease processes that threaten the sufferer's physical health and well-being, often adversely affecting the quality of life not only of the patient but also the family. This paper presents the problem of tinnitus, the probability of which increases with age. This is a complex, multidimensional process which is related to a variety of factors. The article describes the etiological factors that may affect tinnitus. It also presents the relationship between tinnitus and disorders of the temporomandibular joint on the basis of the literature. The increase in muscle tension results in a significant increase in the burden on the temporomandibular joint, thereby overloading numerous tissues and muscles. In the final part of this article, attention has been paid to modern methods of treatment

    Analysis of predisposing factors for complications after surgical removal of mandibular wisdom teeth

    Thrid molar extractions are very common and difficult procedures in oral surgery. Mandibular wisdom are removed for therapeutic or prophylactic reasons. Aim. The aim of the study was to assess the incidence of complications after surgical extraction of mandibular third molars, such as alveolitis, sensation disorders, lockjaw, abscesses, prolonged bleeding and intensified swelling sustained for over 7 days. Material and methods. Data was obtained from patient records from the Department and Clinic of Oral Surgery, Poznan University of Medical Sciences in 2004–2008. Results. Complications occurred in 51 patients (17,5%) with 291 third molar extraction in the mandible. The most common were alveolitis, sensation disorders and intensified swelling. Postoperative complications were observed more frequently in the women. Conclusions. 1. Early extraction of the third molar in the mandible reduces the risk and extent of the treatment and the procedure is better tolerated by the patient. 2. Extraction of third molars in the mandible in eldery patients undergoing periodic clinical and radiological examination should only be carried out when absolutely necessary. 3. Postoperative complications in women are generally more often reported

    The Self Adjusting File – a review. New system, a new look into the root canal space?

    The Self Adjusting File (SAF) is the only representative among endodontic files which enables preparation of the surface of a tooth with irrigation. Nickel-titanium alloy in the form of a grid allows to adjust the flexible file to the canal wall and expand according to its three-dimensional anatomy. Simultaneous rinsing increases the efficacy of cleaning. Unlike most of the available systems endodontic rotary motion was replaced by movement in the vertical axis. The cutting blade characteristic for conical cores turned into an abrasive surface. The combination of in and out movements with the abrasive surface resembles the effect of sandpaper. The first part of the paper takes into account the description of The Self Adjusting File and its the manner of operation while the second is a description of its characteristics in the clinical aspect

    Evaluation of the radiopacity of the materials used for sealing root canals with a wide apical gap

    Introduction. Optimal canal filling in the apical region poses a challenge for the dentist and determines therapeutic success. The choice of an appropriate material whose physico-chemical properties enable X-ray visualization of the quality of the filling is of great importance. The higher the density of the material, the better the contrast and thus the possibility of correct assessment of the filling. Aim. This study was undertaken to assess radiopacity of materials currently used in endodontics for sealing root canals with a wide apical gap. Material and methods. MTA+ (Cerkamed), Biodentine (Septodont), and gutta-percha with AH Plus paste were used for sealing 27 single-root human teeth. The canals were cleaned with K files to achieve ISO size 40. Next, Gates-Glidden drills size 2 were inserted up to their full working length to enlarge the apex. Each material was used to seal nine canals. X-ray images were taken with the X-Mind DC (Satelec) generator and were recorded, scanned, and analyzed with the Digora digital radiography system. Radiopacity of the image was evaluated on the basis of color change using the 256 levels grayscale. Radiographs of the same roots with a glass plate imitating cheek tissues with a bone fragment were obtained for comparison. Results. The lowest optical density was found for Biodentine. This material demonstrated the smallest X-ray contrast as compared with dentin. The best radiographic visibility was found for gutta-percha with AH Plus paste. In spite of the slightly greater optical density values of MTA+ in comparison with Biodentine, the contrast and radiographic visibility of both materials was poor. Optical density of the materials was reduced in the presence of the glass plate imitating cheek tissues with a bone fragment. Conclusions. MTA+ and Biodentine are characterized by lower optical densities than gutta-percha with AH Plus paste

    Diagnostics for upper impacted canines – based on the literature

    Impacted canines are the second most common (after the third molars) impacted teeth. Good diagnostics for impacted canines is possible at approx. 10 years of age, when the incorrect angulation of the crown of the tooth can be seen on the panoramic X-ray. The initial assessment of impacted canines includes a basic dental examination (presence of persistent deciduous teeth, lack of erupted permanent canines in the dental arch after the end of tooth eruption) and a palpative examination of alveolar bone in the vestibulum. The diagnostics is based mainly on X-rays: mainly panoramic images, tooth X-rays and occlusal plate pictures. The new tool in the diagnostics of impacted teeth is computed tomography, especially CBCT. The study collates diagnostic methods based primarily on panoramic X-ray images for the assessment and prediction of the spontaneous eruption chances of maxillary canines

    Fetal alcohol syndrome case report and description based on the literature

    Fetal Alcohol Syndrome (FAS) is a pattern of mental and physical defects that can be expressed as mental retardation, mainly craniofacial anomalies, as well as psychological disorders in children whose mothers consumed alcohol during pregnancy. The article presents the most characteristic clinical features occurring in patients who were under irreversible prenatal exposure to alcohol, which crosses the placental barrier very easily. Ethyl alcohol and its main metabolite acetalaldehyde are teratogens which may cause various fetal disorders depending on the amount of alcohol consumed and the stage of pregnancy. The article describes the case of a seven-year-old patient who came for an orthodontic consultation at an out-patient clinic. The patient’s behaviour, facial features, malloclusion and positive medical history with frequent alcohol exposure during fetal development prompted the authors to focus on the problem of FAS

    Selected methods for the early diagnostics of oral malignant lesions – a literature review

    Early diagnostics of oral cancerous lesions plays a crucial role in the process of treatment and evaluation of the patient's chances for recovery. The article presents new techniques of identifying and detecting abnormal lesions within oral mucosa, such as oral lumenoscopy (ViziLite), TBlue (toluidine blue marking system), VELscope screening system, OralCDx and the brush biopsy. Their application does not allow a definite diagnosis, nor does it replace biopsy or histopathologic assessment, but it permits a more accurate examination of the oral cavity, as well as oral lesion detection at the early stage of progression
