Relationship between perioral muscle evaluation using ultrasonography and the feeding function and nutritional status of older adults
Get PDFThe purpose of this study was to examine the potential utility of ultrasonography in assessing muscle thickness and echo-intensity as a means of evaluating the feeding function and nutritional status of older adults requiring long-term care. This study included 44 older individuals （29 women and 15 men; mean age, 83.3±7.6 years） requiring long-term care. The tongue and masseter muscles were examined using ultrasonography, and muscle thickness and echo-intensity were determined. Feeding function was classified based on the foods that could be ingested using the Functional Oral Intake Scale. To assess the nutritional status of these individuals, we utilized a body composition analyzer to estimate the skeletal muscle mass index. No differences in muscle thickness were observed according to sex, feeding function, or nutritional status. We found no differences in echo-intensity based on sex or feeding function, although significantly higher values were observed in individuals suspected of having sarcopenia than in normal individuals based on their nutritional status （p＜0.01）. Multiple regression analysis revealed a significant association between masseter muscle brightness and age （p＜ 0.01）. These findings suggest that the assessment of masseter muscle echo-intensity via ultrasonography is useful or evaluating sarcopenia in older adults requiring long-term nursing care. 熊本機能病院における40年間の外傷性切断肢指趾1,430例の受傷背景および再接着術の治療経験
Get PDF1981年4月より2021年3月までの40年間に熊本機能病院にて経験した切断肢指趾1,430症例，再接着術977例を診療録及び手術記録に基づき後向きに調査し，患者受傷背景および再接着手術，二次的再建手術を年代別ともに検討した．上下肢ともに指趾の切断が多く，中でも指切断が1,163例と大半を占めた．上肢を年代別に見ると，年代別切断指症例数は徐々に減少し，2016年以降は1981年代より約1/3となった．上腕では2000年以降，前腕では2006年以降減少しているのに対して，手の切断は減少していなかった．再接着を施行した切断肢指趾の手術成績は，指では1981年～2000年で95％以上の生着率で，それ以降はほぼ100％となった．指以外では足趾が88％，手が98％で，大腿を除いてその他の上腕，前腕，下肢，足は100％の生着率であった．受傷年齢は，就業年齢の20～64歳に多かったが，9歳以下の受傷も多かったことは受傷予防の観点からも重要な結果であった．受傷指は示指，中指，環指，母指の順で多かった．曜日別では日曜日に少なく，時間帯では9～17時に多かった．受傷原因は，工場又は工事における機械機器が最も多く，次に電気のこぎり，農機具と続いた．受傷職種では農林業が工場における製造業より約2倍の多さで，農林業の盛んな熊本県の特徴と言える．再接着に至らなかった症例に対する二次的再建手術は足趾を移植するToe to finger法とWrap around flap法で行った．いずれの方法も手術成績は100％の生着率であったが，最小限のドナーの犠牲で行える手術としてはWrap around flap法の有用性が高いと考察した．departmental bulletin pape
仙骨前面に発生した気管支原性嚢胞の1例 —仙骨前面の腫瘍切除術におけるアプローチ方法の検討—
Get PDF仙骨前面に腫瘍性病変が存在する場合，組織学的に皮様嚢胞，類表皮嚢胞，重複腸管，奇形腫などを呈する場合が多い．一方，気管支原性嚢胞は主に前胸部に形成されることが多い良性腫瘍である．今回，われわれは仙骨前面に生じた腫瘍性病変に対して，低侵襲であり，大きな腫瘍の手術においても有利である経仙骨アプローチでの手術を施行し，病理学的診断の結果，気管支原性嚢胞と診断された1例を経験した．われわれが渉猟しうる限りでは仙骨前面に気管支原性嚢胞を認めた報告例は2例のみであり，極めて稀な病態と思われる．仙骨前面部に生じた腫瘍に対する手術でのアプローチ方法についての考察も含め，若干の論文的考察を交えつつ報告する．departmental bulletin pape
中足骨遠位直線状骨切り術変法を併用し裂隙閉鎖を行った両側裂足の1例
Get PDF裂足は，前足部の1趾列またはそれ以上の趾列欠損を特徴とする非常に稀な先天異常であり，趾間が深くなるだけのものから複数の中足骨が欠損するものまで病態はさまざまである．さらなる変形を防ぐため，小児期の早い時期に裂隙閉鎖，必要があれば骨矯正を含めた外科的治療を行うことが望ましい．本報告において，両側裂足を有する14歳女性は歩行を含む日常生活を問題なく行えており，当科に初診となるまで未治療であった．足幅が広いため市販の靴は変形し，整容面で問題を抱えていた．Flap-bag techniqueによる裂隙閉鎖術と，Digital Linear Metatarsal Osteotomyに準じた骨矯正術を行い良好な経過を得られたため，若干の文献的考察を含めて報告する．departmental bulletin pape
