Endobronchial hamartoma: Diagnostic challenges and clinical implications of a rare benign lung tumor

    No full text
Introduction: Endobronchial hamartomas are rare benign lung tumors, representing only 0.025%–0.032% of all lung tumors and 1.4% of hamartomas. Originating in the bronchial passages, these tumors comprise cartilage, fat, fibrous tissue, and epithelium. Despite their benign nature, they can cause bronchial obstruction, post-obstructive pneumonia, and atelectasis. Case Report: A 68-year-old male presented with chronic cough and hemoptysis. Imaging revealed a calcified mass in the right lower bronchus, causing partial obstruction. Bronchoscopy identified a smooth, obstructive mass without bleeding or necrosis. Histopathological examination confirmed an endobronchial hamartoma, showing benign bronchial mucosa, cartilage, adipose tissue, submucosal glands, and myxoid stroma. Discussion: Diagnosing endobronchial hamartomas is challenging due to their rarity, nonspecific symptoms, and nonspecific imaging findings. Advanced imaging techniques, such as computed tomography (CT) scans, can identify calcifications or fatty components, aiding diagnosis. However, histopathological confirmation is crucial to differentiate these lesions from malignant or other benign pulmonary tumors. Early recognition and treatment are essential to prevent complications like recurrent infections and irreversible airway damage. Conclusion: Although rare, endobronchial hamartoma is a clinically significant benign lung tumor due to its potential to cause notable complications. The diagnostic challenge lies in its often nonspecific initial imaging findings. However, combining advanced radiology modalities, such as computed tomography (CT) and histopathological evaluation. Early recognition of this tumor from other pulmonary lesions is crucial for treatment and better patient outcomes

The combined effect of neonatal hypothyroidism and low family income on intellectual disability: A review of mechanisms and interventions

    No full text
This review investigates the combined effect of neonatal hypothyroidism and low family income on the prevalence of intellectual disability, drawing insights from existing literature, including specific findings from studies involving neonatal cases such as 15 cases at Wangaya Regional Hospital Denpasar. Neonatal hypothyroidism significantly impacts cognitive development due to the critical role of thyroid hormones in brain maturation. Concurrently, low family income acts as a risk factor for intellectual disability by limiting access to essential healthcare, education, and developmental resources. The review explores the epidemiology of neonatal hypothyroidism, highlighting global and regional variations influenced by genetic, environmental, and socioeconomic factors. It delves into the mechanisms by which neonatal hypothyroidism affects brain and cognitive function, leading to deficits in intelligence, memory, attention, and executive function. Furthermore, the review examines the relationship between socioeconomic status and intellectual disability, emphasizing the role of resource scarcity and environmental factors associated with poverty. Findings from the reviewed literature suggest a compounding effect of neonatal hypothyroidism and low family income on a child's cognitive development and intellectual disability risk. Addressing this complex interplay necessitates a multifaceted approach, including early detection, prompt treatment, and integrated interventions spanning healthcare, education, and social support services. The review underscores the importance of equitable access to newborn screening, comprehensive early intervention programs, and family-centered support for children from low-income households

Association between cardiothoracic ratio and aortic arch calcification with estimated glomerular filtration rate in hypertensive patients

    No full text
Hypertension remains a leading cause of global cardiovascular mortality, including in Indonesia. Target organ damage, particularly renal impairment, underscores the need for easily obtainable biomarkers for early detection. Although not explicitly recommended in current guidelines, the cardiothoracic ratio (CTR) and aortic arch calcification (AAC), both assessable via chest radiography, represent readily available and cost-effective screening tools. This study aimed to evaluate the association between these radiographic markers and decreased estimated glomerular filtration rate (eGFR) in hypertensive patients at Royal Prima Hospital, thereby addressing a crucial knowledge gap in resource-limited settings with restricted access to advanced imaging modalities. A cross-sectional analysis was conducted involving 175 hypertensive participants, stratified according to their chronic kidney disease (CKD) stage. Baseline demographics, hypertension status, CTR, AAC, and eGFR were assessed. Bivariate correlation and multivariate regression analyses were performed to determine the relationships between CTR, AAC, and eGFR. The study population exhibited a high prevalence of cardiovascular comorbidities, with 82.3% demonstrating cardiomegaly (mild-to-severe CTR) and 61.7% presenting with AAC. Bivariate analysis revealed a strong inverse correlation between eGFR and both CTR (r = −0.418, p < 0.001) and AAC score (r = −0.457, p < 0.001). Multivariate regression confirmed that both CTR (B = -1.738, p < 0.001) and AAC (B = -16.127, p < 0.001) were significant predictors of eGFR decline. Progressive CKD stages were associated with increased CTR (52.3% in Stage 1 vs. 58.7% in Stage 5) and greater AAC severity (0% advanced calcification in Stage 1 vs. 34% in Stage 5). Age, sex, and blood pressure did not show significant correlations with eGFR (p > 0.05). CTR and AAC are strongly associated with renal dysfunction, reinforcing the evidence of an interaction between cardiovascular pathology and CKD progression. These markers have the potential to serve as accessible biomarkers for identifying high-risk patients, thereby facilitating early intervention in resource-limited settings

Regenerative and adjunctive surgical strategies in peri-implantitis: A mini-review

    No full text
Peri-implantitis is a major biological complication that threatens the long-term success of dental implants. The progression from peri-implant mucositis to advanced bone loss often necessitates a shift from non-surgical to surgical treatment, although the most effective management protocols remain under debate. This mini-review aims to evaluate and compare clinical outcomes of three distinct approaches to peri-implantitis management: the diagnostic necessity of surgical access, conventional guided bone regeneration (GBR) combined with antibiotics, and adjunctive topical oxygen therapy. A narrative review was conducted, and three representative clinical case reports were purposively selected for detailed analysis. These cases illustrate different therapeutic strategies currently applied in clinical practice. The findings emphasize that surgical intervention is crucial for halting disease progression when non-surgical access proves inadequate. Conventional GBR supplemented with local antibiotics provided exceptional stability during a 17-year follow-up period. In contrast, the application of a topical oxygen gel (BlueM) yielded encouraging 5-year outcomes, demonstrating reduced inflammation and enhanced bone regeneration without the use of antibiotics. The success of all treatment modalities depended heavily on meticulous surface decontamination and thorough defect debridement. Surgical management of peri-implantitis should be tailored to the specific defect morphology. While GBR with antibiotics remains a well-established long-term reference, adjunctive oxygen therapy offers a promising non-antibiotic alternative for infection control. Further randomized controlled trials are warranted to establish standardized clinical protocols

Enhancing esthetics and functionality of dental implants: A mini-review of titanium-based materials and techniques

    No full text
Dental implant placement in the esthetic zone is among the most challenging procedures in contemporary restorative dentistry and requires meticulous interdisciplinary planning to ensure long-term success. This success largely depends on the presence of adequate peri-implant bone volume, which is often compromised by post-extraction resorption, periodontal disease, or traumatic injury. This comprehensive mini-review consolidates and critically evaluates recent clinical case reports, controlled studies, and relevant literature to assess the efficacy of titanium and its alloys when used with advanced surgical techniques, with particular emphasis on Guided Bone Regeneration (GBR) employing titanium mesh. A synthesis of documented cases demonstrates that titanium mesh functions as a highly effective space-maintaining device, preventing soft tissue ingrowth and promoting substantial horizontal ridge augmentation. One representative case reported an average ridge width increase from 4.56 mm to 7.23 mm. Dental implants from various manufacturers, including Biotec BTK and Straumann Roxolid, both primarily composed of titanium, consistently achieved successful osseointegration without significant complications, even in patients with notable risk factors such as chronic smoking. The discussion highlights the multifactorial nature of implant success, which depends not only on the inherent biocompatibility and corrosion resistance of the material but also on precise surgical technique, comprehensive prosthetic planning, and diligent postoperative maintenance. Titanium and its alloys remain the gold standard biomaterials for dental implants due to their well-established osseointegrative properties. Their strategic use in combination with GBR techniques involving titanium mesh enhances clinical outcomes, yielding predictable and durable results in both esthetic and functional dental rehabilitation. Future research should focus on nanoscale surface modifications of titanium to further promote bioactive healing and on long-term clinical evaluation of these advanced treatment protocols

α-Glucosidase inhibitor activity of endophytic fungi isolated from oil palm leaves (Elaeis guineensis Jacq)

    No full text
Indonesia ranks seventh globally in diabetes prevalence, with an estimated rate of 4.8%, and more than half of the cases remain undiagnosed. By 2030, the number of individuals with diabetes in Indonesia is projected to reach 21.3 million. Alpha-glucosidase inhibitors are commonly used to prevent the progression of prediabetes; however, their use is often associated with adverse side effects. Therefore, research is needed to identify natural sources of alpha-glucosidase inhibitors. This study aimed to evaluate the in vitro alpha-glucosidase inhibitory activity of endophytic fungi isolated from oil palm leaves. The research consisted of fungal isolation, microscopic characterization, and analysis of α-glucosidase inhibitory activity. Two pure endophytic fungal isolates, designated JES1 and JES2, were successfully obtained. Isolate JES1 exhibited a denser mycelial network with well-defined septation, whereas JES2 displayed loosely arranged, curved hyphae, suggesting that the variation was likely due to differences in growth phase or sampling location rather than taxonomic divergence. Microscopic observations identified JES1 and JES2 as Colletotrichum sp. and Fusarium sp., respectively. The α-glucosidase inhibitory assay showed that isolates JES1 and JES2 demonstrated inhibitory activities of 79.4% and 20.8%, respectively, while the positive control, acarbose, exhibited an activity of 81%. These results indicate that endophytic fungi isolated from oil palm leaves have potential as natural sources of bioactive compounds with α-glucosidase inhibitory properties
