Antidiabetic potential of andaliman: Histopathological and glycemic improvements in diabetic rats
Background: Diabetes mellitus (DM) is a metabolic disorder characterized by chronic hyperglycemia and pancreatic dysfunction, particularly involving the Islets of Langerhans. Andaliman (Zanthoxylum acanthopodium) is a plant rich in flavonoids and antioxidants, which are believed to have antidiabetic effects. This study aimed to evaluate the effectiveness of the ethanol extract of Andaliman fruit on pancreatic histopathology in alloxan-induced diabetic rats.
Methods: This study employed a true experimental design with a pre-test and post-test control group structure. Twenty-four male Wistar rats were randomly assigned to six groups: a normal control, a positive control (metformin 9 mg/kg), a diabetic control (alloxan 90 mg/kg), and three treatment groups induced with alloxan followed by administration of Andaliman extract at doses of 150, 250, and 350 mg/kg body weight, respectively. Treatments were administered for 14 days, after which blood glucose analysis (pre- and post-treatment) and pancreatic histopathological assessments were performed.
Results: Statistical analysis revealed a significant reduction in blood glucose levels following treatment (p = 0.001). Histopathological examination showed that Andaliman extract ameliorated alloxan-induced damage to the Islets of Langerhans. The 350 mg/kg body weight dose resulted in the most pronounced tissue regeneration compared to the other treatment groups.
Conclusion: The ethanol extract of Andaliman fruit effectively improves pancreatic histopathology and lowers blood glucose levels in diabetic rats

Diabetic foot ulcer is a common and serious complication of diabetes mellitus that requires effective wound management to accelerate healing and prevent further complications. Objective: This study aimed to evaluate the effectiveness of moist wound healing using hydrogel in improving wound healing outcomes among patients with diabetic foot ulcers at Asri Wound Care Center Medan. Methods: This study employed a case study design with purposive sampling. A total of five patients with diabetic foot ulcers stage 4 were included, consisting of three patients in the treatment group and two patients in the control group. Wound healing outcomes were assessed using the Bates-Jensen Wound Assessment Tool (BWAT) over seven wound care sessions. Results: The treatment group demonstrated a more significant improvement in wound healing compared to the control group. BWAT scores in the treatment group decreased from 43 to 24 (R1), 40 to 20 (R2), and 39 to 20 (R3), indicating marked reductions in necrotic tissue, slough, biofilm, and exudate, along with improved wound base and granulation tissue formation. In contrast, the control group showed smaller reductions in BWAT scores, from 40 to 34 (R4) and 41 to 33 (R5), reflecting a slower healing process with residual necrotic tissue and exudate. Conclusion: Moist wound healing using hydrogel was more effective than conventional wound care in accelerating the healing of diabetic foot ulcers, as evidenced by greater BWAT score reductions and improved wound characteristics. These findings support the use of hydrogel as an effective dressing in diabetic wound management
The Relationship Between Family Support and Anxiety Levels in Preoperative Patients with Reponible Hernia
Patients undergoing surgery often experience anxiety due to concerns about surgical outcomes, pain, and potential complications. Family support plays a vital role in helping patients cope with anxiety before surgery. This study aimed to determine the relationship between family support and anxiety levels among preoperative hernia reponibel patients in Nyi Ageng Serang Room 1 at RSUD Sekarwangi. A quantitative cross-sectional design was employed. The sample consisted of 32 preoperative patients with reducible hernia, representing the total population treated in the study setting during the data collection period. Patients selected using a total sampling technique. The research instruments used were the Multidimensional Scale of Perceived Social Support (MSPSS) to assess family support and the Hamilton Anxiety Rating Scale (HARS) to measure anxiety levels. Data were analyzed using the Chi-Square test with a significance level of p < 0.05. The results revealed a significant relationship between family support and patients’ anxiety levels (p = 0.000). Patients who received strong family support predominantly experienced mild anxiety (87.5%), while those with low family support tended to experience moderate to severe anxiety. These findings indicate that greater family support is associated with lower anxiety levels among preoperative patients. This study contributes evidence that family support is a significant determinant of preoperative anxiety among patients with reducible hernia in a regional hospital setting. The findings enrich nursing science by highlighting the importance of family-centered care as a contextual psychological support strategy in preoperative management, particularly in non-tertiary hospitals

Analysis of Psychological and Organizational Factors in the Implementation of Electronic Medical Records by Medical Records Staff at Welas Asih Regional Hospital
This study aims to analyze the psychological and organizational factors that influence the implementation of Electronic Medical Records (EMR) by medical records staff at Welas Asih Regional Hospital. This research method uses a descriptive qualitative method in the Medical Records installation of Welas Asih Regional Hospital. A sample of 33 people was taken using a purposive sampling technique, namely officers who actively use the EMR system. Data were collected using questionnaires, observation, and documentation. Although the implementation of EMR aims to improve efficiency and quality of service, challenges that arise are changes in work culture and psychological factors such as initial anxiety. From the organizational side, the main challenges are the need for additional training and constraints on infrastructure/system smoothness. Quantitative results show that psychological factors in the form of internal motivation are at a very strong level (average score of 4.87), in line with the high awareness of officers regarding the benefits of EMR (average score of 4.67). However, aspects of initial anxiety were still identified (average score of 2.90). Organizationally, there was a strong indication of the need for additional training (average 4.41) and the persistence of technical operational barriers (average 3.31), indicating the need to optimize facility support. Optimizing RME adoption requires a holistic strategy, including the provision of psychological support programs to mitigate anxiety, as well as targeted organizational investments in improving the quality of practical training and maintaining a smoother and more stable system infrastructure
Optimization Of Data Interoperability Between Simrs And The JKN Mobile Application As An Effort To Digital Transform Health Services At Welas Asih Regional Hospital
Digital transformation in Indonesia's healthcare sector is increasingly developing through the implementation of information system integration that supports accelerated service delivery, particularly in healthcare facilities with high visitor loads. This study analyzes the dynamics of interoperability between the Hospital Management Information System (SIMRS) and the JKN Mobile application in the patient registration process at Welas Asih Regional Hospital, a hospital with approximately 2,000 daily visits, of which approximately 1,500 use JKN Mobile. This study used a qualitative approach with a case study design, through in-depth interviews and direct observation of the registration process. The results indicate that the SIMRS–JKN Mobile integration is effective when the system is functioning normally, characterized by a fast barcode check-in process, automatic queue number synchronization without re-input, and consistent receipt number locking across all service stages up to claims. However, several obstacles still arise, particularly BPJS server disruptions, internet network instability, GPS reading issues, and limited patient digital literacy. Staff overcome these obstacles through adaptive strategies such as GPS calibration, switching to the hospital's Wi-Fi network, arrival time validation, and the use of the SIMRS approval feature. This study demonstrates that successful interoperability is determined not only by technological readiness but also by organizational readiness, staff operational competence, and patient capability in using digital applications. These findings provide a novel scientific contribution by providing comprehensive field evidence on the implementation of interoperability in high-volume healthcare facilities and providing recommendations for technical improvements and service governance for hospitals and policymakers
