EFFECTIVENESS OF BACK SCHOOL EXERCISES IN NON SPECIFIC LOW BACK PAIN
Background: Worldwide, 65 - 80% of the population experience low back pain at some stage of their lives. The most common cause of disability is related to the musculoskeletal system; particularly low back pain and spinal disorders. It is convenient to classify low back pain into two groups i.e. specific and nonspecific low back pain. Aim: To study effectiveness of back school exercises in nonspecific low back pain. Methods: A sample size of 50 between age group of 20 - 60 years was selected. Pre intervention VAS and RMDQ score was noted with back school intervention for four weeks followed by post intervention VAS and RMDQ score. Results: After back school intervention the VAS score for pain intensity was reduced at rest and on activity, the RMDQ scores was also reduced significantly, it shows the low back pain disabilities was reduced in all age groups. Conclusion: From the study it is concluded that the back school exercises are effective in nonspecific low back pain. The back school interventions can improve workability of patient
PRIMARY OVARIAN PREGNANCY IN A PRIMIGRAVIDA: A CASE REPORT
Primary ovarian pregnancy is a rare form of ectopic pregnancy due to the implantation of the gestational sac in the ovary. It is reported to occur in 1 in 25000-40000 pregnancies following natural conceptions and accounts for 0.3- 3% of all ectopic pregnancies. The preoperative diagnosis is difficult and is mostly diagnosed following surgery or histo pathological examination. Here we present a case of ovarian pregnancy which was managed surgically and its review of the literature is discussed. KEYWORDS: Ectopic pregnancy; Extrauterine pregnancy; Primary ovarian pregnancy; Diagnosis; Management

RELATIONSHIP BETWEEN ORGANIZATIONAL INTELLIGENCE AND HOSPITALS AGILITY OF SHIRAZ UNIVERSITY OF MEDICAL SCIENCES
Introduction: Regarding the changing environment of hospitals and necessity of providing services in the shortest possible time and with acceptable quality and cost for patients, attention to innovative and smart approaches and using maximum mental abilities of the organization for increasing hospitals agility seems necessary. In fact, this study aims to answer this question that is there a significant relation between organizational intelligence (and its components according to Albrecht model) and hospitals agility?Method: The present study is applied in respect of aim and descriptive in regard of nature and method and for this purpose, a 368 persons sample was selected by stratifies sampling method among all 8247 personnel working in 15 active hospitals. For data collection in the field of organizational intelligence Albrecht organizational intelligence standard questionnaire and for organization agility, agility standard questionnaire which has been designed based on Goldman agility model were used. Results: Findings showed that there is a positive and significant relation between organizational intelligence and its seven components (strategic perspective, shared fate, desire to change, spirit, knowledge application, union and agreement and performance pressure) with hospitals agility and except intervening variable of educations other intervening variables like age, gender, marital status and working background have no impact on organizational intelligence and hospitals agility. Conclusion: Regarding the results of this study, choosing some solutions for codifying dynamic strategic programs in hospitals and holding training courses of organizational intelligence for informing personnel and managers could lead to increasing of hospitals agility level and providing effective service for patients
EFFECTIVENESS OF CHEST PHYSIOTHERAPY IN PRONE POSITION ON RESPIRATORY FUNCTIONS IN VENTILATED NEONATES: A CASE SERIES
Background: A child\u27s risk of dying is highest in the first 28 days of life (the neonatal period). The central function of Chest physiotherapy in ventilated neonates is to assist in the removal of tracheobronchial secretions, remove airway obstruction, reduce airway resistance, enhance gas exchange, and reduce the work of breathing. Positioning is used with the aim to improve of ventilation/perfusion (V/Q) matching, lung volumes and mucociliary clearance and to reduce the work of breathing (WOB). Objective: To find out effectiveness of Chest Physiotherapy in prone Position on Respiratory Functions in Ventilated Neonates. Method: 6 ventilated Neonates (n=6) between day 1 to day 28 fitting the inclusion criteria were selected. They received chest physiotherapy in prone position which was given to each participant in for 240 minutes/day in two divided sessions of 120 minutes each with a gap of 6 hours (10am -12 pm & 6pm – 8pm) for consecutive three days. (i.e. six intervention sessions). Primary outcomes were Oxygen saturation (SpO2), Partial pressure of arterial oxygen (PaO2) & Peak Inspiratory Pressure (P.I.P.). Outcomes were recorded Pre & Post of Last (6th) 120 minutes session (0 Min & 120 Mins). Results: On comparison of three parameters in two groups using paired t test we found that there was significant difference (p < 0.05) in SpO2, PaO2 and P.I.P at baseline and post intervention. Conclusion: The study concluded that chest Physiotherapy combined with prone positioning is a cost effective, non-invasive and affordable intervention which has a significant impact on improvement of SpO2, PaO2 and PIPin ventilated neonates. A randomized clinical trial evaluating the efficacy of this intervention seems warranted
SIGNIFICANCE OF SERUM TOTAL ALKALINE PHOSPHATASE LEVELS IN BREAST CANCER
Background: Breast cancer has shown an increased prevalence worldwide, both in industrialized and developing countries. Unfortunately, most cancers do not produce any symptoms until the tumors are either too large to be removed surgically or metastasis has taken place. Therefore, there is need for simple biochemical investigations, which can be easily assayed, less expensive and can predict stages and prognosis. This study was undertaken to know the usefulness of estimation of serum Alkaline Phosphatase (ALP) in assessing the prognosis and staging of breast cancer in smaller laboratories where sophisticated technology is not available. Materials & Methods: A total number of 95 subjects participated in the present study which included 60 breast cancer cases and 35 controls. Serum Alkaline Phosphatase levels were estimated in preoperative & postoperative breast cancer subjects of different stages and control. ALP Levels were compared between control and breast cancer. Also comparison was done between different stages and further, ALP levels compared pre and post operatively. Results: Rise in the ALP was found in breast cancer when compared to controls. ALP levels increased significantly with the stage of cancer. Postoperatively, there was significant decrease in ALP levels in each stage. Conclusion: Though less sensitive than imaging procedures, measurement of serum ALP is cost effective and may be useful in smaller laboratories for staging, to know the prognosis and monitor the treatment. Serial analysis using plasma Alkaline phosphatase isoenzymes combined with other parameters like Gamma glutamyl transferase (GGT) for the detection of metastasis would seem to be justified
STUDY OF LIPID PROFILE AND NON-HDL CHOLESTEROL AS A BIOMARKER OF CARDIOVASCULAR RISK IN TYPE 2 DIABETES MELLITUS
Introduction: The reduction of cardiovascular risk by lowering low-density lipoprotein cholesterol (LDL-C) is well documented, and LDL-C remains the main target of lipid lowering therapy. However, not all patients with cardiovascular risk have elevated LDL-C. There is growing recognition that non-high density lipoprotein cholesterol (Non-HDL-C) is strongly related to cardiovascular risk. Aim: This study was done to evaluate the importance of Non-HDL C in predicting cardiovascular risk in type 2 Diabetes Mellitus. Methods: 100 type 2 diabetic patients were taken as subjects. Fasting and post meal blood sugar, lipid profile, and Non-HDL C was analysed in these patients. The patients were divided in two groups depending on their Non- HDL C level; ≤130 mg/dl and > 130mg/dl. Result and discussion: In this study it was seen that, age <60 years, being female, BMI >25 kg/m2 and LDL cholesterol >100mg/dl were associated with having Non-HDL cholesterol >130mg/dl. Conclusion: The results showed positive correlation between Non-HDL and LDL cholesterol. It also showed significant non achievement of Non-HDL cholesterol targets of ≤130mg/dl even if LDL cholesterol targets were achieved i.e. <100mg/dl suggesting the importance of measuring Non - HDL Cholesterol to predict the risk of cardiovascular disease in type 2 diabetes
EFFECT OF DIABETES MELLITUS TYPE II ON ACTIVATED PARTIAL THROMBOPLASTIN TIME AND PROTHROMBIN TIME
Diabetes mellitus contributes for initiation and progression of micro vascular and macro vascular complications. Complication includes coagulation impairment however Shortened activated partial thromboplastin time (aPTT) values may reflect hypercoaguable state, which is associated with increased thrombotic risk and adverse cardiovascular events. Objective: The present study was conducted to study the aPTT and PT levels in patients with diabetes mellitus type II. Material and method: A sample of 50 persons were selected, 30 patients suffering from DM as type II from Al-wahda teaching hospital, Derna-Libya. At the same time a group of 20 randomly selected healthy adults to participate in the study as control group. Result: The mean value of APTT in T2DM individuals was significant lower (28.95 ±7.54) seconds as compare with control, (34.12±2.82) seconds (P = 0.06). Â The mean value of prothrombin time (PT) among T2DM individuals was (14.04 ±2.96) seconds and the mean value of PT among healthy individuals was (13.5 ± 1.54) seconds. There was no significant difference in PT of T2DM individuals P < 0.05. Conclusion: From this study, it may be concluded that shorted APTT confirms that T2DM is a hyper-coagulable state due to which there is increased risk of thrombotic events
IMPACT OF HEALTH EVOLUTION PLAN ON BETWEEN- HOSPITAL DISPATCHES OF ORTHOPEDIC SERVICE IN SHIRAZ MEDICAL SCIENCES UNIVERSITIES IN 2013-2014
Introduction: Today, health system, like any other event dependent to social conditions, requires effective evaluations for promoting condition and function improvement. The present study aims to examine the impact of health evolution plan on between- hospital dispatches of orthopedic service in Shiraz Medical Sciences Universities in 2013-2014. Methodology: This study was performed with descriptive- analytical method in time period of 2013 and 2014. The research population included all orthopedic service dispatches which have been accomplished from hospitals of Fars province cities to hospitals settled in Shiraz. The research sample was coincident with the mentioned statistical population. The research information was received from Shiraz Medical Sciences University, medical care monitoring center and included data relating to dispatches such as non-existence presence of specialized physician in the hospital. Statistical analysis was performed using descriptive statistics and Chi-square, Mann-Whitney test and Z-test and by SPSS 22 software. Results: The number of dispatches with average age of 41 years (SD=22/78) before health evolution plan has been 209 cases and after evolution plan it has reached 279 cases with average age of 43 years (SD=22/38). The results showed that there is a significant difference between before and after health evolution plan based on non- existence of specialist and also need to operation room (p<0.05). also, the results revealed that there is no significant difference between before and after evolution plan based on specialist presence, empty bed, department or respective service, tendency to private hospital, the patient financial problem, time of getting admission and time of admission registration (p<0.05). Conclusion: Health evolution plan has been successful in the field of specialist existence in cities while, in the field of reducing dispatches from cities to center of province has not performed successfully. Regarding the importance of between- hospital dispatch issue and expectations envisaged from evolution plan, it seems that despite the obtained changes, fundamental amendments are necessary to achieve the optimal result
