    Wtrętowe zapalenie mięśni – opis przypadku z odniesieniem do kryteriów diagnostycznych z 2024 roku

    W pracy opisano przypadek 82-letniej kobiety z zaburzeniami chodu, osłabieniem kończyn dolnych oraz z dysfagią, u której na podstawie badań neurofizjologicznych, biopsji mięśnia i wyników laboratoryjnych rozpoznano wtrętowe zapalenie mięśni (IBM, inclusion body myositis). W artykule omówiono najnowsze koncepcje IBM jako choroby autoimmunologicznej ale także zwyrodnieniowej. Wtrętowe zapalenie mięśni cechuje się stopniowym osłabieniem mięśni, często asymetrycznym, z zajęciem głębokich zginaczy palców, prostowników kolan. Choć spotykane są i inne, mniej typowe wzorce zajęcia mięśni. Zagadnienie badań pomocniczych niezbędnych do przeprowadzenia diagnostyki różnicowej zostało omówione na podstawie najnowszych kryteriów diagnostycznych wtrętowego zapalenia mięśni z 2024 roku

    An unusual case of pityriasis rosea Gibert

    Pityriasis rosea Gibert typically follows a recognizable clinical course. However, some cases may pose diagnostic complexities due to their atypical features. A 71-year-old patient with a history of arterial hypertension and type 2 diabetes, presented at the dermatology department with persistent skin lesions over the past 4 weeks. The skin lesions were initially located on the forearms and lower legs, later spreading to the trunk and head. The patient did not associate the onset of the lesions with any specific trigger. Dermatological examination revealed numerous papular-erythematous-oedematous lesions of ambiguous character located on the head, trunk, and upper and lower limbs, accompanied by massive bilateral blepharedema. Laboratory investigations revealed deviations related to the systemic diseases of the patient inter alia normocytic anaemia, elevated creatinine and prostate-specific antigen (PSA) concentration and proteinuria. A skin biopsy dispelled doubts and led to the diagnosis of pityriasis rosea Gibert. Untreated kidney disease could have potentially led to the atypical course of the dermatosis. Treatment with topical steroids resulted in the gradual resolution of skin lesions. This case contributes to the body of knowledge surrounding pityriasis rosea Gibert by highlighting the diagnostic challenges associated with atypical presentations. The histopathological findings underscore the importance of biopsy for accurate diagnosis even of this common dermatosis

    The role of Anti-Müllerian hormone in women health

    Anti-Müllerian hormone (AMH), also known as Müller duct inhibitory factor and primarily known for its role in sexual differentiation. In female fetuses, AMH production by granulosa cells begins around the 36th week of gestation and continues in women until menopause. It is becoming more significant in the endocrine and gynecological diagnosis of adult women. The suppressive effect of AMH on follicle-stimulating hormone (FSH)-induced aromatase production likely plays a role in hyperandrogenism in polycystic ovary syndrome (PCOS) and may increase insulin resistance. Female adolescent with type 1 diabetes (T1D) have an increased likelihood of developing PCOS, but it is not known whether they also show elevated AMH levels. The elevated AMH levels observed in prepubertal girls with T1D suggest that there are more small follicles in their ovaries, probably in response to insulin treatment. Elevated levels of androgens and anti-Müllerian hormone have been previously reported in non-pregnant women with diabetes. The increased AMH concentrations can be associated with reduced systemic inflammation [lower c-reactive protein (CRP) values], irrespective of the type of diet and greater insulin sensitivity in old obese males. AMH can also serve as a valuable marker for granulosa cell tumors (folliculomas) and their recurrence.     Evaluating the impact of red blood cell parameters on outcomes of dinoprostone-induced labor: a retrospective study

    Objectives: This study explores the relationship between red blood cell parameters and perinatal outcomes in dinoprostoneinsert-induced labor at the Department of Obstetrics and Perinatology of the Jagiellonian University MedicalCollege Cracow, Poland.Material and methods: Conducted between May 2019 and February 2021, this retrospective analysis included data onmaternal age, pre-pregnancy BMI, hemoglobin level, red blood cell count, mean cell volume, and red blood cell distributionwidth, alongside newborn birthweight and Apgar scores. The study evaluated the impact of anemia, a hemoglobinlevel lower than 12 g/dL, on the likelihood of cesarean section, labor duration, delivery time, and neonatal outcomes.Results: With 183 participants, findings indicate no significant difference in perinatal outcomes between anemic andnon-anemic groups, suggesting that mild anemia may not significantly affect dinoprostone insert-induced labor outcomes.Conclusions: The study highlights the importance of a nuanced approach to managing anemia in pregnancy, emphasizingthe necessity for individualized treatment strategies. Our findings contribute to the ongoing discourse on anemiamanagement during pregnancy, underscoring the need for more targeted research to develop evidence-based guidelines
