Are local public expenditure reductions associated with increases in inequality in emergency hospitalisation? Time-series analysis of English local authorities from 2010 to 2017

Background Reductions in local government funding implemented in 2010 due to austerity policies have been associated with worsening socioeconomic inequalities in mortality. Less is known about the relationship of these reductions with healthcare inequalities; therefore, we investigated whether areas with greater reductions in local government funding had greater increases in socioeconomic inequalities in emergency admissions. Methods We examined inequalities between English local authority districts (LADs) using a fixed- effects linear regression to estimate the association between LAD expenditure reductions, their level of deprivation using the Index of Multiple Deprivation (IMD) and average rates of (all and avoidable) emergency admissions for the years 2010–2017. We also examined changes in inequalities in emergency admissions using the Absolute Gradient Index (AGI), which is the modelled gap between the most and least deprived neighbourhoods in an area. Results LADs within the most deprived IMD quintile had larger pounds per capita expenditure reductions, higher rates of all and avoidable emergency admissions, and greater between- neighbourhood inequalities in admissions. However, expenditure reductions were only associated with increasing average rates of all and avoidable emergency admissions and inequalities between neighbourhoods in local authorities in England’s three least deprived IMD quintiles. For a LAD in the least deprived IMD quintile, a yearly reduction of £100 per capita in total expenditure was associated with a yearly increase of 47 (95% CI 22 to 73) avoidable admissions, 142 (95% CI 70 to 213) all- cause emergency admissions and a yearly increase in inequalities between neighbourhoods of 48 (95% CI 14 to 81) avoidable and 140 (95% CI 60 to 220) all- cause emergency admissions. In 2017, a LAD average population was ~170 000. Conclusion Austerity policies implemented in 2010 impacted less deprived local authorities, where emergency admissions and inequalities between neighbourhoods increased, while in the most deprived areas, emergency admissions were unchanged, remaining high and persistentVersión publicad

Recommendations for Implementing the INTERACT3 CareBundle for Intracerebral Hemorrhagein Latin America: Results of a Delphi Method

    Get PDF
Introduction: The third Intensive Care Bundle with Blood Pressure Reduction in Acute Cerebral Hemorrhage Trial (INTERACT3) showed that the implementation of a care bundle improves outcomes after acute intracerebral hemorrhage (ICH). We aimed to establish consensus-based recommendations for the broader integration of the care bundle across Latin American countries (LAC). Methods: A 3-phase Delphi study allowed a panel of 32 healthcare workers from 14 LAC to sequentially rank statements relevant to 7 domains (training, resources/infrastructure, patient education, blood pressure, temperature, glycemic control, and anticoagulation reversal). The pre-defined consensus threshold was 75%. Results: A total of 43 statements reached consensus by the third round, with 12 new statements emerging through rounds. The highest-ranked statements in each domain emphasized critical aspects, but successful implementation requires appropriate resourcing. Key priorities were continuous training of all healthcare workers in ICH management, establishing protocols aligned with available resources, and collaborative interdisciplinary care supported by institutional networks. Statements related to anticoagulation reversal had the highest priority. Conclusions: Consensus statements are provided to facilitate integration of the INTERACT3 care bundle to reduce disparities in ICH outcomes in LAC.Versión publicad

Management of chronic pleural tuberculosis and non-tuberculous empyema in the 21st century

    Get PDF
This review addresses complexities in managing pleural tuberculosis (TB) and non-TB empyema. Historically, TB, often termed “the mother of thoracic surgery”, has witnessed a dramatic transformation from invasive surgical interventions to the dominance of medical therapy with the advent of effective anti- TB agents in the mid-20th century. This shift marked a pivotal moment in the treatment landscape, with medical therapy taking precedence and surgical methods reserved for diagnostic purposes or complications. In contrast, the history of non-TB empyema, dating back to Aristotle, evolved from open drainage and open surgery to minimally invasive surgery and fibrinolytic therapy. The review delves into the nuances of these conditions, exploring their clinical manifestations and the state-of-the-art management strategies that have emerged over time. Advancements in imaging modalities, such as computed tomography (CT), complemented by diagnostic laboratory techniques like the GeneXpert MTB/RIF assay, have revolutionized the diagnosis of TB, enhancing accuracy and expediting the process. The review also highlights the distinct clinical presentations of pleural TB and non-TB empyema, underscoring the challenges and intricacies in their management. It thoroughly explores the historical evolution, current diagnostic approaches, and advanced management strategies for these conditions, offering valuable insights for the learner.Versión Publicad

Translating the behaviour change technique taxonomy version 1 into Spanish: Methodology and validation

    Get PDF
Background: Precise and unequivocal specification of intervention content is key to facilitating the accumulation and implementation of knowledge. The Behaviour Change Technique Taxonomy v1 (BCTTv1) is the most widely used classification of behaviour change techniques (BCTs), providing a shared, standardized vocabulary to identify the active ingredients of behavioural interventions. However, the BCTTv1 is only available in English and this hampers its broad use and adoption. The aim of the present article is to report the process of translation of the BCTTv1 into Spanish. Methods: A bilingual team led the translation of the BCTTv1, involving seven iterative steps: (i) establish a Committee, (ii) forward translation from English to Spanish, (iii) back translation from Spanish to English, (iv) comparison of original BCTTv1 and back translation, (v) opportunistic comparison against an independent BCTTv1 translation, (vi) empirical testing, and (vii) final Committee review. Results: Changes as a result of the translation process included relabelling BCTs, amending definitions, and fixing conceptual and grammatical inconsistencies, yielding the final version. Very satisfactory inter-coder reliability in BCT identification was observed as part of the empirical testing (i.e., prevalence and bias-adjusted kappa scores > 0.8). Conclusions: This work provides the Spanish-speaking population with a rigorous and validated BCTTv1 translation which can be used in both research and practice to provide a greater level of intervention detail for evidence synthesis, comparison, and replication of behaviour change interventions. The translation process described here may prove helpful to guide future translation efforts in behavioural science and beyond.Versión publicad

Beyond the law. Virginity in the genealogy of liberalism

    No full text

Saccharomyces cerevisiae as a model for the identification of modifier genes of paraquat toxicity

    Get PDF
Thesis submitted to the Faculty of Medicine of the Universidad del Desarrollo for the Academic Degree of Doctor of Science and Innovation in MedicineParaquat (PQ) is a potent herbicide that induces oxidative stress and mitochondrial dysfunction. In humans, it is highly toxic, and it can induce Parkinson's Disease (PD). PD is a chronic and progressive neurodegenerative disease, with a worldwide prevalence of 315 per 100,000 people of all ages. Recent studies have revealed that neurons from PD patients exhibit stress responses, mitochondrial dysfunction, and metabolic deficits involving ATP and nicotinamide adenine dinucleotide (NAD+). While studies have demonstrated the potential of nicotinamide riboside (NR) to mitigate age- and disease-related metabolic decline in PD, the specific effects of NR on PD induced by pesticide exposure, such as paraquat, have not been extensively explored. Saccharomyces cerevisiae is a model organism that has allowed the study of relevant biological processes over time. It exhibits remarkable genetic diversity, making it an ideal model for studying the genetic basis of phenotypic variation. In this study, we hypothesized that genetically diverse Saccharomyces cerevisiae strains exposed to paraquat exhibit cellular and molecular responses that reveal potential modifier genes associated with its toxicity. We treated 1,011 isolates and 96 segregants from the cross of SA x WE founder Saccharomyces cerevisiae strains with PQ (75 μg/mL). We measured their growth curves and calculated the specific growth rate (μMax), used as a phenotypic trait for the genome-wide association studies (GWAS) and quantitative trait loci (QTL) mapping. We performed mixed-model association analysis using FaST-LMM for GWAS and linkage analysis using R/qtlsoftware, calculating LOD scores with a nonparametric model. We used diploid strains (603 isolates) for GWAS and 96 segregants for QTL mapping, identifying variants and markers that exceeded the significance threshold. Using these variants and markers, we identified candidate genes for validation. We validated NRT1 in a yeast mutant and subsequently showed that NR treatment provided significant protection against PQ-induced damage in both S288C yeast and iPSC-derived dopaminergic neurons from GBA-PD patients. In conclusion, this study provides valuable insights into the genetic and metabolic factors underlying PQ resistance in S. cerevisiae. The findings highlight the importance of NAD+ metabolism and mitochondrial function in mitigating the toxic effects of PQ. The identification of Nrt1 as a key transporter of NR, suggests potential therapeutic targets for interventions aimed at preventing or treating PQ-induced toxicity in PD

Introduction: Sudden hearing loss (SHL) is rare and its etiopathogenesis is still not clear. Corticosteroid therapy is of choice based on expert recommendations due to its theoretical effects and no based on proved clinical efficacy. Objectives: To assess whether there is a correlation between the final auditory outcome of patients with SHL treated with corticosteroids and the in vitro response of their leukocytes to corticosteroids, measured as differences in the relative expression of glucocorticoid receptor target genes. Material and Method: Case-control (total recovery and not recovered respectively) study of patients with SHL treated with corticosteroids at Clinical Hospital Universidad de Chile between 2017 and 2019. DNA was obtained and stored in the Biobanco de Tejidos y Fluidos de la Universidad de Chile (BTUCH). Peripheral blood mononuclear leukocytes were purified and cultured and then exposed to hydrocortisone. The difference in the relative expression of target genes (GFBP1, CAT, HSD17B12, APOA2), by Q-RTPCR was determined. Results: Thirty-five patients were recruited, 24 were included for the analysis: 11 cases and 12 controls, with and average age of 54.9 and 50.,8 years respectively, homogeneous sex distribution. There was no statistically significant difference in the relative expression of the target genes, upon in vitro exposure to corticosteroids, between both groups. Conclusion: In our study, model and evaluation system, no effects of corticosteroids were evidenced. With a larger number of patients, other target genes or other study protocols, we cannot rule out that differences could be detected.

    No full text
