【背景・目的】 
薬局薬剤師が行う疑義照会の情報源の約 3割は、患者からの情報提供に
よることが報告されており[1]、患者の安全な薬物療法を行うためには、患
者からの情報提供が不可欠である。 
2015年の「患者のための薬局ビジョン」の公開以降、薬局薬剤師の役割
は対物から対人業務へと構造的転換が推進されているが［2］、患者への周知
は十分とは言えない。薬局薬剤師から自身の情報を聞かれることに不満を
感じる患者も少なくない[3]。先行研究で、薬剤師の薬物療法への寄与に対
する患者の認識は、情報提供の意欲と関連していることが報告されている
が[4,5,6]、これまでの研究は一時点での評価に過ぎない。 
本研究は、薬剤師の対人業務を動画で周知することによる患者の情報提
供意欲の変化、および情報提供意欲の向上に影響要因の探索を目的とした。 
【方法】 
1. 動画作成 
薬局薬剤師が協議し、４つの対人業務をテーマとした短編アニメーショ
ン動画を作成した。動画のテーマは、動画 1. 薬物相互作用の確認、動画
2. 検査結果に基づく用法・用量の確認、動画 3. 副作用のモニタリング、
動画 4. ポリファーマシーに関する介入である。 
2. 調査概要 
2020年 12月に web 質問票調査を行った。対象者は Web アンケート画面

上で 4つの動画のうち 1つを視聴し、その前後に薬剤師への情報提供の意
欲に関する質問（Q3）に回答した。 
3.対象者 
定期的に医療機関を受診し、薬局で処方薬（家族への処方を含む）を受
け取っている 20 歳以上の者とした。サンプルサイズは G-power 3.11   
を用いて算出し[7]、各動画群 150 人、合計 600 人とした。男女比は 1：1、
年齢は 5 区分（≦39 歳、40～49歳、50～59歳、60～69 歳、70～89歳）に
均等割付とした。  
4. 質問項目 
Q1. 薬剤師に対する信頼感、Q2. 医療従事者としての薬剤師の認識、Q3. 
薬剤師への情報提供意欲(7段階リッカートスケール)[6]、Q4. 動画の視覚
的評価、Q5. 動画の質的評価、Q6. 動画の長さの適切さ、Q7. 動画の内容
に関する認識、Q8. 基本属性、Q9. かかりつけ薬剤師による説明の有無、
Q10. 医師が定期的に処方する薬の種類数、Q11. 定期的に利用している薬
局の数、Q12. 定期的に病院を受診し、処方箋をもらっている病気とした。 
5. 解析  
A. 動画視聴前後の情報提供意欲の比較 
Wilcoxon 符号付順位検定を行い、効果量を算出した。また、動画視聴前の
業務の認識が低い者と高い者に分け評価した。 
B. 薬局薬剤師への情報提供意欲の向上に影響する要因 
動画視聴前の情報提供意欲の評価 11 項目の中央値以下の低い者を抽出し
た。動画視聴後の合算値が中央値以下は向上した、中央値より高い値は向
上しないの 2 値を目的変数として多重ロジスティック回帰分析を行った。
説明変数は、年齢、性別、かかりつけ薬剤師による説明の有無、最終学歴、
薬剤師への信頼感、医療従事者としての薬剤師の認識、薬物療法における
薬剤師の寄与への認識、医師から定期的に処方される医薬品種類数、定期
的に訪れる薬局数、ハイリスク疾患（心臓疾患、糖尿病、呼吸器疾患、精
神疾患、がん）の有無とした。 
【結果】 
4つの異なる動画を視聴したグループ（n = 600）間で、属性に有意な差
はみられなかった。 
A. 動画視聴前後の情報提供意欲の比較 
4動画全てにおいて、動画視聴後の情報提供意欲は視聴前と比較して有
意に増加した（P < 0.001）。さらに動画の内容の認識が低い群、認識が高
い群に分けて比較した結果、全ての群において動画視聴後の患者の情報提
供意欲が有意に高かった（P < 0.001）。効果量は 0.40～0.58 で中程度以

上であった[8,9,10]。 
B. 薬局薬剤師への情報提供意欲の向上に影響する要因 
視聴前の情報提供意欲の中央値は 45 であり、視聴後の情報提供意欲が向
上した、しないの 2 値を目的変数とした多重ロジスティック回帰分析の結
果、薬剤師への信頼感のオッズ比は 1.10、かかりつけ薬剤師からの説明は
2.25であり、正の影響を示し、糖尿病の有のオッズ比は 0.34と負の影響
が示された。 
【考察】 
すでに薬剤師の役割を認識している患者も薬剤師の業務が自身の薬物
治療にどのように影響するかを具体的には知らなかった可能性があり、動
画の視聴が薬剤師の対人業務の理解に役立ったと考えられる。 
薬剤師への信頼に基づく患者と薬剤師の関係が患者の情報提供意欲に
影響を与えることが示された。患者の安全な薬物治療のため、薬局薬剤師
は患者から情報を得る必要があり、対人業務の周知と患者との信頼関係の
構築が求められる。

内容要旨 
【背景・目的】加齢に伴う口腔乾燥症の多くは薬剤の服用に関連すること
が知られており、唾液腺の老化に伴う重篤化の可能性が指摘されているが、
その詳細なメカニズムについては未だ不明である。そこで、単一細胞レベ
ルで遺伝子発現を網羅的に解析可能である single cell RNA sequencing 
を用いて老齢マウス顎下腺における遺伝子発現解析を行なった。 
【方法】成体(12週齢)および老齢(94週齢)の雌性マウス(C57B6/J)の顎下
腺を単離し、酵素処理により分散化した。その後単一細胞かつ上皮マーカ
ー(EpCAM)陽性細胞集団を選別した後、ライブラリーを作成し sequence 
を行なった。データ解析には R を用いた。 
【結果】成体および老齢マウス顎下腺のデータを統合し、UMAP を週齢別
に描図すると、老齢マウス顎下腺では腺房細胞の割合が減少傾向にあるこ
とが明らかとなった。Gene Ontology 解析の結果、老齢マウス顎下腺では
導管構成細胞や筋上皮細胞に、MHC class I 関連遺伝子の発現上昇が認め
られた。老化関連遺伝子の p21(Cdkn2a)、p16(Cdkn2a) 、SASP 遺伝子の 
Tgfb1 は基底細胞や線状部導管で、わずかではあるが発現の上昇を示した。 
【考察】本研究では老齢マウス顎下腺における MHC class I の発現上昇
が示唆された。今後 MHC class I の発現をコントロールすることが可能
になれば、老化に起因した唾液分泌障害の新たな治療的アプローチになり
うると考えられる。本研究では唾液腺上皮における遺伝子発現プロファイ
ルを作出したため、今後は間葉細胞も含めた検索が必要であると考える。

Impact of scanning range and image count on the precision of digitally recorded 
intermaxillary relationships in interocclusal record using intraoral scanner

Abstract 
Purpose: The effect of scan range and the number of scanned images on 
the precision of in vivo intermaxillary relationship reproduction was evalu­
ated using digital scans acquired with an intraoral scanner. 
Methods: The study involved 15 participants with normal occlusion. l\vo 
different interocclusal recording settings were employed using the intraoral 
scanner (TRIOS 4):'M瓜'focusing on the minimal scan range of the first 
molar region, and'MAX,' including the scan range from the right first pre­
molar to the right second molar. These settings were combined with three 
different image cmmts, resulting in six experimental conditions. Interocclu­
sal recordings were performed four times for each condition. Dimensional 
discrepancies between datasets were analyzed using three-dimensional 
morphometric software and compared using two-way analysis of variance. 
Results: Median din1ensional discrepancies (interquartile range; !QR) of 
39.2 (30. 7-49.4), 42.2 (32.6-49.3), 30.3 (26.8-44.1), 20.1 (16.0-34.8), 21.8 
(19.0-25.1), and 26.6 (19.9-34.5)µm were found for M邸200, MIN/400, 
MIN/600, MAX/200, MAX/400, and MAX/600, respectively. Significant 
differences in dimensional discrepancies according to scan range were 
found. Wilcoxon signed-rank test showed significant differences between 
MAX and MIN (P < 0.01). 
Conclusion: Scan range may affect the precision ofintermaxill町relation­
ship reproduction. Thus, scanning of the most extensive region practically 
achievable is recommended

Introduction 
The trueness and precision of three-dimensional images acquired through 
intraoral scanners (IOSs) have been the subject of multiple investigations 
[1-13]. Generally, previous studies have found that the trueness and preci­
sion of digital scans acquired using !OS are either clinically acceptable or 
superior to those obtained with conventional impression methods, particu­
larly when the scanning range is restricted [1,4,6,8, 11,12]. However, when 
the scan incorporates the entire dental arch, the trueness and precision of 
the digitally acquired data appear to be less favorable than those achieved 
via conventional impression methods [2,3,5,7-10,12,13]. 
In the context of interocclusal recording, digital recordings through 
IOSs involve capturing the buccal surfaces of the upper and lower dental 
arches while they are in maximal intercuspal contact. U叫ke traditional 
methods, this digital approach obviates the need for using occlusal record­
ing materials and mounting stone casts onto an articulator—factors that 
introduce dimensional inaccuracies. Consequently, this digital method for 
interocclusal recording is theoretically susceptible to fewer d血ensiooal
errors. Consistent with this hypothesis, multiple studies have demonstrated 
the superior reliability of digital scans acquired using !OS for reproduction 
ofintennaxillary relationships. Zimmermann et al. [14] and Iwauchi et al.

[ 15] reported that digital interocclusal recordings offered better precision 
than conventionally acquired data. In addition, Edher et al. (16] evaluated 
the accuracy of interocclusal recordings using a master model and reported 
that the accuracy of interocclusal scans of complete arches was worse than 
that of quadrant arch scans. Tllis may be due to the scan range dependence 
of digital scanning with 10S, as described above (2,3,5,7-10,12,13]. 
Digital interocclusal recording requires three-dimensional alignment 
of digital scans of the mandibular and maxillary arches, utilizing lateral 
buccal scan data. In this context, inaccuracies may occur during this align­
ment process. Although 10S so和'lare can generally achieve alignment 
with m血mal lateral buccal scan data, such as a single set of opposing 
teeth, the extent of this scan data can affect the trueness and precision of 
the subsequent interocclusal recording. Limited scan data may compro­
mise registration quality due to a dearth of reference points. In clinical 
settings, several commercially available intraoral scanner systems prompt 
a notification suggesting completion of the registration when a皿nimal
lateral scan record for as small as one tooth antagonistic pair is obtained. 
However, completing an interocclusal recording with such a minimal scan 
range might also affect its trueness and precision 
Conversely, extending the scan range-albeit still within the limits 
of the quadrant arch—increases the scan duration and increases the risk 
of positional shifts in the mandible during the procedure. In the present 
context, determination of an optimal scanning methodology, in terms of 
both range and duration, remains an unresolved issue. 
The aim of this study was to critically assess the effect of variations rn 
scan range and scan duration, measured by the number of captured images, 
on the fidelity of digitally replicated intermaxillary relationships in a living 
individual. Using !OS digital scan methodology, the sh1dy sought to either 
confirm or refute the null hypothesis that the scan range and the number 
of scanned images would have no discernible effect on the precision of 
reproduced intermaxillary relationships. 
Materials and Methods 
Participants and settings 
The study sample comprised 15 participants (seven males and eight 
females; mean age 26.8土2.2 years) exhibiting complete natural denti­
tion and classified as skeletal Angle Class 1. All individuals were in good 
general health, as indicated by an American Society of Anesthesiologists 
physical status of I, and had not undergone any dental rehabilitation. Any 
individuals with periodontitis, temporomandibular joint dysfunction, oro­
facial pain, or any forrn of acute oral disease were excluded from the study. 
Digital scan and intcrocclusal recording using 10S 
The experimental design is depicted in Fig. I. A single operator with four 
years of clinical experience performed the digital scans using JOS. All 
scanning activities were conducted in a laboratory environment with regu­
lated conditions: room temperature 26.0°C and humidity 35%. Participants 
underwent preliminary training to maintain a maximal intercuspal position 
using consistent light occlusal force, guided by visual electromyographic 
feedback from the masseter muscle (PowerLab, ADinstruments Pty Ltd., 
Bella Vista, Australia). Subsequently, a digital scan of the right mandibular 
and maxillary molar regions was captured for each participant using an 
10S (TRJOS 4, 3Shape NS, Copenhagen, Denmark). Figure 2 illustrates 
two distinct scanning settings: the minimal scan range focused on the right 
first molar region (MIN) and an extended range from the first premolar 



data consistently indicated that larger scan ranges contributed to more pre­
cise reproductions of in term axil I町relationships across all participants. 
IOS software typically includes a feature that notifies users when inter鰤
occlusal recording is ostensibly complete. Often, this notification arises 
after scanning only a minimal range, such as a single pair of occluding 
teeth. The findings of the present study challenge this approach, suggest­
ing that scanning should continue to encompass the entire quadrant of the 
dental arch for enhanced precision, even after the notification is displayed. 
Regarding the metric of the scarming time, the number of scanned 
images was quantified in this study. W皿e these terms are nearly 
synonymous, it is crucial to recognize that they may not be entirely 
interchangeable. The image count captured per unit time may vary due 
to several factors, including computational delays caused by processing 
limitations when constructing 3D images in real time. In this study, this 
variable was mitigated by employing a computer with the highest available 
processing power. 
In the literature, Zimmermann et al. [14] reported that the precision of 
digital interocclusal recording had a range of 61-84µm, depending on the 
IOS model. Jaschouz et al. [19] and Iwauchi et al. [15] reported a precision 
of 42µm and 31µm, respectively. While these results cannot be compared 
directly between the studies because of methodological differences, the 
precision of the digital method was consistently reported to be superior to 
that of the conventional method (99 µm by Zimmermann et al. [14], 135 
µm by Jaschouz et al. (19], and 128µm by Iwauchi et al. (15]), suggesting 
that the precision of digital interocclusal recording is clinically acceptable. 
Furthermore, when compared to the results of the previous study [15], 
where the study protocol was identical and thus direct comparisons are 
possible, they are inferior to the results obtained under the MAX condi­
tions and superior to the results obtained under the MIN conditions in this 
study. These findings are not unexpected because, in the previous study, 
the lateral interocclusal recording range was not specified, and it was sug­
gested that the precision of the interocclusal recording would be improved 
by scanning the fulJ range of the impression area. 
Regarding the study's limitations, the digital scans using !OS for both 
maxillary and mandibular dentitions were acquired only once, serving as 
the reference for evaluating intermaxillary relationships. This protocol 
does not fully emulate actual clinical scenarios, where both the impres­
sion-taking process and the recording of intermaxillary relationships are 
sequential activities. In such settings, the trueness and precision of the 
impressions could affect those of the intermaxillary relationship reproduc­
tions. A further limitation lies in that this study did not assess the trueness of 
the method for recording digital intermaxillary relationships. To accurately 
determine trueness in an in vivo setting, a gold standard would need to 
be established; however, this is feasible only in model-based experiments 
Accordingly, the present study focused solely on evaluating the precision 
of the employed metl1odology. 
In conclusion, this study has shown that the extent of the lateral scan­
oiog range is crucial for achieving better precision of digital interocclusal 
recordings. Within the above-mentioned limitations, the present results 
suggest that clinicians should expand the scanning range within the 
quadrant arch to achieve the highest level of precision of the interocclusal 
records.

【背景・目的】頭蓋顎顔面の成長と発達は、周囲の軟組織の機能的な状態
や患者の遺伝的要素に依存すると考えられてきた。近年、Cone Beam 
Computed Tomography(以下 CBCT)を用いて下顎骨形態や顎顔面形態に関し
て様々な報告がされている 1–5。過去の研究において垂直顔面形態と咬筋、
垂直顔面形態と咬合力、さらに咬合力と下顎頭形態の間に関連性があるこ
とが明らかとなっている。しかし、下顎頭形態と咬筋の関係については未
だ報告がない。また、過去の報告ではメカニカルストレスは下顎の成長に
とって重要な刺激であると述べられている 6。このことから顎変形症の適
応となるような顎顔面の変形は遺伝的要因だけでなく、後天的な要因とし
て咀嚼筋の牽引方向も関与していると考えられる。そこで本研究では垂直
顔面形態ごとに分類された各群内、また、各群間における下顎頭形態と咀
嚼筋との相関性を明らかにするために，下顎頭体積、下顎頭表面積と咬筋
断面積の関連性について検討を行った。 
【方法】対象は 2013 年 1 月から 2022 年 10 月までに昭和大学歯科病院で
矯正歯科診断のために CBCT 撮影を行い、下顎頭の成長が終了していると
見込まれる成人患者 46 名とした。顔面正中線に対して下顎の正中が 3mm
以上偏位のある患者 7、顎関節症患者、下顎頭の大きさに左右差がある患
者は除外した。過去の研究より成長期における顔面の左右非対称や奇形が
ない患者の両側の咬筋断面積の大きさに左右差が認められないと報告さ
れているため、本研究では咬筋断面積、下顎頭体積、下顎頭表面積の計測
を右側のみ行った。FMA の角度によって垂直顔面パターンの分類を行い、
Brachyfacial type（22°未満）、Mesofacial type（22°～29°）、
Dolichofacial type（29°以上）とした 8。 
【 結 果 】 咬 筋 断 面 積 で は 、 Mesofacial type と Dolichofacial 
type(p=0.0070)、Brachyfacial type と Dolichofacial type(p=0.0037)
の２群間で有意差が認められ、下顎頭体積では、Brachyfacial type と
Dolichofacial type（p=0.0459）、下顎頭表面積では、Brachyfacial type
と Dolichofacial type（p=0.0418）で有意差が認められた。垂直顔面パタ
ーン内での咬筋断面積と下顎頭体積、咬筋断面積と下顎頭表面積では有意
差が認められなかった。また、垂直顔面パターン内での下顎頭体積と下顎
頭表面積ではすべての群間で有意差が認められた。 
【考察】咬筋断面積において、Mesofacial type と Dolichofacial type、
Brachyfacial type と Dolichofacial type の２群間で有意差が認められ
たことより、咬筋断面積が大きい患者では、顔面の垂直方向の成長が弱い
ことが分かる。このことより、顔面の下方向の成長が咬筋断面積の大きさ
と関連していることが示唆された。下顎頭体積、下顎頭表面積では
Brachyfacial type と Dolichofacial type で有意差が認められたことよ
り、下顎頭の軟骨成長は咀嚼筋の牽引方向と強さと関係していることが示
唆される。さらに、下顎頭の表面積では Brachyfacial type と
Dolichofacial type で有意差が認められたことより下顎頭の軟骨性成長
には至適な分圧が存在すると考えられる 9。また過去の報告では、成長期
のラットの両側咬筋を切断することにより下顎頭形成が損なわれたとい
う報告があるが 10、今回の計測では、垂直顔面パターンごとの咬筋断面積
と下顎頭体積、咬筋断面積と下顎頭表面積ではそれぞれ相関は認められな
かった。下顎頭体積、下顎頭表面積には個人間の大きさの差はないが、咀
嚼筋の牽引方向のベクトルにより形に影響を及ぼすと考えられる。即ち、
垂直顔面パターンの分類だけでは解明出来ない原因があると示唆される。

内容要旨 
【背景・目的】レヴィ小体病の原因蛋白であるαシヌクレイン(αS)は凝
集過程で細胞傷害を発揮し，細胞外へ放出され，隣接細胞へ伝播する．細
胞内αS による細胞傷害機構は報告されているが，細胞外αS による機序
は解明されていない．本研究では神経細胞外の高分子αSオリゴマー(HMW-
αSo)による細胞膜傷害，細胞死誘導機序の解明を目的とした． 
【方法】Human αS ペプチド(LMW-αS)を incubation し，サイズ排除クロ
マトグラフィーにより HMW-αSo を分取した．分化誘導させたヒト神経芽
細胞腫に両αS を処置し，細胞毒性，細胞膜損傷(細胞膜リン脂質過酸化,
細胞膜流動性,電気生理学的変化)，酸化ストレス，細胞内 Ca2+([Ca2+]i)
の変動，アポトーシス細胞検出および細胞死関連因子を測定した． 
【結果】HMW-αSo は LMW-αS に比べて細胞膜リン脂質過酸化の上昇，細
胞外からの[Ca2+]i の持続的増加，細胞膜抵抗や細胞膜容量の低下，細胞
膜を脱分極誘発を示した．さらに caspase 3・8 および酸性スフィンゴミ
エリナーゼ活性の上昇を示しアポトーシスを誘導した． 
【考察】HMW-αSo は LMW-αSに比べて強い細胞毒性を示した．細胞外の
HMW-αSo は伝播するだけでなく，直接的な細胞膜傷害および外因性アポ
トーシスを誘導し，神経細胞傷害を誘発することが明白になった．

本論文は骨粗鬆症患者におけるロコモ度 2 とロコモ度 3 の病態と転倒リスクの違いを明らか
にしたものである.2020 年にロコモの臨床診断基準にロコモ度 3 が追加された.ロコモが細分
化されたことにより転倒リスクにどのような効果があったかを調べることを目的とした.年齢
別では ,年齢が高くなるとロコモ度が高くなった .ロコモ度別に分けた結果では ,握力 ,開眼片脚
起立時間 ,転倒スコア ,大腿骨頚部骨密度 T スコア ,骨折の既往の有無 ,ロコチェック該当数に差
があった .転倒リスク因子をロコモ度別に検討したところ ,ロコモ度 3 では開眼片脚起立時間 ,
骨折の既往歴の有無 ,ロコチェック該当数に有意差があった .結果としてロコモ度分類は，筋力
や運動・日常生活能力のほか，骨密度や転倒・骨折の既往の違いを反映していた.転倒リスク
については，骨折の既往のほか，ロコモ度 3 では開眼片脚起立時間の短縮やロコチェック該当
数が転倒高リスク因子であった. 
以上よりロコモ度 3 はロコモ度 2 より転倒リスクが高く , 重点を置いて治療を行って行く必
要があることが明らかになった .

 本論文は、精神科外来に通院する患者の肥満症の有病率を把握し、関連する因子を検討した
研究報告である。 
 石井らは、精神科外来に通院中の患者に対し、身長、体重、罹病期間、治療期間、生活習慣
に関するアンケート調査を行った。また、診療録より、内服薬の種類についての情報を統合し、
統計学的検討を行った。 
 その結果、精神科外来に通院している患者の肥満症の有病率は、一般人口における有病率と
比較して、やや高い可能性が示された。また、実臨床における精神科外来通院患者において肥
満症に関連する因子は、男性であること、治療期間が増えること、外食日数が増すこと、SGA
（第二世代抗精神病薬）を処方されていること、FGA（第一世代抗精神病薬）を処方されてい
ることである可能性が示唆され、年齢が上がることは BMI の減少と関連する可能性が示され
た。 
 これまでに精神科外来患者の肥満症の有病率を調査した大規模な研究は多く存在せず、今
回得られた知見により、向精神薬や生活習慣と肥満症との関連が明確となり、適切な治療、生
活指導を行う上で有益であると考えられた。 
本論文は本学大学院学位論文(博士)審査基準を満たしており、学位論文に値すると判断し

内容要旨  
【背景・目的】我々は眼窩下壁骨折に対して、眼窩下壁を支持するために、
約 2 週間上顎洞にバルーンを留置し、その後抜去する治療を行っている。
今回の研究では、抜去前後の眼球突出度と眼球運動障害の程度を評価し、
バルーンの支持効果を検討した。 
【方法】2015 年 7 月 1 日から 2018 年 6 月 30 日の間で、眼窩下壁骨折整
復術および上顎洞バルーン挿入術を施行した症例を抽出した。評価項目は、
骨折のタイプ、バルーンの留置期間、バルーン抜去前後における術眼と非
術眼の眼球突出度、HAR%、バルーン挿入時と抜去時のバルーンの容量変化
とした。 
【結果】今回の研究対象となったのは 64例 64眼（男性 44 例、女性 20例、
平均年齢 38.9±14.7 歳）であり、骨折のタイプは開放型 55 例、閉鎖型 9
例であった。平均バルーン留置期間は開放型の集団では 11.6±2.1日、閉
鎖型の集団では 12.6±1.6日であった。両群ともにバルーン抜去前後で眼
球突出度に統計学的有意差はなかった。眼球運動障害の評価として用いた
HAR%では、両群ともに有意に改善を認めた。また、全症例において挿入時
と抜去時のバルーン容量の観点から眼球突出度に差が生じるかについて
も検討した。抜去時に挿入時と比較して 50%以上の漏れが認められた症例
は 15 例あり、バルーン内容の漏れが 50%未満の症例と比較したが、眼球
突出度には有意な差は認められなかった。 
【考察】短期的な評価では 2 週間以内のバルーン留置の有効性が示された。
