Patients recovering from COVID‑19 who presented with anosmia during their acute episode have behavioral, functional, and structural brain alterations
Patients recovering from COVID-19 commonly exhibit cognitive and brain alterations, yet the specific neuropathological mechanisms and risk factors underlying these alterations remain elusive. Given the significant global incidence of COVID-19, identifying factors that can distinguish individuals at risk of developing brain alterations is crucial for prioritizing follow-up care. Here, we report findings from a sample of patients consisting of 73 adults with a mild to moderate SARS-CoV-2 infection without signs of respiratory failure and 27 with infections attributed to other agents and no history of COVID-19. The participants underwent cognitive screening, a decision-making task, and MRI evaluations. We assessed for the presence of anosmia and the requirement for hospitalization. Groups did not differ in age or cognitive performance. Patients who presented with anosmia exhibited more impulsive alternative changes after a shift in probabilities (r = - 0.26, p = 0.001), while patients who required hospitalization showed more perseverative choices (r = 0.25, p = 0.003). Anosmia correlated with brain measures, including decreased functional activity during the decision-making task, thinning of cortical thickness in parietal regions, and loss of white matter integrity. Hence, anosmia could be a factor to be considered when identifying at-risk populations for follow-up.Versión publicad

Towards closing socio-economic status disparities in COVID-19 premature mortality: a nationwide and trend analysis in Chile
Background: Socio-economic status (SES) disparities in coronavirus disease 2019 (COVID-19) mortality have been reported but complete information and time trends are scarce. In this study, we analysed the years of life lost (YLL) due to COVID-19 premature mortality during the pandemic in Chile and its evolution according to SES and sex compared with a counterfactual scenario [cerebrovascular accidents (stroke)].
Method: We used Chile's national mortality databases from 2020 to 2022. YLL and age-standardized YLL and mortality rates by sex and by epidemic waves were determined. The 346 communes were stratified into SES groups according to their poverty index quintile. Negative binomial regression models were used to test trends.
Results: In >2 years of the pandemic, the COVID-19 YLL was 975 937, corresponding to 61 174 deaths. The YLL rate per 100 000 inhabitants was 1027 for males and 594 for females. There was a heterogeneous distribution of YLL rates and the regional level. Communes in the most advantaged SES quintile (Q5) had the highest YLL during the first wave compared with those in the lowest SES quintile (Q1) (P < 0.001) but the opposite was true during the second wave. COVID-19 YLL trends declined and differences between Q1 and Q2 vs Q5 converged from the second to the fourth waves (0.33 and 0.15, Ptrend < 0.001 and Ptrend = 0.024). YLL declined but differences persisted in stroke (-0.002, Ptrend = 0.979).Aceptad
Assessing the role of corporate governance in non-financial performance: An Empirical Examination
A thesis submitted at the School of Business and Economics, Universidad Del Desarrollo, Chile, in fulfillment of the requirements for the degree of Doctorate in Business Economics.Research on corporate governance has garnered increased scholarly and practitioner interest since 2000, driven by rising stakeholder pressures regarding the role and objectives of firms. While traditional economic literature emphasizes the central role of financial performance, new societal paradigms are prompting organizations to prioritize non-financial performance, including environmental and societal dimensions. Despite prior research exploring the role of corporate governance in driving organizational non-financial performance, there have been inconclusive and contradictory empirical findings. This lack of consensus has spurred a growing call to employ robust, sophisticated methods to enhance our understanding of this phenomenon. This thesis responds to the recent call to develop robust empirical analyses by incorporating novel theoretical developments and employing advanced econometric methods.
The thesis aims to evaluate the effect of corporate governance on non-financial organizational performance through three research papers. The first paper (Chapter 2) delves into the literature from 1990 to 2022, shedding light on prevailing intellectual trends and debates in this research field. The second paper (Chapter 3) unveils a dynamic social mechanism that shapes and constrains corporate governance mechanisms, influencing non-financial performance. The third paper (Chapter 4) untangles the complementarities of various internal corporate governance mechanisms and their effect on non-financial performance. This thesis provides novel theoretical and methodological approaches to explore this complex phenomenon, offering practical and policy implication

Assessing the effectiveness of static heuristics for scheduling lumber orders in the sawmilling production process
Although optimization models can be used to plan the production process, in most cases static heuristics, such as earliest due date (E), longest processing time (L), and shortest processing time (S), are used because of their simplicity. This study aims to analyze the production cost of the static heuristics and to determine how this cost relates to the size of the production orders in the sawmilling industry. We set a planning problem with different orders and due dates and solved it using two cost-minimization models to compare their solutions. The first was a planning model (PL) where orders were split up into products demand by period, and the second, a planning scheduling (PS) where the sequence of processing orders based on static heuristics was assumed as known. In the latter, the minimum production cost for each static heuristic was found. In both models, the same resource constraints were assumed. The costs showed no significant changes based on order sizes. However, 0,5 % of orders were delayed using PS-E, and 17 % of orders were delayed using PL. PL was an efficient solution method when changing the orders´ size and when looking for the best static heuristic to process the orders. However, PS-E showed the ability to reduce the backlog close to zero while the PL backlog ratio was 17 %. No penalties were applied to backlogs due to their subjective nature; however, when shortages occurred, the demand was unmet or backlogged with substantial costs. Thus, in case the proposed method is adopted using a conservative backlog cost, a sawmill producing under the cut-to-order environment that produces 300000 m3 /year would reduce backlogged orders by 51000 m3. If the holding lumber cost is 2 408000.Versión publicad
Analysis of the hospital discharge rate for chronic obstructive pulmonary disease between 2019-2022 in chile
Introduction: Chronic Obstructive Pulmonary Disease is a chronic lung condition characterized by respiratory symptoms such as dyspnea, cough, sputum production, and exacerbations that may require hospitalization. This study describes the Hospitalization Discharge Rate for Chronic Obstructive Pulmonary Disease in Chile between 2019 and 2022. Methodology: Descriptive ecological study with data from the Department of Health Statistics and Information. Chronic Obstructive Pulmonary Disease deaths in Chile between 2019 and 2022 were analyzed by sex and age group (n=29,805). The Hospitalization Discharge Rate was calculated and did not require an ethics committee. Results: The Hospitalization Discharge Rate for the period was 58.19/100,000 inhabitants. The year with the highest Hospitalization Discharge Rate was 2019 with 87.68. The years with the lowest Hospitalization Discharge Rate were 2021 with 39.54 and 2020 with 43.25. Males had the highest Hospitalization Discharge Rate with 59.86. The age group of 80 years and older had the highest Hospitalization Discharge Rate with 545.56. Discussion: The study showed a decrease in the Hospitalization Discharge Rate for Chronic Obstructive Pulmonary Disease in 2020 compared to 2019, possibly due to the impact of the COVID-19 pandemic, which reduced visits to medical centers. Males had the highest Hospitalization Discharge Rate, likely due to higher smoking rates in men. The age group of 80 years and older had the highest Hospitalization Discharge Rate, possibly due to increased comorbidities in older patients. Conclusion: Epidemiological studies are essential to understand risk factors and guide primary and secondary health intervention measures for patients with Chronic Obstructive Pulmonary Disease
Introduction: Breast cancer progression involves physiological mechanisms such as metastasis. Delays in diagnosis and treatment increase the risk of mortality and are associated with barriers to healthcare access. In Chile, breast cancer is highly prevalent, and early diagnosis has improved, although disparities in the disease evolution persist. This study characterized diagnostic and staging tests, waiting times, and sociodemographic profiles to identify delays and inequities in care.
Methods: Survey study. Using a non-probabilistic sample, a questionnaire was applied in an encrypted platform with prior informed consent. The instrument collected data on requested tests, associated times, staging, and sociodemographic characteristics. These variables were analyzed using descriptive statistics, tests of association, confidence intervals, and comparison tests using bootstrapping.
Results: A sample of 263 persons was obtained. The most requested tests were biopsy (99.62%) and blood tests (80.23%). The median number of tests requested was six (Q1:4, Q3:8), with a mean of 5.87 (standard deviation: 2.24). No significant differences were observed in the percentage of persons from whom the total number of examinations were requested according to the studied variables. The day-hour-result intervals ranged from 1 to 365 days. The median day-hour-result of the biopsy was 15 days (Q1:10, Q3:30). People between 40 and 49 years old, non-residents of the capital city, belonging to income quintile I, with high school education, from the public health system, with late-stage diagnosis had higher median day-hour-result in biopsy. There was no significant difference in the number of requested tests according to staging (I and II, or III and IV).
Conclusions: Biopsy in Chile is the test of choice for diagnostic confirmation in breast cancer. Other tests are requested regardless of the diagnosis stage, contrary to the recommendations of clinical guidelines. Cancer prognosis is crucial, especially in countries with greater inequalities.Versión Publicad
