Sex differences in blood pressure variability in office, home and ambulatory measurements
Background: Hypertension is widely recognized as a significant risk factor for cardiovascular diseases. Beyond merely focusing on blood pressure levels, there is growing recognition of the importance of considering blood pressure variability (BPV).
Aims: The aim of this study was to compare factors influencing BPV in men and women in office and out-of-office measurements.
Methods: The study enrolled 120 women and 99 men recruited from an outpatient clinic between 2021 and 2022. All participants underwent a medical interview. Subsequently, office blood pressure measurements were conducted using two methods - unattended and attended measurements Following this, 24-hour ambulatory blood pressure monitoring was performed, and participants were instructed to conduct home blood pressure measurements for 7 days. Laboratory tests, echocardiography and carotid artery ultrasound were performed thereafter.
Results: BPV was higher in women, in older patients, those with diabetes and smokers. Female sex remained significant determinant of higher BPV in multiple regression analysis (b = –0.299; P = 0.002) after adjustment for age (b = 0.247; P = 0.01), body mass index (b = 0.012; P = 0.89), diabetes (b = –0.155; P = 0.08), smoking (b = 0.063; P = 0.48) and blood pressure values (b = 0.478; P <0.001). BPV is associated with parameters of subclinical organ-damage and this relationship is stronger for women than men.
Conclusions: Women exhibit higher BPV despite having lower blood pressure. Reducing the BPV, a multifaceted phenomenon related to organ damage, necessitates integrated intervention focused on optimizing blood pressure values on one hand, and managing metabolic risk factors and lifestyle modifications, notably including tobacco cessation, on the other
Allergic reactions during coronary angiography or PCI in Poland: Occurrence, trends, and long-term perspective based on the Polish ORPKI registry
Background: Allergic reactions during coronary angiography and percutaneous coronary intervention (PCI) in patients with acute coronary syndromes can affect their outcomes.Aims: We aimed to investigate the incidence of allergic reactions and predictors of their occurrence in the catheterization laboratory.Material and methods: The study included 1 812 994 treatment cases available in the ORPKI Polish National Registry, of which 659 372 were for women and 1 144 689 for men. Data were collected from 161 interventional cardiology centers in Poland between 2014 and 2022 and included in the retrospective analysis. An allergic reaction was defined as any hypersensitive response of the immune system manifesting as bronchospasm, asthma exacerbation, conjunctivitis, urticaria, eczema, angioedema, or anaphylactic shock.Results: There was a decreasing incidence of allergic reactions overall, in both sexes and all age subgroups. The incidence of allergic reactions each year oscillated between 0.02% and 0.11% (P <0.001), and the most recent one was 0.02% in 2022. The incidence of allergic reaction was associated with low-molecular-weight heparin (LMWH) (OR, 23.5 [95% CI, 18.92–29.19]), glycoprotein IIb/IIIa inhibitors (GPI IIb/IIIa) (OR, 2.31 [95% CI, 1.92–2.78]), previous PCI (OR, 1.55 [95% CI, 1.34–1.8]), radiation dose (OR, 1.25 per 1000 mGy [95% CI, 1.19–1.31]), and contrast dose (OR, 1.17 per 100 ml [95% CI, 1.08–1.28]).Conclusions: The overall incidence of acute allergic reactions in the cath lab is low and decreased significantly between 2014 and 2022. The decrease in frequency did not depend on age or sex. Independent predictors of allergic reaction incidence were the use of GPI IIb/IIIa, LMWH, and dose of contrast agent and radiation

Terapia ta umożliwia również bezpieczną rezygnację u części chorych ze standardowej chemioterapii przy zachowaniu wysokiej skuteczności leczenia. W artykule przedstawiono opis przypadku chorego zakwalifikowanego do uzupełniającego leczenia ozymertynibem w ramach Programu Wczesnego Dostępu do produktu leczniczego.
Assessment of the impact of VDR polymorphisms on selected hormonal, metabolic and mineral balance markers in young women with hyperandrogenism
Objectives: Hyperandrogenism is a frequently recognized endocrine imbalance in which there is excessive production of androgens. The purpose of the study was to investigate the impact of vitamin D receptor (VDR) gene polymorphisms on chosen bone metabolism and biochemical parameters in women with hyperandrogenism. Material and methods: Eighty young females with hyperandrogenism were enrolled in the study, in whom selected parameters of bone turnover, endocrine and metabolic parameters were determined. Two polymorphisms of the VDR gene were analyzed: rs731236 (TaqI) and rs1544410 (BsmI), using real-time polymerase chain reaction (PCR). Statistical tests were performed in this research with the program SPSS Statistics 17.0 for Windows. Results: The rs731236 and rs1544410 polymorphisms of the VDR gene turned out to be statistically significantly related to the concentration of insulin determined in the 60 'glucose tolerance test. There was no relationship between the studied polymorphisms of the VDR gene and the determined parameters of bone metabolism and other biochemical parameters. Conclusions: The research presented that VDR gene variants may influence disturbances in carbohydrate metabolism in young women with hyperandrogenism
Comparison of VCare and SecuFix uterine manipulator in total laparoscopic hysterectomy: a prospective randomized trial
Objectives: To compare the Vcare and SecuFix uterine manipulators (UM) used in total laparoscopic hysterectomy (TLH) with regard to operating time, duration of hospital stay, the time for insertion of the manipulator, time for completion of colpotomy, and maintenance of pneumoperitoneum, estimated blood loss, and perioperative complications.
Material and methods: This prospective, single-center, randomized controlled trial was conducted on patients who underwent TLH for benign indications between June 2023 and January 2024. Patients were randomized to either the Vcare UM or SecuFix UM group. Both groups were compared for operative time, colpotomy time, intrauterine manipulator insertion time, pneumoperitoneum maintenance, blood loss, complications, and length of hospital stay.
Results: A total of 100 patients were enrolled, 50 with VcareUM and 50 with SecuFix UM. SecuFix UM demonstrated shorter colpotomy time (p < 0.001) and better pneumoperitoneum maintenance (p = 0.008) compared to VCare. VCare had shorter insertion time (p = 0.001) but a higher uterine rupture rate (p = 0.001). No significant differences were found in operative time, blood loss, complications, or length of hospital stay (p > 0.05).
Conclusions: SecuFix may offer advantages in colpotomy and pneumoperitoneum maintenance during TLH compared to VCare. The Vcare UM demonstrated a shorter insertion time than the SecuFix UMs, yet a higher rate of uterine rupture was observed with its placement
