DEXMEDETOMIDINE PREMEDICATION WITH KETAMINE AND PROPOFOL DURING BURNS DEBRIDEMENT AND DRESSINGS
Background: Burn patients undergo frequent extensive burn debridement and painful dressing changes. Ketamine and Propofol are the most common anesthetic used along with Opioids and Benzodiazepines in burns dressings. Studies have shown that concomitant use of Dexmedetomidine with Propofol and Ketamine. Objectives: To study the effects of Dexmedetomidine as premedication with Ketamine and Propofol as sole anesthetic agents during burns debridement and dressing. Materials and methods: Total 60 Patients of scheduled for elective burn debridement and dressings at P.R.H. Loni admitted in the wards were enrolled for the study. Patients satisfying the following eligibility criteria were selected and grouped those who received Ketamine and Propofol (Group B) with and without Dexmedetomidine (Group A) and both the groups were assessed to find out difference in the dose requirement, haemodynamic variables and recovery time (using Ramsay Sedation scale). Results: The haemodynamic parameters like heart rate, systolic and diastolic Blood pressure was significantly higher in Group A as compared to Group B. The recovery time in Group A was 12.9 mins as compared to 9.5 mins in Group B. It was observed that dose requirement of Ketamine (228.8±21.9) and Propofol (263.2 ± 22.5) was significantly more in Group A as compared to Group B (101.1±20.3 and 120.8±22.4 respectively).Conclusion: Dexmedetomidine (1μg/kg IM dose) is a good anaesthetic adjuvant that decreases the requirement of Propofol and Ketamine during burns debridement and dressings, maintains stable intraoperative haemodynamics and also has an excellent recovery profile.

CYTOMEGALOVIRUS IgG AND IgM ANTIBODIES AMONG SUDANESE PATIENTS WITH ACUTE MYELOID LEUKEMIA: RELATION TO HEMATOLOGICAL PROGNOSTIC MARKERS
Background: Viral oncogenesis has remained an area of interest in cancer biology. Viruses have been great tutors of cancer biology, helping researchers to uncouple many signaling pathways and identifying critical therapeutic targets. Aim: The aim of this study was to assess the incidence of cytomegalovirus (CMV) infection and its impact on hematological prognostic markers of Acute Myeloid Leukemia (AML) among Sudanese populations. Method: The seroprevlance of CMV infection in AML patients was assessed in 100AML and 100 age and gender-matched controls. The associations of total white cell count and absolute blast count with the seroprevlance were examined. Results: The prevalence of CMV infection was 81% in patients and 17% in control subjects. Total white cell count and blast count were higher in AML CMV positive patients than AML CMV negative patients. Conclusion: Our findings indicate a high incidence of CMV infections in AML and its worse association with hematological markers could emphasize the role of CMV in the progression of AML.

DAY-TRADERS WORKING IN STOCK MARKET & RISK PROFILE OF NONCOMMUNICABLE DISEASES
Background: A Day-trader who is involved typical working day. He/she would have to endure extremes of unpredictable market fluctuations placing them at a greater risk of non-communicable disease. The data regarding these health issues among day-traders working in stock market in India is deficient. Objectives: This study was devised with the aim of studying the risk profile among day-traders and compare them with a comparative group. Materials & Methods: A total of 128 (64 Day-traders; 64 IT-Professionals) were interviewed using the WHO-Steps Questionnaire. Proportions, Mean values, Odds ratio and p-values were calculated with the help of statistical package STATA version 14. Results: The overall mean age of the study participants was 42.24 ±6.61 with the mean age 41.78 ±5.16 and 42.7 ±7.82 respectively. The prevalence rates of alcohol use, tobacco use, inadequate physical activity and inadequate diet among day traders were 68.6%, 56.3%, 64% and 89% respectively. More than 50% were overweight or had abdominal obesity and more than three-fourths had high blood pressure. Conclusions: The burden of NCD risk factors was high in the study population compared to the national averages. The prevalence of risk factors was high in day-traders as compared to IT professionals. Use of tobacco and prevalence of obesity & hypertension were found to be high among day-traders.

BRIDGING THE GAP BETWEEN R&D AND COMMERCIALIZATION IN PHARMACEUTICAL INDUSTRY: ROLE OF MEDICAL AFFAIRS AND MEDICAL COMMUNICATIONS
The launch of a new drug into the market requires a unique and complex process initiating from development (R&D) to its commercialization (marketing authorization and selling of these products). Due to increased burden by regulatory authorities, the medical affairs evolved as separate medical organization to bifurcate medical and commercial functions. It is closely associated with development but works mainly on post-approval activities by providing both the scientific and clinical expertise. The core functional areas of medical affairs (MA) include: medical communications/publications, medical science liaisons (MSLs) and medical information. In the recent years ethical and evidence-based medical communications has attained increasing importance in emerging pharmaceutical markets. A successful product launch is based on both promotional (medico-marketing) and non-promotional (medical information services, publications) material to support the product and to report clinical trials in peer-reviewed international journals. Local clinical or observational studies and medical surveys can provide sufficient information about the product. Besides, a number of medical communication like product monographs, brochures, slide kits, leave behind literature (LBL), continuous medical education (CME) slides, training modules, key opinion leader documents(promotional inputs) and conference posters, abstracts, and journal articles (research based inputs)are other ways to aid in an effective product launch. In the last 10 years medical writers in India have developed the capability to meet this challenge through their training and work experience making India hub for medical communications services.

CLINICAL OUTCOME OF PATIENTS WITH ACUTE SCROTAL PAIN AT A TERTIARY RURAL CENTRE IN MAHARASHTRA
Introduction: Acute scrotum is a common reason for emergency consultation. Early scrotal exploration is mandatory if torsion of the testis suspected clinically. Objectives: To study management and clinical outcome of patients admitted with acute scrotal condition. Methods: This was a prospective observational study involving 60 patients hospitalized for Acute scrotum at tertiary rural centre, from September 2014 to September 2016. Patients admitted with complaints of acute pain and swelling of scrotum irrespective of age and gender were included in the study. Patient with painless scrotal swelling and those with chronic scrotal pain were excluded from the study. Patients were monitored post-operatively till discharge and followed for 1 month for any complications. Results: Of the 60 patients in our study, acute epididymo-orchitis (36.6%) was the dominant cause of acute scrotal swelling followed by scrotal abscess (23.3%). 51.6% of the patients were managed conservatively, which were diagnosed as epididymoorchitis and haematocele. 14 patients underwent incision and drainage who diagnosed as scrotal abscess; 7 patients diagnosed as fournier\u27s underwent debridement; 5 underwent orchidectomy or orchidopexy while 3 underwent B/L orchidopexy. Out of 8 cases of torsion of testis, bilateral orchidopexy was done in 3 cases (37.5%) in which we were able to salvage the affected testis. Out of the 60 patients, maximum patients (41 patients) required a hospital stay of 1-10 days. All patients were followed up for a period of 1month after discharge. None of the patients had any serious complications. Conclusion: Acute scrotum can be successfully managed by consevative methods after exclusion of Torsion of testis. Early exploration can salvage the testis in cases of torsion of testis.

SERUM ADIPONECTIN LEVEL IN OBESE AND NON OBESE TYPE 2 DIABETES MELLITUS
Back ground: Worldwide diabetes mellitus is no more an epidemic; rather it has turned into a pandemic health hazard. Association of obesity in type 2 diabetes mellitus is an established fact. This association could be partly mediated by altered secretion of adipokines by adipose tissue. Among all adipokines, adiponectin has been considered as an important factor in obesity induced insulin resistance. This new hormone produced exclusively by adipocytes differs from its predecessors in at least one important feature. While all of the other adipose tissue derived hormones related to insulin resistance are increased in obesity, adiponectin production and concentration actually decreases in obese subjects. Objective: the study was taken up to investigate the relationship between adiponectin in obese and non obese type 2 diabetes, and also to find out the correlation between adiponectin and serum lipids in urban south Indian population. Method: The cases chosen for the study group were already diagnosed cases of type 2 diabetes mellitus undergoing treatment. All subjects were interviewed regarding a full medical history that included age, sex, occupation, duration and family history of diabetes mellitus. The general physical examination procedure included measurement of height, weight, waist circumference and hip circumference. The biochemical parameters such as fasting plasma glucose, serum adiponectin, HDL and triglyceride were measured. Result: The reduction of serum adiponectin in study group was highly significant as compared to control group but its difference between the two subgroups divided based on BMI did not give a statistically significant result. But on correlation of BMI with adiponectin in study group, the correlation co-efficient was found to statistically highly significant. Similar correlation of waist-hip ratio (W/H) with adiponectin did not give a significant result. Conclusion: Adiponectin is lowered in obese non-diabetics affecting lipid metabolism showed by increase of LDL and TG and decrease of HDL. In obese diabetics adiponectin is high but dyslipidemia is still present possibly due to improper function of existing serum adiponectin
IN-VITRO BIO-AVAILABILITY OF IRON FROM IRON FORTIFIED COW MILK-OAT MILK BLENDED YOGHURT
Yoghurt is a dairy product obtained by fermentation of milk using starter culture Streptococcus thermophilus and Lactobacillus delbrueckii ssp. bulgaricus. Application of dietary fibre and iron salt fortification in the manufacture of yoghurt enhances its nutritive and therapeutic value. Oat (Avena sativa) is a cereal that contains soluble fibre - glucan renders several health benefits. In the present investigation, an attempt was made to develop iron salt fortified, cow milk and oat milk blended yoghurt and in-vitro bioavailability of the iron from the yoghurt was explored. The optimization of oat milk tried at different levels and was accepted at 20% level and fortification of cow milk - oat milk blended yoghurt with ferrous sulphate fortified at different levels and was accepted at 10mg. Experimental yoghurt samples tested for sensory, chemical parameters, microbiological test and in-vitro bio-availability. The yoghurt samples showed (12%) of iron was maximum bio-availability from yoghurt fortified with ferrous sulphate followed by control and cow milk-oat milk blended yoghurt showed 10.2% and 8.52% respectively. The bio-availability of iron was reduced due to interfere of dietary fibre in yoghurt
VIEW POINT OF MANAGERS IN YASUJ UNIVERSITY OF MEDICAL SCIENCES, ABOUT INTELLECTUAL CAPITAL AND DETERMINATION IT\u27S EFFECTIVE FACTORS
Background and Objectives: Intellectual capital is a topic that is formed in the shadow of economic, social and managerial development in recent years. Its purpose is trying to use of knowledge effectively in management and upgrading and improving the organizations, and it has attracted much attention in today\u27s environment. This study was conducted and implemented to evaluate intellectual capital and effective factors on it in the Yasuj University of Medical Sciences of Kohgiluyeh and Boyer-Ahmad. Materials and methods: This is a cross-sectional study and all employees, including managers, Yasuj University of Medical Sciences of Kohgiluyeh and Boyer-Ahmad province, selected for statistical population. This study was done in September 2015. 150 manager of university were participated in this project and completed the questionnaire. Collected and coded information was analyzed using SPSS-ver. 22 software. We used tables and diagrams for describing the statistical population and qui-square test and t-test for data analysis. Significance level was 0.05 in this study. Results: Mean and standard deviation of intellectual capital, human capital, costumer, and structural capital in study population were 61.3 (9.3), 60.6 (10.5), 62.7 (10.4) and 61.6 (11.2) respectively. Sex has had a significant relationship with the intellectual capital but; Age, job tenure and education have had no significant relationship with the intellectual capital. Conclusion: These scores indicate the middle and low level of intellectual capital in Yasuj University of medical sciences in Kohgiluyeh and Boyer-Ahmad province. It is necessary to plan for upgrading and improving the intellectual capital in the university
