Risk factors for complications in bolt-connected external ventricular drains
Introduction. This study was aimed at identifying risk factors for the main complications following bolt-connected external ventricular drain (EVD) insertion.
Material and methods. This was a single-centre cohort study. All patients who underwent bolt-connected EVD placement between March 2015 and February 2024 were included. The complications analysed were infection, haemorrhage, misplacement, obstruction, and accidental pull-out. The need for EVD replacement was also recorded. Univariable and Cox regression multivariate analyses were performed.
Results. A total of 119 procedures were analysed. The duration of ventriculostomy (OR 1.14; 95% CI 1.02–1.27) and the occurrence of cerebrospinal fluid leak (OR 409.86; 95% CI 1.36–12,353.36) or system obstruction (OR 31.44; 95% CI 2.04–484.85) were confirmed to be independent risk factors for infection. No risk factors were identified for misplacement or obstruction. Thicker catheters (OR 25.56; 95% CI 2.28–286.33) and antiplatelet or anticoagulant use (OR 7.29; 95% CI 1.75–30.41) were found to be independent risk factors for EVD-related haemorrhage. Men showed a 72% increased risk of accidental EVD pull-out. Finally, involuntary pull-out (OR 79.36; 95% CI 8.32–756.99), misplacement (OR 39.38; 95% CI 3.21–482.64), and obstruction (OR 31.55; 95% CI 2.70–368.40) were found to be independent risk factors for a new drain replacement.
Conclusions. We have confirmed the duration of ventriculostomy, cerebrospinal fluid leak, and catheter obstruction to be independent risk factors for infection. Thicker catheters and antiplatelet or anticoagulant drug use were identified as independent risk factors for EVD-related haemorrhage. Male gender increased the risk for involuntary catheter pull-out. Finally, accidental removal, obstruction, and misplacement were confirmed as independent risk factors for re-inserting a new EVD. Neither surgeon's experience nor bedside placement in the intensive care unit increased the risk.
Clinical implications. Our study has identified the risk factors for the most common complications associated with a specific type of ventricular catheter (bolt-connected EVDs), which allows the targeting of preventive measures. This is the first study to have analysed this specific group of drains, which are increasingly being used in clinical practice
Acalabrutinib in treatment of patients with chronic lymphocytic leukemia including those at high genetic risk
Acalabrutinib belongs to a class of Bruton’s tyrosine kinase inhibitors (BTKis) which have become one of the most widely used drugs for the treatment of B-cell malignancies including chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL), mantle cell lymphoma, Waldeström’s macroglobulinemia, and marginal zone lymphoma. Chronic lymphocytic leukemia is the most frequently diagnosed hematological malignancy in the Western world, with a median age of 72 at diagnosis. BTK inhibitors are recommended in monotherapy until disease progression or unacceptable toxicity. Acalabrutinib shows high specificity towards its target, with minimal off-target activity regarding other kinases. This leads to fewer treatment-related adverse events compared to the first-class BTK inhibitor, ibrutinib. Moreover, acalabrutinib has proven its efficacy in numerous clinical trials i.e. ACE-Ly-004, STELLAR, ASCEND, ELEVATE-TN, and ELEVATE-RR, including in high-risk patients. The most clinically relevant high-risk feature of chronic lymphocytic leukemia is a disruption of TP53 signaling via either 17p deletion or TP53 mutation. Another significant adverse genetic marker is unmutated IGHV. Recent trials have aimed at studying novel combination regimens that lead to a negative measurable residual disease status and allow for time-limited highly-active chemotherapy-free treatment, with acalabrutinib as one of the key players
Hypofractionated whole and partial breast irradiation: Brazilian Society of Radiotherapy (SBRT) consensus
Background: Breast cancer is the most common cancer in women worldwide, with 73,610 new cases expected annually in Brazil between 2023 and 2025. Post-operative radiation therapy (PORT) is a critical component of treatment, and recent advances have allowed for shorter treatment times that can help overcome shortages in low- and middle-income countries. The Brazilian Society of Radiotherapy (SBRT) updated its consensus on hypofractionated whole-breast radiotherapy and included recommendations for partial breast irradiation.
Materials and methods: The SBRT convened a national panel of experts to develop updated recommendations. Using a modified Delphi method, the panel reached a consensus through structured rounds of voting. Recommendations were categorized based on the strength of available evidence.
Results: The consensus supports hypofractionation, which offers shorter, cost-effective treatment schedules, and partial breast irradiation (PBI), which targets high-risk areas while sparing healthy tissue. Despite high-quality evidence, adopting these techniques has been inconsistent. The panel’s recommendations provide evidence-based guidance to clinicians, tailored to the Brazilian context, emphasizing safety and efficacy.
Conclusion: The updated SBRT consensus presents hypofractionation and PBI as practical alternatives to conventional radiation therapy, offering improved access and reduced costs. These recommendations aim to guide clinicians in adopting these approaches and help address barriers to access
Quality evaluation of synthetic CT using a gamma analysis for prostate cancer MR-only radiotherapy
Background: This study evaluates the quality of synthetic computed tomography (sCT) images for MR-only radiotherapy in prostate cancer using gamma analysis. A software tool, MRGamma, was developed to address challenges like the absence of electron density maps and registration uncertainties between magnetic resonance imaging (MRI) and planning CT (pCT).
Materials and methods: Aplication developed in MATLAB assesses Hounsfield units (HU) discrepancies between CT and sCT images via 2D and 3D gamma analysis (GA). GA computes a gamma index (γ) for each voxel based on HU differences and distance to agreement (DTA). The study analyzed prostate cancer patients using the MRCAT algorithm to generate sCT images. Preprocessing included registration, resampling, and cropping to standardize image dimensions. GA was performed with thresholds of 100 HU and 70 HU for HU differences and 3 mm for DTA to evaluate image quality.
Results: Aplication demonstrated high agreement between sCT and CT images. For 3D GA (100 HU, 3 mm), 95.5 ± 1.8% of voxels passed the threshold, with slightly lower rates for 70 HU. Discrepancies were most pronounced at tissue interfaces and air cavities, where HU variations were more significant. Mean γ values for 3D GA were 0.19 ± 0.04 (milder parameters) and 0.23 ± 0.05 (stricter parameters), showing consistent trends in 2D GA. Maximum γ values confirmed good overall agreement.
Conclusions: Aplication effectively evaluates sCT quality, supporting the feasibility of MR-only radiotherapy. By providing detailed HU comparisons, the tool enhances MR-based treatment planning, reducing costs and ionizing radiation exposure. Its implementation may improve workflow efficiency and patient safety in clinical practice
Impact of ultrasound-guided brachytherapy on optimizing planning outcomes and target volume definition in cervical cancer
Background: Combined external beam radiotherapy (EBRT) and intracavitary brachytherapy (ICBT) are the standard treatments for cancer cervix. 3D-based image-guided brachytherapy has changed the face of intracavitary applications and allowed better dosimetric outcomes. While magnetic resonance imaging (MRI)-based therapy is the current standard, computed tomography (CT) with ultrasound measurements could represent an acceptable alternative with less cost and real-time imaging advantage.
Material and methods: Our study included sixty-four patients with uterine cervix cancer (stages IB to IVA) who underwent concurrent chemoradiotherapy (CCRT), followed by high dose rate (HDR)-ICBT 8 GY/FR/weekly. Patients were randomized into two arms: Arm A (application done ultrasound-guided with each fraction in 32 cases) and Arm B (without image guidance). The rate of optimal insertion is defined as tandem centralized within the uterine cavity without perforation; the needle insertion rate is also calculated between both arms.
Results: Ultrasound guidance significantly improved the rate of optimal insertion and reduced the rate of uterine perforation by 12% (from 14.6% to 1.8%). Optimal insertion positively impacted isodose distribution, resulting in numerically higher Dmax values for organs at risk (OARs) in the non-ultrasound-guided group. However, it was not statistically significant. On the other hand, MRI and US measurements were found to be comparable, with the largest mean difference being 2.3 mm, which is not found to be clinically significant.
Conclusion: The use of ultrasound is strongly recommended during brachytherapy as it minimizes the rate of suboptimal insertions. Also, US measurement strongly correlated with MRI, which may support its use in a radiotherapy setting with a lack of MRI availability
Kliniczne i biologiczne implikacje mutacji genu TP53 w ostrych białaczkach szpikowych i nowotworach mielodysplastycznych
The TP53 gene encodes the p53 protein, a transcription factor that regulates cellular processes, DNA repair, and apoptosis. Mutations in this gene occur in over 50% of cancers, highlighting its crucial role as a tumour suppressor gene. In hematologic malignancies, TP53 mutations (TP53mut) are less frequent than in solid tumours; however, they significantly correlate with poor prognosis. The frequency of TP53mut ranges from 5–10% in myelodysplastic syndromes (MDS) and acute myeloid leukaemia (AML) de novo, 25–40% in therapy-related cases, and up to 50% in complex karyotypes. Most TP53 mutations are inactivating missense mutations found in several dominant hotspots. Missense, nonsense, and frameshift TP53 mutations, along with chromosomal loss involving TP53, lead to a loss of function of the normal p53 protein and a dominant negative effect (DNE) on the wild-type/unmutated TP53 variant. Treating patients with MDS and AML TP53mut remains a significant challenge and an unmet need in haematologic therapy. Despite significant advances in understanding the pathogenesis of MDS and AML TP53mut, an effective treatment that substantially prolongs survival has yet to be introduced. TP53-mutant clones demonstrate resistance to standard chemotherapy and clonal expansion is observed under the selective pressure of chemotherapy. In MDS and AML, median survival ranges from 5 to 10 months, irrespective of the therapy used, even when a treatment response is noted. Comprehensive TP53mut diagnostics, including allelic status, mutation type, and localisation, co-mutations, are crucial for prognostic assessment and therapeutic decision-making, including patient eligibility for clinical trials. This study highlights the impact of TP53mut on the pathogenesis and treatment outcomes of patients with MDS and AML, as well as diagnostic criteria for these diseases based on the current classifications of haematologic malignancies.Gen TP53 koduje białko p53, czynnik transkrypcyjny niezbędny do regulacji procesów komórkowych, naprawy DNA i apoptozy komórek. Mutacje TP53 występują w ponad 50% nowotworów, podkreślając kluczową rolę tego genu jako supresora nowotworowego. W nowotworach hematologicznych mutacje TP53 (TP53mut) są rzadsze niż w nowotworach litych, jednak znacząco korelują z gorszym rokowaniem. Częstość występowania TP53mut wynosi 5–10% w nowotworach mielodysplastycznych (MDS) oraz ostrych białaczkach szpikowych (AML) de novo, 25–40% w postaciach AML/MDS zależnych od terapii oraz do 50% w AML/MDS z kariotypem złożonym. Większość występujących TP53mut to inaktywujące mutacje typu missense, zlokalizowane w kilku dominujących hotspotach. Mutacje TP53 typu missense, nonsense, frameshift lub utrata chromosomu zawierającego TP53, powodują utratę funkcji prawidłowego białka p53 oraz dominujący potencjał negatywny (DNE) nad dzikid/niezmutowanyd wariantem TP53. Leczenie pacjentów z MDS- i AML-TP53mut stanowi duże wyzwanie oraz niezaspokojoną potrzebę w zakresie terapii hematologicznych. Pomimo ogromnego postępu w zakresie patogenezy MDS i AML-TP53mut, nie udało się wprowadzić do leczenia skutecznej terapii, która wydłużałaby przeżycia chorych. Klony TP53mut wykazują oporność na standardową chemioterapię, a pod wpływem selekcyjnego działania chemioterapii często obserwowana jest ekspansja klonalna. W MDS i AML, nawet jeśli odpowiedź na leczenie jest obserwowana, mediany przeżycia wynoszą od 5 do 10 miesięcy, niezależnie od zastosowanego leczenia. Kompleksowa diagnostyka mutacji TP53, w tym statusu allelicznego, rodzaju i lokalizacji mutacji, obecności mutacji współwystępujących, jest kluczowa dla określenia rokowania i podejmowania decyzji terapeutycznych, w tym kwalifikacji pacjentów do badań klinicznych. W niniejszej pracy przedstawiono wpływ TP53mut na patogenezę i wyniki leczenia chorych na MDS i AML, jak również kryteria diagnostyczne wspomnianych jednostek chorobowych w aktualnych klasyfikacjach nowotworów układu krwiotwórczego
Eltrombopag jako składnik wielolekowej terapii u niemowlęcia z dramatycznym przebiegiem małopłytkowości immunologicznej
Małopłytkowość immunologiczna w populacji pediatrycznej ma w większości przypadków łagodny, niekiedy samoograniczający się przebieg. Brak odpowiedzi na terapię pierwszego rzutu oraz ciężkie krwawienia zdarzają się w niej wyjątkowo rzadko. Wobec tego, oporna na leczenie ITP u dzieci stanowi każdorazowo duże wyzwanie terapeutyczne. W niniejszym artykule zaprezentowano przypadek 3-miesięcznego niemowlęcia z nowo rozpoznaną małopłytkowością immunologiczną powikłaną krwawieniem śródmózgowym oraz krwawieniem z przewodu pokarmowego. Osiągnięcie ostatecznej remisji było możliwe dzięki wielolekowej terapii złożonej z immunoglobulin dożylnych, glikokortykosteroidów, dwóch agonistów receptora trombopoetyny, rytuksymabu, winkrystyny, a także oseltamiwiru. Pomimo dramatycznego przebiegu małopłytkowości, chłopiec po niespełna trzech tygodniach terapii wszedł w remisję i pozostaje w niej do dnia dzisiejszego. Opisany przypadek uzasadnia korzyści płynące z wczesnego włączenia terapii łączonej małopłytkowości opornej na leczenie, w której istotną rolę odgrywają agoniści receptora trombopoetyny
Oporna na leczenie pierwotna małopłytkowość immunologiczna
W artykule przedstawiony został opis przypadku pacjentki z oporną na leczenie pierwotną małopłytkowością immunologiczną (pITP). Pomimo zastosowania kilku linii leczenia — steroidoterapii, agonistów receptora trombopoetyny, splenektomii — nie uzyskano prawidłowej liczby płytek krwi. Ponowne włączenie do leczenia eltrombopagu przyniosło zadowalający efekt. Pacjentka przyjmuje lek i pozostaje pod stałą opieką poradni hematologicznej
Zapobieganie i leczenie działań niepożądanych związanych z terapią skojarzoną ibrutynibem i wenetoklaksem w przewlekłej białaczce limfocytowej
Skojarzenie ibrutynibu i wenetoklaksu jest pierwszą doustną terapią o ustalonym czasie trwania, wykorzystującą inhibitor kinazy tyrozynowej Brutona i inhibitor BCL-2 u dorosłych pacjentów z nieleczoną przewlekłą białaczką limfocytową. Podstawą do zatwierdzenia schematu stały się wyniki badań klinicznych fazy II CAPTIVATE i fazy III GLOW. Omawiane skojarzenie charakteryzuje się wysoką skutecznością, ograniczonym czasem trwania leczenia oraz akceptowalnym profilem działań niepożądanych, choć wyniki badań klinicznych mogą rodzić pewne obawy w odniesieniu do tolerancji terapii. Ryzyko wystąpienia działań niepożądanych można zminimalizować poprzez staranny dobór pacjentów, podejmowanie działań profilaktycznych oraz racjonalne zarządzanie toksycznością związaną z leczeniem. W związku z powyższym, celem autorów niniejszej pracy było dokonanie podsumowania opublikowanych danych z badań klinicznych i obserwacji z praktyki klinicznej na potrzeby optymalizacji skuteczności i tolerancji terapii skojarzonej ibrutynibu z wenetoklaksem
Rekomendacje diagnostyczne i terapeutyczne dla przewlekłej białaczki limfocytowej w 2025 roku — Raport Grupy Roboczej PTHiT i PALG-CLL
Przewlekła białaczka limfocytowa (PBL) jest chorobą osób starszych z medianą wieku podczas rozpoznania około 70 lat. Naturalny przebieg choroby jest bardzo zróżnicowany, a chorzy z bezobjawową i nieprogresywną postacią białaczki nie wymagają terapii. Wskazaniem do leczenia pozostaje zaawansowana i/lub progresywna PBL. Wyniki leczenia PBL uległy istotnej poprawie dzięki wprowadzeniu nowych, skutecznych leków: inhibitorów receptora limfocytu B (BCR, B-cell receptor) oraz inhibitorów BCL2 (B-cell lymphoma 2). Leki te w monoterapii stosuje się w sposób ciągły, a w schematach skojarzonych przez czas określony. Wenetoklaks w połączeniu z przeciwciałami anty-CD20 jest stosowany przez 24 (z rytuksymabem) lub 12 (z obinutuzumabem) 28-dniowych cykli a leczenie ibrutynibem i wenetoklaksem przez 15 28-dniowych cykli. Wybór metody leczenia jest uzależniony przede wszystkim od oceny delecji 17p/mutacji TP53 oraz w pierwszej linii, statusu mutacji genów dla części zmiennej łańcucha ciężkiego immunoglobulin (IGVH, immunoglobulin variable heavy chain). Rola immunochemioterapii w leczeniu chorych na PBL jest obecnie marginalna. Chorzy ze wskazaniami do leczenia, zarówno w pierwszej, jak i kolejnych liniach terapii powinni otrzymać nowe terapie celowane, które są obecnie dostępne w Polsce w ramach programu lekowego NFZ. W niniejszym artykule przedstawiono aktualizację standardów postępowania w diagnostyce i terapii w roku 2025
