    STUDYING INCIDENCE OF OBESITY AND DEMOGRAPHIC FACTORS RELATED TO IT IN 30 TO 60-YEAR-OLD WIVES OF STAFF WHO ARE WORKING IN OIL COMPANY OF SHIRAZ

    Introduction: obesity is one of health problems and risk factor for manifestation of other illnesses. The prevalence of this complication is increasing in many industrial countries specially America and developing nations. The present research aims to study prevalence of obesity and demographic factors related to it in 30 to 60-yearold wives of husbands who are working in Oil Company of Shiraz. Materials and Methods: this is a descriptive-analytic and sectional study. At first, by taking letter of recommendation of Islamic Azad university of Marvdasht to health service canters of Oil Company, necessary permissions were taken from clinics of Oil Company. The population form was prepared from all wives of Oil company staff in Shiraz and sampling was done randomly. 500 subjects were obtained by the formula. In order to measure the weight, digital scale (with error of 100 g) was used and its validity was confirmed. The weight and height were measured by a skilful person without shoes with light and thin cloths and body mass index was calculated. In order to analyze statistical data, SPSS version 21 was used in addition to Chi-square test, Fisher Exacttest and ANOVA. Results and Conclusion: results showed that among 500 subjects under study, the weight of 30% (154 persons) was normal, the weight of 3% (15 persons) was low, 34.8% of them had overweight (174 persons), 14% (75) had obesity type 1, 13.2% had obesity type 2 (66 persons), 4.2% had obesity type 3 (21persons). Mean age was 47.3 ±8.1, mean weight was ± 72 ±5.6 and mean height was 159.3±5.6. 425 of women were housewives and others had jobs. Also among the husbands of the women under study, 204 husbands were retired (40.8%) and 296 husbands (59.2%) were still working in Oil company. the salary of 376 persons (75.2%) was monthly higher than three million tomans and others were paid approximately three million tomans per month. 249 persons (49.8%) were walking continuously as an exercise, 201 persons (40.2%) did no exercises and others did trivial activities such as aerobic and swimming (10%)

    COMPARISON OF PREDICTORS OF DIFFICULT INTUBATION

    Context: Unanticipated difficult laryngoscopy and tracheal intubation always remain a primary concern for an anaesthesiologist as the failure to maintain a patent airway during induction of anaesthesia may lead to anaesthesia related morbidity and mortality. Aims: The aim of our study was to predict difficult intubation and to identify best predictor(s) among them and also to compare the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of various airway parameters.Airway parameters taken in our study were Modified Mallampati Classification (MMT), Thyromental Distance (TMD), Sternomental Distance (SMD), Interincisor Gap (IIG), Upper Lip Bite Test (ULBT), Degree of Neck Extension (DNE), Anterior Subluxation of Mandible (ASM) and Protruding Teeth (PT). Methods and Material: 350 patients of ASA Grade 1 and 2 scheduled for various elective surgeries under general anaesthesia were included in our study and were assessed preoperatively for different airway parameters. Intraoperatively all patients were classified as difficult and easy intubation group according to Cormack and Lehane laryngoscopic view. Clinical data of each test was collected, tabulated and analyzed to obtain the sensitivity, specificity, positive predictive value and negative predictive value. Results: The upper lip bite test had the highest sensitivity (80%); Anterior subluxation of mandible had highest specificity (99.06%) and both of above were most accurate tests. The overall incidence of difficult intubation was 8.57%. Conclusion: Upper lip bite test was the best predictor of difficult intubation and it should be included as a routine test along with Modified mallampati test in preanaesthetic evaluation

    EFFECT OF PRANAYAMA AND YOGASANAS ON LIPID PROFILE IN NORMAL HEALTHY VOLUNTEERS

    Background: The present study was conducted to assess the impact of Pranayama and Yogasanas on healthy individuals in Patanjali yoga centre, Kurnool. Methodology: 30 subjects in the age group of 25-35 years, of either sex were included in the study and none of them had a history of Lipid Metabolism Disorders. The scope and objectives of the present study were explained to the subjects and their written consent was obtained for participation in the study. The Institutional Ethical Committee had approved the study protocol and design. Pranayamas and yogasanas were practiced by the subjects for One and half hour on each day for three months. The Lipid profiles were compared one day prior and one day after experimental period. Result: A Significant reduction was observed in Total Triglycerides, Total Cholesterol, VLDL-cholesterol and LDL Cholesterol and a significant elevation of HDL-cholesterol was seen in the healthy volunteers at the end of 3 months. Conclusion: It was concluded from the results of the study that yoga practice has brought positive changes in the lipid profile in healthy individuals.     A RARE CASE OF PULMONARY ASPERGILLOMA

    Pulmonary aspergilloma is unprecedented disorder affecting lung parenchyma in already existing cavity in healed pulmonary tuberculosis patients. Typically, it is resulting from Aspergillus fumigates leading to development of fungal ball. Common presenting complaints are Haemoptysis, Dyspnoea, Cough, Chest pain and Fever. We are reporting a case of Pulmonary aspergilloma a sequel of Pulmonary tuberculosis, has been recognized on basis of clinical findings, chest X-ray, CT thorax in which Fungal ball is seen in pre-existing cavity. It has been managed with antifungal drug Amphotericin B and Itraconazole. It must be differentiated from different clinical entity specifically Lung carcinoma on basis of relevant examination and research to treat successfully

    A CASE OF MULTIORGAN DYSFUNCTION DUE TO SCRUB TYPHUS INFECTION

    Rickettsia tsutsugamushi causes an acute febrile illness known as scrub thypus. Humans get infected when they accidentally encroach the mite infested areas (mite islands) mainly in rural and sub-urban areas1. Scrub typhus is being increasingly reported in India. It should be considered in the differential diagnosis of patients with acute febrile illness, including those with thrombocytopenia, abnormalities in liver function tests, altered sensorium, atypical pneumonia, acute respiratory distress syndrome. We report a case Multi Organ Dysfunction due to scrub typhus infection. A thorough knowledge of scrub typhus including varied presentations and its complications is important for providing life saving treatment for patients

    EVALUATING SOCIAL WORKERS WORKING LIFE QUALITY WORKING AT BUSHEHR UNIVERSITY OF MEDICAL SCIENCES IN 2015-16

    Background and Objective: Working life quality is a combination of working environment variables which could be effective on job satisfaction increase, personnel retention and finally qualitative and quantitative promotion of services in each organization. So, Ministry of Health is not exempt from this rule. The present study has been performed with the aim of determining social workers working life quality working in Bushehr province health centers. Methodology: This study has been conducted in cross- sectional method (descriptive - analytical) on 467 social workers of the province health center. Statistical population selection and participation is in census form. Data collecting tool is Walton two-part questionnaire. The first part of questionnaire includes social workers demographic information which includes 7 questions. The second part of questionnaire which contains social workers working life quality includes 35 questions. Regarding wide usage of this questionnaire, its reliability and validity is confirmed and it is in an acceptable level. The collected information was analyzed using SPSS version 20 software and the results were analyzed using descriptive - analytical statistics, Pierson correlation and unilateral variance analysis. Findings:The average total score of social workers working life quality 97.5 indicates desirable level. Considering all cases, also inclusion of confounding cases in regression model , in respect of demographic information only the variables of city (p=0.026) , age (p=0.006), working background( p=0.33) determine social workers working life quality. Conclusion: Working life quality of Boushehr province social workers is in desirable level and the impact of this variable is considerable in increasing job satisfaction rate and social workers motivation and as a result the society health level promotion. Regarding the role of some factors especially demographic factors in more promotion of this variable, necessity of identifying these factors and trying to improve working life quality has special significance

    Rheumatoid arthritis is an autoimmune disorder. It is a chronic progressive disease resulting in inflammation of joints and painful deformity and immobility of various joints. Being an autoimmune disease, there\u27s lacuna in proper management of the disease. Current options like steroids and DMARD\u27S (disease modifying anti-rheumatic drugs) are the cornerstone in therapy of the disease, but have their own limitation. New drugs and better methods for management of rheumatoid arthritis are still evolving. The present review highlights the possible involvement of Montelukast, an antagonist of leukotriene receptors and Cetrizine, an antihistaminic drug in amelioration of the progression of the disease

    COMPARITIVE STUDY OF DELAYED PRIMARY CLOSURE VERSUS PRIMARY CLOSURE OF SKIN IN CONTAMINATED AND DIRTY ABDOMINAL WOUNDS/ INCISION

    Background: It is still a matter of debate whether delayed primary  closure (DPC) of contaminated abdominal incision reduces surgical site infection compared with primary closure (PC ). The rate of wound infection for dirty abdominal wound is approximately 40%, but the optimal method of wound closure remains controversial. Aims and objectives: To determine whether delayed primary skin closure of contaminated and dirty abdominal wounds reduces the rate of surgical site infection (SSI) compared with primary skin closure. Method: Patient diagnosed as acute peritonitis and posted for exploratory laparotomy during the period of October 1 2013 to September 1 2015 were included. The study was conducted at Shri B M Patil Medical College and Hospital, Bijapur. In this series a total of 100 patients were included and were divided in two groups. Each group had 50 patients. For primary closure group, wounds were closed with monofilament interrupted suture. For Delayed primary closure, skin and subcutaneous tissue are left open and packed with 10 % (betadine) povidone iodine soaked gauge, which was changed daily to prevent excessive collection of exudates. The outcome of wound was assessed on post - operative days. Result: In this entire series, wound infection developed after incision closure was 33% .The primary group had a higher rate of wound infection 54% and  delayed primary closure was  12% (P<0.001) and longer length of hospital stay 19.4 days in primary closure group  and 16.5 days in delayed primary closure group (P<0.002). Conclusion: Laparotomy wound complications are multifactorial, it depends on many factors. A strategy of DPC of dirty abdominal wound, clinically appears to decrease the rate of wound infection, when compared with PC without increasing the length hospital of stay
