Pseudomonas aeruginosa Bloodstream Infections Presenting with Septic Shock in Neutropenic Cancer Patients: Impact of Empirical Antibiotic Therapy
This large, multicenter, retrospective cohort study including onco-hematological neutropenic patients with Pseudomonas aeruginosa bloodstream infection (PABSI) found that among 1213 episodes, 411 (33%) presented with septic shock. The presence of solid tumors (33.3% vs. 20.2%, p < 0.001), a high-risk Multinational Association for Supportive Care in Cancer (MASCC) index score (92.6% vs. 57.4%; p < 0.001), pneumonia (38% vs. 19.2% p < 0.001), and infection due to multidrug-resistant P. aeruginosa (MDRPA) (33.8% vs. 21.1%, p < 0.001) were statistically significantly higher in patients with septic shock compared to those without. Patients with septic shock were more likely to receive inadequate empirical antibiotic therapy (IEAT) (21.7% vs. 16.2%, p = 0.020) and to present poorer outcomes, including a need for ICU admission (74% vs. 10.5%; p < 0.001), mechanical ventilation (49.1% vs. 5.6%; p < 0.001), and higher 7-day and 30-day case fatality rates (58.2% vs. 12%, p < 0.001, and 74% vs. 23.1%, p < 0.001, respectively). Risk factors for 30-day case fatality rate in patients with septic shock were orotracheal intubation, IEAT, infection due to MDRPA, and persistent PABSI. Therapy with granulocyte colony-stimulating factor and BSI from the urinary tract were associated with improved survival. Carbapenems were the most frequent IEAT in patients with septic shock, and the use of empirical combination therapy showed a tendency towards improved survival. Our findings emphasize the need for tailored management strategies in this high-risk population.Versión Publicad
Croí: A Process for Core and Responsible Graphic Communication Design Education
Research has shown that addressing personal values and conviction is crucial for long-lasting Education for Sustainable Development. However, there is a shortage of theory and evidence-based value-focused processes in Graphic Communication Design Education literature. This article presents a novel personal value thinking and doing process called Croí (pronounced Cree) that can be used as a precursor to Education for Sustainable Development in Graphic Communication Design Education. Croí aims to stimulate sustainable transitions by disrupting behaviour development. Over five years, five rounds of Action Research were conducted with third-level educators and students to explore how Croí could facilitate value (or core) design, with the broad aim to encourage sustainable Graphic Communication Design. The initial Croí prototype developed over Cycles One and Two is briefly summarised, with focus on Cycles Three, Four, and Five, where Croí was further developed and evaluated. Data collection included researcher field notes, semi-structured interviews, and written reflections, analysed through Thematic Analysis. Key discussion points include fundamental process elements, Croí's impact on core thinking and doing and responsible thinking and doing, and its potential to facilitate behaviour development. The research concludes that Croí promotes core and responsible thinking and core doing, increasing the likelihood of responsible doing. It offers educators an innovative way to facilitate plural, practical, and core development for graphic communication design students, potentially influencing change in the profession. Croí is not a panacea for the complex issue of Sustainable Development, but it provides a novel and meaningful foundation for disrupting behaviour
¿Cuáles son las estrategias efectivas que el liderazgo escolar puede implementar para desarrollar procesos adecuados de gestión de convivencia escolar y fortalecer las capacidades de los funcionarios en el colegio RobertoWhite Gesell?
Proyecto de grado presentado a la Facultad de Educación de la Universidad del Desarrollo para optar al grado académico de Magíster en Dirección y Gestión EscolarEste proyecto se contextualiza en un establecimiento educacional de educación parvularia, básica, media y especial, ubicado en Palena, Región de Los Lagos. Su Proyecto Educativo Institucional (PEI) se enfoca en brindar oportunidades educativas inclusivas, promoviendo la sana convivencia y el desarrollo integral. No obstante, la institución ha enfrentado desafíos debido al estallido social y la pandemia, afectando los procesos pedagógicos. Los resultados académicos han sido dispares, con una tendencia a la baja en el SIMCE. Los indicadores de desarrollo personal y social muestran debilidades en autoestima, clima escolar y participación ciudadana. Los problemas de convivencia escolar han aumentado, evidenciando la necesidad de una gestión más efectiva y de formación continua para los funcionarios. La pregunta de investigación aborda: ¿Cuáles son las estrategias efectivas que el liderazgo escolar puede implementar para mejorar la
gestión de convivencia escolar y fortalecer las capacidades de los funcionarios en el Colegio Roberto White Gesell?
A partir de lo expuesto, se propone un Plan de Innovación y Mejora cuyo objetivo es favorecer la convivencia escolar en el CRWG mediante el desarrollo e implementación de una política institucional de convivencia escolar (PICE) y un programa de acompañamiento que fortalezca las capacidades de los miembros de la comunidad educativa.
Para ello, se realizó una revisión sistemática de la literatura con un enfoque estructurado, siguiendo los pasos: a) selección de bases de datos confiables; b) definición de palabras claves; c) filtrado por tipo de fuente; d) criterios de inclusión y exclusión; e) sistematización de la información, y f) proceso de lectura y registro.
Posteriormente, se describen los hallazgos en la literatura sobre los principios de actuación destacados en la convivencia escolar. Las estrategias clave incluyen: liderazgo distribuido, fomentando un liderazgo colaborativo, participativo y democrático; desarrollo de capacidades profesionales, invirtiendo en formación continua con foco en diversidad, aprendizaje cooperativo, educación en valores y mediación escolar; y prácticas efectivas de convivencia escolar, incluyendo evaluación y seguimiento, y prácticas pedagógico- curriculares, organizativo-administrativas y socio-comunitarias que fortalezcan el clima escolar. Estas estrategias se reflejan en un marco conceptual esquemático.
En las reflexiones, se exponen las fortalezas del plan y sus oportunidades de mejora. Finalmente, se concluye que mejorar la gestión de la convivencia escolar
requiere un liderazgo escolar efectivo, distribuido y colaborativo, que invierta en el desarrollo profesional continuo e implemente prácticas efectivas respaldadas por una evaluación constante
The roles of age, gender, and migration in shaping adolescent student satisfaction within Chilean schools
This study has a quantitative cross-sectional design that aims to investigate the relationships between gender, age, status (migrant or Chilean-born), educational satisfaction, and overall life satisfaction among adolescent students in 7th and 8th grades of the Chilean educational system. The sample includes 406 students from four municipal public educational centers located in the Santiago district of the Metropolitan Region of Chile, with at least 20% migrant enrollment. The data were analyzed using quantitative methods using the R language, with descriptive analysis, cross-tabulation analysis, and independence tests. The packages used were: ggplot2, tidyverse and ggstatplot. The study found that age has an impact on the level of satisfaction with education and relationships formed with peers. Specifically, younger students expressed higher levels of satisfaction in these areas compared to older students. Moreover, foreign students showed a statistically significant difference in relation to age range, suggesting that as these students grow older, they tend to become less satisfied with the relationships they form with their classmates. In contrast, no statistically significant difference was found among Chilean students, indicating that the findings discussed here may not be generalizable to this specific population. The analysis also indicates a significant difference in the entire sample, suggesting a correlation between age range and level of satisfaction with schooling.Versión publicad
Validation of an instrument on exposure of migrant communities to hate speech in the Chilean media ecosystem: preliminary results
Introducción: Investigaciones advierten un aumento de las formas de ciberodio hacia grupos migrantes en contextos digitales. Sin embargo, escasas herramientas relevan la experiencia de quienes enfrentan tales agresiones. Metodología: El objetivo es validar un instrumento diseñado para recoger la exposición a discursos de odio, así como los efectos en la participación de comunidades migrantes en el ecosistema mediático chileno. Se recurre al método Delphi, mediante juicio de expertos, así como entrevistas cognitivas.
El cuestionario está compuesto por 26 ítems y es aplicado a una muestra piloto de 453 personas migrantes en Chile, de las cuales un 51% se ubica en el rango etario de 30 a 59 años, 58% se identifica con el género femenino y el 60% proviene de Venezuela. Resultados: Se alcanza un Alpha de Cronbach de 0.95 y se constata que un 62% de los encuestados ha recibido mensajes de odio a través de Instagram (56%) y Facebook (45%), vinculados a su nacionalidad (33%) y bajo el encuadre de seguridad (43%), experimentando incomodidad (53%) y desesperanza (56%). En consecuencia, un 41% “a veces” elimina cuentas de medios de su dieta informativa digital y sólo un 7% participa en entornos mediáticos “frecuentemente”. Discusión: Se observa
que el ciberodio hacia personas migrantes podría producir desinformación, por evitación noticiosa, e incidir en la participación mediática de estas comunidades. Conclusiones: Se obtiene un cuestionario validado para recoger la exposición de personas migrantes a discursos de odio y sus posibles efectos.Versión publicad
Private green areas in urban planning: The risk of regulatory expropriation and/or misuse of power
Este trabajo trata sobre el establecimiento de áreas verdes de carácter privado, vale decir, sobre predios de propiedad privada en los instrumentos de planificación urbana. Para tales efectos, se analizan las propuestas de áreas verdes privadas que contiene la imagen objetivo realizada y propuesta en el marco del proceso de modificación del plan regulador comunal que desarrolla actual-mente la Municipalidad de Lo Barnechea. Dicho análisis abarca las propuestas propiamente tales, la fundamentación y motivación que dio el municipio para estas, la regulación de ese tipo de áreas en nuestro ordenamiento jurídico y la normativa que se aplicaría a las áreas verdes nuevas propuestas, en caso de que estas se establecieren finalmente en el plan regulador comunal de Lo Barnechea. En una segunda instancia, se aborda el concepto de «regulación expropiatoria» y su tratamiento en el derecho chileno. Por último, se analiza la afectación a los derechos de los propietarios, que podría resultar del establecimiento de este tipo de áreas, en tanto podrían constituir una regulación expropiatoria y/o desviación de poder.Versión publicad

El presente proyecto propone diseñar un material con propiedades acústicas y
térmicas a partir de la cáscara de naranja y aglutinantes derivados de polímeros naturales.
La investigación se desarrolla en tres etapas, la primera aborda temáticas como la sostenibilidad, ODS, fruto cítrico, características de la industria frutícola nacional y por último, los escenarios futuros hacia el desarrollo de nuevos materiales compuestos sostenibles.
La segunda etapa agrupa el procedimiento por el cual se desarrollo la
investigación, integrando fases de experimentación con la materia prima,
caracterización del material a través de pruebas normadas y validadas mediante ensayos aplicados.
La última parte explica los resultados obtenidos en la investigación, evaluando los hitos y decisiones en cada una de las etapas mencionadas anteriormente para dar paso a la conclusión y diseño a concretar del nuevo material acústico y térmico desarrollado
The Role of River Vigilance Committees to Address New Socio-Climatic Conditions in Chile: Insights from Ostrom’s Design Principles for Common-Pool Resource Institutions
Chile is currently facing a mega-drought, which is expected to lead to a significant increase in the water stress level. Social conflicts related to water use are linked to the effects of climate change and a governance system marked by the privatization of the natural resources of public interest. This study aims to analyze whether the current Chilean water governance scheme can adapt to the effects of climate change through a critical observation of the role of the River Vigilance Committees (RVCs; private user organizations exercising the public function of water management), from the perspective of Ostrom’s design principles for long-enduring Common-pool Resource (CPR) institutions. We analyze legal approaches, management mechanisms, and decision-making processes under the socio-climatic conditions that the country is currently facing. The results indicate that, with a few exceptions, the Chilean governance system does not allow RVCs to effectively incorporate the design principles—and, therefore, to achieve adaptation—due to dispersed functions, the exclusion of water users, and a lack of planning at different levels. We propose that water governance should consider the creation of River Basin Boards with broader planning powers, as well as the incorporation of different relevant stakeholders.Versión publicad
Radiotherapy-induced vitiligo in a patient with breast cancer, a case report
Vitiligo is a disease characterised by the autoimmune destruction of melanocytes, manifesting as depigmentation of the skin. We present the case of a female patient with a history of breast cancer who developed vitiligo in the area of the treatment field 12 months after the end of radiotherapy. It has been reported in the literature that vitiligo can occur in patients with a history of vitiligo after radiotherapy, attributable to the Koebner phenomenon, where some treatments can induce new vitiligo lesions in the patient.Versión Publicad
Challenges for the prevention of hypertension among international migrants in Latin America: prioritizing the health of migrants in healthcare systems
Among the health priorities of international migrants, non-communicable diseases such as hypertension are of major interest due to their increasing prevalence, mainly in low- and middle-income countries. Previous evidence has reported a significant risk of hypertension in international migrants derived from multiple exposures during the migration process and at the destination, such as living conditions, health literacy and access to preventive services. Also, poorer disease control has been found compared to the local population. Considering existing deficiencies in access and use of healthcare services related to hypertension prevention and continuity of care of migrants globally, we aimed to offer a Latin American perspective of the challenges faced by international migrants residing in Latin America in accessing hypertension preventive care from a human rights, equity, and universal primary healthcare approaches. From a health systems perspective, we conducted a scoping review of scientific literature on hypertension prevention and control among international migrants in Latin America and the Caribbean. Based on the findings, we discuss the potential influence of migration and health policies on healthcare systems and individual and structural barriers to healthcare access, including lack of insurance, linguistic barriers, limited intercultural competence, and geographical and financial barriers. From existing evidence related to hypertension, we highlight the particular healthcare needs of migrants and their implications for regional public health goals. This aligns with promoting culturally tailored interventions considering the migration process, lifestyle patterns, structural vulnerabilities, and gender particularities in hypertension prevention, diagnosis, and treatment. We advocate for developing universal, voluntary, and systemic regional screening and disease control initiatives in Latin America for hypertension and other chronic conditions.Publicad
