Gallbladder Cancer: Is It Time to Modify the Explicit Health Guarantees (GES) Program?
Chile presenta en la actualidad una de las más altas tasas de mortalidad por cáncer de vesícula en el mundo. En 2006 el Ministerio de Salud Chileno incluyó la colelitiasis (pacientes 35-49 años) y el tratamiento con colecistectomía dentro del programa de Garantías Explícitas de Salud (GES).
Objetivo: Evaluar los resultados del programa GES "Colecistectomía preventiva del cáncer de vesícula biliar" con la intención de proponer cambios en el programa.
Método: Estudio retrospectivo de datos oficiales obtenidos de fuentes abiertas y mediante solicitud por transparencia desde el Ministerio de Salud e Instituto Nacional de Estadísticas.
Resultados: Desde su inicio el año 2006, se han efectuado un total de 284.139 notificaciones de pacientes portadores de colelitiasis pertenecientes al rango etario entre 35 y 49 años. Lo anterior puede correlacionarse al número de colecistectomías practicadas como parte del programa. De estos, 229.546 fueron a pacientes mujeres. Durante los últimos 20 años, la mortalidad ajustada de la enfermedad ha mostrado una caída desde 15.16 muertes/100.000 habitantes hasta 6.72/100.000 habitantes el año 2021. La disminución en la tasa de mortalidad comenzó antes de la implementación del programa. A lo largo del país, se observan áreas claramente identificadas como de alta incidencia, especialmente en la zona centro sur. Mientras se identifican otras de baja incidencia en el norte". En cuanto a notificaciones GES existen regiones con alta notificación que no siempre se correlacionan con la incidencia. Existiendo áreas de alta incidencia y baja notificación.
Conclusión: La disminución en la mortalidad del cáncer de vesícula en Chile, requiere que el programa de colecistectomía preventiva del cáncer de vesícula biliar sea modificado. Los recursos existentes debieran focalizarse en pacientes portadores de colelitiasis de áreas de alta incidencia además de portadores de factores de riesgo.
Chile currently has one of the highest mortality rates from gallbladder cancer in the world. In 2006, the Chilean Ministry of Health included cholelithiasis (patients aged 35-49) and treatment with cholecystectomy within the Explicit Health Guarantees (GES) program.
Aim: To evaluate the results of the GES Preventive Cholecystectomy Program for Gallbladder Cancer as to consider the need to propose changes to the model.
Methods: retrospective analysis of official data from the Chile Ministry of Health and National Statistics Institute.
Results: Since the program's inception in 2006, 284.139 notifications have been issued to patients aged between 35 and 49 with gallstones. Notifications can be correlated with the number of cholecystectomies performed under the program. Gallbladder cancer standardized mortality has shown a decrease over the last two decades, even before the program was implemented. High and low incidences are observed throughout the country. Regarding notifications, there are areas with a high incidence that are not always correlated with the areas with high notifications.
Conclusion: The decrease in the standardized mortality of GBC in Chile requires modifying the present GES Preventive Cholecystectomy program for Gallbladder Cancer. Resources should be oriented to patients living in high-incidence areas with known risk factors.Versión Publicad
Adherence to the Mediterranean diet and risk of stroke in a Chilean population: a case-control study
Introduction: Ñuble is the region of Chile with the highest stroke incidence rates in the country. The aim was to estimate the association between adherence to the MED diet and the first ischemic stroke in adult patients living in the Ñuble region.
Methods: a case-control hospital-based study. The cases (n = 89) were patients with first ischemic stroke, and controls (n = 178) were admitted to the same hospital during the same month the cases were recruited. We selected two controls for one case and paired them for sex and age (± 5 years). We used the food frequency questionnaire and the adherence Mediterranean diet questionnaire. A descriptive analysis of the variables and a conditional logistic regression to determine the association between variables.
Results: 71 % of the sample was ≥ 65 years old and 64 % were male. Cases smoked (11.2 %), consumed at least one drink per month (41.6 %), and had a diagnosis of hypertension (76.4 %) more frequently than controls. In the model adjusted for all variables, it is observed that those who are in quartile 2 of adherence (6-7 points) are 42 % less likely to have a cerebral infarction compared to those who have a lower score (p < 0.005).
Conclusions: our findings suggest that moderate adherence to a Mediterranean diet, defined by the PREDIMED score and adjustment for other variables, reduces the probability to first ischemic stroke.
Objetivo: estimar la asociación entre la adherencia a la dieta Mediterránea y un primer accidente cerebrovascular isquémico en adultos de la región de Ñuble.
Métodos: estudio de casos y controles con base hospitalaria. Los casos (n = 89) fueron pacientes con primer accidente cerrebrovascular (ACV) isquémico, y los controles (n = 178) fueron ingresados en el mismo hospital durante el mismo mes en que se reclutaron los casos. Se seleccionaron dos controles para un caso y se emparejaron por sexo y edad (± 5 años). Se utilizó el cuestionario de frecuencia de consumo de alimentos y el cuestionario de adherencia a dieta mediterránea. Se realizó un análisis descriptivo y una regresión logística condicional para determinar la asociación entre variables.
Resultados: el 71 % de la muestra tenía ≥ 65 años y el 64 % eran hombres. Los casos fumaban (11,2 %), consumían al menos una bebida al mes (41,6 %) y tenían diagnóstico de hipertensión arterial (76,4 %) con mayor frecuencia que los controles. En el modelo ajustado por todas las variables, se observa que aquellos que se encuentran en el cuartil 2 de adherencia (6-7 puntos) tienen un 42 % menos de probabilidades de sufrir un infarto cerebral en comparación con los que tienen una puntuación más baja (p < 0,005).
Conclusiones: nuestros hallazgos sugieren que la adherencia moderada a una dieta mediterránea, definida por la puntuación PREDIMED y el ajuste por otras variables, reduce la probabilidad de un primer ACV isquémico.Versión Aceptad
Enseñar y aprender competencias socioemocionales en la formación inicial docente: caso de estudio de Artes visuales y Matemática
La competencia socioemocional (en adelante, CSE) es clave en la formación docente y se sugiere enseñarla de manera explícita junto con aspectos declarativos del currículo.
Investigaciones en el área demuestran un déficit de dichas competencias en los programas de estudio y es necesario abordarlas curricularmente. En el marco de un Programa de Formación Pedagógica para docentes de enseñanza media planteamos la siguiente pregunta de investigación: ¿Cómo enseñan los docentes y cómo aprenden los estudiantes competencias socioemocionales en el curso de Didáctica y Prácticas para Artes visuales y Matemáticas? Las dos macrocategorías a considerar son: competencia socioemocional y proceso de enseñanza-aprendizaje. El objetivo es relevar el proceso de enseñanza y aprendizaje de las CSE en un programa de formación pedagógica para licenciados en el ámbito de las Didácticas y Prácticas en Artes visuales y Matemáticas. La metodología es
cualitativa, con trabajo etnográfico de aula, entrevistas en profundidad a docentes y grupo focal con estudiantes. Los resultados arrojan un abordaje indirecto y no necesariamente planificado de las competencias. La inclusión curricular de las CSE como contenido didáctico parece fundamental en la formación inicial docente, donde las simulaciones prácticas en el aula emergen como una oportunidad.Versión publicad
Intragastric administration of short chain fatty acids greatly reduces voluntary ethanol intake in rats
Alcohol use disorder (AUD) represents a public health crisis with few FDA-approved medications for its treatment. Growing evidence supports the key role of the bidirectional communication between the gut microbiota and the central nervous system (CNS) during the initiation and progression of alcohol use disorder. Among the different protective molecules that could mediate this communication, short chain fatty acids (SCFAs) have emerged as attractive candidates, since these gut microbiota-derived molecules have multi-target effects that could normalize several of the functional and structural parameters altered by chronic alcohol abuse. The present study, conducted in male alcohol-preferring UChB rats, shows that the initiation of voluntary ethanol intake was inhibited in 85% by the intragastric administration of a combination of SCFAs (acetate, propionate and butyrate) given before ethanol exposure, while SCFAs administration after two months of ethanol intake induced a 90% reduction in its consumption. These SCFAs therapeutic effects were associated with (1) a significant reduction of ethanol-induced intestinal inflammation and damage; (2) reduction of plasma lipopolysaccharide levels and hepatic inflammation; (3) reduction of ethanol-induced astrocyte and microglia activation; and (4) attenuation of the ethanol-induced gene expression changes within the nucleus accumbens. Finally, we determined that among the different SCFAs evaluated, butyrate was the most potent, reducing chronic ethanol intake in a dose-response manner. These findings support a key role of SCFAs, and especially butyrate, in regulating AUD, providing a simple, inexpensive, and safe approach as a preventive and intervention-based strategy to address this devastating disease.Versión Publicad
Biobanks and data interoperability in Latin America: engendering high-quality evidence for the global research ecosystem
Currently, each biobank in Latin America operates with its own set of standards for database creation and management, resulting in a lack of regional and international interoperability. Furthermore, regulations concerning data protection, curation, and the transfer of biological samples and associated data vary significantly from country to country, by complicating efforts to create a unified data-sharing platform. To address these challenges, Latin America should promote the development of an integrated regional network of biobanks to generate high-quality evidence within the global research ecosystem. This initiative will combine regulatory science—focused on interoperability standards across semantic, technical, legal, and organizational dimensions—and meta-science, which assesses the quality of scientific practice. Evidence indicates that harmonized standards in biobanks lead to higher-quality, more reliable data, thereby facilitating the reproducibility of scientific studies. This paper aims to identify and address existing regulatory, policy, and infrastructure gaps in Latin America to establish harmonized interoperability criteria essential for reproducing biomedical studies. Additionally, it seeks to propose minimum standards for regulating biobank networks, which will promote the development of medical products on a global scale, thereby engendering high quality evidence for the global research ecosystem and enhancing Latin America’s integration into it.Versión Publicad
Translation and Cross-Cultural Adaptation of the Toronto Extremity Salvage Score (TESS) for Latin American Spanish-Speaking Patients With Limb Sarcoma: Latin American Spanish TESS Adaptation
Background and Objectives: This study aims to translate and culturally adapt the Toronto Extremity Salvage Score (TESS) for Latin American Spanish-speaking patients, enhancing the tool's accessibility for evaluating postsurgical functional outcomes in sarcoma patients across Latin America. Methods: The TESS questionnaires for lower extremity (LE) and upper extremity (UE) were translated and adapted following international guidelines. The process included forward and backward translation, expert committee review, and pretesting with cognitive interviewing. Patients treated for bone or soft tissue tumors in LE or UE were recruited to complete the adapted questionnaires. Test-retest reliability was evaluated by having participants complete the questionnaire again 2 weeks after the initial assessment. Results: A total of 89 participants completed the questionnaires. The study found high internal consistency, with Cronbach's alpha values reaching 0.9437 for LE and 0.9402 for UE. An agreement rate of 98.4% for the global score of TESS-LE (95% confidence interval [CI]: 0.909-1.059) and 93.9% for TESS-UE (95% CI: 0.882-0.995) was observed, demonstrating strong test-retest reliability. Conclusions: The Latin American Spanish version of TESS for both lower and upper extremities is a reliable and culturally appropriate tool for assessing physical function in limb sarcoma patients. Further validation across diverse Latin American populations is encouraged to strengthen its broad applicability.Versión Aceptad
Actividad física, comportamiento sedentario y su asociación en personas con infarto agudo al miocardio: análisis secundario de la ENS 2017
Actividad de Grado presentada a la Facultad de Medicina de la Universidad del Desarrollo para optar al grado académico de Magíster en Terapia Física y Rehabilitación (MAKI)Antecedentes: La Actividad Física (AF) constante y la reducción del Comportamiento Sedentario (CS) están estudiados en prevención y tratamiento de Enfermedades Crónicas No Trasmisibles. El Infarto Agudo al Miocardio (IAM) es una de las principales causas de mortalidad y discapacidad en Chile, siendo la AF y el CS factores claves en su rehabilitación.
Objetivo: Analizar la asociación entre el cumplimiento de las recomendaciones de AF y CS (>4 horas diarias) en personas con IAM mediante un análisis secundario de la Encuesta Nacional de Salud.
Método: Estudio de base poblacional de la ENS-2017, muestra= 6233 participantes donde 300 presentaron IAM. La AF y el CS se reportaron mediante el Global Physical Activity Questionnaire. Se utilizó el test Chi cuadrado para la comparación de variables y modelos de regresión logística para el análisis del objetivo con un nivel de significancia al 5%.
Resultados: Entre la población con IAM, el 49% presentó niveles bajos de AF, 21% niveles moderados y el 23% niveles vigorosos. El 25% reportó un CS >4 horas diarias. Las personas con IAM tuvieron menos probabilidades de cumplir las recomendaciones de AF (OR=1,5; IC95%=1,2-2,0) en comparación con la población sin IAM en el modelo univariado. No existe asociación entre el IAM y CS >4 horas en modelo univariado y múltiple.
Conclusiones: La presencia de IAM se asoció a un bajo cumplimiento de recomendaciones de AF. No se observaron diferencias significativas de CS en esta población
Emotional text messages affect the early processing of emoticons depending on their emotional congruence: evidence from the N170 and EPN event related potentials
Emoticons have been considered pragmatic cues that enhance emotional expressivity during computer-mediated communication.
Yet, it is unclear how emoticons are processed in ambiguous text-based communication due to incongruences
between the emoticon's emotional valence and its context. In this study, we investigated the electrophysiological correlates
of contextual influence on the early emotional processing of emoticons, during an emotional congruence judgment task.
Participants were instructed to judge the congruence between a text message expressing an emotional situation (positive or
negative), and a subsequent emoticon expressing positive or negative emotions. We analyzed early event-related potentials
elicited by emoticons related to face processing (N170) and emotional salience in visual perception processing (Early Posterior
Negativity, EPN). Our results show that accuracy and Reaction Times depend on the interaction between the emotional
valence of the context and the emoticon. Negative emoticons elicited a larger N170, suggesting that the emotional information
of the emoticon is integrated at the early stages of the perceptual process. During emoticon processing, a valence effect
was observed with enhanced EPN amplitudes in occipital areas for emoticons representing negative valences. Moreover,
we observed a congruence effect in parieto-temporal sites within the same time-window, with larger amplitudes for the
congruent condition. We conclude that, similar to face processing, emoticons are processed differently according to their
emotional content and the context in which they are embedded. A congruent context might enhance the emotional salience
of the emoticon (and therefore, its emotional expression) during the early stages of their processing.Versión publicad
