Retrospective study of patients applied to Düzce University department of forensic medicine for the purpose of assessing criminal liability
Amaç: Ceza sorumluluğunun değerlendirilmesi, kişinin işlediği ve sonucunda zarar oluşmuş olan bir fiille ilgili olarak, yasada cezaya hükmedilmesinin gerektiği hallerde kişinin kusuru olup olmadığının tanımlanmasını sağlayan değerlendirmedir. Türk Ceza Kanunu'nun 32. maddesinde akıl hastalığı-zayıflığı nedeniyle kişinin işlemiş bulunduğu fiilin anlam ve sonuçlarını algılayamadığı veya işlediği fiille ilgili olarak irade yeteneğinin bulunmadığı halleri, 34. maddesinde geçici bir neden, irade dışı alınmış alkol veya madde etkisiyle ceza sorumluluğunun kalktığı durumları kapsamaktadır. Bu maddelere ilişkin değerlendirmeler ülkemizde tıbbi bilirkişi olarak adli makamların görevlendirdiği hekimlerce yapılmaktadır. Çalışmamızda bu değerlendirmenin yapılması amacıyla Adli Tıp Anabilim Dalımız üzerinden Adli Tıp Uzmanlar kurulumuza 2021-2023 yıllarında yönlendirilen suç tarihleri itibariyle 18 yaş ve üzeri olguların; sosyademografik veriler, suç tipi, psikiyatrik hastalıkları gibi parametreler retrospektif olarak değerlendirilerek olguların analizlerinin yapılması amaçlanmaktadır. Yöntem: Çalışmamız retrospektif bir çalışma olup, 01.01.2021- 31.12.2023 tarihleri arasında Düzce Üniversitesi Adli Tıp Anabilim Dalına ceza sorumluluğunun değerlendirilmesi amacıyla başvuran suç tarihi itibariyle 18 yaş ve üzeri olguları kapsamıştır. Çalışmamızda olgulara ait sosyodemografik ve klinik özellikler, suç teşkil ettiği iddia edilen fiilleri, suçların işlendiği mekan, suçun failinin mağduru ile yakınlık durumu, adli sürece ait özellikler, adli muayene bulguları gibi özellikler değerlendirilmiştir. Sonrasında olgular TCK 32. Maddeden yararlanma ve yararlanmama durumlarına göre iki gruba ayrılmış ve gruplar arasında fark olup olmadığı incelenmiştir. Verilerdeki sayısal sonuçlar aritmetik ortalama ve standart sapma, kategorik sonuçlar ise yüzde olarak değerlendirilmiştir. Grup karşılaştırmalarında verilerin dağılımına uygun olarak istatistiksel analizler uygulanmıştır. İstatistiksel analizler "SPSS (Statistical Package for Social Sciences) for Windows 26.0 (SPSS Inc, Chicago, IL)" aracılığıyla değerlendirilmiştir. Bulgular: Çalışmada ilgili yıllarda suç tarihi itibariyle 18 yaş ve üzeri olan, rapor sonuçları itibariyle ceza sorumluluğuna dair kanaate ulaşılmış, dosya içeriği açısından değerlendirilen parametrelerin yer aldığı toplam 446 olgu değerlendirmeye alınmıştır. Tüm olgular belirlenen parametrelerle değerlendirilmiş, sonrasında veriler TCK 32. maddeden yararlananlar ve yararlanmayanlar olarak ayrılarak farklılıklar tespit edilmiştir. Bu farklılıklar arasında TCK 32. maddeden yararlanan olguların yararlanmayanlara göre; çiftçi olma durumları, iletişim araçlarıyla daha çok suç işledikleri, askerlikten akıl hastalığı nedeniyle daha sık muaf oldukları, öğrenim durumlarının farklılaştığı, akıl hastalıklarının farklılaştığı, suç tipleri açısından cinsel suçlar ve mala zarar verme suçunun daha sık olduğu, işledikleri fiilin mağdurunun 1. derece akraba olma oranının daha sık olduğu, suçu kendi iş yerlerinde işleme oranının daha yüksek olduğu, ceza sorumluluğu değerlendirmesine daha erken yönlendirildikleri tespit edilmiştir. Sonuç: Çalışmamızın sonuçlarının literatürle uyumlu yönleri olmakla birlikte, literatürde yeterince konuşulmamış bazı sonuçlara ulaşılmıştır. Ceza sorumluluğuna yönlendirilen olguların sosyodemografik özellikleri ülke genelindeki pek çok çalışmayla benzer özellikler gösterdiği tespit edilmiştir. Askerlik durumlarının ayrıntılı sorgulanması, suçun mağdurları, suçun gerçekleştiği mekan, suçun iletişim aracıyla işlenmiş olup olmaması gibi durumların farklılaşması dikkate alındığında daha doğru sonuçlara ulaşmak için odaklı çalışmalara ihtiyaç olduğu tespit edilmiştir.Aim: Evaluation of criminal responsibility is an evaluation that enables the definition of whether a person is at fault in cases where he/she should be punished for the crime committed. Article 32 of the Turkish Penal Code covers situations where a person cannot perceive the meaning and consequences of the act he/she has committed due to mental illness or weakness, or lacks the ability to willpower regarding the act he/she has committed, and Article 34 covers situations where criminal liability is abolished due to a temporary reason, involuntarily consumed alcohol or substance. Evaluations regarding these items are made by physicians appointed by the judicial authorities as medical experts in our country. In order to make this evaluation in our study, cases aged 18 and over as of the crime dates referred to our Board of Forensic Medicine Experts through our Department of Forensic Medicine in 2021-2023; It is aimed to analyze the cases by retrospectively evaluating parameters such as sociodemographic data, crime type, and psychiatric diseases. Materials and Methods: Our study is a retrospective study and includes cases aged 18 and over as of the date of the crime who applied to Düzce University Department of Forensic Medicine between 01.01.2021 and 31.12.2023 for the evaluation of criminal responsibility. In our study, sociodemographic and clinical characteristics of the cases, acts alleged to constitute a crime, the place where the crimes were committed, the relationship between the perpetrator of the crime and the victim, characteristics of the judicial process, and forensic examination findings were evaluated. Numerical results in the data were evaluated as arithmetic mean and standard deviation, and categorical results were evaluated as percentages. Statistical analyzes were applied in accordance with the distribution of the data in group comparisons. Statistical analyzes were evaluated using "SPSS (Statistical Package for Social Sciences) for Windows 26.0 (SPSS Inc, Chicago, IL)". Results: In the study, a total of 446 cases were evaluated in the relevant years, who were 18 years old and over as of the date of the crime, whose criminal responsibility was reached according to the report results, and whose file content was evaluated according to the parameters. All cases were evaluated with the determined parameters, and then the data were separated into those who benefited from Article 32 of the TCK and those who did not, and differences were determined. Among these differences, it was determined that the cases who benefited from Article 32 of the TCK were more farmers, committed more crimes through communication tools, were exempted from military service more often due to mental illness, their educational status differed, their mental illnesses differed, in terms of crime types, sexual crimes and property damage were more common, the rate of the victim of the act they committed was more frequent than the rate of being a first-degree relative, the rate of committing the crime in their own workplace was higher, and they were directed to the criminal responsibility assessment earlier. Conclusion: Although the results of our study are compatible with the literature, some results that have not been discussed enough in the literature have been reached. It has been determined that the sociodemographic characteristics of the cases referred to criminal responsibility show similar characteristics to many studies across the country. Considering the differences in situations such as detailed questioning of military service status, victims of the crime, the place where the crime took place, whether the crime was committed through a communication tool or not, it has been determined that focused studies are needed to reach more accurate results
An Exploratory Study of Serum Vasorin Levels in Polycystic Ovary Syndrome: A Novel Potential Biomarker for Diagnosis and Pathogenesis
Objective: This study aims to investigate the potential role of vasorin as a novel biomarker in the pathogenesis of polycystic ovary syndrome (PCOS) by evaluating serum vasorin levels in women diagnosed with PCOS. Methods: A prospective study was conducted at D & uuml;zce University Faculty of Medicine between March and July 2024, including 92 women with PCOS, diagnosed based on the 2003 Rotterdam criteria, and 68 age- and BMI-matched healthy controls. Serum vasorin levels were measured using an enzyme-linked immunosorbent assay (ELISA) and compared between the two groups. Additionally, correlations between vasorin levels and metabolic, inflammatory, and hormonal parameters were analyzed. Results: Women with PCOS had significantly lower serum vasorin levels (median: 0.70 pg/mL) compared to the control group (median: 2.36 pg/mL, p < 0.001). No significant correlation was found between vasorin and metabolic or hormonal parameters in the PCOS group. However, a weak positive correlation with prolactin was observed in the control group (r = 0.264, p = 0.030). Although vasorin is involved in inflammatory and oxidative-stress pathways, its association with insulin resistance and lipid metabolism remains unclear based on this study. Receiver Operating Characteristic (ROC) curve analysis demonstrated a high diagnostic performance for vasorin in distinguishing PCOS from healthy individuals (AUC = 0.918, p < 0.001, 95% CI: 0.869-0.967). The optimal cutoff value for vasorin (1.285 pg/mL) yielded 92.6% sensitivity and 87.0% specificity. Conclusions: These findings suggest that vasorin may serve as a promising biomarker for PCOS, potentially linking hormonal dysregulation, inflammatory responses, and ovarian dysfunction. However, further validation is required through longitudinal studies, multi-center cohorts, and mechanistic investigations. Additionally, comparative assessments with established biomarkers such as anti-M & uuml;llerian hormone (AMH) and androgen levels are warranted to determine vasorin's diagnostic and prognostic utility in clinical practice
Response Surface Method Optimization of Hard Machining with Cryogenically Treated Cutting Tools: Surface Roughness, Cutting Force, Tool Wear, Rietveld Analysis, and Metallurgical Properties
The study consists of two stages: machinability tests and metallographic examinations. In the first stage of the study, response surface methodology (RSM) optimization was performed for the main-cutting-force (Fc) and average surface roughness (Ra) values obtained as a result of turning tests. In the second stage, the wear performance of cutting tool tips with and without deep cryogenic treatment was explained with the support of SEM images, EDX, XRD, and microhardness analyses. As a result of ANOVA analysis, it was found that the most effective parameter on Fc was the feed rate, with 78.89%. As a result of RSM-optimization, the lowest Fc value was found to be 339.99 N. The most effective parameter on Ra was found to be the feed rate at 69.38%. In terms of tool life, 15% and 11% improvements were achieved in uncoated and coated carbide cutting tools, respectively. As a result of Rieltveld's analysis, it was determined that cryogenic treatment increased the carbide percentages in the tools. Optimal planning of solar PV-based electric vehicle charging stations empowered by energy storage system: Feasibility and green charge potential
The rapid growth of electric vehicle (EV) adoption and declining photovoltaic (PV) costs have accelerated global efforts to integrate renewables into EV charging infrastructure. In emerging economies like T & uuml;rkiye, these developments pose both opportunities and challenges due to limited PV installation areas, variable charging demand, and the capital-intensive nature of energy storage systems (ESS). This study presents a techno-economic and environmental optimization of hybrid solar-powered EV charging stations (EVCS) across 12 climatically diverse Turkish cities. Results show that with flexible PV sizing and moderate demand, grid dependency can be reduced by up to 66.7%, while the renewable fraction (RF) can reach 89%. In high-irradiance regions (>4.5 kWh/m(2)/day), ESS becomes economically viable below $150/kWh, achieving discount payback periods under 10 years. Conversely, in areas with limited space and high demand, RF may drop to 16%, increasing grid reliance and CO2 emissions. Cities like & Idot;zmir and Gaziantep demonstrate 23% lower energy costs than less favorable locations like Trabzon, emphasizing climate impacts. The study offers a replicable framework for planning efficient, low-carbon EVCS tailored to regional conditions, supporting policymakers, utilities, and investors in advancing T & uuml;rkiye's energy transition and decarbonization strategies

Amaç: Bu çalışmanın temel amacı, işe yeni başlayan sağlık çalışanlarında iş doyumu, nomofobi düzeyi, depresyon ve yaşam kalitesi arasındaki ilişkilerin incelenmesidir. Gereç-Yöntem: Bu araştırma, tanımlayıcı ve kesitsel bir çalışma olarak Düzce Üniversitesi Araştırma ve Uygulama Hastanesi'nde gerçekleştirilmiştir. Çalışmaya, 01.01.2024 ile 31.12.2024 tarihleri arasında işe yeni başlayan toplam 100 sağlık personeli dâhil edilmiştir. Katılımcıların sosyodemografik özellikleri, nomofobi düzeyleri, depresyon puanları, yaşam kalitesi puanları ve iş doyumu değerlendirilmiştir. Veri toplama sürecinde, katılımcılara anket formları uygulanmıştır. Bu formlar, bireylerin belirtilen değişkenler açısından değerlendirilmesini sağlayan Nomofobi, Beck Depresyon, Yaşam Kalitesi ve Minesota İş Doyumu ölçeklerinden oluşmaktadır. Bulgular: Çalışmaya toplam 100 sağlık personeli katılmış olup, katılımcıların %64,5'i (n=71) kadın, %35,5'i (n=39) erkektir. Son bir hafta içinde, sağlık çalışanlarının günlük ortalama ekrana bakma süresi ise 259.1±115.1 dakika olarak saptanmıştır. Araştırmada, katılımcıların nomofobi düzeyleri işe yeni başladıkları dönemde ve başladıktan üç ay sonraki süreçte karşılaştırılmıştır. İşe yeni başlayan sağlık çalışanlarının %2.7'sinde (n=3) nomofobi tespit edilmemiş, %41.8'inde (n=46) hafif düzeyde, %45.5'inde (n=50) orta düzeyde ve %10'unda (n=11) yüksek düzeyde nomofobi saptanmıştır. İşe başladıktan üç ay sonra yapılan ölçümde ise sağlık çalışanlarının %0.9'unda (n=1) nomofobi bulunmazken, %45.5'inde (n=50) hafif düzeyde, %43.6'sında (n=48) orta düzeyde ve %10'unda (n=11) yüksek düzeyde nomofobi görülmüştür. Bununla birlikte, üç aylık süreçte nomofobi düzeylerinde istatistiksel olarak anlamlı bir değişiklik tespit edilmemiştir (p>0.05). Katılımcıların nomofobi düzeyleri arttıkça depresyon düzeylerinin de yükseldiği, buna karşılık yaşam kalitesinin azaldığı saptanmıştır (p0.05). Ayrıca, yaşam kalitesindeki artışın depresyon düzeylerinde azalmaya neden olduğu belirlenmiştir (p0.05). Sonuç: Elde edilen bulgular, nomofobi düzeyinin azalmasının yalnızca iş doyumunu artırmakla kalmayıp, aynı zamanda depresyon düzeylerini düşürdüğünü ve yaşam kalitesini yükselttiğini göstermektedir. Bu doğrultuda, sağlık çalışanlarında nomofobi düzeyini azaltmaya yönelik bilinçlendirme ve müdahale stratejilerinin iş doyumunu artırmada etkili olabileceği öne sürülmektedir. Sağlık çalışanlarında nomofobi düzeylerini azaltmaya yönelik yapılacak iyileştirme çalışmalarının, onların rol model olma potansiyeli sayesinde hasta ve hasta yakınlarında da olumlu etkiler yaratabileceği düşünülmektedir.Aim: The main aim of this study was to examine the relationships between job satisfaction, nomophobia level, depression and quality of life in newly recruited healthcare workers. Materials and Methods: This study was conducted as a descriptive and cross-sectional study at Düzce University Research and Application Hospital. A total of 100 newly recruited health personnel between 01.01.2024 and 31.12.2024 were included in the study. Participants' sociodemographic characteristics, nomophobia levels, depression scores, quality of life scores and job satisfaction were evaluated. During the data collection process, questionnaire forms were applied to the participants. These forms consisted of Nomophobia, Beck Depression, Quality of Life and Minnesota Job Satisfaction scales, which enabled the evaluation of individuals in terms of the specified variables. Results: A total of 100 healthcare professionals participated in the study and 64.5% (n=71) of the participants were female and 35.5% (n=39) were male. In the last week, the average daily screen viewing time of healthcare professionals was 259.1±115.1 minutes. In the study, the nomophobia levels of the participants were compared at the time they started their new jobs and three months after they started their new jobs. Nomophobia was not detected in 2.7% (n=3) of the newly recruited healthcare workers, 41.8% (n=46) had mild nomophobia, 45.5% (n=50) had moderate nomophobia and 10% (n=11) had high nomophobia. Three months after starting work, 0.9% (n=1) had no nomophobia, 45.5% (n=50) had mild nomophobia, 43.6% (n=48) had moderate nomophobia and 10% (n=11) had high nomophobia. However, there was no statistically significant change in nomophobia levels during the three-month period (p>0.05). As the nomophobia levels of the participants increased, their depression levels also increased, whereas their quality of life decreased (p0.05). In addition, it was determined that the increase in quality of life led to a decrease in depression levels (p0.05). Conclusion: The findings show that decreasing the level of nomophobia not only increases job satisfaction, but also decreases depression levels and improves quality of life. Accordingly, it is suggested that awareness-raising and intervention strategies to reduce nomophobia levels in healthcare workers may be effective in increasing job satisfaction. It is thought that improvement efforts to reduce nomophobia levels in healthcare professionals may also have positive effects on patients and their relatives thanks to their potential as role models
Energy, exergy, thermoeconomic, exergoeconomic, and sustainability analyses of hydrogen-enriched diesel-biodiesel nano fuels
This work intends to discuss the thermodynamic, economic, and sustainability analysis of hydrogen-enriched biodiesel-diesel and nanoparticle blends. In this direction, sunflower methyl ester (20 %) and diesel fuel (D100) are volumetrically blended (B20). Then, cerium oxide (CeO2) nanoparticles are added to B20 at a mass fraction of 100 ppm (B20CeO2). Furthermore, hydrogen of 5 lpm (B20CeO2H5) and 10 lpm (B20CeO2H10) is introduced into the combustion chamber from the intake manifold by mixing with the fresh air. The test engine is loaded from 15 to 60 Nm with increments of 15 Nm while it runs at a constant engine speed of 2000 rpm. The results show that the highest energy efficiencies are 28.9 % for B20CeO2H10 at 45 Nm. Besides, the highest exergy efficiencies are 27.01 % and B20CeO2H10 at 45 Nm. On the other hand, the best results are generally obtained for the B20CeO2H10 test fuel in terms of exergoeconomic and sustainability results. Crankshaft work has a particular work specific exergy cost of 138.9 US/GJ for B20CeO2H10. The sustainability index is determined as 1.37 for B20CeO2H10. In conclusion, these results reveal that adding nanoparticles and hydrogen considerably enhances the thermodynamic, economic, and sustainability metrics of the engine fuelled with diesel-biodiesel fuels
Solitary Metastasis Mimicking Glioblastoma in a Patient with Fahr's Disease
Fahr's disease is defined as the accumulation of bilateral idiopathic calcification in the basal ganglia, dentate nucleus, and centrum semiovale. The coexistence of Fahr's disease and intracranial tumors is extremely rare, with only five cases documented in the literature. This study aimed to present a rare case, discussing both similarities and differences with previously reported cases. A 70-year-old female patient presented to our hospital with a progressive headache over the past month. Brain magnetic resonance imaging (MRI) with intravenous contrast was performed for further evaluation. Bilateral basal ganglia, thalamus, and dentate nuclei showed calcifications consistent with Fahr's disease. After excluding toxic, infectious, and endocrine causes of calcification, the patient was diagnosed with Fahr's disease. Magnetic resonance imaging also revealed a midline localized, irregularly contoured, and heterogeneously intense mass with contrast enhancement at the centrum semiovale level, accompanied by peripheral edema. Radiologically, the mass resembled a high-grade glioma or metastasis, and the presence of a lipid peak further suggested metastasis. Based on radiological findings, the patient was diagnosed with brain metastasis of breast carcinoma. The patient received chemotherapy for metastasis, which resulted in substantial regression of the mass. In patients with known Fahr's disease, new-onset headaches and related symptoms should warrant evaluation for possible brain tumors. In these cases, imaging findings should be assessed along with physical examination findings and the patient's medical history to support the diagnostic process
Obesity-Related Disorders in Turkiye: A Multi Center, Retrospective, Cross-Sectional Analysis from the OBREDI-TR Study
Objectives: Obesity is a significant public health concern, as it is associated with the development of numerous chronic diseases. The prevalence of obesity and attendant diseases has been increasing over recent years. This study attempted to ascertain the frequency of chronic diseases in obese patients in Turkiye for the first time on this scale. Methods: A retrospective study was conducted, with patients admitted to the internal medicine outpatient clinics or obesity centers between December 2023 and December 2024 included in this study. Participants were recruited from seven regions, 20 provinces, and 28 centers, and the inclusion criteria were met by those aged 18 years and over with a body mass index (BMI) of 30 kg per square meter (kg/m(2)) or above. Their status, with respect to chronic diseases, and their anthropometric parameters were documented. Results: The total number of patients was 10,121, with a mean age of 45.2 +/- 13.92. Of these, 7222 (71.35%) were female. The prevalence of type 2 diabetes mellitus (T2DM), hypertension (HT), dyslipidemia (DL), coronary artery disease (CAD), obstructive pulmonary disease (OPD), obstructive sleep apnea syndrome (OSAS), and fatty liver disease (FLD) was found to be 35.01%, 78.19%, 12.37%, 10.32%, 5.88%, and 75.12%, respectively. A subsequent analysis of the prevalence of these diseases by region revealed a statistically significant variation between regions (p < 0.001 for all regions). Assessment of serum glucagon-like peptide-1 and dipeptidyl peptidase-4 levels in patients with migraine
Background Migraine is a prevalent neurovascular disorder affecting over one billion people globally, imposing significant daily life limitations. Migraine headaches are linked to the activation of trigeminal nerve endings. Glucagon-like peptide-1 (GLP-1) is a hormone involved in insulin regulation and plays neuroprotective roles in the central nervous system (CNS). Dipeptidyl peptidase-4 (DPP-4) is a protease enzyme that degrades GLP-1, rendering it inactive, and modulates metabolic and neurological pathways. This study investigates the association between serum GLP-1 and DPP-4 levels and migraine pathophysiology to propose a novel therapeutic approach for migraine. Methods The study included 42 migraine patients and 42 healthy controls. After fasting for at least 8 h, blood samples were collected. Serum GLP-1 and DPP-4 levels were measured using ELISA, and statistical analyses were conducted with SPSS 24.0. Results Serum GLP-1 and DPP-4 levels were significantly lower in migraine patients compared to controls (p < 0.001). Patients with migraine with aura had significantly lower GLP-1 levels compared to those with migraine without aura (p = 0.016). A significant decrease in GLP-1 levels was observed in patients experiencing pain localized in the occipital lobe (p = 0.01). Conclusions Serum GLP-1 and DPP-4 levels were lower in migraine patients. Given the role of GLP-1 in the central nervous system, reduced GLP-1 may contribute to migraine pain. Similarly, low DPP-4, which metabolizes GLP-1, may be linked to these findings and could trigger attacks by increasing blood CGRP levels

Klasik Türk edebiyatı metinleri üzerine farklı tahlil çalışmaları yapılmaktadır. Bu tahlil çalışmaları arasında önemli bir başlık da metinlerde yer alan tipler ve kişiliklerin tespitidir. Klasik Türk edebiyatı metinlerinde şairlerin şiirlerinde ortak olarak bulunan tipler ve kişiliklerin şair üslubu özelinde değerlendirilmesi, onun üslubunun farklılıklarını ortaya koymak bakımından önemlidir. Bu çalışmada 16. yüzyıl divan şairlerinden Lâmiʿî Çelebi'nin Dîvân'ında tipler ve kişilikler üzerine bir yoğunlaşma söz konusudur. Çalışmada Lâmiʿî Çelebi Dîvânı'ndaki unsurlar tespit edilecek ve gelenek içindeki diğer kullanımlar ile olan benzerlik ve farklılıklar ortaya konulmaya çalışılacaktır.Different analysis studies are carried out on classical Turkish literature texts. An important topic among these analysis studies is the identification of the types and personalities in the texts. Evaluating the types and personalities that are common in the poems of poets in classical Turkish literature texts, specifically the poet's style, is important in terms of revealing the differences of his style. In this study, there is a concentration on the Divan of Lamiʿi Çelebi, one of the 16th century divan poets, through types and personalities. With this study, the elements in the Divan of Lamiʿi Çelebi will be determined and the similarities and differences with other uses in the tradition will be tried to be revealed
