    Dissection d’artère cervicale ::proposition d’arbre décisionnel à l’usage des thérapeutes manuels

    Introduction : les dissections d’artere cervicale sont les évenements indésirables les plus redoutés lors des thérapies manuelles cervicales. Leur incidence a probablement été longtemps sous-estimée et s’éleve a 8,93/100 000 habitants/an (IC 95 %, 6,53–11,34). Leur présentation clinique varie grandement, pouvant notamment prendre la forme d’une cervicalgie d’apparence mécanique ou d’une céphalée d’apparence cervicogénique, induisant un risque d’apparente indication au traitement manuel cervical erronée. Les arbres décisionnels standardisés existants ne permettent pas une pratique sécuritaire face a ce phénomene. Un nouvel arbre s’adaptant a la complexité du raisonnement clinique est donc proposé. Développement : l’arbre décisionnel proposé est basé sur la présentation clinique et les préférences du·de la patient·e recueillies a l’anamnese ainsi qu’aux examens cliniques cardiovasculaires et neurologiques effectués. Six cas de figure sont décrits avec leurs implications pratiques, de la réorientation en urgence a la prise en charge sous condition et surveillance de l’évolution. Discussion : face au défi diagnostique que pose cette problématique, aucune certitude ne peut etre établie. Il est donc préférable d’adopter une approche prudente, en tenant compte des valeurs du·de la patient·e. Conclusion : cet arbre décisionnel pourrait participer a une prise en charge du·de la patient·e se présentant avec une cervicalgie ou une céphalée sécuritaire, et suivant les principes de l’evidence based practice en incluant les meilleures données actuelles disponibles. Une étude portant sur ses propriétés psychométriques serait pertinente afin de tester cette hypothese

    Locally specific genome‐wide signatures of adaptation to environmental variation at high resolution in an alpine plant

    Microevolutionary processes shape adaptive responses to heterogeneous environments, where these effects vary both among and within species. However, it remains largely unknown to which degree signatures of adaptation to environmental drivers can be detected based on the choice of spatial scale and genomic marker. We studied signatures of local adaptation across two levels of spatial extents, investigating complementary types of genomic variants—single-nucleotide polymorphisms (SNPs) and polymorphic transposable elements (TEs)—in populations of the alpine model plant species Arabis alpina . We coupled environmental factors, derived from remote sensed digital elevation models (DEMs) at very high resolution (0.5 m), with whole-genome sequencing data of 304 individuals across four populations. By comparing putatively adaptive loci detected between each local population versus a regional assessment including all populations simultaneously, we demonstrate that responses of A. alpina to similar amounts of abiotic variation are largely governed by local evolutionary processes. Furthermore, we find minimally overlapping signatures of local adaptation between SNPs and polymorphic TEs. Notably, functional annotations of candidate genes for adaptation revealed several symbiosis-related genes associated with the abiotic factors studied, which could represent selective pressures from biotic agents. Our results highlight the importance of considering different spatial extents and types of genomic polymorphisms when searching for signatures of adaptation to environmental variation. Such insights provide key information on microevolutionary processes and could guide management decisions to mitigate negative impacts of climate change on alpine plant populations

    Healing beyond the surface ::empathy’s role in chronic wound care a qualitative study

    Aim : This study aimed to investigate health professionals’ perceptions of the impact of empathy on healing outcomes, patient satisfaction and delivery of wound care. Materials and Methods : A descriptive qualitative approach was used, employing semi-structured face-to-face interviews with 10 healthcare professionals specializing in wound care in Australia. Participants were purposively sampled, and interviews lasted between 22 and 47 minutes. Data collection concluded upon achieving saturation. Interviews were transcribed verbatim using CORV, a secured transcription tool, and analyzed using Braun and Clarke’s six-phase thematic framework. Rigor was ensured by employing COREQ guidelines, triangulating data analysis, and including direct quotes for confirmability. Results : Three key themes emerged: (1) Empathy as a Foundation for Trust, Engagement, and Collaboration where participants emphasized that empathy promotes trust, improves patient adherence, and aligns treatment goals. (2) The Dual Role of Empathy in Addressing Emotional and Physical Barriers to Healing where empathetic care alleviated psychological distress, built resilience, and improved adherence to care regimens. (3) Barriers and Facilitators to Empathy in Wound Care where time constraints, emotional fatigue, and cultural differences were identified as barriers, while mentorship, training, and supportive environments facilitated empathetic care. Conclusion : Health professionals considered empathy to be a critical factor in enhancing patient satisfaction and supporting healing outcomes in wound care. By providing trust, addressing emotional barriers, and aligning care with patient needs, empathy plays a meaningful role in the care process. Additionally, addressing systemic barriers and thoughtfully integrating empathy training into clinical practice can contribute to improved outcomes in chronic wound management

    This paper presents an unpublished collection of Aénts Chicham artefacts that were discovered in the former ethnographic collections of the University of Fribourg, Switzerland. Besides traditional material culture from the Ecuador and Peruvian border area, it contains items that incorporate Western materials such as textiles and glass beads. Highly appreciated by indigenous societies, these were exchanged over great distances through a system of trade partners. The collection is discussed together with its possible link to the Jesuit Mainas missions and to Swiss emigration waves of the 19th century

    SLANet-1M ::a lightweight and efficient model for table recognition with minimal computational cost

    Modern approaches for table recognition consist of an encoder for feature extraction and one or more decoders for structure recognition and cell box detection. Recent advancements in this field have introduced Transformers, initially in the decoders and more recently in the encoder as well. While these improvements have enhanced performance, they have also increased model complexity, requiring larger datasets for training, a pre-training step, and higher inference time. In this paper, we explore SLANet, a lightweight Transformer-free model originally trained on PubTabNet. By training it on the SynthTabNet dataset, we improve its S-TEDS score by 0.47%, we named this model SLANet-1M. Additionally, SLANet-1M achieves an S-TEDS score on PubTabNet that is only 0.41% lower than the state-of-the-art UniTable Large, while using nearly 14 times fewer parameters. On SynthTabNet, its S-TEDS score is just 0.03% below UniTable Large. Moreover, SLANet1M outperforms large vision-language models (VLMs) such as GPT-4o, Granite Vision, and Llama Vision in this specific task. SLANet-1M is also more efficient during inference, offering faster processing and CPU-friendly execution, eliminating the need for a GPU

Despite the potential benefits of service companies using robots, there are also important ethical considerations to be taken into account to ensure that consumers accept these smart devices. This study examines how participants’ assessment of ethical issues affects the risks perceived and their intention to use a service robot. Several other variables such as ease of use, level of usefulness, emotional appeal, and social presence are also integrated into our model

    The Galapagos Islands, known as a leading sustainable tourism destination in the world, was hit by the COVID pandemic in 2020. Tourism numbers fell by 73% from 2019 causing tremendous challenges to local tourism stakeholders. The study revealed that the UNESCO World Heritage Site, despite its excellence in the management of environmental sustainability, has been falling short concerning social and economic sustainability of local tourism stakeholders. Through an exploratory study with 586 completed semi-structured surveys in the local population, alongside interviews with diverse stakeholders and leading tourism authorities, the article explains what social and economic impacts and challenges the pandemic has generated in the local tourism sector. Additionally, the study also elucidates, on the one hand, what sociocultural modifications the studied stakeholders needed to adopt to survive during this unprecedented period in tourism history, and, on the other hand, what tourism authorities propose to revitalize tourism in the post-COVID period

    In ophthalmology, Optical Coherence Tomography (OCT) has become a daily used tool in the diagnostics and therapeutic planning of various diseases. Publicly available datasets play a crucial role in advancing research by providing access to diverse imaging data for algorithm development. The accessibility, data format, annotations, and metadata are not consistent across OCT datasets, making it challenging to efficiently use the available resources. This article provides a comprehensive analysis of different OCT datasets, with particular attention to dataset properties, disease representation, accessibility, and aims to create a catalog of all publicly available OCT datasets. The goal is to improve accessibility to OCT data, increase openness about the availability, and give important new perspectives on the state of OCT imaging resources.
