A CASE OF PRIMARY SJOGREN`S SYNDROME WITH DISTAL RENAL TUBULAR ACIDOSIS PRESENTING AS HYPOKALEMIC PARALYSIS
Renal tubular acidosis secondary to autoimmune interstitial nephritis is quite common in patients with Sjogren`s syndrome. Here we present a case of 24 year old female who presented with Acute Hypokalemic Quadriparesis and was later diagnosed with distal RTA. Patient did not have features of xerostomia or xerophthalmia but was diagnosed to have Primary Sjogren`s syndrome fromserological findings,in this case renal involvement preceded subjective Sicca syndrome.Patient recovered after giving i.v potassium. Patient was treated with corticosteroids and is asymptomatic during the one year follow up period
THE SUGAR BUSH- MAPLE SYRUP
Maple syrup is the syrup made from the xylem sap of trees like sugar maple, red maple, black maple and some other maple species. From one of nature\u27s true phenomena the process of maple syrup production gets started. During spring when the nights are very cold, the maple tree absorbs water from the soil. In the daytime due to warmer temperature a pressure is created which pushes the water again to the tree bottom which helps in easy collection of the maple syrup which is very precious actually. It takes almost 12 to 20 days to collect the sap depending on the area / region.

STUDY OF SERUM LIPID PROFILE AND FASTING BLOOD SUGAR IN POLYCYSTIC OVARIAN SYNDROME
Background: Polycystic ovarian syndrome (PCOS) is the multisystemic disorder and most common reproductive endocrinopathy of women during their childbearing years, expressed in wide varieties of clinical signs and symptoms. It is characterized by a varied and often complex array of metabolic and endocrine abnormalities, including hyperinsulinaemia, hyperglycaemia, glucose intolerance and obesity which put women with PCOS at a higher risk for diabetes, hypertension, dyslipidemia, and cardiovascular disease. Objectives: To estimate Fasting blood glucose and lipid profile in women with PCOS and normal females. Materials and Methods: After Ethical Committee Approval, blood samples were collected from 50 diagnosed PCOS cases and 50 healthy controls (premenopausal women); aged 18 to 40 years. Fasting plasma glucose and lipid profile were investigated in both PCOS patients and controls. The correlation between these biochemical parameters were then studied in the PCOS group. Data analysis done using student\u27s t- test. Results: There was a significant increase in fasting plasma glucose levels in PCOS patients as compared to controls. PCOS women had higher BMI with increased total cholesterol, TGL, LDL-C, VLDL-C and lower HDL-C (P < 0.05) as compared to the controls which was statistically significant. The levels of glucose showed significant positive correlation with total cholesterol (P<0.01), triglycerides(P<0.05), LDL-C (P<0.01) whereas non-significant negative correlation with HDL-C. Conclusion: The findings of this study confirms the association between Glucose, BMI and dyslipidaemia in PCOS and may help to identify women with PCOS at risk of cardio metabolic syndrome thereby confirming the association between PCOS and cardiovascular risk factors
EVALUATION OF RISK MANAGEMENT STATUS IN OPERATION ROOM OF NAMAZI HOSPITAL OF SHIRAZ CITY
In this study, the awareness rate from risk management was examined in the personnel of operation room and anesthesia in Shiraz Namazi hospital. For performing this study, a questionnaire which was previously designed by Zaboli et.al in Tehran Bagheiatallah Hospital was used. This study sample content was 123 persons and data relating to the individuals awareness rate from risk management and also demographic and job features were collected. Finding of this study showed that most participating people had a moderate awareness from risk management and awareness rate in women was more than men and by income increase people awareness increased. No significant relation was found between awareness and age, working experience and so on. Regarding lack of relation between working experience rate and type of job and age with risk management, holding some classes and training all personnel about risk management seems necessary
HEPARIN INDUCED THROMBOCYTOPENIA AND HEMODIALYSIS
Unfractionated heparin is the most commonly used anticoagulant for hemodialysis (HD). It is well-known that heparin can cause immune-mediated thrombocytopenia due to immunoglobulin antibody formation against the complex of platelet factor 4 (PF4) and heparin. Heparin may also contribute to HD-associated platelet activation, thrombocytopenia, and increased PF4 release from platelets during a heparin dialytic session. The present study was conducted to study the effect of unfraction heparin as anticoagulant in newly treatment hemodialysis patients. Material and method: A sample of 72 people were selected, 32 patients on dialysis for first time from unite of kidney dialysis. At the same time a group of 40 randomly selected healthy adults to participate in the study as control. By Automated cell counter (sysmex X 21) platelets from all patients on dialysis before starting heparin and after one month later were estimated. Result: The mean value of platelets in patients after treated with heparin was significant lower (192.3 ± 20.7 ) — 109/l as compare before treated with heparin 203 ± 20.7 — 109/l ( P = 0.001). Conclusion: From this study, heparin as anti-coagulant has effect on decrease platelets count but still patients have no thrombocytopenia platelets level 150 — 109/l.
BIOCHEMICAL BASIS AND EMERGING MOLECULAR TARGETS TO TREAT DIABETIC RETINOPATHY
Diabetes being considered as an epidemic, long term untreated complicated diabetes resulting in retinopathy will be a leading cause of blindness worldwide. Many cross-sectional studies reported a strong relationship between chronic hyperglycaemia and development, progression of retinopathy, however the underlying mechanism that cause retinal microvascular damage following prolonged hyperglycaemia, yet to be revealed. Continued research worldwide focuses on understanding the molecular basis with the ultimate goal to prevent diabetic retinopathy by developing newer therapeutic targets. This article reviews multiple biochemical pathways that are implicated in diabetic retinopathy. Recent advancement in the molecular basis of the disease as well as clinical trials undertaken to target these molecules in order to block the signalling cascade prevailing in diabetic retinopathy is also discussed. This review highlights the recent therapeutic targets to prevent the onset as well as the progress of retinopathy in diabetes
HUMORAL IMMUNE RESPONSE IN NIGERIANS WITH LONG STANDING TYPE 2 DIABETES MELLITUS
Introduction: Deranged immunologic capability has been widely implicated in diabetic subjects. It is not well documented if dysfunctional humoral antibodies that occur in DM leads to susceptibility to infections as a result of poor glycaemic control or a reaction that occurs when the infection has already set in. We sought to evaluate the pattern of humoral immune response in Nigerians with Diabetic mellitus with and without complications and its association with glycaemic control indices. Methods: This was a cross sectional analytical study conducted on 150 people with type 2 DM between the ages of 38 and 80 years and 75 age and sex matched healthy controls. Presence of co morbidities and complications was sought for in the subjects. DM subjects were subdivided into early onset (less than five years duration) and long standing (greater than five years duration). Glycaemic control was assessed using fasting plasma glucose, fructosamine and glycosylated haemoglobin. Plasma immunoglobulins A, G, and M were estimated using elisa method. Results: The mean levels of all the studied immunoglobulins were comparable in DM and healthy controls save for immunoglobulin M which was significantly lower in DM. A significantly inverse association was observed between immunoglobulin G with fructosamine (r = - 0.356, p = 0.030) and glycosylated haemoglobin (r = -0.352, p = 0.026). Immunoglobulin M was negatively associated with systolic blood pressure (r = - 0.269, p = 0.034 ) and diastolic blood pressure (r = - 0.257, p = 0.044). Conclusion: Plasma levels of Immunoglobulin M are lower in subjects with DM than in people without DM. Plasma Immunoglobulin G and M levels are significantly and inversely associated with glycaemic control indices and blood pressures respectively in DM subjects.

EVALUATING MANAGERS LEADERSHIP STYLE, CAREER SELF-EFFICACY AND BURNOUT AMONG NURSES
Introduction: The present study examined the relation between managers leadership style and burnout through career self-efficacy in the framework of a scientific model. Methodology: The study participants included 240 nurses (205 women, 35 men) from Shiraz city hospitals who were selected based on random multistage cluster sampling method. The participants completed three questionnaires of leadership style inventory, career self-efficacy and burnout. The model was analyzed using multiple regression method in simultaneous style based on Baron & Kenni stages. Results & Conclusion: The results showed that: a) managers leadership style is predictive of burnout dimensions, b) managers leadership style is predictive of career self -efficacy dimensions. Totally, the present study findings and Sobel test results showed that career self-efficacy has a moderate role between managers leadership style and burnout
STUDY OF THE RELATIONSHIP BETWEEN WORKPLACE DESIGNING AND INDIVIDUAL EFFICIENCY AMONG THE PERSONNEL OF TEHRAN SHAHID MODARRES HOSPITAL IN 2015
Introduction: Today, one of main priorities of managers is increasing efficiency and improvement of provided services in health and treatment domain for increasing people satisfaction. By increasing efficiency in their organizations, managers could pave the way for achieving organizational aims and the country development in health and treatment domain. Regarding the different designing of workplace and factors effective on personnel efficiency in various organizations, this study has been performed with the aim of examining the relation between workplace designing and personnel efficiency. Materials and Methods: The present descriptive study was performed in 2015 in cross-sectional form. The study statistical population was constituted from 750 persons of Tehran Shahid Modarres hospital personnel. For collecting data, scholar- made questionnaire was used whose validity was confirmed. In this study, 107 men and 175 women randomly completed the questionnaire that totally 282 people were examined. The questionnaire validity was calculated using Cronbach alpha between 0.691 and 0.701. Data obtained from the questionnaire was analyzed using SPSS software and statistical analysis (variance analysis, dependent T-test, Pierson correlation test, regression). In data analysis the highest and lowest score was 1 and 5. Results: Findings indicate that workplace physical factor including furniture, temperature, noise and music had significant relations with efficiency and lighting and spatial arrangement did not have any significant relation with efficiency. Generally, temperature with correlation (0.241) had the highest impact and noise (0.137) had the least impact on efficiency. Conclusion: Improvement and suitable designing of workplace leads satisfaction and increasing personnel efficiency
A COMPARITIVE OVERVIEW OF HISTOPATHOLOGY OF GRANULOMATOUS LESIONS OF SKIN
Granulomatous lesions of the skin and subcutaneous tissue are common in India. They are known as "Dermal Granulomas". In present study following types of Dermal Granulomas were included Leprosy, Cutaneous Tuberculosis, Syphilis, Fungal, Actinomycosis, Foreign body Granuloma, Granuloma annulare and Sarcoidosis. Histopathology remains a time tested tool for establishing a correct diagnosis. Clinical lesions often reveal surprising underlying pathology. Hence carrying out skin biopsies and microscopic study with routine haematoxylin and eosin (H & E) as well as special stains are must in these disorders so that the type and aetiological agent of the granuloma are properly identified. Besides, follow-up biopsies after the commencement of treatment help in evaluation of the response to therapy. Methods: 52 cases were studied over a period of 2 and a half years. Specimens were collected from the patients of skin, surgery and ENT department. A brief clinical history with age presenting symptoms and signs were recorded. Routine investigations like ESR were performed. Comparison of our study was done with others around the country. Results: Most common type of dermal granuloma was leprosy (59.6%) followed by cutaneous tuberculosis (21.1%), Foreign Body (5.7%), Granuloma Annulare (5.7%), Fungal (5.7%), Actinomycosis (1.9%) and Sarcoidosis (1.9%). Dermal granulomas were most common between the age group of 31 - 50 years. In leprosy and cutaneous tuberculosis, males were affected more as compared to females. Conclusion: Leprosywas most common than other dermal granuloma. Most common type of leprosy and tuberculosis were lepromatous leprosy and lupus vulgaris respectively
