Analysis of amputations occurring in the earthquakes centered in Kahramanmaraş: Adıyaman experience
Purpose: Amputations are a common surgical procedure resulting from trauma during earthquakes, leading to severe disability. This study aims to investigate surgical outcomes specific to amputations that occurred in Adıyaman after the Kahramanmaraş earthquakes. Methods: This descriptive study included amputees who presented to Adıyaman University Training and Research Hospital. Between March 6, 2024 and March 29, 2024, amputees were contacted by phone and asked the questions in the form. Data analyzed included demographic information, number and level of amputated extremities, phantom limb pain, stump infection, extrication time, time to initiation of rehabilitation, number of revision, and whether a prosthesis was fitted. Results: The study reached 75 amputees. The mean age was 37.9 ± 19.2, and the most frequently amputated age group was adults. Stump infection was observed in 40 amputees (53.3%), phantom limb pain in 47 (62.6%), and revision in 29 (38.7%). The median extrication time was 36 h and initiation of rehabilitation time was 45 days. It was noted that 35 amputees (62.5%) used prostheses. A statistically significant relationship was found between fasciotomy and stump infection (p =.000). Infection was detected in 65% of those who underwent fasciotomy. Conclusion: Earthquake-related amputations most frequently affected the adult age group and primarily involved lower extremity amputations, such as transfemoral and transtibial amputations. Phantom limb pain, need for revision, and infection are common in earthquake-induced amputations. Delayed fasciotomy increases the risk of stump infection. The data obtained in this study will help plan local health services to coordinate amputation care in disasters. © The Author(s) under exclusive licence to SICOT aisbl 2024
Pozitif psikoterapi ve kabul ve kararlılık terapisi açısından değerler kavramının incelenmesi
Değerler, kişiyi istediği hedeflere yönlendiren, kişinin eylemlerine rehberlik eden inanç ve bilişsel yapılardır. Kişinin davranışlarına yön vermesi nedeniyle değerler kavramı başta postmodern yaklaşımlar olmak üzere psikoterapi ekollerinde son yıllarda çalışılmaya başlanmıştır. Bu sebeple bu çalışmanın temel amacı, değerler kavramının pozitif psikoterapi (PPT) ve kabul ve kararlılık terapisi (KKT) açısından nasıl ele alındığını ve psikoterapide ne şekilde kullanıldığını incelemektir. Çalışmanın genel amacı doğrultusunda, her iki yaklaşımın insan doğasına olan bakış açısı ele alınmış ve değerler bağlamında benzer ve farklı yanlarına vurgu yapılmıştır. Her iki yaklaşımda da kişinin anlamlı bir hayat yaşaması adına değer odaklı eylemleri planlamanın belirleyici bir rolü olduğu görülse de pozitif psikoterapi ve kabul ve kararlılık terapisini birbirinden ayıran bazı hususlar vardır. En temel noktalardan bir tanesi, pozitif psikoterapide danışanın iyi ve mutlu bir hayat için olumlu duyguları olumsuzlara kıyasla daha sık yaşaması vurgusudur. Kabul ve kararlılık terapisinde ise olumsuz yaşantıyla birlikte değerler doğrultusunda bir yaşamın sürdürülebileceğidir. Diğer bir nokta ise danışan değerler doğrultusunda adım atarken farklı yapıların devreye girebileceği göz önünde bulundurularak danışana ek beceriler öğretilmesidir. Pozitif psikoterapiye kıyasla kabul ve kararlılık terapisinin bu yapıları ele alması daha bütüncül bir yaklaşım olarak görülmüştür. Bu doğrultuda, ruh sağlığı uzmanlarının değerler ile çalışırken danışanın içsel deneyimlerine yönelik engellerini de açığa çıkararak incelemesi önem arz etmektedir
A machine learning approach to predict foot care self-management in older adults with diabetes
BackgroundFoot care self-management is underutilized in older adults and diabetic foot ulcers are more common in older adults. It is important to identify predictors of foot care self-management in older adults with diabetes in order to identify and support vulnerable groups. This study aimed to identify predictors of foot care self-management in older adults with diabetes using a machine learning approach.MethodThis cross-sectional study was conducted between November 2023 and February 2024. The data were collected in the endocrinology and metabolic diseases departments of three hospitals in Turkey. Patient identification form and the Foot Care Scale for Older Diabetics (FCS-OD) were used for data collection. Gradient boosting algorithms were used to predict the variable importance. Three machine learning algorithms were used in the study: XGBoost, LightGBM and Random Forest. The algorithms were used to predict patients with a score below or above the mean FCS-OD score.ResultsXGBoost had the best performance (AUC: 0.7469). The common predictors of the models were age (0.0534), gender (0.0038), perceived health status (0.0218), and treatment regimen (0.0027). The XGBoost model, which had the highest AUC value, also identified income level (0.0055) and A1c (0.0020) as predictors of the FCS-OD score.ConclusionThe study identified age, gender, perceived health status, treatment regimen, income level and A1c as predictors of foot care self-management in older adults with diabetes. Attention should be given to improving foot care self-management among this vulnerable group
Deep eutectic solvent dispersive liquid–liquid microextraction methods for the analysis of chlorophyll natural colorant (E140) via microwave assisted sample preparation
In the current research paper, extraction and analysis of chlorophyll, one of the bioactive compounds that is usually utilized as natural pigments (E140) and obtained from many leafy green plants and seaweeds, have been carried out via deep eutectic solvent-based dispersive liquid–liquid phase microextraction (DES-UA-DLLME) from plants and food pruducts. The DES-UA-DLLME method was developed by utilizing deep eutectic solvent (DES) which was prepared by heating process with a 1:1 ratio of choline chloride and urea (ChCl: U). Analytical parameters such as pH, DES volume, sample volume, centrifugation and mixing time, DES type, matrix effect, etc. were optimized to ensure the extraction efficiency of chlorophyll. The limit of detection (LOD), limit of quantification (LOQ), intra-day and inter-day relative standart devision (RSD) were found as 0.007 µg/mL, 0.023 µg/mL, 1.3 %, and 1.9 %, respectively. The accuracy of the method was tested with water sample addition/recovery studies. The DES-UA-DLLME method has been applied to food, water, and plant samples. The food products and plants were prepared with the help of ultrasonic and microwave extraction for spectrophotometric analysis of chlorophyll. According to literature survey, the DES-UA-DLLME method and the MWAE method combination was used to the first time for extraction of chlorophyll (E140) from plants. © 2023 Elsevier B.V
Deep eutectic solvent liquid phase microextraction, powered by ultrasonic system, for determination of ?-carotene in food samples
The research work is mainly concerned with the extraction of beta-carotene, which is a terpenoid compound, from food and fruit samples using deep eutectic solvent based liquid phase extraction. Deep eutectic solvent liquid phase microextraction method, powered by ultrasonic mixing, (DES-UP-LPME) was carried out with deep eutectic solvent (DES) consisting of tetrabutylammoniumbromide (TBAB) and decanoic acid (DA). Key parameters such as pH, sample volume, tetrahydrofuran (THF) volume, DES type and volume, matrix effect, ultra-sonication/centrifugation time were optimized to increase the extraction efficiency of beta-carotene. Quantitative recoveries were obtained when pH, TBAB:DA ratio and sample volume were 3, 1:3 and 30, respectively. The limit of detection (LOD) and limit of quantification (LOQ) were found to be 0.005 mu g/mL and 0.015 mu g/mL, respectively. Various food samples were analyzed spectrophotometrically by using the developed DES-UP-LPME at 450 nm. The addition/recovery studies were also performed on water samples to evaluate the accuracy of the method
Highly efficient family of two-step simultaneous method for all polynomial roots
In this article, we constructed a derivative-free family of iterative techniques for extracting simultaneously all the distinct roots of a non-linear polynomial equation. Convergence analysis is discussed to show that the proposed family of iterative method has fifth order convergence. Nonlinear test models including fractional conversion, predator-prey, chemical reactor and beam designing models are included. Also many other interesting results concerning symmetric problems with application of group symmetry are also described. The simultaneous iterative scheme is applied starting with the initial estimates to get the exact roots within the given tolerance. The proposed iterative scheme requires less function evaluations and computation time as compared to existing classical methods. Dynamical planes are exhibited in CAS-MATLAB (R2011B) to show how the simultaneous iterative approach outperforms single roots finding methods that might confine the divergence zone in terms of global convergence. Furthermore, convergence domains, namely basins of attraction that are symmetrical through fractal-like edges, are analyzed using the graphical tool. Numerical results and residual graphs are presented in detail for the simultaneous iterative method. An extensive study has been made for the newly developed simultaneous iterative scheme, which is found to be efficient, robust and authentic in its domain. © 2023 the Author(s), licensee AIMS Press
Taking stock of far-right terrorism through manifestos: Glorification of identity
This research delves into the identity construction and violence justification within the context of far-right lone-actor terrorism, particularly motivated by white supremacist ideologies. Employing a qualitative analysis of manifestos compiled by five lone-actor terrorists, this study adopts a model to unveil the nuanced processes behind the justification of violence and glorification of collective identities. The model has been formed for the purpose of the study, drawing from social identity and identity fusion approaches, including steps such as group alignment, exclusion, threat, virtue, and celebration. The analysis of these manifestos illuminates a progression through each phase of the violent act, meticulously crafted through textual expression. Central to the terrorists' objectives is the creation of a rhetorical platform aimed at fomenting violence against non-white, ethnic, and religious groups. Their motivation arises from the perceived threat of the 'white race' being supplanted by immigrant communities across various social, political, and economic domains. This justification of violence hinges on the portrayal of themselves as protectors of the majority society, pitted against these minority groups. Strikingly, the terrorists celebrate their actions by commemorating past white supremacists who employed violence against marginalised communities
Family opinions on resuscitation and participation in end of life care in the emergency department: A cross sectional study
OBJECTIVE: The study looked into emergency department family members’ (FMs) views on being present during resuscitation and contributing to end‑of‑life care. METHODS: A cross‑sectional study with 467 FM volunteers of mildly injured or ill patients was conducted at a research hospital between October 2021 and May 2022. Data were collected using a questionnaire administered by a clinical psychologist. The analysis employed SPSS 22.0 with a significance threshold of P < 0.05. The study was conducted according to the STROBE criteria. Statistical significance was set at P < 0.05. RESULTS: The mean FMs’ age was 34.3 ± 10.43; 64.2% were male, 62.1% were married, and 76.9% had nuclear families. About 61% wanted the option of being present during resuscitation, with 47.5% desiring participation in both resuscitation and end‑of‑life care. Significant differences were observed in opinions based on education, work status, and resuscitation training (P = 0.015, P = 0.001, P = 0.002). CONCLUSION: Many FMs sought the choice to be present during resuscitation, and nearly half preferred participation in both resuscitation and end‑of‑life care. © 2024 Turkish Journal of Emergency Medicine | Published by Wolters Kluwer - Medknow
Do natural resource dependence, economic growth and transport energy consumption accelerate ecological footprint in the most innovative countries? The moderating role of technological innovation
Climate change coupled with environmental degradation is primary concern of the modern society. Natural resource dependence has been widely recognized as a principal stimulator of environmental degradation. However, little is known whether natural resource dependence may take part in environmental sustainability of the most innovative countries via the channel of technological innovation. Against this backdrop, this study aims to explore whether natural resource dependence, transport energy consumption, economic growth, and technological innovation explain ecological footprint of the seven most innovative countries under the environmental Kuznets curve (EKC) framework in the period from 1990 through 2018. This study utilizes both, the patent applications and research, development and demonstration budget as proxies for the technological innovation. By employing panel Method of Moments Quantile Regression (MMQR), this study's outcomes outlined that both transport energy consumption and natural resource dependence significantly increase ecological footprint while technological innovation reduces environmental degradation. Moreover, empirical findings of this study verified the legitimacy of EKC hypothesis. Additionally, natural resource dependence with interactive term of technological innovation provided evidence to reduce the harmful environmental effect of natural resources. The findings are robust to the selection of technological innovation proxy variable. The present empirical outcomes shed new light on the key role of technological innovation in attaining environmental sustainability clearly outlining that the most innovative countries should align the natural resources use, economic growth and transport energy consumption with the Sustainable Development Goals (SDGs). Analysis of dementia research trends in nursing using text mining approach
This research aimed to analyze dementia research trends in nursing using a text mining approach. In this study, text mining approach was used to analyze the studies on dementia in the field of nursing around the world. In this study, summaries of all available publications in English between the years 1990–2023 were extracted from the database and transferred to an Excel table by using the ‘Nursing’ AND ‘Dementia’ query from the Thomson Reuters-ISI Web of Science database. According to the results of this research, a total of 21 concept groups were identified, four of which were the most important. It was determined that the most frequently used word in all of the studies obtained from the database was ‘cost’. In this direction, scientific research on dementia in the field of nursing can be planned by evaluating the most frequently used concepts. © 2024 Japanese Psychogeriatric Society
