    12-year clinical observation of telescopic anchors applied in removable dentures – case reports

    The study presents a clinical evaluation of the conical telescopic anchors applied in order to enhance the stability of the lower partial removable denture. Main clinical and labolatory stages in performance of the removable denture were described and the method of evaluation of the denture’s retention was presented. The telescopic anchors were applied as the attachments for the removable dentures, the framework of the denture was made from chromium-cobaltmolibdenium alloy, and the external telescopic crowns were veneered with composite resin. After a 12-year clinical observation it was concluded, that this system has many advantages which make it highly recommendable in elderly patients with reduced vision and motion abilities. The characteristic features of this system include: satisfying retention and stability of dentures during mastication, splinting effect, accessibility for oral hygiene maintenance, ease of repair and durability. It was also underlined that this system demands follow-up examinations and visits preventing the abutments from occlusal overload

    Early prosthetic rehabilitation of a juvenile patient with hereditary ectodermal dysplasia – a case report

    Ectodermal dysplasia is the term used to describe a large and heterogenic group of congenital disorders. Ectodermal dysplasia affects at least two ectoderm-derived structures. The dental abnormalities associated with ectodermal dysplasia are hipodontia, oligodontia and even anodontia. Aim of the study. The authors describe a clinical case of 5-yearold boy with hereditary hidrotic ectodermal dysplasia. Materials and methods. The first stage in the clinical treatment of a young patient affected by ectodermal dysplasia is presented. Complete dentures were made. Conclusion. Early prosthetic rehabilitation of juvenile patients with ectodermal dysplasia is a very complex problem. Early rehabilitation and an interdisciplinary approach are essential for successful treatment

    Difficulties in the diagnosis of Biermer's disease – a case report

    Biermer’s disease (pernicious anemia PA), is an autoimmune disease inducing vitamin B12 deficiency by malabsorption. The clinical presentation of Biermer’s disease (pernicious anemia), is often insidious because the onset is difficult to establish and progression is very slow. It rarely occurs before the age of 30, the mean age at which it is diagnosed being about 60. Patients may seek medical advice due to non-specific symptoms such as weakness, decreased mental concentration, and headaches. Biermer’s disease may be accompanied by other autoimmune diseases. Longstanding Helicobacter pylori infection probably plays a role in many patients with PA. In this case report a 22-year old woman with Biermer’s disease coexisting with Hashimoto thyroiditis and a Helicobacter pylori infection has been described. This case indicates the important role of dentists in the early diagnosis of this disease, especially in respect of later neurological complications

    Urgent analgo-sedation of a patient with polytrauma done by a basic medical emergency team in prehospital care

    Medical Rescue Teams during their daily work often face the need to analgo-sedate patients, the victims of road accidents. This is especially important in patients with cranial and brain trauma and is designed to prevent secondary damage to the brain. This procedure should begin as early as the prehospital phase at the scene or during transport to a Hospital Emergency Department. In the light of the Law on State Emergency Medical Teams, basic crews themselves cannot give drugs to provide urgent analgo-sedation and brain protection. The case describes a 65-year-old patient, a participant in a traffic accident, in whom the initial examination showed multiple traumatic injuries, of which injury to the chest and head trauma proved to be the most dangerous. During pre-hospital activities, due to a significant deterioration of the patient and the emergence of life-threatening symptoms, it was necessary to implement emergency rescue procedures. There was a pneumothorax, possible to decompress by members of the basic Medical Rescue Team, as well as increasing psychomotor agitation with indications for urgent analgo-sedation requiring the consent of a coordinating physician at the Crisis Management Centre. The coordinating doctor consented to the administration of Mannitol and Thiopental to protect the central nervous system (CNS) of the casualty. Neither drug is on the list of drugs that may be administered by paramedics alone. They must be authorised by a doctor, whose decision, in this case, undoubtedly contributed to increasing the chances of the patient's survival, safe delivery to a Hospital Emergency Department, transfer to the Department of Intensive Care Medicine, and the Department of Trauma Surgery

    Effectiveness of laser therapy in the elimination of prosthetic stomatitis symptoms in a group of female patients with osteoporosis and unfavourable prosthetic foundation conditions

    Introduction. Irreversible changes in bone mass and structure of the masticatory system occurring in patients with osteoporosis affect the results of prosthetic treatment and many patients suffer due to stomatopathy symptoms. Laser therapy has an anti-inflammatory, analgetic, and anti-edematous effect. Aim of the study. The aim of the study is to evaluate the effectiveness of laser therapy in the elimination of prosthetic stomatitis symptoms in a group of female patients with osteoporosis and unfavourable prosthetic foundation conditions. Material and methods. The study concerned two groups of patients (20 women); one with osteoporosis or osteopenia and the other healthy group with an unfavourable state of prosthetic foundation. Prosthetic treatments were performed. Patients were asked to assess the presence of pain. A clinical assessment of the state of the prosthetic foundation was performed. As an additional comprehensive treatment biostymulation laser therapy was performed using a Doris CLT-1106 laser. Results. The study revealed a reduction of pain and prosthetic stomatitis symptoms in both groups. A regeneration of tissues in the prosthetic foundation was observed. The questionnaire study confirmed the positive effects of laser therapy. The functional aspects of prosthetic appliances in patients with osteoporosis were rated as satisfactor

    Symptoms of ectodermal dysplasia according to Freire-Maia and Pinheiro

    Ectodermal dysplasia (ED) is a congenital syndrome of developmental disorders in tissues derived from the ectoderm. The inheritance of ED may be autosomal dominant or recessive. The distinctive features of this syndrome involve the skin and appendages, dentition, eye, ear, adrenal glands, nervous system and facial part of the skull. Over 200 forms of this disease are described, about 120 include symptoms located in the oral cavity and dentition. One of the several classifications, by Freire-Maia and Pinheiro, is based on the presence of four leading clinical symptoms: hair disorders, dental anomalies, nail disorders and dyshidrosis. ED needs multidisciplinary treatment because of the multitude and variety of its symptoms

    Complications during the surgical removal of a wisdom tooth – a case report

    Wisdom teeth eruption or malposition may cause many pathological conditions such as recurrent pericoronitis, abscesses, osteomyelitis, radiating spontaneous pains, cysts, and others. The disorders mentioned above are indications for third molar removal. The degree of difficulty of the procedure is connected with the impaction stage of the tooth. Mesioangular and distoangular impactions are the least favourable positions. The surgical procedure may involve many complications such as excessive bleeding, alveolitis, nerve paresis, bone fracture, damage, luxation, or unintentional adjacent tooth removal. A displacement of the whole or of fractured fragments of the tooth to the adjacent fascial spaces, though rarely described in the literature, is a very serious complication. The success of the operation depends not only on the surgeon’s knowledge and experience but also on effective complications management. In this paper the authors present the case of a patient who after the surgical removal of a wisdom tooth was treated for second molar subluxation with ligature immobilization and endodontic treatment

    Pierre Robin sequence: a case report

    Introduction. Pierre Robin Sequence (PRS) is a congenital abnormality associated with the first brachial arch malformation. The etiology of PRS is not fully known, though both genetic and environmental factors are taken into account. PRS symptoms (micrognathia, glossoptosis and cleft palate) have a significant influence on the proper development of a child. The severity of the disease determines the treatment plan and its results. Aim. The aim of this report is to present the case of a five-year old boy diagnosed with PRS. Material and methods. A clinical examination of the oral cavity was performed, an orthopantomogram was taken. Results. Visible changes in facial features, bird face appearance, scar after surgical reconstruction of soft palate, dental caries. Conclusions. The course of treatment and PRS patient care demand long-term monitoring as well as a multidisciplinary team approach

    Breastfeeding and dental caries in infants and toddlers – a literature review

    Natural feeding has a number of benefits for general health of the child due to, inter alia, presence of immune factors and antibacterial, anti-inflammatory and immunomodulatory effect. In addition, breastfeeding stimulates the muscles responsible for normal development of the stomatognathic system and consequently also the articulation. However, the literature data indicates various opinions of the authors concerning the relationship between natural feeding and the occurrence of early childhood caries. The aim of the paper was to provide information on the impact of mother's milk to the child's dentition. Cited authors emphasize that guidelines concerning feeding of infants and young children do not show a clear and optimal deadline for completion of breastfeeding. There is a lack of unanimity on the effects of prolonged breastfeeding as a key factor to promote the development of caries due to considerable complexity of the situation related to the expansion of the diet and individual features
