    The use of the CO2 laser in treatment of lipomas

    Introduction. Lipomas are the most common benign soft tissue tumors, but their occurrence in the oral cavity is rarely recorded. They are generally asymptomatic and painless, very rarely giving recurrence. Microscopically cells resemble mature adipocytes, divided by connective tissue. Aim. Review of the CO2 laser treatment of lipomas. Material and methods. 43 patients were treated because of lipomas in 2011–2012, in the Department of Oral Surgery, Medical University of Lodz. This group consisted of 11 (25.6%) women and 32 (74.4%) men, whose age ranged from 45 to 68 years. The presence of lesions appearing as insolated, mobile, non-tender, subcutaneous or submucous masses measuring 1–3.5 cm in diameter, were found within: skin above buccal muscle – 4 (9.3%) cases, the forehead – 6 (13.95%) patients, lower lip – 11 (25, 6%), floor of the mouth – 8 (18.6%), the duct of parotid gland – 8 (18.6%), tongue – 4 (9.3%) and in the alveolar part of the mandible – the premolars and molars area – 2 (4.65%) cases. In local infliltration anaesthesia with 0,3 ml of 2% Lidocaine, the focused continuous laser beam of 3 W was used. The mucosa was incised and subsequently prepared. Afterwards the tumor was enucleated alongside the surrounding fibrous capsule. The material was sent for histopathological evaluation. Results. The results of histopathological examination confirmed the initial diagnosis of lipoma in 15 (34.9%) cases, whereas in 13 (30.2%) patients – fibrolipoma, in 10 (23.3%) – cutaneous lipoma, and in 5 (11.6%) cases angiolipoma. The follow up examination after 14 days showed that healing was uneventful, and after 12 and 24 months no signs or symptoms of recurrence developed. The functional and esthetical result was satisfactory

    Obesity and its exponents in the oral cavity – a literature review

    Obesity has reached pandemic proportions, according to the World Health Organization over the last years. Not only adults suffer from the disease but more often children and young people. One of the main causes of overweight and obesity is excessive food intake, particularly highly processed carbohydrates. This type of food, taken in large quantities, without observing the gaps between meals leads to negative changes also in the mouth. Among researchers, there is a discussion of the relationship of obesity with dental caries, periodontal disease and xerostomia. More and more scientific reports draw attention to the changes in the microflora of the oral cavity in the course of obesity. The paper presents the latest views on the diagnosis of obesity, the impact of the disease on the state of the hard and soft tissues of the oral cavity and its exponents found in saliva, as an excellent diagnostic material

    A diagnostics point of view on impacted maxillary canines – a literature review

    Disorders of eruption in the anterior maxilla is associated with permanent canines which is believed to be caused by: the premature loss or impaired resorption of deciduous teeth. Treatment of retained maxillary canines depends on their location in the bone, the amount of space in the dental arch, the patient's age, type of braces used. The aim of this paper is to diagnose the possibility of retained maxillary canines in terms of clinical and radiological threats. Based on the analysis of literature from the years 1990–2012 made its review of the 3 databases (Pub Med., Medline, Polish Base) using as keywords: retained tooth, permanent teeth of the jaw, diagnosis. Due to the size of a thematic issue of detainees about retained canines, analyzed the literature only for the diagnosis of the location of these teeth in the bone. Reviewed publication indicates that the correct diagnosis of retained canines should be performed on the basis of clinical examination and radiological (teeth radiograph, panoramie X-Ray, computer tomography, occlusal radiograph). Recent diagnostic techniques such as volumetric imaging – to a three-dimensional assessment of the position of teeth in the bone, therefore, is one of the most accurate methods to assess the position of retained canines in the jawbone. Comprehensive analysis of the exact teeth allows for precise determination of their location in relation to neighboring structures in the jaw bone as well as the proper development plan of orthodontic treatment

    Orthodontic and surgical treatment of a patient with mandibular prognathism – a case report

    This study describes the course of interdisciplinary treatment for mandibular prognathism in an adult patient with a severe Class III malocclusion and a reverse overjet of 9 mm that involved an orthodontic and surgical orthognathic treatment. The orthodontic treatment involved attaching a fixed appliance to the upper and lower dental arches to prepare the patient for the mandibular surgery. Following the orthodontic treatment, there was performed a bilateral sagittal split ramus osteotomy (BSSRO) in order to reduce the length of the mandible and to reposition the mandibular anterior segment posteriorly. The treatment turned out to be successful. The mandible was shortened and the profile was corrected. The orthognathic surgery was followed by the second step of the the orthodontic treatment of the mandible and maxilla. Now, following three years of treatment and observation, the patient shows a good face profile and occlusion as well as the acceptance of the facial aesthetics

    A comparison of force-deflection relationships in half-round dental clasps, in vitro, fabricated by two casting methods

    Introduction. From a clinical stand point it is important to know if the different casting techniques, using the same alloy, may change the stiffness of the cast denture clasp. Although rotary and vacuum-pressure casting methods have the same induction melting means, any similarity ends at this point. In the rotary method, the melting and casting process is carried out in air but for the vacuum-pressure method, melting is conducted in the vacuum and casting is done in forming gas. Aim of the study. The aim of this study was to compare the influence of two casting methods: a rotary-centrifugal method and a vacuum-pressure method on the deflection force of a straight clasp profile in vitro and disclose any differences in microstructure of cobalt chromium alloy cast profiles caused by using different casting technology. Material and Methods. A commercial cobalt-chromium alloy (Wironit® extrahard BEGO, Bremen, Germany) was used. The tested material were the straight clasp profiles of a half-round section, cast in standard wax patterns (BEGO, Bremen, Germany), by the use of two inductive casting devices: a vacuum-pressure microprocessor assisted method (VP) (in device Nautilus®-BEGO, Bremen, Germany) and a rotary (R) manually operated method (in device Fornax®-BEGO, Bremen, Germany). Thirteen castings were fabricated by each of the respective methods. The custom made measuring instrument consisted of a micrometer screw and an force into voltage extensometer converter was applied. After deflection force measurement chosen profiles undergone metallographic examination to reveal internal grain arrangements in them. Results. On the level of significance p = 0.05 the observed difference between the means was proved to be statistically significant with better results for vacuum-pressure method. The microstructure varied, and these differences correlated with differences in deflection force measurement with greater deflection force for bigger grains.     The properties and use of galvanoforming are presented on the basis of the literature. Evaluating the possibilities of applying galvanoforming technology for making accurate restorations. Restorations performed by galvanoforming are characterised by very good marginal tightness, biocompatibility, low weight and ensure smaller plaque deposition. Homogeneous gold with a purity of 99.99% eliminates the risk of corrosion or allergy associated with traditional dental alloys. Galvanoforming technology provides considerable opportunities for precision retention elements, and increases the mechanical strength and resistance to deformation of final restorations

    Hydrodynamic maxillary sinus lift technique with immediate implant placement: a review of new techniques and a presentation of clinical cases

    There are many ways to litf the maxillary sinus mucosa in order to obtain adequate bony conditions prior to implantation. Among these "sinus graft" and "sinus lift", are the methods of raising the sinus’ mucous membrane practiced for 25 years. But with increasing demands from both patients and surgeons research into new treatment techniques is in progress. Due to the fact that the sinus lifting procedure in a traditional way is unpleasant for the patient, less invasive techniques to improve the conditions of the bone before the prosthetic treatment are being researched. In this paper sinus physiolift and crestal approach sinus kit methods are presented as an alternative, atraumatic hydrodynamic techniques of sinus lift. The aim of the study is to present the hydrodynamic technique of sinus lift on basis of available literature and our own observations. Among the 24 patients qualified for sinus lift, the procedure was performed by means of hydrodynamic technique. The use of closed hydrodynamic methods of sinus lift has improved the bone conditions within the maxillary alveolar process and permitted planned simultaneous implantation

    Ergonomics in the professional work of the dentist – a questionnaire survey

    Aim of the study. The objective of the present study was to assess the occurrence of pain, ergonomic skills in the dental workplace and the frequency of health-promoting behaviour among dentists. Material and methods. The survey was based on an anonymous questionnaire. The group comprised 223 dentists (172 women and 51 men). The respondents gave answers to 3 groups of questions covering demographic data, working methods, experience of pain and the methods dentists used to treat such pain. Results. The survey confirmed that the occurrence of pain in the musculoskeletal system is a universal problem. The cause lies in the failure of dentists to observe the basic rules of ergonomics as well as poor work organisation. 81.86% of dentists and 19.13% of graduate dentists reported musculoskeletal pain over the course of the previous 12 months. Chronic pain affected 30.09% of the respondents. Pain was most frequently located in the lumbosacral region (35.91%). The study revealed a correlation between years of working experience and the percentage of dentists reporting pain. 71.82% of the respondents had undergone treatment for pain, 34% of whom sought the assistance of a doctor. Treatment helped improve the problem in 90.96% of cases. Conclusion. The study showed that dentists already require instruction in ergonomics during their undergraduatetraining and they should observe these rules during their professional careers. Regular relaxation exercise reduces or eliminates pain in the musculoskeletal system

    Traumatic occlusion treatment through canine guidance reconstruction with the use of T-Scan III diagnosis – a case report

    This study describes the treatment of a patient with traumatic occlusion and pain syndrome of the left temporo-mandibular joint. Traumatic occlusion was diagnosed by clinical examination, i.a. due to the lack of mutual canine guidance. Diagnosis was enhanced with the use of a T-Scan III System. Selective teeth grinding and rebuilding the canines with composite material were done during treatment. After 3 and 6 months the effects of treatment were evaluated. Pain does not occur any more and the reconstructed occlusal conditions remain normal

    Non-carious tooth loss in terms of erosion – a literature review

    The loss of tooth structure caused by the process of chemical dissolution and mechanical abrasion is becoming a great problem in modern dentistry. Patient awareness as well as diagnostic alertness of a dentist are crucial for proper prevention and treatment. Late diagnosis and treatment of extensive loss of tooth structure caused by erosion, abrasion or abfraction poses many difficulties for clinicians. Chronically progressive loss of tooth structure can cause loss of vertical dimension of the bite, secondary orthodontic complications and also aesthetic and functional problems. The treatment is often associated with long-term diagnosis and interdisciplinary, expensive rehabilitation of the entire stomatognathic system. The aim of this study was to review current publications on the prevalence of tooth wear in developmental age, with focus on etiology, risk factors, prevention and treatment methods
